Gasroşizis ve omfalosel karın ön duvarının en sık malformasyonlarındandır ve her ikisi de yüksek morbidite ve mortalite ile birliktedir. Bazen bu durumların tanısı hekimler için zor olmakta ve aile de farkına varamamaktadır. Bu makalede bir omfalosel ve bir gastroşizis vakasının iki ve üç boyutlu ultasonografik tanısal özellikleri sunulmuştur. Her iki durum ve bu durumların tanı ve tedavisinde üç boyutlu ultrasonografinin yeri ve klinik yönetimi tıbbi literatür ışığında tartışılmıştır. Üç boyutlu ultrasonografi omfalosel ve gastroşizisin tanı ve ağırlığını belirlemede yardımcıdır., Gastroschisis and omphalocele are the two of the most common malformations of the anterior abdominal wall that both of them associated with high morbidity and mortality. Sometimes the diagnosis and the realization of the situations by physicians and the parents are difficult. Herein, one omphalocele and one gastroschisis cases were presented with the two-dimensional and three-dimensional ultrasonographic diagnostic features. The two entities and the role of three-dimensional ultrasound in diagnosis and the clinical management of the situations are discussed in the view of medical literature. The three-dimensional ultrasound helps in the diagnosis and detection of the severity of the omphalocele and the gastroschisis.

Ureters more than 7 mm in diameter are called megaureters. Most of the patients have an abnormal finding on routine prenatal ultrasound and it is possible to identify the fetuses having the increased risk of developing urinary pathologies. Urinary tract infection, abdominal pain, abdominal mass, and hematuria are the general observed signs of the entity. The prenatal diagnosis of the primary megaureter is generally possible by using the conventional two-dimensional ultrasound. However, in some cases the nearby fluidfilled intestines may obscure the dilated ureters and cause misdiagnosis. In this condition, three-dimensional ultrasound may help to clarify the pathology. In this report, we present a case of primary megaureter. We could have the final diagnosis by using the three dimensional ultrasound., Üreterlerin çapının 7mm'yi aşması megaureter olarak adlandırılır. Hastaların pek çoğunda prenatal anormal bulgu vardır ve üriner patoloji gelişebilecek fetüsleri ayırtetmek mümkündür. Üriner sistem enfeksiyonları, karın ağrısı, karında kitle ve hematüri antitenin görülen genel bulgularıdır. Magaureterin prenatal tanısı konvansiyonel iki boyutlu ultrasonografi kullanılması ile genellikle mümkündür. Ama bazı olgularda yakındaki sıvı içerikli barsaklar genişlemiş ureterleri maskeleyebilir ve yanlış tanılara sebep olabilir. Bu durumlarda üç boyutlu ultrasonografi patolojinin aydınlatılmasında yardımcı olabilir. Bu yazıda bir primer megaureter olgusu sunuyoruz. Olguda üç boyutlu ultrasonografi kullanılarak kesin tanı konulabilmiştir.

Ovarian cysts over 5 and 15 cm are described as large and giant, respectively. Ovarian cysts over 5 cm without regression in 6-8 weeks are indicated for surgery. Although there are published data about the laparoscopic or laparoscopy assisted management of the large and giant cysts, especially in cases of giant cysts midline laparotomy is preferred by many surgeons. In this paper, we present the management of a 32 cm mucinous ovarian cyst by laparoscopy-assisted mini-laparotomy., Over kistleri 5 cm'i aştıklarında büyük, 15 cm'i aştıklarında dev olarak adlandırılırlar. 5 cm'den büyük over kistleri 6-8 hafta içinde gerilemezse cerrahi tedavi gerekir. Büyük ve dev over kistlerinin yönetiminde yayınlanmış laparoskopik ya da laparoskopi asiste yöntemler olsa da, özellikle dev kistlerde laparotomi pek çok cerrah tarafından tercih edilir. Bu yazıda 32 cm'lik dev bir over kistinin laparoskopi asiste mini-laparotomi ile sağaltımını sunuyoruz

