7. The Impact of the COVID-19 Pandemic on Orthopedic Trauma Management; A Cross-sectional Study.

Objective: The aim of this study was to compare emergency orthopedic trauma admissions between the pre- and pandemic periods and detect changes in orthopedic trauma epidemiology. Methods: A total of 40,700 patients admitted within 1 year between March 2019 and March 2020 comprised the pre-pandemic group and 16,935 patients admitted between March 2020 and March 2021 comprised the pandemic group. Demographic characteristics of the patients, such as age and gender, were recorded. In addition, the injury mechanisms of the patients, diagnosis, injured extremity or anatomical region, broken bone, fracture classification, multitrauma rates, trauma-related complications, hospitalization days, and treatment modalities were examined. Results: In the pre-pandemic period, the mean age of the patients was younger, and the distributions of pediatric/adolescent patients and female patients were higher compared with the pandemic period (p=0.001, p=0.001, and p=0.001; respectively). Fractures and dislocations were more frequent in the pandemic period, whereas soft tissue injuries were more common in the pre-pandemic period (p=0.001). Home accidents increased and occupational accidents decreased during the pandemic period (p=0.001). The rate of surgical treatment statistically increased during the pandemic period (p=0.001). Conclusion: Considering the epidemiology of orthopedic trauma, estimating the trauma burden and optimizing resource use and allocation are very important for maintaining safe and effective treatment services for patients in extraordinary situations such as the coronavirus disease-2019 pandemic, which can cause serious disruptions in the healthcare system. Due to its high mechanical properties, Vanadis 8 cold work tool steel is used to cutting and forming applications in industry. In this study; Vanadis 8 cold work tool steel was subjected to cryogenic treatment and conventional heat treatment, and the effects of these samples on mechanical properties and microstructure were investigated. The samples were cooled by cryogenic treatment at a constant rate of 2°C/min from room temperature to -140°C and kept at this temperature for 24 hours. The hardness values of the samples were measured in Rockwell under 200 g load with a 10 second application time. Compression test was carried out on a tensile tester with a loading capacity of 100 kN at a constant delivery speed of 1 mm/min. The microstructures of the samples were evaluated by obtaining SEM images. When the experimental results were examined, hardness and the compressive strength values of the cryogenic sample were increased. After cryogenic treatment, martensite phase, residual austenite phase, vanadium and chromium based carbides were determined in microstructure., Vanadis 8 soğuk iş takım çeliği endüstride yüksek mekanik özellikleri nedeni ile kesme ve form verme işlemlerindeki uygulamalarda karşımıza çıkmaktadır. Bu çalışmada; Vanadis 8 soğuk iş takım çeliğine kriyojenik işlem ve geleneksel ısıl işlem uygulanmış olup, bu numunelerin mekanik özellikleri ve mikroyapı üzerindeki etkileri incelenmiştir. Numuneler kriyojenik işlem ile 2°C/dk sabit hızla oda sıcaklığından -196°C’ye soğutulmuş ve bu sıcaklıkta 24 saat boyunca bekletilmiştir. Numunelerin sertlik değerleri Rockwell cinsinden 200 gr yükün altında 10 sn uygulama süresi ile ölçülmüştür. Basma deneyi 100 kN yükleme kapasiteli çekme test cihazında sabit 1 mm/dk basma hızında gerçekleştirilmiştir. Numunelere ait mikroyapılar SEM görüntüleri elde edilerek değerlendirilmiştir. Deneysel sonuçlar incelendiğinde, kriyojenik işlem uygulanan numunenin sertlik ve basma dayanımı değerlerinde artış gözlenmiştir. Kriyojenik işlem sonrası mikroyapıda martenzit fazı, kalıntı östenit fazı, vanadyum ve krom esaslı karbürler belirlenmiştir.

