Acute bronchiolitis is a lower respiratory tract infection caused by viral agents in children aged under two years. Treatment includes hydration, oxygen, nebulized salbutamol, and intravenous steroids. This study aimed to determine the clinically related factors, the effect of viral agents on the clinical picture, and the efficacy of treatment methods in patients admitted with acute bronchiolitis.Patients aged under two years of age who were hospitalized with a diagnosis of moderate/severe acute bronchiolitis between March 2015 and March 2019 were included in the study. Demographic data, hospitalization time, body temperature, presence of congenital heart disease, history of atopy, acute-phase reactants, mean platelet volume values, and respiratory virus panel results were recorded. The treatment modalities, length of hospitalization, intensive care hospitalization, and high-flow nasal cannula oxygen therapy (HFNC) were recorded.Four hundred twenty-two patients were included in the study. The duration of hospitalization was found to be significantly longer in patients aged under one year and in patients with acyanotic congenital heart disease. A single viral agent was detected in 69 (51.9%) patients. Rhinovirus was detected in 70 patients and RSV was detected in 37. The duration of hospitalization was found to be significantly shorter in patients who received only oxygen and/or intravenous fluid treatment compared with those who received nebulized salbutamol and/or intravenous steroids. In addition, and there was no significant difference between the groups in terms of HFNC and hospitalization in the intensive care unit.Rhinovirus was the most common cause of acute bronchiolitis in our study. It was observed that congenital heart disease prolonged the length of hospitalization. In the treatment approaches, it was observed that hydration and oxygen therapy were sufficient treatment methods for the patients, in accordance with the recommendations of the American Academy of Pediatrics, and giving nebulized therapy prolonged the hospitalization period due to the treatment discontinuation steps.Akut bronşiolit iki yaş altı çocuklarda, viral ajanlar ile oluşan alt solunum yolu enfeksiyonudur. Tedavisinde hidrasyon, oksijen, nebulize salbutamol, damariçi steroid gibi yaklaşımlar bulunmaktadır. Çalışmamızda akut bronşiolit ile yatırılan olgularda klinik ilişkili etmenlerin belirlenlenmesi, viral ajanların kliniğe etkisi ve tedavi yöntemlerinin etkinliklerinin karşılaştırılması amaçlanmıştır.Çalışmamıza Mart 2015–Mart 2019 tarihleri arasında orta/ağır akut bronşiolit tanısı ile yatarak tedavi gören iki yaş altı olgular alındı. Olguların demografik verileri, yatış zamanı, vücut sıcaklığı, doğuştan kalp hastalığı olup olmaması, atopi öyküsü, akut faz reaktanları, ortalama trombosit hacmi, solunum virus panel sonuçları kaydedildi. Uygulanan tedavi yöntemleri, hastanede yatış süreleri, yoğun bakım yatışları ve yüksek akışlı nazal kanül oksijen tedavisi (YANKOT) alıp almadığı kaydedildi.Çalışmaya 422 hasta alındı. Bir yaş altı olguların ve asiyanotik doğuştan kalp hastalığı olan olguların yatış süreleri anlamlı olarak uzun saptandı. Olguların 69’unda (%51,9) tek viral etken saptandı. Olguların 70’inde Rinovirüs, 37’sinde respiratuar sinsityal virüs saptandı. Sadece oksijen ya/ya da damariçi sıvı tedavisi alanların yatış süreleri, bu tedavilerin yanında nebulize salbutamol ya/ya da damariçi steroid alanlarda anlamlı düzeyde kısa olduğu, YANKOT ve yoğun bakım yatış için iki grup arasında anlamlı fark olmadığı görüldü.Çalışmamızda akut bronşiolit etkenleri arasında en sık Rinovirüs saptandı. Doğuştan kalp hastalığının yatış süresini uzattığı görüldü. Tedavi yaklaşımlarında ise Amerikan Pediatri Akademisi önerileri ile uyumlu olarak, hidrasyon ve oksijen tedavisinin olgular için yeterli tedavi yöntemi olduğu ve nebulize tedavi vermenin tedavi kesme basamakları nedeni ile yatışı süresini uzattığı görüldü.