AMAÇ: İntrauterin büyüme geriliği bulunan yüksek riskli gebe grubunda yapılan umblikal arter Doppler incelemesi sonucunda ters akım veya diastol sonu akım kaybı bulunan gebelerde perinatal sonuçları incelemek. YÖNTEM: Çalıșmaya gebelik haftası ≥30 olan 44 gebe alındı. Bütün gebelerde intrauterin gelișme geriliği ve/veya preeklampsi vardı. Umbilikal arter Dopplerinde ters akım veya diastol sonu akım kaybının gebelik sonuçlarını predikte edebilmesi araștırıldı. İstatistiksel analizde Pearson Ki-kare ve Fisher Exact testleri kullanıldı. BULGULAR: Ortalama gebelik haftası; son adet ve ultrasonografiye göre sırasıyla 34,4 ve 29 bulundu. Ortalama yeni doğan yoğun bakımda kalma süresi 8,1 günken, ortalama doğum ağırlığı 1604 gramdı. 26 gebede preeklampsi ve 24 gebede oligohidroamniyos saptandı. Neonatal ölüm 9 olguda ve intrauterin ölüm 1 olguda gözlendi. 13 gebede ters akım veya diastol sonu akım kaybı tespit edildi (%29,5). Bu fetüslerde 5. dakika Apgar skoru ve doğum sırasındaki gebelik haftası anlamlı olarak daha düșüktü. Benzer olarak bu olgularda, yenidoğan yoğun bakım ünitesine gereksinim süresi ve perinatal ölüm oranları daha yüksekti. SONUÇ: Umbilikal arter Doppler incelemelerinde ters akım veya diastol sonu akım kaybı görülmesi yüksek riskli gebeliklerdeki kötü prognozu belirlemede oldukça yararlı bir parametredir., AIM: To evaluate the perinatal outcome of high risk pregnancies with intrauterine growth restriction with reverse fl ow or absence of end-diastolic fl ow velocity in umbilical artery Doppler fl owmetry. METHODS: Pregnant women (n=44) with gestations of ≥30 weeks were included. They all had intrauterine growth restriction and/ or preeclampsia. Doppler fl owmetry including reverse fl ow or absence of end-diastolic fl ow velocity were used to predict the perinatal outcomes. Statistical analysis was performed using Pearson Chi-Square and Fisher’s Exact tests. RESULTS: The mean gestational age according to the last menstrual date and ultrasound evaluation were 34.4 and 29 weeks, respectively. The mean duration of neonatal intensive care unit requirement time was 8.1 days and the mean birth weight was 1604 gram.Preeclampsia and oligohydramnios were seen in 26 and 24 of the fetuses, respectively. There were 9 neonatal and 1 intrauterine fetal deaths. Reverse fl ow or absence of end diastolic fl ow velocity was detected in 13 patients (29.5%). These fetuses had signifi cantly lower 5th minute Apgar’s score and gestational age at labor. Similarly, the duration of neonatal intensive care requirement and the perinatal death rate were signifi cantly higher. CONCLUSION: Reverse fl ow or absence of end-diastolic fl ow velocity in fetal umbilical artery Doppler fl owmetry is a useful parameter in predicting poor perinatal outcome of high risk pregnancies

AMAÇ: Son yirmi yıldaki deneysel hayvan kullanımı eğilimlerini ve laparoskopik cerrahiyi ilerletmek için yapılmıș yayınlardaki etik bașvuru oranlarını incelemeyi amaçladık. YÖNTEM: Çalıșma ulașılabilen on-line arama motorları kullanılarak yapıldı. 1990 ve 2010 yılları arasında hayvanları kullanan biomedikal çalıșma oranlarını belirlemek için PubMed’i inceledeik. İngilizce özeti olan dergiler en eskiden en yeniye doğru kronolojik olarak sıralandılar. Arama yalnızca “laparoscopy” anahtar kelimesini içerecek biçimde daraltıldı. Hayvanlar üzerinde yapılan 3023 çalıșma elde ettik. Bir laparoskopi tekniği, cihazı ya da aleti bulmak, geliștirmek ya da değiștirmeyi amaç edinen çalıșmaları içerdik. Laparoskopik yaklașımı geliștirmeyi amaç edinmeyen çalıșmalar çıkarıldılar. Kalan 82 çalıșma; çalıșma yılı, hayvan türü, çalıșmaların bașında ve sonundaki hayvan sayısı ve etik bașvuru durumu açısından incelendiler. BULGULAR: Çalıșmada 1990-2000 yılları arasındaki 10 yılda yayınlanan 21 ve 2000-2010 yılları arasındaki 10 yılda yayınlanan 61 çalıșma yer aldı. Toplam 2132 hayvandan 496’sı ilk 10 yıldaki ve 1636’sı ikinci 10 yıldaki çalıșmalarda kullanılmıșlardı. En çok kullanılan hayvan olan domuzları sırasıyla ratlar, tavșanlar ve köpekler takip ediyordu. İki on yıllık süre içerisinde çalıșma bașına kullanılan hayvan sayısı ve hayvan ölüm oranları belirgin olarak farklı değillerdi (p > 0,05). Etik bașvuru oranları artsa da, bu artıș belirgin fark olușturmak için yeterli değildi (p > 0,05). SONUÇ: Laparoskopik cerrahide gelișme sağlamak için araștırmacılar çoğunlukla domuzlar üzerinde deneyler yapmaktadırlar. Son iki 10 yıllık sürede, etik bașvuru oranında küçük bir artıș ve gereksiz hayvan ölüm oranında küçük bir azalma olsa da, hayvan çalıșmalarında etik bașvuru oranı %70’lerde olduğu için hala kat edilmesi gereken çok yol vardır, AIM: We aimed to