Original Article - Abstract
OBJECTIVE: Mammographic screening and management of breast cancer (BC) in elderly women are controversial and continue to be an important health problem. To investigate, through members of the Senologic International Society (SIS), the current global practices in BC in elderly women, highlighting topics of debate and suggesting perspectives. MATERIALS AND METHODS: The questionnaire was sent to the SIS network and included 55 questions on definitions of an elderly woman, BC epidemiology, screening, clinical and pathological characteristics, therapeutic management in elderly women, onco-geriatric assessment and perspectives. RESULTS: Twenty-eight respondents from 21 countries and six continents, representing a population of 2.86 billion, completed and submitted the survey. Most respondents considered women 70 years and older to be elderly. In most countries, BC was often diagnosed at an advanced stage compared to younger women, and age-related mortality was high. For this reason, participants recommended that personalized screening be continued in elderly women with a long life expectancy. In addition, this survey highlighted that geriatric frailty assessment tools and comprehensive geriatric evaluations needed to be used more and should be developed to avoid undertreatment. Similarly, multidisciplinary meetings dedicated to elderly women with BC should be encouraged to avoid under- and over-treatment and to increase their participation in clinical trials. CONCLUSION: Due to increased life expectancy, BC in elderly women will become a more important field in public health. Therefore, screening, personalized treatment, and comprehensive geriatric assessment should be the cornerstones of future practice to avoid the current excess of age-related mortality. This survey described, through members of the SIS, a global picture of current international practices in BC in elderly women.

Original Article - Abstract
OBJECTIVE: Therapeutic management of ductal carcinoma in situ (DCIS) is heterogeneous among countries worldwide, and some treatment indications are still controversial. To investigate DCIS management in different countries; identify both consensual practices and controversial topics; and survey opinions about the future management of DCIS. MATERIALS AND METHODS: The Senologic International Society network members participated to an online survey using a questionnaire, between November 2021 and February 2022. RESULTS: Twenty-two responses from 20 different countries showed that organized breast cancer screening programs were present for 87% participants, and DCIS cases represented 13.7% of all breast cancers. Most participants used the grade classification (100%), the morphological classification (78%) and performed immunohistochemistry assays (73%). In case of conservative treatment, the mean re-excision rate was 10.3% and clear margins of mean 2.5 mm were considered healthy. Radical mastectomy rate was 35.5% with a breast reconstruction rate of 53%. Tumor bed boost indications were heterogeneous, and 73% of participants indicated hormone therapy for hormone-positive DCIS. Surgery and radiotherapy omission for some low-risk DCIS were considered by 73% of participants. Multigene assays were used by 43% of participants. Concerning future changes in DCIS management, participants mostly answered surgical de-escalation (48%), radiotherapy de-escalation (35) and/or active surveillance for some cases (22%). CONCLUSION: This survey provided an overview of the current practices of DCIS management worldwide. It showed that some areas are rather consensual: incidence increases over time, treatment in young women, pathological classifications, definition of healthy margins, the skin-sparing mastectomy and immediate breast reconstruction. However, some topics are still debated and result in heterogeneous practices, such as evolution in the age of diagnosis, the benefit of de-escalation in low-risk DCIS among elderly women, indications for hormone therapy, radiotherapy omission, or multigene assays. Further evidence is needed to reach consensus on these points, and innovative approaches are still under evaluation in clinical trials. The International Senologic Society, by its members, encourages precision medicine and personalized treatments for DCIS, to avoid overtreatment and overdiagnosis, and provide better healthcare to women with DCIS.

