Introduction: Insulinoma is a rare disease, however the most common cause of hypoglycemia due to excess insulin secretion. Diagnosis and localization can be challenging. This study evaluated the clinical features, diagnostic workup, management, and outcomes of patients with insulinoma. Methods: The records of 13 patients with insulinoma who were followed up at İstanbul University, İstanbul Faculty of Medicine were retrospectively reviewed. Results: The mean age of the 13 patients (female/male: 11/2) was 43.9±12.5 years at diagnosis. The mean tumor diameter was 14.3±6.7 mm and localized at the head in 30.8%, at the tail and/or body in 61.6%, and at both the head and body in 7.6% of patients. The tumor was correctly localized by magnetic resonance imaging in 10/13 patients, 68Ga DOTATATE positron emission tomography/ computed tomography in 4/8, endoscopic ultrasound in 3/7, and selective arterial calcium stimulation in 4/4 patients. Eleven patients were operated. Distal pancreatectomy was performed in 4 patients, distal pancreatectomy plus splenectomy in 3, and enucleation in 4 of the patients. The median follow-up duration was 4 years. In 8 patients, cure was achieved with surgery alone. Somatostatin receptor analog (SSRA) treatment was initiated in 2 cases and one of whom developed lymph node metastasis 2.5 years after surgery under SSRA treatment and she was reoperated. These patients had stable disease at the last visit. Conclusion: Insulinomas are usually small tumors, but they can cause severe symptoms. A multidisciplinary approach is required for diagnosis and treatment. In some patients, different imaging modalities may be necessary for tumor localization. [ABSTRACT FROM AUTHOR]

Objective: Craniopharyngioma (CP) is benign, but its treatment is challenging because of its occurrence in a critical area. Hypothalamic-pituitary dysfunction (HPD) is an important complication that is related to tumour itself or to therapy. This study identified the long-term outcomes of CPs. Material and Methods: Records of 44 patients with CPs followed up at the Istanbul Faculty of Medicine were retrospectively reviewed. Results: The mean age of 34 patients (M/F: 19/15) at diagnosis was 24±13.3 years. The most common symptoms were headache (60.3%) and visual impairment (45.5%). The tumour was localised in suprasellar region in 76.6%, in sellar-suprasellar in 16.7%, and in the sellar region in 6.7% of the patients. The mean tumour diameter was 32.7±10.4 mm (n: 19). Surgery was the first line of treatment in all patients except one, and 13 patients underwent more than one operation. Ten patients received radiotherapy. The median duration of follow-up was 6.5 years. Anterior pituitary dysfunction was observed in all patients: hypocortisolism in 88.2%, hypothyroidism in 97%, hypogonadism in 88.2%, growth hormone deficiency in 68.2%, and prolactin deficiency in 20.6% of the patients. Diabetes insipidus occurred in 97% of the patients. Recurrence developed in 2 patients (after 5-6 years). At the end of all treatments, 15 patients had residual disease. Conclusion: The curative treatment of CP is surgery. HPD is associated with increased mortality and morbidity and decreased quality of life. It is essential to protect the hipothalamo-pituitary axis and to choose the best treatment option for each patient. [ABSTRACT FROM AUTHOR]

The association of subacute thyroiditis (SAT) and papillary thyroid carcinoma is a rare finding. In this study, we aimed to investigate the prevalence of differentiated thyroid cancer in a cohort of patients followed with the diagnosis of SAT.

Objective: Investigation of differences in the remission and recurrence rates of microadenoma and macroadenoma patients with Cushing's disease following transsphenoidal surgery (TS). Material and Method: Records were reviewed for 31 microadenoma and 9 macroadenoma patients who had undergone TS for Cushing's disease over a 12 year period and their remission and recurrence rates were compared. Results: Remission was achieved in 26 (83.9%) of 31 microdenoma patients following TS. 3 (11.5%) of these 26 patients developed recurrent disease. One of them achieved remission following a second TS, the other underwent adrenalectomy and last one didn't came to outpatient clinic visit after recurrence. Adrenalectomy was performed for 2 of the 5 microadenoma patients with persistant disease, other 3 patients underwent a second TS. Two of them achieved remission. Adrenalectomy and radiotherapy was performed for the patient who had persistent disease. Remission was achieved in 8 (88.8%) of 9 macrodenoma patients following initial TS and 1 patient achieved remission after second TS. Two (25%) of the macroadenoma patients who had recurrent disease, had radiotherapy after adrenalectomy. The mean duration of follow up for patients who remained in remission was 43.6±51.1 months in microadenoma and 24.7±11.9 months in macroadenoma patients. The mean time to recurrence was 28.7±10.6 months in microadenoma and 37.0±15.6 months in macroadenoma patients. Conclusion: We could not find significant differences between microadenoma and macroadenoma patients in achieving remission, the mean duration of follow up for patients who remained in remission and recurrence rates in patients with Cushing's disease following TS. Down's syndrome is the most common genetic disorder. It is well established that there is an increased risk of autoimmune endocrine diseases in Down's syndrome. Defect of a gene located on chromosome 21q22.3 causes autoimmune polyglandular syndrome type 1 and this might be related with susceptibility to autoimmune diseases in patients with Down's syndrome. We reported a 20-year-old male with Down's syndrome who was brought to emergency room by his family and had a diagnose of diabetic ketoacidosis and Graves' disease. Underlying mechanisms in co-existing of autoimmune diseases in Down's syndrome, management of such patients and the importance of making the family to be aware of the symptoms of possible subsequent components of the autoimmune polyglandular syndrome are discussed.