analyze the trends in experimental animal use and the ethical approval rates of the publications dealing with the improvement of the laparoscopic surgery during the last two decades. METHODS: The study was performed by using the available online medical search engines. We searched the PubMed for the proportion of biomedical publications used animals between 1990 and 2010. Journals with English Abstracts, organized in chronological order from the earliest to the oldest. The search was narrowed to include only the key word “laparoscopy”. We gathered 3023 studies performed on animals. We included the studies with objectives to innovate, improve or modify a laparoscopic technique, instrument or device. The publications in which the objective of the study was not to improve the laparoscopic approach were excluded. The remaining 82 studies were analyzed for the data including the year of the study, animal species, and the number of the animals at the initial phase and at the end of the studies, and the ethical approval status. RESULTS: The study included 21 studies in 1990-2000 decade and 61 studies in 2000-2010 decade groups. A total of 2132 animals, 496 in 1990-2000 decade and 1636 in 2000-2010 decade, were used in the studies. Most widely used animals were pigs followed by rats, rabbits and dogs. The numbers of animals per study and animal death rates were not significantly different between two decades (p > 0.05). Although, the ethical committee approval rate increased, the increase was not sufficient enough to create a significant difference (p > 0.05). CONCLUSION: Researchers mostly perform experiments on pigs to achieve improvements in laparoscopic surgery. Although, there was a tendency towards to a small increase in ethical approval rate and a small decrease in unnecessary animal death rate in the last decade, we still have a long way to go as the ethical approval rate fluctuates around 70% in animal studies.

Cantrell pentalojisi; kısmi ya da tam ektopia kordis, supraumbilikalkarın ön duvar duvar defekti, konjenital kalp defektleri, sternum altucu, perikardium ve ön diyafram defektleri birliktelikleri olan ve nadirgörülen bir sendromdur. Olgumuzda; gebeliğinin 20. haftasındagastroșizis ön tanısıyla refere edilen bir gebenin fetüsünde Cantrellpentalojisi ve sol üst ekstremitede fokomeli saptanmıștır. In this case, a woman at 20th week of her pregnancywas referred to our clinic with the diagnosis of gastrochisis,however diagnosed with a Cantrell pentalogy associated with aphocomelia of the left upper extremity. Following the terminationof the pregnancy postmortem examination confi rmed the prenataldiagnosis. Gebelik kadınlarda önemli biyolojik, psikolojik ve sosyolojik deği- șikliklere yol açar. Gebelik haftası, anne ve fetüste gelișebilecek yan etkiler, gebelerin psikolojik bozukluklarının yönetiminde de dikkate alınması gereken önemli faktörlerdir. Bu nedenle gebelikteki psikiyatrik yaklașımda bazı farklılıklar vardır. Bu yazıda gebelikte elektrokonvüsif tedavi kullanımını özellikle anne ve fetüse etkileri açısından, tıbbi literatürü gözden geçirerek incelemeyi amaçladık., Pregnancy results in major biological, psychological and sociological changes of the pregnant women. Gestational week, and probable maternal and fetal adverse effects are important factors that should be considered carefully during the management of psychological disturbances of a pregnant woman. Therefore, there are some differences in the psychiatric approach during pregnancy. In this paper, we aim to analyse the use of electroconvulsive therapy during pregnancy, particularly with its effects on the mother and the fetus, by reviewing the existing medical literature.

Moreover, neither the insertion of a second intrauterine device in addition to an existing one or the co-insertion of two devices at the same time has ever, to our knowledge, been reported previously in the literature. In this report, we aim to present the co-existence of two intrauterine devices in a woman who had been suffering from chronic lower abdominal pain.A national patient record system and close adherence to the available guides will improve the quality of the family planning services while decreasing complication rates.

