Objectives: This study aims to evaluate subclinical atherosclerosis using the Ankle-Brachial Index (ABI) in patients with psoriatic arthritis (PsA). Patients and methods: This case-control study included 51 PsA patients (24 males, 27 females; median age 47; range, 41 to 52 years) recruited at our hospital’s outpatient clinics between October 2016 and January 2017 and 50 healthy controls (24 males, 26 females; median age: 48.5; range, 40.7 to 56 years). Anthropomorphic measurements and laboratory results were recorded. In patients, the 66 swollen/68 tender joints count, dactylitis score, Leeds Enthesitis Index, Health-related Quality of Life, the Psoriasis Area and Severity Index, and Dermatology Life Quality Index were evaluated. Ankylosing Spondylitis Quality of Life and Bath Ankylosing Spondylitis Disease Activity Index were applied to patients with axial disease. Then, Composite Psoriatic Disease Activity Index was determined. A Doppler probe and a standard blood pressure cuff were used to calculate the ABI values for each participant. Results: Patients had lower right ABI (median, 1.05 vs. 1.1, p<0.01), lower left ABI (1.04 vs. 1.09, p<0.01) and lower overall ABI (1.03 vs. 1.09, p<0.01) compared with healthy subjects. Twelve (23.5%) patients had borderline ABI, but none of the controls (p<0.01). Patients with borderline ABI had a longer duration of psoriasis (25 vs. 15 years, p=0.03). The distribution of borderline ABI value was statistically significant between patients with axial disease and peripheral disease only (42.1% vs. 12.5%, p=0.02). Disease activity was found as an independent risk factor for borderline ABI in a binary logistic regression (odds ratio 6.306, 95% confidence interval 1.185 to 33.561, p=0.031). Conclusion: Lower ABI was found in PsA patients than healthy controls even in those matched with traditional cardiovascular risk factors. Amaç: Kronik bel ağrısı sık karşılaşılan önemli bir halk sağlığı sorunudur. Kronik bel ağrısının ekonomik yükünün oldukça yüksek olduğu bildirilmekle birlikte Türkiye için ne olduğu net olarak bilinmemektedir. Bu çalışmanın amacı kronik bel ağrılı hastaların toplam ekonomik yükünü araştırmaktır Materyal ve Metod: Çalışmaya kronik bel ağrısı olan 18 yaş üzeri 211 hasta dahil edildi. Hastaların sosyo demografik özellikleri, son 6 ay içindeki sağlık sistemi kaynakları kullanımı, son 3 ay içerisindeki çalışamama durumları anketlerle sorgulandı. Tüm direkt ve indirekt harcamalar hesaplandı. Direkt harcamaların içine doktor muayeneleri, tetkikler, ilaçlar, hastane yatışları, ortopedik yardımcı cihaz kullanımları, fizik tedavi ve son 6 aydaki bel ağrısına bağlı sakatlık nedenli hasta bakım ödemeleri dahil edildi. Çalışmamızda indirekt harcamalar son 3 ay içindeki üretim kaybı hesaplanarak tahmin edilmeye çalışıldı Bulgular: Kronik bel ağrısı için yıllık direkt ekonomik yük 2011 değerlerine göre 823.91 TL (346.14 Euro, 443.39 USD), indirekt ekonomik yük ise 5501 TL (2311.34 Euro, 2960.71 USD) olarak hesaplandı. Sonuç: Kronik bel ağrısına bağlı indirekt harcamaların direkt harcamalara göre çok daha fazla olduğu görülmektedir. Bel ağrısının etkili tedavisiyle hasta rapor kullanımına bağlı üretim kaybının ve harcamaların azaltılacağı sonucuna varılmıştır., Background: Chronic low back pain (LBP) is a common and important health problem. The economic cost of LBP is very high and its burden in Turkey is not exactly known. The aim of this study is to research the total economic burden of the LBP among the chronic LBP patients. Material and Methods: 211 patients over 18 years of age having chronic LBP were included. Patients sociodemographics, healthcare resource use in last 6 months, inability to work in last 3 months were collected by using questionnaires. We calculated all direct and indirect costs. Direct costs include medical visits, investigations, medications, hospitalizations, orthopedical aids, physical therapy and home payments during the last 6 months. Indirect costs in our study were evaluated mostly with productivity losts for the last 3 months. Results: The annual direct costs for chronic LBP per patient were estimated at 823.91 TL (346.14 Euros or 443.39 USD) and the indirect costs were estimated at 5501 TL (2311.34 Euros or 2960.71 USD) in 2011 prices. Conclusion: The indirect costs for chronic LBP seems to be higher than the direct costs. The productivity losses due to sick leave could be reduced with effective treatments and could help cost savings.