For the illustration of the design procedure, the hydraulic type double- sided press frames are considered in the thesis. The method can also be ussed for mechanical presses, with small alterations. This thesis has six sections. In- the introduction section, an evaluation of the press sector of the machine tool industry is done. ı In the second section, the theoretical and experimental studies on the classified press types done up today are summar- ized. In the same section the characteristics of the double- sided, welded steel hydraulic press frames which have no com plete method of design calculations, but are widely manufactu red in our country are given. Further in the section, the design criteria for such designs are criticised and among these criteria, the limitation of maximum deflection on the bed plate is determined as the suitable design criterion. In the third section, the basic equations of the theory of plates are introduced and the reduction of the problem of bending of a plate to that of deflection of a membrane is shown. In the fourth section,. the method of obtaining the de flections of the double-sided frames by general methods of applied mechanics and the reduction of the frames to a simply supported deep beam for mostly used geometrical ratios, are given. In the same section, Finite Differences Equations are obtained to calculate deflections of a rectangular plate supported elastiçally along the edges by elastic supports ofIX different rigidity, loaded by the press `forces, and the approx imate boundary values calculated before are used to determine deflections of the press bed plate.. In the fifth section, the total permitted deflection limited by the design criterion is divided between the frame and the bed plate in order to be used in. dimensioning them. - In thi,s way-, by equating the divided value of criterion to the deflection of the members calculated in the previous section, design formulae are obtained.. ; For the bed plate thickness t, the dimension to be calculated by the designer, the formula is obtained as. Fb '. - ' `, E ` where F is the press force, b is the bed length, E is the modulus of elasticity of the material used and a is a tabulated- coefficient. For the frame- face plate thickness s, the most important dimension to be calculated by the designer, the design formula established is F ' s =? 6 ( - ) Eb where (3 is a tabulated coefficient. During the derivation of the formulae, the dimensions minimizing the total mass of the frame and the bed plate are found in the infeasible region, so the rigidities are selectedto give feasible dimensions and thus the design is partially optimized. As a result, double-sided presses up to 1000 tonnes of press power is classified in six groups according to the optimum rigidity needed and within each group a suitable rigidity ratio is chosen to be a characteristic of the group. The suitable rigidity ratios of the six groups are chosen as follows: At the end of the section the formulae are tabulated in order to expedite the design calculations to be done by the press designers. In the sixth and the last section, the measured; deflec tions and forces on two typical examples of 150 tonnes double- sided welded steel hydraulic drawing presses are given and compared with the calculated values in the thesis. The deflections and the forces are measured by dial and strain gauges applied to the examples. The deflection values of the first example are found quite above the calcu lated values in the thesis. The differences are interpreted as the result of the 'discontinuities, such as holes and open ings, on the lower beams.XI The deflection values of the second example which have no openings on the face plates are found quite close to the calculated values in the thesis. Lastly, in the same section, the dimensions of a press frame having the same nominal dimensions as the 150 tonnes example are calculated by the developped formulae and the tables given in the previous section. The rigidity and material saving obtained through the new formulation are shown./ In Appendix 1, the derivation of the deflections of the beams carrying the bed plate are given. Several analytical methods are being employed related to various research projects and routine workson water samples in hydrogeochemistry laboratory of General Directorate of Mineral Research and Exploration. Somestatistical evaluations are necessary in order to obtain accurate results for the projects related. In order to control theprecision and accuracy of the chemical determinations in our laboratory, standard water samples have been requested fromEPA (Environmental Protection Agency, USA). The samples donated by EPA have been analyzed for 28 species using ourstandard methods. Results of the determinations were compared with the true values supplied by EPA using «Studend's ttest». R values have been used to investigate the presence of systematic errors at 95 % an 99 % confidence levels. Accuracyand precision of the analytical methods used have been statistically determined. No systematic error was observed for43 results of the total of 54. Evaluation for systematic error could not be realized for 11 results. Relative percent errorswere found to be in the ranges at 0-10, 11-20, 21-50 and more than 50 for 33, 10, 6,5 results respectively. As a resultof this study, by applying the statistical tests for accuracy and precision, it has been determined that accurate resultsare obtained in most of the methods, only two methods require further development.

