WOS: 000481718800004, PubMed ID: 31418412, Background/Aims: Patients with colorectal cancer continue to present with relatively advanced tumors that are associated with poor oncological outcomes. The aim of the present study was to assess the association between localization, symptom duration, and tumor stage. Materials and Methods: A prospective, multicenter cohort study was conducted on patients newly diagnosed with a histologically proven colorectal adenocarcinoma. Standardized questionnaire-interviews were performed. Data were collected on principal presenting symptoms, duration of symptoms (time to first presentation to a doctor and time to diagnosis) and treatment, diagnostic procedures, tumor site, and stage of the tumor (tumor, node, and metastasis (TNM)). Results: A total of 1795 patients with colorectal cancer were interviewed (mean age: 60.76 +/- 13.50 years, male patients: 1057, patients aged >50 years: 1444, colon/rectal cancer: 899/850, right side/left side: 383/1250, stage 0-1-2/stage 3-4: 746/923). No statistically significant correlations were found between duration of symptoms and either tumor site or stage. Principal presenting symptoms were significantly associated with left colon cancer. Patients who had "anemia," "change in bowel habits," "anal pruritus or discharge," " weight loss," and "tumor in right colon" had a significantly longer symptom time. Conclusion: Symptom duration is not associated with localization, nor is the tumor stage. Diagnosis of colorectal cancer at an earlier stage may be best achieved by screening of the population.

High serum YKL-40 level is associated with poor prognosis in patients with lung cancer Introduction: YKL-40 is a glycoprotein that plays role in inflammation and malignant processes. High serum YKL-40 levels are associated with short survive in cancer and chronic obstructive pulmonary disease (COPD) is another reason to increase its’ level. However, limited knowledges are known in YKL-40 along with lung cancer and COPD. Materials and Methods: One hundred patients were involved to study with lung cancer (84 men, 16 women, and median age 62). Results were compared with 30 healthy volunteers. Thirteen patients were small cell lung cancer (SCLC), 87 patients were nonsmall cell lung cancer (NSCLC). 62% of patients were inoperable. results: Median YKL-40 level was 222.7 ± 114.1 ng/mL in patients and was 144.5 ± 105.7 ng/mL in controls (p< 0.001). Stage, tumour size, lymph node involvement and distant metastasis weren’t associated with serum YKL-40 level. Above all cut-off values (133.159 and 162 ng/mL) survival was shorter (p> 0.05). Patients with COPD had worse survive above all cut-off values (p< 0.05), especially according to 133 ng/mL (p= 0.01). Conclusion: YKL-40 level is useful in lung cancer however it’s not related to cell type and prognosis. Akciğer kanseri ve kronik obstrüktif akciğer hastalığı (KOAH)’nın serum YKL-40 düzeyi ile ilişkisinin birlikte incelendiği yayın sayısı kısıtlıdır. Materyal ve Metod: Çalışmaya 84 erkek ve 16 kadın olmak üzere 100 hasta dahil edildi. Yaş ortalaması 62 idi. Sonuçlar 30 sağlıklı gönüllü ile karşılaştırıldı. Hastalardan 13 tanesi küçük hücreli akciğer kanseri (KHAK) 87 küçük hücre dışı akciğer kanseri (KHDAK) tanısı aldı. Yüz hastanın 42’sinde KOAH eşlik ediyordu. bulgular: Hasta grupta ortalama YKL-40 düzeyi 229 ± 110.7 ng/mL iken kontrol grubunda 114.5 ± 105.7 ng/mL saptandı (p< 0.001). Evre, tümör çapı, nodal infiltrasyon ve uzak metastazla YKL-40 düzeyi arasında ilişki saptanmadı. Tüm eşik değerler üzerinde (133.159 ve 162 ng/mL) yaşam süresi daha kısa bulundu (p> 0.05). Özellikle 133 ng/mL‘ye göre KOAH tanılı 42 hastada KOAH olmayanlara göre yaşam süresi anlamlı derecede daha kısaydı (p= 0.01). sonuç: YKL-40 akciğer kanserinde yararlı bir belirteç ancak hücre tipi ve yaşam süresiyle ilişkisiz. Amaç: Çalışmanın amacı Jefferson Doktor Empati Ölçeği öğrenci versiyonunun güvenirlik ve geçerliliğinin değerlendirilmesi, cinsiyet ve eğitim yıllarına göre farklılıklarının analiz edilmesi için tıp fakültesi öğrencilerine adaptasyonunun yapılmasıdır.Gereç ve Yöntem: Jefferson Doktor Empati Ölçeği öğrenci versiyonu iki yönlü çeviri yapılarak Türkçe'ye çevrilmiş ve 1.