- Abstract
Objective: In this study, it was aimed to evaluate the relationship between vitamin D level and pain severity, localization and duration in patients with non-specific musculoskeletal pain. Materials and Methods: Patients who applied to physical medicine and rehabilitation outpatient clinics due to non-specific muscle pain in 19 in Turkey were retrospectively screened. Three thousand four hundred fourpatients were included in the study, whose pain level was determined by visual analog scale (VAS) and the painful region, duration of pain and vitamin D level were reached. D group was found to be D deficient (group 1) when 25 (OH) D level was 20 ng/mL or less and group D 2 (vitamin D deficiency) was higher than 30 ng/mL (group 3). The groups were compared in terms of pain duration, localization and severity. In addition, the correlations of pain localization, severity and duration with vitamin D levels were examined. Results: D vitamin deficiency was detected in 2202 (70.9%) of 3 thousand four hundred and four registered patients, and it was found that vitamin D deficiency in 516 (16.6%) and normal vitamin D in 386 (12.4%). The groups were similar in terms of age, body mass index, income level, duration of complaint, education level, family type and working status (p>0.05). There was no statistically significant difference between groups in terms of VAS, pain localization and duration scores (p>0.05). Conclusion: Our study shows that vitamin D deficiency in patients with nonspecific musculoskeletal pain is not associated with the severity and duration of pain. 21. Osteopathic manipulative treatment improves function and relieves pain in knee osteoarthritis: A single-blind, randomized-controlled trial.

Objectives: In this study, we aimed to compare the efficacy of osteopathic manipulative treatment (OMT) to exercise treatment in the knee osteoarthritis (OA). Patients and methods: A total of 100 patients (9 males, 76 females; mean age 54.8±8.5 years; range, 40 to 70 years) with Stage II-III bilateral knee OA enrolled to the study and randomized into two groups between January 2015 and June 2015. Group 1 performed exercise and received OMT and Group 2 performed exercise alone. We assessed the clinical parameters with Western Ontario MacMaster Questionnaire (WOMAC) pain score, WOMAC joint stiffness score, WOMAC physical function score, Visual Analog Scale (VAS) and 50-m walking time. All patients were assessed at the beginning of the study, just after the treatment, and four weeks after the treatment. Results: There was no significant difference between groups in terms of physical examination and clinical assessment parameters before treatment. Functional improvement (p<0.05) and pain relief (p<0.05) were significantly higher in the exercise + OMT group. Conclusion: Based on our study results, OMT is a particular treatment used by osteopathic physicians to complement conventional treatment of OA of the knee. Objective: The objective of this study was to examine the association between 25-hydroxyvitamin D [25(OH)D] and cognitive function in younger women. Materials and Methods: One hundred four women aged 18 to 65 years were recruited in the study. The demographic data were recorded and the Standardized Mini Mental State Examination (SMMSE) was administered. Patients were divided into three groups; 25(OH)D level <20 ng/mL regarded as vitamin D deficiency, 20-29 ng/mL as vitamin D insufficiency, ≥30 ng/mL as normal. Results: There was no statistically significant relationship between SMMSE scores and serum 25(OH)D levels. Conclusion: We found no evidence of associations between lower 25(OH)D levels and cognitive functions in younger women. [ABSTRACT FROM AUTHOR]