Sezaryen sonrası analjezi için pek çok merkezde intramusküler meperidin ve tramadol kullanıldığı halde bu ilaçlarla ilgili karşılaştırmalı klinik çalışmalar azdır. Bunun için meperidin ve tramadolün elektif sezaryen doğumlar sonrası kullanımını karşılaştıran bir prospektif randomize çalışma tasarladık. Yöntem Eylül 2009 ve Mayıs 2010 tarihleri arasında elektif sezaryen doğum yapan 101 kadın çalışmaya alındı. Kadınlar meperidin ve tramadol grubuna ayrıldılar. Postoperatif analjezi için kadınların 50 tanesi tramadol ve 51 tanesi meperidin aldı. Her iki ilaç da uyandırılma sonrası 50 mg intramusküler uygulandı. Ağrı değerlendirilmesi için ilk uygulamanın 1,4,8,12 ve 24. Saatlerinde vizüel analog skala kullanıldı ve her iki ilaç için VAS değeri 4’ten büyük bulunduğunda 25 mg’lık ek dozlar yapıldı. Bütün kadınlar vizüel analog skala değerlendirmeleri sırasında bulantı, kusma, kaşıntı ve diğer yan etkiler açısından gözlendiler. Demografik veriler için ortalama ve standart sapma değerleri hesaplandı. Vizüel analog skala değerleri,ek doz ihtiyacı ve yan etkiler iki grup açısından bağımsız değişkenler Student t testi ile karşılaştırıldılar. P0,05). 1,4,8,12 ve 24 saatlerdeki vizüel analog skala ağrı değerleri, bulantı, kusma, kaşıntı oranları ve ek doz kullanım oranları açısından iki grup karşılaştırıldı. Vizüel analog skala ağrı değerleri tramadol grubunda, bulantı, kusma ve kaşıntı meperidin grubunda daha yüksek oranda gözlendiyse de, bulgular istatistiksel fark oluşturmadı (p>0,05). Tramadol grubunda meperidin grubuna göre daha çok sayıda ek doza ihtiyaç duyulduysa da, fark istatistiksel olarak anlamlı değildi (p>0,05). Sonuç Elektif sezaryen doğumlar sonrası analjezide tramadol ve meperidin intramusküler kullanımda aynı analjezik etkinlik ve yan etki oranına sahiptir. İki ilaç da birbirinin yerine tercih edilebilir. Postoperatif ilk saatlerde ek doz gereksinimi her iki grupta da daha fazladır., AimAlthough meperidine and tramadol are widely used after cesarean sections in many centers, comparative clinical studies are rare. Therefore, we designed a prospective randomized study to compare tramadol and meperidine following elective cesarean births.MethodsFrom September 2009 to May 2010, 101 women who had elective cesarean section were included in the study. The women were allocated into meperidine and tramadol groups. There were 51 women in meperidine group and 50 women in tramadol group. Both agents were used in 50mg doses at the first hour following the termination of the anesthesia.Visual analogue scale was used for pain scoring at the 1st,4th,8th,12th and 24th hours following the initial analgesic dose. If the pain scores were higher than 4, an additional dose of 25 mg was supplied for each agent.All the participants were observed for nausea, vomiting and other side effects during visual analogue scale scorings.Means and standard deviations were calculated for demographic data. The parameters of visual analogue scale scores, additional dose requirements and side effects were compared in the two groups by using independent variations Student t test. p value 0.05).In the comparison of the parameters of visual analogue scale pain score, nausea, vomiting, itching and the requirements for additional doses, the two groups did not show significant differences from each other at 1st,4th,8th,12th and 24th hours (p>0.05), although tramadol group had higher pain scores and meperidine group had higher nausea, vomiting and itching rates.ConclusionIntramuscular tramadol and meperidine have similar analgesic efficacy and side effects following elective cesarean births. Both agents can be substituted for each other and additional dose requirements are higher in the early postoperative hours.