-5. sınıf düzeylerindeki 752 tıp fakültesi öğrencisine uygulanmıştır. Ölçeğin boyutluluk yapısının değerlendirilmesinde kategorik verilerde doğrulayıcı faktör analizi (DFA) kullanılmıştır. İç tutarlılık Cronbach alfa katsayısı ile değerlendirilmiştir. Alt ölçek puanları eğitim yıllarına göre farklılıkları karşılaştırılmıştır.Bulgular: DFA, bir madde hariç ölçeğin mevcut üç boyutlu yapısını doğrulamıştır. Alt ölçekler için iç tutarlılık katsayıları sırasıyla 0,83, 0,70, ve 0,60 olarak bulunmuştur. "Perspektif alma" ve "şevkatli bakım" puanları açısından cinsiyet ve eğitim yılları arasında istatistiksel olarak anlamlı farklılık bulunmuştur.Sonuç: Türkçe'ye adaptasyonu başarılı biçimde yapılan Jefferson Doktor Empati Ölçeği öğrenci versiyonu, Türkiye'de tıp fakültelerinde öğrenciler üzerinde kullanılabilir. Background: An important aspect of the "description" of a variable is the shape of its distribution, which tells you the frequency of values from different ranges of the variable. Typically, as most of the statistical tests are based on the normality assumption, a researcher is interested in how well the distribution can be approximated by the normal distribution. Unless there are extreme violations of the normality assumptions, approved statistical tests usually provide accurate results. Although simple descriptive statistics can provide some information relevant to this issue, more precise information can be obtained by performing one of the tests of normality to determine whether the sample comes from a normally distributed population or not. Aim: Lilliefors corrected Kolmogorov-Smirnov, Shapiro-Wilk, D'Agostino Pearson and Jarqua-Bera tests were aimed to be compared in terms of Type I error and power of the tests. Materials and Methods: The simulation was run 1000 times for 23 different sample sizes and for 8 different distributions. Lilliefors corrected Kolmogorov-Smirnov, Shapiro-Wilk, D'Agostino Pearson and Jarqua-Bera tests were compared in terms of Type I error and power of the tests. Results: The most powerful results for normal distributions were given by the Jarqua-Bera and for non-normal distributions by the Shapiro-Wilk test. Conclusions: As it had the lowest Type I error rate, the Jarqua-Bera test was superior for normal and standard normal distributions. For nonnormal distributions, achieving sufficient power at smaller sample sizes, the Shapiro-Wilk was the most powerful.

Although parametric methods are commonly used for longitudinal data, the use of these methods depends on several assumptions. In recent years, nonparametric methods have been developed to investigate the effect of multiple factors for the longitudinal data that do not meet parametric test assumptions and/or that include ordinal variables. The aim of this study is to introduce the nonparametric method for the analysis of longitudinal data which was developed by Brunner and colleagues and to implement it on a data set from the field of physical medicine and rehabilitation. Marginal distributions are used for obtaining parameter estimates and hypothesis testing, in the analysis of longitudinal data with nonparametric methods. While ANOVA or Wald type statistics are used to test the hypotheses depending on the sample size, relative treatment/marginal effects are usedas descriptive statistics. For the application, Duruöz Hand Index (DHl) scores are compared between two independent groups (control and treatment) at three time points; pre-treatment, during treatment and after 3 months of treatment, by using the FI-LD-FI design. The method developed by Brunner and colleagues offers solutions to different trial designs which may arise in the analyses of longitudinal data. Not requiring any test assumptions and any constraints in terms of sample size can be summarized as the advantages of this method., Uzunlamasına verilerin analizinde parametrik yöntemler oldukça yaygın olarak kullanılsa da bu yöntemlerin kullanımı, belirli varsayımların sağlanmasına bağlıdır. Son yıllarda parametrik test varsayımlarının sağlanmadığı verilerde ve/veya sıralı kategorik değişkenler kullanılarak yapılan değerlendirmelerde birden çok faktörün etkisini incelemek üzere parametrik olmayan yöntemler geliştirilmiştir. Bu çalışmanın amacı, uzunlamasına verilerin analizinde Brunner ve arkadaşları tarafından geliştirilen parametrik olmayan yöntemin tanıtılması ve Fiziksel Tıp ve Rehabilitasyon alanından bir veri seti üzerinde uygulamasının yapılmasıdır. Uzunlamasına verilerin parametrik olmayan yöntemlerle analizinde, parametre tahminlerinin elde edilmesinde ve test edilmek istenen hipotezlerin oluşturulmasında marjinal dağılımlar kullanılır. Hipotezlerin test edilmesinde örneklem büyüklüğüne bağlı olarak ANOVA tipi veya Wald tipi istatistik kullanılırken; tanımlayıcı istatistik olarak göreli deneme/marjinal etkileri yorumlanır. Uygulama için tedavi öncesi, tedavi sırasında ve tedaviden 3 ay sonra elde edilen Duruöz El İndeksi değerleri bağımsız iki grupta (tedavi ve kontrol) FI-LD-FI tasarımı kullanılarak karşılaştırılmıştır. Brunner ve arkadaşları tarafından geliştirilen yöntem, uzunlamasına verilerin analizinde ortaya çıkabilecek farklı deneme tasarımlarına çözüm sunmaktadır. Herhangi bir test varsayımı gerektirmemesi ve örneklem büyüklüğü bakımından kısıtı olmaması yöntemin avantajları olarak özetlenebilir.