26. Bilateral and atypical localized elastofibroma dorsi in a patient with shoulder and back pain: A case report.

- Abstract
A 61-year-old woman presented with localized back pain at the left inferomedial border of the scapula and left shoulder pain. On physical examination, a soft tissue mass was detected on the left scapula. Magnetic resonance imaging showed a mass at the left suprascapular region and another mass in the left infrascapular region. A Tru-Cut biopsy of the lesion at the left suprascapular region was obtained due to atypical localization. Pathological examination was consistent with an elastofibroma. During wire-marking, another mass was detected at the right infrascapular region. Three mass lesions were excised on consecutive operations. There is a little evidence in the literature regarding the effectiveness of Botulinum Toxin type A injections as an adjuvant therapy to customised rehabilitation programs in pediatric patients with hereditary spastic paraplegia. This is a report on two siblings aged 4 and 6, who underwent 3 sessions of botulinum toxin A injections to the hip adductors and ankle plantar flexor muscles. Additionaly, patients were given a home exercise program consisted of stretching the agonist muscles and strengthening of the antagonist muscles. Dynamic ankle foot orthoses were also prescribed and used regularly Both patients showed remarkable improvements in muscle tone and gross motor function measurements. There has been a recent interest in calcitonin as a potential treatment for osteoarthritis, based on its metabolic activities in both bone turnover and cartilage. The aim of this study was to evaluate the effects of nasal form calcitonin on knee osteoarthritis and quality of life in women who receive calcitonin treatment for postmenopausal osteoporosis. Two hundred and twenty postmenopausal women, aged between 55 and 65 years with knee pain and knee osteoarthritis, graded II-III by using Kellgren-Lawrence radiographic scoring system, were included. Western Ontario and McMaster Universities (WOMAC) osteoarthritis index, the quality of life questionnaire of the European Foundation for Osteoporosis (QALEFFO-41) and visual analog scale were used for the algofunctional assessments. Need of rescue analgesic was recorded. Pain ( P < 0.001), stiffness ( P < 0.05), functional capability ( P < 0.05) and total score of WOMAC ( P < 0.05) revealed statistically significant improvements after 3 months of the treatment and remained consistent throughout 1 year of the treatment period. Participants experienced significant reductions in WOMAC perceptions of pain (−53 %), joint stiffness (−44 %) and limitations in physical function (−49 %) at the end of 1 year of calcitonin treatment. Need of rescue analgesic intake was reported to have decreased approximately by 60 % at the end of the 1-year treatment period. QUALEFFO_41 scores improved: 37.6 (baseline), 30.9 (3 months), 28.0 (6 months) and 24.4 (1 year). In conclusion, nasal calcitonin treatment provided dual action on osteoporosis and osteoarthritis with significant improvements in quality of life and algofunctional results in knee osteoarthritis. Results: There was a statistically significant relation-ship between body mass index, wearing style, physical activity and 25(OH)D levels (p<0.01). Also, there was a statistically significant correlation between L2-L4 z-scores and 25(OH)D levels (p<0.05). Although there was a statistically significant correlation between Modified Romberg test and 25(OH)D levels (p<0.05), there was no correlation with Timed Get Up and Go test and 10 meter Walk Test (p>0.05). Objective: The aims of this study were to evaluate the burnout of primary caregivers of children with cerebral palsy (CP), and to assess the relationship between burnout levels and the variables. Material and Method: Demographic characteristics, CP type, ambulation status, Gross Motor Functional Classification System (GMFCS) levels, accompanying problems were recorded. Caregivers were asked to fill Maslach Burnout ınventory (MBı). Results: 115 patients