Amaç:Talasemi taşıyıcılığı hafif anemi ile seyreden, ülkemizde özellikle Akdenizbölgesinde çok sık gözlenen ve klinik belirtileri hafif olsa da kalıtsalaktarım açısından oldukça önemli bir talasemi formudur. Yapılan çalışmalarda talasemitaşıyıcısı erişkin bireylerde depresyon ve kaygı oranının arttığı gözlenmişancak alan yazında talasemi tayıcısı ergenlerin ruh sağlığı ile ilgili herhangibir çalışmaya rastlanmamaktadır. Bu çalışmamızda talasemi taşıyıcısı ergenbireylerde depresyon ve kaygı düzeyini belirleyerek psikiyatrik destekalmalarının gerekliliği değerlendirilmiştir.Gereçve Yöntem: Çalışmamıza çocuk sağlığı ve hastalıklarıpolikliniğinde hemoglobin (Hb) elektroforezi ile talasemi taşıyıcısı teşhisikonan 73 ve herhangi bir hemoglobinopatisi olmayan 70 sağlıklı kontrol olmaküzere toplam 143 kişi dahil edildi. Katılımcılara Hızlı depresif belirtienvanteri-ergen- öz bildirim formu’ ve ‘Çocuklar için Spielberger Durumluk-Sürekli Kaygı Envanteri’ uygulandı. Gruplar arasındaki ölçek skorlarıkarşılaştırıldı.Bulgular:Çalışmaya katılan talasemi minör tanılı ergenler ile sağlıklı ergenler arasındadepresyon ve kaygı düzeyleri açısından anlamlılık saptanmadı. Sonuç: Bu çalışma ergentalasemi taşıyıcılı bireylerde kaygı ve depresyon düzeyini inceleyen ilkçalışmadır. Bulgularımız talasemi taşıcısı ergenlerde kaygı ve depresyonskorlarının kontrollere göre değişmediği göstermiştir.Objective: Betathalassemia is a form of thalassemia that is very common in the Mediterraneanregion. Although it has mild clinical signs, it is very important in terms ofhereditary transference. In the literature, adolescents with beta thalassemiamajor were found to have higher depression and anxiety rates. Studies on themental health of thalassemia trait individuals are very limited. In this study,the necessity of psychosocial support by determining depression and anxietylevels in adolescents with thalassemia trait was evaluated.Material and Methods: A total of 143 subjects, 73 of whom werediagnosed as thalassemia trait with hemoglobin (Hb) electrophoresis and 70healthy individuals without any hemoglobinopathy, were included in the study.The signed informed consent was obtained for each participant. The QuickInventory of Depressive Symptomatology self-report form and SpielbergerState-Trait Anxiety Inventory for Children were applied to the participants.Results: In ourstudy, the scores of anxiety and depression in the thalassemia trait group werenot significantly different compared to the healthy controls.Conclusion: This is the first study toexamine the level of anxiety and depression in adolescents with thalassemiatrait. Our results showed that anxiety and depression levels were not increasedin adolescents with thalassemia trait.

Introduction: this article examines a boy who was diagnosed with anorexia nervosa at eight years old. Pediatricians and family physicians encountered difficulties during the diagnosis and treatment stages due to the fact that early onset of the disease is rare. Case report: a boy aged eight years and four months presented with bradicardia, malnutrition and dehydration as far away from the preliminary diagnosis of anorexia nervosa investigated for possible organic pathologies at the pediatric ward. Finally, he was diagnosed with early-onset anorexia nervosa and anxiety disorder based on the Diagnostic and Statistical Manual of Mental Disorders 5. Discussion: the case highlights two important points. First, this case report demonstrates a limited awareness of anorexia nervosa and of a psychosomatic or psychiatric origin of weight loss in children among physicians. Second, disrupted eating behaviors and mood changes may be predicting factors for the development of full-blown anorexia nervosa. Resumen Introducción: se presenta el caso clínico de un niño diagnosticado de anorexia nerviosa a los 8 años. Tanto los pediatras como los médicos de familia tuvieron problemas durante el diagnóstico y en el tratamiento debido a que su aparición a tan corta edad es poco frecuente. Caso clínico: un varón de ocho años y cuatro meses ingresó por bradicardia, desnutrición y deshidratación en un hospital pediátrico. Tras el despistaje de un origen orgánico, se diagnosticó de anorexia nerviosa y trastorno de ansiedad según DSM-5. Discusión: el caso pone de relieve dos aspectos. El primero, demuestra la atención limitada que prestan los médicos a un origen psicosomático o psiquiátrico, incluyendo la anorexia nerviosa, en una pérdida de peso en un niño. El segundo, que una alteración en la conducta alimentaria acompañada de cambios de carácter pueden ser factores predictores del desarrollo de un cuadro de anorexia nerviosa florido.