Kas-iskelet sistemi hastalıkları, kronik, özürlülüğe yol açan sorunlardır. Bu hastalıkların, gerek yüksek prevalansları gerekse sağlık hizmetlerindeki yüksek maliyetleri nedeniyle toplumdaki ekonomik yükü de fazladır. Kas-iskelet sistemi sorunlarının tedavi ve rehabilitasyonunda hedef, hastalık aktivitesini kontrol altına almak, fonksiyonları korumak ve geliştirmek ve sonuçta özürlülüğü engellemektir. Özürlülük değerlendiriminin etkin bir şekilde yapılabilmesi, ancak geçerli, güvenilir, değişime duyarlı ve kolay uygulanabilir değerlendirme araçları ile mümkündür. BUT, kalibrasyonu yapılmış soru bankalarından sadece en fazla bilgiyi sağlayan maddeleri seçerek, ölçüm kesinliğinin artmasını ve hastaların bıkkınlık düzeylerinin azalmasını sağlar.Bu çalışmanın amacı, toplumda sık görülen ve özürlülüğe yol açan kas-iskelet sistemi hastalıklarından bel ağrısı ve diz osteoartritinde BUT yöntemiyle hastaların özürlülük düzeylerini belirlemektir. Bu amaca yönelik olarak öncelikle ICF sınıflaması temel alınarak bel ağrısı ve diz osteoartritinde özürlülük değerlendirimi için birer soru bankası oluşturulmuş; 266 bel ağrısı ve 225 diz osteoartriti hastası, sırasıyla 108 ve 110 maddeye yanıt vermiştir. Soru bankalarının boyut yapısı kategorik veriler için AFA ve DFA analizleri ile psikometrik özellikleri kısmi kredi modeli ile değerlendirilmiştir. İkinci aşamada da final soru bankaları kullanılarak, geliştirilen yazılımla sağlık alanında yeni bir uygulama olan BUT yöntemiyle ?en çok olabilirlik yöntemi? kullanılarak hastaların özürlülük düzeyi belirlenmiştir.Elde edilen sonuçlar, soru bankasındaki tüm maddeler ile elde edilen özürlülük düzeylerinin, BUT yöntemi kullanıldığında daha az madde ile belirlenebildiğini ve elde edilen sonuçlar arasında yüksek derecede uyum olduğunu göstermiştir. Bu sonuçlar, özürlülüğün BUT yöntemi kullanılarak doğru ve klinik olarak etkin bir şekilde değerlendirilebileceğini kanıtlamaktadır. Musculoskeletal diseases are chronic and disabling disorders. The economic burden of these disorders on the society is great due to both high prevalence and high cost of the diseases to the health care system. The main goals in the treatment and rehabilitation of musculoskeletal disorders are to control the disease activity, maintain and improve function and consequently prevent disability. Assessment of disability in an effective manner will only be possible with valid, reliable, responsive and easily applicable assessment instruments. Computerized Adaptive Tests (CAT) promise to enhance measurement precision and reduce respondent's burden by only selecting the most informative items from a calibrated item bank.The purpose of this study is to determine the disability levels of patients with low back pain and knee osteoarthritis, common and disabling musculoskeletal diseases, by using computerized adaptive testing (CAT) method. To achieve this purpose, firstly, two item banks each for the assessment of disability in low back pain and knee osteoarthritis have been developed based on the ICF model. Two hundred sixty six patients with low back pain and two hundred twenty five patients with knee osteoarthritis have answered 108 and 110 questions respectively. The dimensionality of item banks has been analyzed using exploratory and confirmatory factor analyses for categorical data and the psychometric properties of them have been evaluated by Partial Credit Model. At the second stage, CAT based disability levels of patients have been determined with ?maximum likelihood method? from these final item banks by using the CAT software that had been developed in this study.Results indicate that comparable disability levels to the full-length item banks can be achieved in the CAT version using fewer items and there is a high agreement between them. These results support further consideration of using CAT programs for accurate and effective clinical assessments of disability. 134