were included in the study (56.5% boys, 43.5% girls). Their mean age was 7.84years. Primary caregivers of 111 children were their mothers (96.5%). The neurologic classification were as follows: diplegia 27.8%, tetraplegia 43.5%, hemiplegia 20%. The GMFCS levels were as follows: level 1 15.7%, level 2 13.9%, level 3 17.4%, level 4 23.5%, level 5 29.6%. MBı subdimensions measurement scores were: reduced personal accomplishment: 23.25, emotional exhaustion: 12.21, and depersonalization: 3.81. We didn't detect any significant correlation between the GMFCS levels of children and caregivers' burnout. The presence of pain (69.6%) and chronic disease (31.3%) were releated with emotional exhaustion (p=0.006, p=0.008, respectively). The children's ambulation status was related with emotional exhaustion (p=0.03). Fathers' lack of interest (27%) was related with depersonalization (p=0.0019). Speech disorder was related with emotional exhaustion and depersonalization (p=0.031, p=0.04, respectively). There was no significant relationship between caregivers' burnout and caregivers' age-education status, income level, the presence of epilepsy and mental retardation in child. Conclusion: The presence of chronic disease and pain, fathers' lack of interest, children's ambulation status and speech disorders were detected as the factors that case caregivers burnout. Psychologic and physical health of caregivers affect child. So, not only children with CP but also their caregivers should be considered. [ABSTRACT FROM AUTHOR]

Objective. – To evaluate the efficacy of intramuscular injection of thiocolchicoside (4 mg–2 ml) compared to placebo administered twice daily for five days in patients suffering from acute low back pain.Patients and Methods. – Multicenter, randomized, double-blinded, placebo-controlled trial. The study was conducted between July 1998 and March 2000 in five centers. Hospitalized patients with acute low back pain were included. The primary evaluation criterion was spontaneous pain assessed by visual analogue scale (VAS). Hand- to floor distance, muscle spasm intensity, patients’ global evaluation and analgesic tablet (paracetamol) consumption were used as secondary evaluation criteria.Results. – One hundred forty nine (149) patients were included. Both groups showed improvement on spontaneous pain assessed by Vas at the end of day 1; however improvement was statistically significant in thiocolchicoside group on day 3 (p < 0.001). Hand- to floor distance and muscle spasm determined by palpation decreased significantly on day 5 in thiocolchicoside group (p < 0.0005 for both). According to the patients’ global evaluation, 76.8% of patients in thiocolchicoside group evaluated the treatment as very good/good (p < 0.0005). Also a significant difference was observed in favor of thiocolchicoside group in the total paracetamol consumption (p < 0.001). Treatment was well tolerated in both groups and none of the patients was withdrawn from the study due to an adverse effect.Conclusion. – Twice daily administration of 4 mg thiocolchicoside for five days provides an efficient and safe treatment for patients with acute low back pain accompanied by muscle spasm. Objective: In this study, it was aimed to evaluate the relationship between vitamin D level and pain severity, localization and duration in patients with non-specific musculoskeletal pain. Materials and Methods: Patients who applied to physical medicine and rehabilitation outpatient clinics due to non-specific muscle pain in 19 in Turkey were retrospectively screened. Three thousand four hundred fourpatients were included in the study, whose pain level was determined by visual analog scale (VAS) and the painful region, duration of pain and vitamin D level were reached. D group was found to be D deficient (group 1) when 25 (OH) D level was 20 ng/mL or less and group D 2 (vitamin D deficiency) was higher than 30 ng/mL (group 3). The groups were compared in terms of pain duration, localization and severity. In addition, the correlations of pain localization, severity and duration