Genel olarak, fosil yakıtların öngörülebilir tükenmesi, küresel ısınmaya karşı mücadele gerekliliği, çevrenin korunmasına yönelik farkındalık ve son olarak enerji politikalarındaki sürdürülebilir kalkınmanın dikkate alınması, yenilenebilir enerjileri gezegenimizin geleceği. Ancak şu anda yıllık % 3,5'lik bir ekonomik büyümeyi bilen Cibuti Cumhuriyeti için, ekonomik büyümesini sağlamak ve para birimlerinden tasarruf sağlamak için yenilenebilir enerjilerdeki potansiyelini kullanmak neredeyse hayati önem taşıyor. Ne yazık ki, ülke enerjinin bol olduğu ancak elektriğin nadir olduğu Sahra altı Afrika ülkelerinin aynı enerji durumunu biliyor! Gerçekten de, ülkenin mevcut enerji dengesi güçlü bir şekilde abartılıdır. Nüfusun enerji ihtiyacının % 97'si (çoğunlukla % 85'in üzerinde kentsel) petrol üretimi ithalatıyla karşılanmaktadır ve Cibuti hanelerinin% 90'ı gazyağıyı yerli yakıt olarak kullanmaktadır. Elektrik şebekesinin kapsama oranı çok düşüktür, yaklaşık % 30'dur. Elektrik üretiminin sadece % 0,2'si (toplam kapasite 130 MW kurulu iken) benzersiz bir yenilenebilir kaynaktan yapılmaktadır enerji (fotovoltaik güneş enerjisi). Generally, the predictable exhaustion of the fossil fuels, the necessity of fighting against the global warming, the awareness for the protection of the environment and finally the consideration of the sustainable development in energy policies put the renewable energies in the heart of a strategic stake for the future of our planet. But for the Republic of Djibouti which currently knows an annual economic growth of 3,5 %, it is almost vital to exploit its potential in renewable energies to ensure its economic growth and to realize savings of currencies.Unfortunately, the country knows the same energy situation of the countries of sub-Saharan Africa where the energy is plentiful but the electricity is rare! Indeed, the current energy balance of the country is strongly overdrawn. The 97 % of the energy needs of the population (mainly urban in more than 85 %) are satisfied by the imports of oil productions and 90 % of the Djiboutians households use the kerosene as domestic fuel. The cover rate for the electricity network is very low, about 30 %. Only 0,2 % of the electric production (with a total capacity installed of 130 MW) is made from a unique source of renewable energy ( the photovoltaic solar energy).Nevertheless, Djibouti has a great potential for its renewable energy resources. Obtained solar energy potential at photovoltaic solar energy (PV) level is estimated to be 20 000 MWh. At wind energy level, potential estimation is currently 8 MWh and small or big wind turbine cannot be used in the country. At geothermal energy level, the potential technically is estimated to be between 350-650 MWh. 80

Global dünyada enerjinin dönüşüm içerisinde olması, fosil enerji kaynaklarının sınırlı rezerv durumları ve çevreye vermiş olduğu olumsuz etkileri sebebiyle son yıllarda alternatif yenilenebilir enerji kaynaklarına yönelimi arttırmaktadır.Türkiye'de de sanayileşmenin günden güne gelişip artmasına paralel olarak enerji ve elektriğe olan ihtiyaç da aynı şekilde artmaktadır. Yapılan bu çalışmasında, dünya geneli ve ülkemize ait yenilenebilir enerjinin kaynaklarının güncel durumuna değinilmiştir. HelioScope isimli yazılımla ile çeşitli modellemeler simüle edilip, otopark alanında optimum yatırım ve amortisman ile maksimum verim sağlayacak model aranmıştır. Ele alınan iki çalışmada mevcut alanda ağaç gölge etkisiz ve ağaç gölge etkisi olmak üzere iki ayrı sistem olarak modellenmiştir. Bu iki sistemin, yıllık enerji üretim miktarları, işletme maliyetleri, yatırım maliyetleri incelenmiştir. Turkey also develop in parallel to increase by the day's industrialization is increasing need for energy and electricity in the same way. In this study, the current state of the sources of renewable energy belonging to our country and of the world are mentioned. With HelioScope software, various models have been simulated and the model has been searched for optimum efficiency with maximum investment and depreciation in the parking area. In the two studies discussed, the tree was modeled as two separate systems; the shadow was ineffective and the tree shadow effect was present. Annual energy production amounts, operating costs and investment costs of these two systems are examined. 20. Combining Innovative Techniques: Total Extraperitoneal Approach in Orthopedic Surgery (O-TEP) and Percutaneous Both Column Screw (BCS) Fixation Technique in a Geriatric Acetabular Fracture Case.