The defect occurs with the failure of neural tube toclose within 28 days after conception and leads to fetal loss, still birth, or neonatal death. Prenatal diagnostictool for both anomalies is ultrasound. In most cases two-dimensional conventional ultrasound providessufficient images for the accurate diagnosis. Erken ve sporadik gebelik kayıpları sık karşılaşılan jinekolojik problemlerdir. Gebelikte ise en sık karşılaşılan komplikasyon olduğu bilinmektedir. Gebelik kayıplarının büyük bir kısmı ilk trimester içinde olur ve bu oran bundan sonra hızla düşer. Günümüz şartlarında erken gebelik kaybına sebep olan nedenleri saptayabilmek her zaman mümkün olmamaktadır. Spontan gebelik kayıplarına neden olan maternal faktörler arasında ise genetik bozukluklar, uterin anomaliler, endokrin faktörler, enfeksiyonlar ve bağışıklıkla ilişkili faktörler sayılabilir.Gebeliğin başarılı gidişatı için etkili bir uteroplasental dolaşım şarttır ve bu dolaşım hemostaz bozukluklarından etkilenebilir. Hemostatik hataların plasental yatak damarlarında tıkanıklığa yol açabilmesinden yola çıkılarak, gebelik sırasında koagülasyon faktörlerindeki beklenmeyen değişikliklerin düşük oluşumuna neden olabileceği düşünülebilir.Amaç: Spontan abortus (missed, komplet, inkomplet veya habitual) ve terapötik küretaj (istemli küretaj) olgularında plasenta ve desidua dokusundaki Tromboksan A2 reseptör düzeyini immunohistokimyasal yöntemle karşılaştırılmasıdır.Gereç ve Yöntemler:Çalışmaya Ocak 2010 ile Mart 2012 tarihleri arasında Kafkas Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı'na spontan abortus tanısıyla başvuran 19 hasta ve 25 kontrol hastası dahil edilmiştir. Hastaları çalışma grubuna dahil etmeden önce hastanın yaşı, gebelik sayısı, paritesi, daha önceki gebeliklerindeki abort sayısı, sistemik hastalık varlığı (diabet, hipertansiyon, kalp hastalığı vb.), sigara-alkol-kafein-ilaç vs kullanımı, son adet tarihinin ilk günü öyküsü sorgulandı. Kanama pıhtılaşma öyküsü bulunan , kanama pıhtılaşmayı etkileyen ilaç kullanma öyküsü bulunan ve sistemik hastalığı bulunan olgular çalışmaya alınmamıştır.Bulgular: Spontan düşük yapan 19 hastanın toplam gebelik, doğum ve çocuk sayıları anlamlı olarak istemli düşük yapan 25 hasta grubuna göre daha az, kendiliğinden düşük sayıları ise daha fazla. Yaşlar açısından iki grup arasında anlamlı farklılık yoktur. Tromboksan A 2 reseptör düzeyininimminohistokimyasal boyanma sonuçlarında ; Nukleer boyanma istatistik açıdan uygun olmadığından değerlendirmeye alınmamıştır. Endometrial bezler, desidua, villoz trofoblastlar ve intermediate trofoblastlar icin ayri ayri boyanmalar bakıldı. Intermediate trofoblastlarda hic boyanma izlenmedi. Early and sporadic pregnancy loss are common gynecological problems. This is known that, it is the most common compication of pregnancy. Most of pregnancy loss occurs in the first trimester and this proportion fall rapidly thereafter. In today's terms, the cause of early pregnancy loss is not always possible to detect. Maternal factors that may cause spontaneous pregnancy loss are the genetic factors, uterine abnormalities, endocrine factors, immunological factors and infections.For the successful progress of pregnancy, an effective uteroplacental circulation is mandatory and this circulation can be effected by haemostatic disorders. Considering the fact that haemostatic failures can cause an obstruction at the placental vascular lacunes, it can be figured out that the unexpected changes in coagulation factors during the pregnancy may be the reason of an abortion.Objective: Spontaneous abortion (missed, complete, incomplete or habitual) and therapeutic abortion (voluntary abortion) placenta and decidual tissue in patients with immunohistochemical methods to compare the level of Thromboxane A2 receptor.Materials and Methods: 19 patients with spontaneous abortion diagnosis and 25 control group, who applied to Kafkas University Medical faculty of Obstetrics and Gynecology Department between January 2010 and March 2012, have been involved in the study,. Patients included in the study were questioned beforehand about the patient's age, number of pregnancies, parity, number of previous abortion , the presence of systemic disease (diabetes, hypertension, heart disease, etc.), non-alcohol-caffeine-drug etc. usage, the story of the first day of last menstrual history. Patients with a history of bleeding, clotting and with a history of using drugs that affect coagulation of bleeding and with systemic disease were excluded from this study.Results: : Numbers of pregnancy, childbirth and the number of children with the 19 patients of spontaneous abortoin were significantly less than the group with the 25 patients of voluntary abortion but spontaneous abortion is more.There is no significant difference in ages between the twogroups. The level of thromboxane A 2 receptores immunohistochemical nuclear staining result was not assessed,because it was not appropriate statistically.Endometrial glands, decidua, villous trophoblasts and intermediate trophoblasts were painted separately.Staining was not observed in Intermediate trofoblasts. Variable staining were detected in the villous Sito and sinsityo trofoblasts.Conclusion: Thromboxane A2 receptor level of endometrial and placental tissue as a result of work between the two groups were not significantly different. 88