with vitamin D levels were examined. Results: D vitamin deficiency was detected in 2202 (70.9%) of 3 thousand four hundred and four registered patients, and it was found that vitamin D deficiency in 516 (16.6%) and normal vitamin D in 386 (12.4%). The groups were similar in terms of age, body mass index, income level, duration of complaint, education level, family type and working status (p>0.05). There was no statistically significant difference between groups in terms of VAS, pain localization and duration scores (p>0.05). Conclusion: Our study shows that vitamin D deficiency in patients with nonspecific musculoskeletal pain is not associated with the severity and duration of pain. Amaç: Bu çalışma ile özgül olmaya kas iskelet sistemi ağrısı olan hastalarda; vitamin D düzeyi ile ağrı şiddeti, lokalizasyonu ve süresi arasındaki ilişkinin değerlendirilmesi amaçlandı. Gereç ve Yöntem: Türkiye genelinde 19 merkezde özgül olmayan kas ağrısı nedeniyle fiziksel tıp ve rehabilitasyon polikliniklerine başvuran hastalar retrospektif olarak tarandı. Taranan hastalar arasında ağrı düzeyi visüel analog skala (VAS) ile belirlenmiş olan ve ağrılı bölge, ağrı süresi ve vitamin D düzeyi kayıtlarına ulaşılabilen 3104 hasta çalışmaya alındı. 25(OH)D düzeyi 20 ng/mL’den düşük ise D vitamini eksikliği (grup 1), 21 ile 29 ng/mL arasında ise D vitamini yetersizliği olan grup (grup 2) ve 30 ng/mL’den yüksek ise normal grup (grup 3) olarak kabul edildi. Gruplar ağrı süresi, lokalizasyonu ve şiddeti açısından karşılaştırıldı. Ayrıca D vitamini düzeyleri ile ağrı lokalizasyonu, şiddeti ve süresinin korelasyonları incelendi. Bulgular: Üç bin yüz dört kayıtlı hastanın 2202’sinde (%70,9) D vitamini eksikliği saptanırken, 516’sında (%16,6) D vitamini yetersizliği ve 386’sında (%12,4) normal sınırlarda D vitamini olduğu tespit edildi. Gruplar; yaş, vücut kitle indeksi, gelir düzeyi, şikayet süresi, eğitim düzeyi, aile tipi ve çalışma durumu yönünden benzer olarak saptandı (p>0,05). VAS, ağrı lokalizasyonu ve süresi skorları yönünden gruplar arasında istatistiksel olarak anlamlı fark bulunmadı (p>0,05). Giriş ve Amaç: Mekanik kaynaklı bel ağrılarının oldukça yaygın ve önemli bir sağlık sorunu olduğu bilinmektedir. Ülkemizde bel ağrısına bağlı iş gücü kaybının boyutları ve bel ağrısının maliyeti tam olarak bilinmemektedir. Bu çalışmada, İstanbul Medeniyet Üniversitesi Göztepe Eğitim Araştırma Hastanesi Fizik Tedavi ve Rehabilitasyon kliniğine başvuran kronik bel ağrılı hastalarda, sağlık kaynaklarının kullanımı, iş gücü-üretim kaybı ve hastalığın toplam ekonomik yükünün araştırılması planlanmıştır.Gereç ve Yöntemler: Çalışmaya, İstanbul Medeniyet Üniversitesi Göztepe Eğitim ve Araştırma Hastanesi, Fizik Tedavi ve Rehabilitasyon polikliniklerine başvuran 18 yaş ve üstü, son 3 ay boyunca, günlerin en az yarısında ağrılı olan kronik bel ağrılı 211 hasta dahil edilmiştir. Poliklinikte, sosyo-demografik verileri, son 6 ay içindeki sağlık sistemi kaynaklarının kullanımı, son 3 ay içindeki çalışabilme durumlarını içeren bir anket, fonksiyonel durumu değerlendirmek için Rolland Morris Disabilite anketi (R&M), depresyon durumunun değerlendirilebilmesi içinde Beck Depresyon Ölçeği (BDÖ) dolduruldu. Çalışma sosyal açıdan yapıldığı için, kişinin kendisi, iş vereni ve sağlık sistemi tarafından yapılan her türlü harcama hesaplandı. Direkt harcamaların içine hastane bakımı, poliklinik viziti, tanısal testler, medikal tedaviler, ortopedik yardımcı cihazlar, fizik tedavi ve ödenen ev yardımları dahil edildi. Çalışmamızda kullanılan indirekt harcamalar daha çok üretim kayıplarıyla (sağlık durumundaki değişikliğe ya da işe gitmemeye bağlı olarak üretim gücündeki azalma, iş gücü kaybı, erken emeklilik, refakatçinin işe gidememesi, yol masrafı ya da ev işi için yardım alma gibi) değerlendirildi. İstatistiksel Analiz: İstatistiksel değerlendirmeler SPSS 16.0 paket programı kullanılarak yapıldı. Bulgular: 211 kişilik hasta grubu sonuçlarına dayanarak kronik bel ağrısı için hasta başı yıllık direkt harcama 823.91 TL (346.14 Euro, veya 443.39 USD), toplam harcamanın %13'ü, indirekt