- Abstract
The concepts of both column fixation corridor (BCFC) and both column screws (BCS) along with the orthopedic total extraperitoneal approach (O-TEP) are highly innovative approaches in orthopedic surgery. However, the orthopedic literature lacks sufficient studies on these topics, indicating a significant gap. This case report aims to highlight how combining these innovative techniques can facilitate the use of BCFC and BCS in the treatment of acetabular fractures with O-TEP, thereby enhancing the feasibility of endoscopic techniques. Furthermore, we aim to discuss the potential advantages of BCFC and BCS concepts when used in O-TEP.For this purpose, we present a case of a 74-year-old geriatric patient with a transverse acetabular fracture involving the posterior column. The quadrilateral surface was exposed using the O-TEP approach, and the fracture was reduced using a fully endoscopic approach. The fixation was achieved using BCS and a Magic screw through percutaneous screw placement.The patient's clinical condition was followed for at least 13 months. The clinical outcomes demonstrated the effectiveness of combining these innovative techniques in the treatment of acetabular fractures, with satisfactory healing and functional recovery observed.This case report illustrates that combining BCFC and BCS with the O-TEP approach can facilitate the treatment of acetabular fractures with minimally invasive techniques and enhance the feasibility of endoscopic procedures. - Abstract
Objectives: This study aims to categorize and map the incidence and patterns of upper extremity fractures in children during and after novel coronavirus disease 2019 (COVID-19) quarantine and to identify changes in the demographic characteristics and mechanisms of these fractures., Patients and Methods: Between April 2020 and April 2022, a total of 3,549 upper extremity fractures occurring in 1,028 pediatric patients (682 males, 346 females; median age: 7 years; range, 0 to 18 years) were retrospectively analyzed. Those who presented between the dates of April 1
st , 2020 and April 1st , 2021 (quarantine) were included in Group 1, whereas those who presented between April 1st , 2021 and April 2nd , 2022 (post-quarantine) were included in Group 2. The fracture map also showed the fracture density and location., Results: There were statistically significant differences in terms of age range between Groups 1 and 2 (p<0.01). The 6-11 age range was significantly higher in Group 1, and the 12-18 age range was significantly higher in Group 2., Conclusion: Reducing physical activity during quarantine reduces fractures, particularly in adolescents. The removal of restrictions increases fractures in children in this age range. These findings highlight the importance of considering age ranges and physical activity levels while planning safety measures to prevent injuries in children.- Published

22. Dorsal Metatarsal Closed Wedge Osteotomy in the Treatment of Freiberg's Disease: A Prospective Observational Study.

- Abstract
The aim of this study was to prospectively evaluate the clinical and radiological results of dorsal metatarsal closed wedge osteotomy and headless screw fixation in the surgical treatment of Freiberg's disease.Thirty-four patients who were treated with dorsal metatarsal closed wedge osteotomy (DMCWO) for Freiberg's disease between February 2018 and March 2022 were included in the study. Patients were staged according to the classification system described by Smillie. Clinical outcomes were evaluated using the American Orthopedic Foot and Ankle Society's (AOFAS) lesser metatarsophalangeal-interphalangeal scale, the visual analog scale (VAS), the range of motion (ROM) of the metatarsophalangeal (MTP) joint, and a subjective patient satisfaction questionnaire. For radiological evaluation, the amount of preoperative shortening of the involved metatarsal, the amount of metatarsal shortening developed after osteotomy, and radiological recovery times were recorded.Thirty-two (94.1%) of the patients were female, and two (5.9%) were male. The average postoperative follow-up period for patients was 33.7 months (range: 24 months to 41 months). The mean AOFAS scores increased from 53.24 to 86.26 (p < 0.01). The mean VAS scores decreased from 8.59 to 1.79, and it was observed that the patients' pain improved significantly (p < 0.01). The mean ROM of the MTP joint increased from 48.76 degrees to 70.76 degrees (p < 0.01). An average of 2.5 mm (range 1 mm to 4.1 mm) of shortening of the metatarsal length developed postoperatively (p < 0.01). Arthrosis developed in 1 case (2.9%), and transfer metatarsalgia developed in 2 cases (5.8%).DMCWO is an effective treatment for both the early and advanced stages of symptomatic Freiberg's disease, leading to high patient satisfaction., Competing Interests: The authors declare that they have no conflict of interest., (Thieme. - Abstract
The purpose of this retrospective study was to determine the most appropriate distal fusion level in terms of clinical results and radiological changes in Lenke 3C adolescent idiopathic scoliosis (AIS). Between June 2010 and May 2014, a total of 90 consecutive patients who underwent surgery for Lenke 3C AIS were divided into three groups as L2, L3, and L4 according to the fusion levels and compared in terms of functional and radiological outcomes. Patients were evaluated with Scoliosis Research Society 22 (SRS 22) and the Oswestry disability index (ODI) at the final follow-up. Preoperative standard posteroanterior and lateral whole-spine radiographs and lateral bending radiographs were used to evaluate the curves. All patients were also assessed with lumbar MRI at the final follow-up, and classified for each patient in terms of disc degeneration (DD) and facet joint degeneration (FJD). There was a statistically significant difference between the groups in terms of ODI category (P = 0.001). The rate of minimal disability in the L3 group, moderate disability in the L2 group, and severe disability in the L4 group were higher. Statistically significant differences were observed between the groups in terms of pain, body image, mental health, and total score distribution from the SRS-22 domains (P < 0.05). No significant difference was observed between the groups in terms of DD and FJD in MRI (P = 0.263). Although the preoperative functional scores could not be evaluated due to the retrospective nature of the study, it was observed that the only significant difference between the groups was the SRS-22 score. The best SRS-22 score was obtained in patients in whom fusion was stopped at L3.