Amaç: Gastroözofageal reflü hastalığı tanısı alan adolesanlarda beslenme davranışları sorgulanarak beslenme ve reflü arasındaki ilişkiyi ve bununla birlikte gastroözofageal reflü hastalığı ile anksiyete arasındaki etkileşimi değerlendirmek amaçlanmıştır. Yöntem: Çalışmamıza Çocuk Gastroenteroloji Polikliniğinde gastroözofageal reflü hastalığı tanısı almış ve henüz tedavi başlanmamış 12-18 yaş arasındaki hastalar dahil edildi. Benzer yaş aralığında üst solunum yolu enfeksiyonu nedeniyle başvuran hastalar kontrol grubu olarak alındı. Beslenme ve reflü arasında ilişki Adolesan Beslenme Alışkanlıklarını Kontrol Listesi ile gastroözofageal reflü hastalığı ve anksiyete arasındaki ilişki Çocuklarda Anksiyete Bozukluklarını Tarama Ölçeği kullanılarak değerlendirildi. Bulgular: Yetmiş gastroözofageal reflü hastalığı olan adolesan ve 67 kontrol olgusu çalışmaya dahil edildi. Her iki grup arasında hem beslenme hem de anksiyete durumları değerlendirildiğinde anlamlı fark bulunmadı. Ancak her iki gruptaki olguların büyük çoğunluğunun kötü beslenme alışkanlığı olduğu ve genel anksiyete skorunun yüksek olduğu görüldü. Sonuç: Adolesanlarda gastroözofageal reflü hastalığı ile anksiyete ve beslenme alışkanlıkları arasında ilişki saptanmadı. Bu sonuçlar, reflü hastalığının, psikosomatik olabilen karın ağrısı gibi diğer gastrointestinal yakınmalardan ayrılması gerektiğini düşündürmektedir. Bununla birlikte, kötü beslenme alışkanlıkları, gastroözofageal reflü hastalığı olanlarda kantitatif olarak daha yüksek saptandı. Bu nedenle bu hasta popülasyonunda daha büyük bir örneklem büyüklüğü ile beslenme alışkanlıklarını araştıran ileri çalışmalara gereksinim vardır., Objective: It was aimed to evaluate the relationship between eating habits and reflux by investigating eating habits in adolescent patients with gastroesophageal reflux disease, and also to assess the interplay between gastroesophageal reflux disease and anxiety. Methods: We included patients between the ages of 12 and 18 diagnosed as gastroesophageal reflux disease in the pediatric gastroenterology outpatient clinic but who had not yet been treated. the control group consisted of age matched adolescents who applied for upper respiratory tract infection. the relationship between eating habits and reflux was assessed with Adolescent Nutrition Checklist, and relationship between gastroesophageal reflux disease and anxiety was assessed with Anxiety Disorder Screening Questionnaire. Results: Seventy adolescents with reflux disease and 67 adolescents as the control group were included in the study. There was no significant difference between the groups regarding the eating habits and anxiety scores. However, it was seen that most of the patients in both groups. had poor eating habits and higher general anxiety score. Conclusion: No relation was found between the gastroesophageal reflux disease, anxiety and eating habits in adolescents. These results suggested that reflux disease should be separated from other gastrointestinal complaints such as abdominal pain which may be psychosomatic. However, there was a quantitative increment in poor eating habits in those with gastroesophageal reflux disease. For this reason there is a need for further studies which investigate the eating habits with a larger sample size of this population.

Giriş ve Amaç: Bu çalışmanın amacı krup patogenezinde, sitokinlerin (TNF, IL-6, IL-10, PAF), kortizol ve IgE'nin rolünün saptanması ve krup hastalığının klinik ağırlığını belirleyen etkenlerin araştırılmasıdırMateryal ve Metot: Hasta grubu olarak Eylül 2013-Eylül 2014 tarihleri arasında Çocuk Acil Polikliniği'ne başvuran krup tanısı konulan hastalar ve kontrol grubu olarak 30 sağlıklı çocuk çalışmaya alındı. Tüm hastaların demografik özellikleri, başvuru şikayeti, klinik bulguları, tekrarlayan krup, hastada ve ailede atopi öyküsü kaydedildi. Westley krup skorlama sistemine göre hastalar hafif ve orta/ağır olmak üzere gruplandırıldı. Hastalardan tedavi öncesi ve kontrol grubundan tam kan sayımı ve sitokin düzeyleri için kan örneği alındı. Solunum yolu patojenleri nazofarengeal sürüntü örneğinde PCR yöntemi ile çalışıldı.Bulgular: Toplam 69 krup tanısı alan hasta çalışmaya alındı. Hastaların 34 (%49)'ü hafif, 31 (%45)'i orta, 4 (%6)'ü ağır krup olarak sınıflandırıldı. Krup olgularının 52 (%75,4)'si erkek ve ortanca yaşı 32,0 (16,5-52,0) ay, kontrol grubunun 17 (%56,7)'si erkek ve ortanca yaşı ise 27,5 (15,5-72,0) ay idi (p= 0,064; p=0,486).Hafif ve orta/ağır grup arasında yaş açısından farklılık saptanmadı (p=0,107).Hafif grupta hastaların 18 (%52,9)'inde, orta/ağır grubun 15,0 (%42,8)'inde olmak üzere toplam 33,0 (%47.8) hastada tekrarlayan krup tespit edildi (p=0,405).Hastaların %32'sinin 12.00-18.00 saat aralığında başvurduğu saptandı. Hastaların 45 (%65.2)'inde viral etken izole edildi. Rhinovirüs PCR22 hastada pozitif saptandı.Hasta grup ve kontrol grup karşılaştırıldığında TNF alfa, IL-6, IL-10, PAF ve IgE düzeyi arasında istatistiksel olarak anlamlı fark bulunmadı.