harcama ise 5501 TL (2311.34 Euros, or 2960.71 USD), toplam harcamanın %87'si idi. Yaptığımız BDÖ sonuçları ortalama 17.33 (SD 10.34) idi. Hastalarımızın %59.7'si orta ve ağır düzeyde depresyon ve anksiyete puanı aldı. R&M skorlarına göre sonuçlarımız ortalama 15.67 (SD 5.23) idi. Hastalarımızın %91.42'si orta ve ağır düzeyde fonksiyonel dizabilite puanı aldı. Sonuç: Özellikle iş günü ve iş gücü kaybına bağlı indirekt harcamalar, ilaç, doktor viziti, fizyoterapi ve hastanede yatışa bağlı direkt harcamalardan çok daha fazla gözükmektedir. Daha etkin bir tedaviyle indirekt harcamaların azaltılmasının maliyeti düşüreceğine inanmaktayız. Objective: Mechanical low back pain (LBP) is a common and important health problem. The economic burden of LBP in Turkey is not exactly known. In this study, we aimed to research the health sources usage, work and productivity loss and total economic burden of the LBP among the chronic LBP patients who were admitted to the Physical Therapy and Rehabilitation out patient clinic of İstanbul Medeniyet University Göztepe Education and Research Hospital. Material and Methods: 211 adult (over 18 years, having LBP in at least half of the days of the last 3 months) among the chronic LBP patients who were admitted to the Physical Therapy and Rehabilitation out patient clinic of İstanbul Medeniyet University Göztepe Education and Research Hospital between September 2009-November 2009 were included in to the sudy. Patients socio-demographics, healthcare resource use in last 6 months, inability to work in last 3 months, Rolland&Morris Disability Index for the functional status and psycological health at the time of the study with Beck Depression Scale were collected by using questionnaires. Because of the social aspect of the study we calculated all direct and indirect costs which were done by the patient, employee and health care system. Direct costs included medical visits, investigations, medications, hospitalizations, orthopedical aids, physical therapy and home payments. İndirect costs in our study were evaluated mostly with productivity losts ( decrease in productivity due to abscence in work or health status change, productivity lost, early retirement, lost work days of the company of the patient, getting assistance for housekeping and extra).Statistical Analysis: Statistical analysis were done with SPSS 16.0 programme.Results: For the 211 patients, the total annual direct costs for chronic LBP per patient were estimated at 823.91 TL (346.14 Euros, or 443.39 USD). The indirect costs were estimated at 5501 TL (2311.34 Euros, or 2960.71 USD) per patient. The mean Beck Depression Scale points were 17.33 (SD10.34). %59.7 of patients got severe and moderate depression and anxiety points. The mean Roland & Morris score was 15.67 (SD 5.23). %91.42 of the patients got moderate and severe functional disability points.Conclusions: The indirect costs for chronic LBP seems to be higher than the direct costs. The productivity losses due to sick leave could be reduced with effective treatments and could help cost savings. 93

Amaç: Postmenopozal Osteoporoz (OP) hastalarında sırt ağrısının uyku kalitesi, yaşam kalitesi ve depresyon üzerine olan etkilerini, bu bulguların birbiri ile ilişkisini araştırmak.Materyal ve Metot: Çalışmaya Sağlık Bakanlığı Göztepe Eğitim Ve Araştırma Hastanesi Fizik Tedavi Ve Rehabilitasyon Kliniği Osteoporoz Polikliniğine başvuran ve postmenopozal osteoporoz tanısı alan 105 hasta alındı. Hastalar sosyodemografik özellikler ve risk faktörleri gibi tanımlayıcı özellikler içeren hasta bilgi formu, Avrupa Osteoporoz Derneği Yaşam Kalitesi Anketi (QUALEFFO-41), Beck Depresyon Ölçeği (BDÖ) ve Pittsburgh Uyku Kalite İndeksi (PUKİ) soru formlarını cevaplandırdı. Sırt ağrısının şiddeti, Görsel Analog Ölçek (VAS) ile değerlendirildi. İstatistiksel analizler için NCSS (Number Cruncher Statistical System) 2007&PASS 2008 Statistical Software (Utah, USA) programı kullanıldı.Bulgular: Hastaların yaşları 46 ile 75 arasında değişmekte olup, ortalaması 61,16±7,59 idi. Ağrı skorları yükseldikçe; depresyon skorlarının da arttığı ve uyku kalitesi ile yaşam kalitesinin bozulduğu tespit edildi (p0,05). L1-4 KMY ve T-skorları; yaşam kalitesi, uyku kalitesi ve depresyon ile de ilişkisiz bulundu (p>0,05). Yeterli fiziksel aktiviteye sahip olanların yaşam kalitesi, yeterli fiziksel aktivitesi olmayanlara göre anlamlı derecede iyi olduğu görüldü (p0,05). Ek kronik, sistemik hastalığı bulunanların yaşam kalitesi ise, ek hastalığı bulunmayanlara göre anlamlı derecede kötü idi (p0,05).Sonuç: Postmenopozal OP hastalarında yüksek ağrı skorları, düşük uyku ve yaşam kalitesi ve depresyon ile ilişkilidir. Depresyon, uyku bozukluğu ve düşük yaşam kalitesi ise; birbirini etkilemektedir ve birlikte görülme olasılığı yüksektir. Postmenopozal OP hastalarında temel tedavinin yanı sıra sık görülen bir semptom olan sırt ağrısı ile mücadele edilmesinin, uyku ve yaşam kalitelerinde ve depresyon üzerinde olumlu etkiler sağlayacağı kanısındayız.Anahtar Kelimeler: Ağrı, Depresyon, Postmenopozal Osteoporoz, Uyku Kalitesi, Yaşam Kalitesi Objective: The aims of this study were to asses the impact of back pain on quality of sleep , quality of life and depression in postmenopausal osteoporosis patients, these findings in relation to each other.Material and Method: Onehundered five (105) postmenopausal osteoporosis patients who came to Ministry of Health Goztepe Education and Research Hospital Physical Medicine and Rehabilitation Department Osteoporosis out-patient clinic were included into the study. Patients answered questions in forms that includes distinctive marks (sociodemographic properties, risk factors), Quality Of Life Questionnaire of The Europan Foundation for Osteoporosis (QUALEFFO-41), Beck Depression Scale and Pittsburgh Sleep Quality Index (PUKİ). Vizüel Anolog Scale (VAS) was used to asses respectively back pain. NCSS (Number Cruncher Statistical System) 2007&PASS 2008 Statistical Software (Utah, USA) program and Spearman's rho coefficient of correlation were used to asses statistical accounting.Results: Age of these 105 patients were changing between 46 and 75 with average of 61,16±7,59. It was found that higher pain scores affects qualiy of life, quality of sleep and depression negatively (p0,05). L1-4 T-score and BMD were not related with depression, sleep quality and quality of life (p>0,05). We detected a positive relationship between physical activity and Qualeffo-41 scores (p0,05). The quality of life patients with chronic systemic disease was significantly worse (p0,05).Conclusions: Higher pain scores are associated with low quality of sleep and quality of life and depression. Depression, sleep disorders, lower quality of life affect each other and most likely to ocur with. In addition to basic treatment in patients with postmenopausal osteoporosis is to deal with back pain, the opinion will provide a positive impact on their quality of sleep and quality of life and depression.Key Words: Pain, Depression, Postmenopausal Osteoporosis, Quality of Sleep, Quality of Life 94

ÖZETAmaç: Romatoid artritte Türkçe SF-36'nın güvenilirlik ve geçerliliğini araştırmak.Materyal ve Metot: Hastaların demografik özellikleri kayıt edildikten sonra yaşamkalitesinin ölçümü için romatoid artritli hastalarda kullanılan sağlık değerlendirmeanketi (HAQ) ve SF-36 formları dolduruldu, hastalık aktiviteleri DAS-28kullanılarak hesaplandı. Hastalar 3 ay sonra tekrar çağrılarak kendilerini ilkgelişlerine göre nasıl hissettikleri soruldu ve verdikleri cevaplara göre daha iyi, aynıve daha kötü olmak üzere 3 gruba ayrıldı. Daha sonra ilk gelişlerinde yapılan testlertekrarlandı.Bulgular: Çalışmadaki 141 hastanın %9,9'u (n=14) erkek; %90,1'i (n=127)kadındır. SF-36'nın güvenilirlik çalışmasında ölçek alt boyutlarının Cronbach alfadeğerleri 0,792-0,992 arasında değişmekte olup, bunun sonucunda SF-36 yüksekdüzeyde güvenilir bulunmuştur. Madde-toplam puan korelasyon katsayıları da her biralt ölçek için ilgili maddelerle ayrı ayrı hesaplanmıştır. Fiziksel fonksiyonda 0.436-0.840 arasında, fiziksel rol güçlüğünde 0.887-0.895 arasında; ağrıda 0.861-0.958arasında, sağlığın genel olarak algılanmasında 0.564-0.892 arasında; vitalite(enerji)'de 0.702-0.841 arasında; sosyal fonksiyonda 0.949-0.952 arasında,emosyonel rol kısıtlamasında 0.396-0.473 arasında ve mental sağlıkta 0.456-0.824arasında bulunmuştur. Daha sonra her iki SF-36 ölçümü karşılaştırılmış ve p değerifiziksel fonksiyon için 0.065, fiziksel rol güçlüğü için 0.391, ağrı için 0.555, genelsağlık için 0.215, vitalite için 0,862, sosyal fonksiyon için 0.107, emosyonel rolfonksiyon için 0.616, mental sağlık için 0.112 olarak saptanmış; p>0,05 olduğu içinikinci ölçümlerde ilk ölçümlere göre bir farklılık saptanmamıştır.SF 36 geçerlilik çalışması, beyana dayalı durumları ikinci gelişlerinde ilkine göredeğişmeyen 63 olgu üzerinde yapılmış olup; test tekrar test arasındaki korelasyon,intra-class korelasyon katsayıları ile değerlendirildiğinde her iki uygulama arasında xistatistiksel olarak anlamlı ilişki görülmektedir. - Abstract
Objectives: This study aimed to investigate the effects of exercise-induced lower limb muscle fatigue on postural stability in female patients with fibromyalgia (FMS)., Method: This study included 19 female patients diagnosed with FMS according to the 2010 American College of Rheumatology criteria and 19 age-matched healthy controls. Muscle fatigue was induced by chair sit-to-stand (STS) repetitions in all participants. Postural stability was evaluated before and immediately after muscle fatigue test (MFT) in standard bipedal and tandem stances with eyes open (EO) and eyes closed (EC), and in single leg stance with EO using a foot pressure platform., Results: The mean number of STS repetitions on MFT was lower in FMS patients (p<0.001). Before MFT, plantar center of pressure (CoP) excursions was greater in FMS patients versus controls in all postures tested except in EO bipedal stance (all p<0.01). Postural sway increased post-MFT in both FMS and control groups in all stances (all p<0.001). Post-MFT increase in postural sway was greater in the FMS group versus controls (all p<0.05)., Conclusion: Lower limb muscle fatigue occurs earlier in FMS patients than in healthy individuals and exacerbates postural stability problems. - Abstract
Spasticity is the most common motor disturbance in cerebral palsy (CP). Lockdown in the COVID-19 outbreak has profoundly changed daily routines, and similarly caused the suspension of spasticity treatment plans. Besides, the delay in botulinum toxin (BoNT) injection, which is important in the management of focal spasticity, led to some problems in children. This consensus report includes BoNT injection recommendations in the management of spasticity during the COVID-19 pandemic in children with CP. In order to develop the consensus report, physical medicine and rehabilitation (PMR) specialists experienced in the field of pediatric rehabilitation and BoNT injections were invited by Pediatric Rehabilitation Association. Items were prepared and adapted to the Delphi technique by PMR specialists. Then they were asked to the physicians experienced in BoNT injections (PMR specialist, pediatric orthopedists, and pediatric neurologists) or COVID-19 (pediatric infectious disease, adult infectious disease). In conclusion, the experts agree that conservative management approaches for spasticity may be the initial steps before BoNT injections. 46. Bilateral and atypical localized elastofibroma dorsi in a patient with shoulder and back pain: A case report.

- Abstract
A 61-year-old woman presented with localized back pain at the left inferomedial border of the scapula and left shoulder pain. On physical examination, a soft tissue mass was detected on the left scapula. Magnetic resonance imaging showed a mass at the left suprascapular region and another mass in the left infrascapular region. A Tru-Cut biopsy of the lesion at the left suprascapular region was obtained due to atypical localization. Pathological examination was consistent with an elastofibroma. During wire-marking, another mass was detected at the right infrascapular region. Three mass lesions were excised on consecutive operations.