Sporcuların Psikolojik İyi Oluş Düzeyi ,Proactive Personality ,Psychological Well-being of Athletes ,Proaktif Kişilik - Abstract
Bu konuda alınan yanıtlar, literatür taramalarında karşımıza çıkan benzer görüş ve önerileri de desteklemiştir., It can easily be understood that a proactive athlete has different characteristics than other individuals to the extent that he / she takes responsibility in the face of events in order to best perform his / her role during sports activities. Psychological well-being can be defined within different form of events such as individual's life satisfaction, elevation of life standards, strong posture under stressful environments and positive look. Based on this phenomenon, an athlete has an important role to be able to have success of sports by his/ her own and team's success in terms of increasing of level of well-being of an athlete. The main frame of our thesis is evaluation of the concepts of proactive personality and psychological well-being in the context of sports psychology. In our study, proactive personality has been considered in order to evaluate the impact of the athlete on the level of well-being in the professional context. The Court of Accounts, which inspects the revenues, expenditures and goods of administrations using public resources on behalf of the Grand National Assembly of Turkey and which has the power to prescribe the accounts and transactions of the responsible persons, is not far from the judicial account which is the point of intersection between law and accounting as a continuous requirement in the conduct of this duty. It is thought that the ways that are followed in the face of a criminal act or public loss are not defined as forensic accounting but totally overlaps with this concept. For this reason, it is considered that the Court of Accounts audit will contribute to the judicial accounting profession and the judicial accounting profession will contribute to the Court of Accounts audit. In this context, the aim of more effective use of judicial accounting, which does not yet have a full scope of application in our country, under independent control of the Court of Accounts will also be put forward through theoretical and practical research in order to make contributions to forensic accounting in the experience of the Court of Accounts in public sector supervision. In our work, firstly the Court of Accounts were dealt with in regard to its functions and Powers. Furthermore financial and compliance audits conducted in the scope of the Court of Accounts audit and in connection with judicial accounting have been conceptually explained. At the same time, the concept of forensic accounting has been addressed with the definitions in the literature and its historical development has been assessed within the context of its relation with other branches of science, and its current situation both in the world and in our country. Another concept called the concept of public loss, which is a bridge between the Court of Accounts audit and judicial accounting has been explained and associated with the Criminal Accounting-Court Audit. In addition, the questionnaire survey was conducted to learn about the knowledge and opinions of the Court of Account auditors regarding the forensic accounting / judicial audit and the functions of the judicial audit under the supervision of the Court of Accounts. The data obtained from the questionnaire were evaluated and then our work ended with the chapter including conclusions and recommendations of the general evaluations.Statistical researches have been done to measure forensic accounting perception of the Turkish Court of Account auditors and The relationship of The Turkish Court of Account with forensic accounting. As a result, statistically signifant relationships between these concepts were found in the positive direction. However, it is concluded that in order to benefit more from forensic accounting a number of regulations should be made in the audit legislation. 115

Objective: Breast cancer is the most common type of cancer and the leading cause of cancer-related deaths in women in Turkey. This study presents the characteristics of patients registered in National Breast Cancer Registry Program of Turkish Federation of Breast Diseases Societies. Materials and Methods: The registry contains 242 variables under 10 categories and 699 questions. Patients were recorded (online and offline) from nationwide breast centers around Turkey. Results: Twenty-thousand patients were registered between May 2005 and April 2017 at 36 centers. After data cleaning, 19,503 women were included in the study. The median age at diagnosis was 51 [14-97]; 17.2% were younger than 40 and 37.2% were premenopausal; 13.6% were nulliparous. Breast conserving surgery rate was 39.3%. Histopathology was invasive ductal cancer in 77%. Majority of patients had stage II cancer (48.3%). Estrogen, progesterone and HER-2 receptor positivity rates in invasive breast cancer were 72.5%, 62.5% and 21.8%, respectively. The mean tumor diameter was 2.5±1.7 cm. During the mean 51.6 months of follow-up, the local/regional and systemic recurrence rates were 3.7% and 5.2%, respectively; five and 10-year overall survival rates were 86% and 76%. Conclusion: Despite increasing number of screening centers and free-of-charge mammography (ages 40 to 69) and mobile screening systems in recent years, a significant portion of patients were diagnosed at advanced stage due to lack of breast cancer awareness. In contrast with the study published 5 years ago, there was a decrease in the rate of pre-menopausal women and an increase in the breast conserving surgery. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: Surgical site infection (SSI) is a common complication after surgery and is an indicator of quality of care. Risk factors for SSI are studied thoroughly for most types of gastrointestinal surgeries and especially colorectal surgeries, but accumulated data is still lacking for gastric surgeries. We studied the parameters affecting SSI rate after gastric cancer surgery. Material and Methods: Consecutive patients, who underwent elective gastric cancer surgery between June and December 2013, were included. Descriptive parameters, laboratory values and past medical histories were recorded prospectively. All patients were followed for 1 month. Recorded parameters were compared between the SSI (+) and SSI (-) groups. Results: Fifty-two patients (mean age: 58.87±9.25 [31-80]; 67% male) were included. SSI incidence was 19%. ASA score ≥3 (p<0.001), postoperative weight gain (p<0.001), smoking (p=0.014) and body mass index (BMI) ≥30 (p=0.025) were related with a higher SSI incidence. Also patients in the SSI (+) group had a higher preoperative serum C-reactive protein level (p=0.014). Conclusion: We assume that decreasing BMI to <30, stopping smoking at least 3 weeks before the operation, and preventing postoperative weight gain by avoiding excessive intravenous hydration will all help decrease SSI rate after gastric surgery. Objective: Mammography-screening (MS) rates remain low in underdeveloped populations. We aimed to find the barriers against MS in a low socioeconomic population. Materials and Methods: Women aged 40-69 years who lived in the least developed city in Turkey (Mus), were targeted. A survey was used to question breast cancer (BC) knowledge and health practices. Results: In total, 2054 women were surveyed (participation rate: 85%). The MS rate was 35%. Women aged 50-59 years (42%, p<0.001), having annual Obstetric-Gynecology (OB-GYN) visits (42%, p<0.001), reading daily newspaper (44%, p=0.003), having Social Security (39%, p=0.006) had increased MS rates. The most common source of information about BC was TV/radio (36%). Having doctors as main source of information (42%, p<0.001), knowing BC as the most common cancer in females (36%, p=0.024), knowing that BC is curable if detected early (36%, p=0.016), knowing that MS is free (42%, p<0.001) and agreeing to the phrase "I would get mammography (MG), if my doctor referred me" (36%, p=0.015) increased MS rates. Agreeing that MG exposes women to unnecessary radiation decreased MS rate (32%, p=0.002). Conclusion: To increase the MS rate in low socioeconomic populations, clear messages about BC being the most common cancer in women, MS after 40 years of age not causing unnecessary radiation but saving lives through enabling early detection, and MS being free of charge should be given frequently on audiovisual media. Uninsured women and women aged 40-49 years should be especially targeted. Physicians from all specialties should inform their patients about BC. [ABSTRACT FROM AUTHOR]

AUTOIMMUNE thyroiditis , *BREAST cancer prognosis , *THYROID diseases , *THYROID hormones , *IODIDE peroxidase - Abstract
Objective: The association of breast cancer and thyroid autoimmunity has been suggested by many studies in the literature, but the causality still needed to be proven. With this study we aimed to search the correlation between thyroid autoimmunity and breast cancer prognostic factors. Materials and Methods: To this prospective cohort study 200 consecutive breast cancer patients, who were operated in our clinic were included. Patients' serum thyroid hormone, anti-thyroglobuline (anti-TG) and antithyroid peroxidase (anti-TPO) levels and tumors' prognostic parameters (tumor size, axillary involvement, histological grade, lymphovascular invasion, receptor status, Ki-67 proliferation index) were collected. The correlation between serum thyroid autoantibody levels and tumor's prognostic factors were studied. Results: The prevalence of thyroid autoimmunity (high levels of serum anti-TPO and/or anti-TG) was 18.5% (n=37). Patients with thyroid autoimmunity had a significant lower rate of axillary involvement and a lower rate of Ki-67 proliferation index (22% vs. 46% [p=0,007] and 12.73% vs. 20.72% [p=0.025], respectively) and were more commonly included to the "low-risk" group (<14%) according to their Ki-67 scores (68% vs. 46%; p=0.015). Other parameters did not differ between the two groups. Conclusion: We found a favorable correlation between thyroid autoimmunity and axillary involvement and also Ki-67 proliferation index score, which are two crucial and strongly predictive parameters of breast cancer prognoses. This supports the idea of thyroid autoimmunity being a favorable prognostic parameter. Further studies are necessary to investigate the reasons of protective or predictive effect of high thyroid peroxidase levels in breast cancer patients. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: Anorectal abscess is a clinical condition frequently encountered in daily surgical practice and recurrences may occur despite treatment with adequate incision and drainage. The primary aim of this study was to analyze the variables that may have resulted in recurrent anorectal abscess, retrospectively. Material and Methods: Ninety-three patients out of 149 patients who underwent surgery for anorectal abscess at our center between 2011-2012 were included in this study. Data regarding age, gender, presence of recurrence, time to recurrence, abscess type, presence of fistula, fistula type, drain usage, length of hospital stay and follow-up duration were retrospectively recorded. Results: Patients were divided into two groups: the recurrence group and the treatment group. Eleven patients (11.8%) had a recurrence and the median time to recurrence was 3 months. None of the variables evaluated were found to be significantly associated with the presence of recurrence. Conclusion: Variables such as age, gender, type of abscess, presence of fistula or drain usage were not associated with the development of recurrence in patients who underwent incision and drainage of an anorectal abscess. - Abstract
Objective: To compare the clinical and biochemical outcomes between adequate and inadequate parathyroidectomies in patients with chronic renal failure. Material and Methods: All secondary hyperparathyroidism patients who were previously operated in the Marmara University Hospital Breast and Endocrine Surgery Unit were planned to be included in the study. Patients were divided into two groups according to their extent of surgery: "adequate" and "inadequate" surgery groups. "Adequate surgery" was regarded as either subtotal (3½) or total parathyroidectomy. Removing fewer than 3½ parathyroids was defined as "inadequate surgery." Demographic, preoperative clinical symptoms, and their severity, as well as biochemical (e.g., tCa, PTH) findings, were recorded. Patients were followed monthly. The course of biochemical findings (tCa, PTH, P, ALP) and symptoms (by a scoring system of 1-4) was determined by comparing preoperative findings to those at the patient's last follow-up. Primary outcome of the study was treatment failure (biochemical persistence/recurrence) rates in both study cohorts. Secondary outcomes of the study were the levels of biochemical findings and improvement rates of clinical symptoms after parathyroidectomy, as well as complication rates related to the initial surgery in each surgery cohort. Results: Forty-two patients with secondary hyperparathyroidism who underwent parathyroidectomy were included into the study. Twenty-six were male and 16 were female. Median age was 46. Forty (95%) patients had at least one symptom as the indication for surgery, whereas only 2 (5%) patients were asymptomatic, but biochemical findings were the indication. Twenty-two (52%) patients underwent adequate operation, whereas 20 (48%) patients had inadequate operation. Mean follow-up duration after initial parathyroidectomy was 60 [3-244] months. Significantly more patients (n=15; 75%) in the inadequate surgery group had biochemical persistence/recurrence when compared with those (n=8; 36%) who underwent adequate surgery (OR [odds ratio] 5.25; 95% CI 1.38-19.93; p=0.012). However, symptom improvement rates were similar in both adequate and inadequate surgery groups. Conclusion: Although there is high biochemical treatment failure after inadequate parathyroidectomy in patients with renal hyperparathyroidism, clinical symptom improvement rates are also surprisingly high in this patient group. On the other hand, the adequate surgery group also had an increased biochemical failure rate well above expected in longer follow-ups. [ABSTRACT FROM AUTHOR]

BREAST cancer research , *AUTOIMMUNE thyroiditis , *THYROGLOBULIN , *CONTROL groups , *DISEASE prevalence , *CANCER in women , *ESTROGEN - Abstract
Objective: Recent studies indicated an association between thyroid autoimmunity and breast cancer, but the causality still needed to be proven. The aim of this study was to compare the prevelance of autoimmune thyroid diseases (ATD) between breast cancer patients and the control group. We hypothesised that breast cancer patients have a higher ATD prevalance and higher thyroglobulin gene polymorphism rates than the control group. Materials and Methods: The study was designed as a prospective matchedcohort investigation. Control sample size was determined according to 1:1 matching with cancer cases. The study group included 100 consecutive breast cancer patients admitted to "Marmara University Hospital", while the control group included 100 women admitted to outpatient clinics in whom breast cancer was ruled out. Thyroid autoantibody, thyroid and sex hormone levels were measured and compared between study and control groups. In order to compare Tg gene polymorphism rates between breast cancer and control groups, although cancer tissue samples were obtained from study patients, a different control group with the same age, sex and ethnicity with the study group was constituted from the hospital's tissue bank. Results: Autoimmune thyroid disease prevalances were similar between study (n=25) and control (n=18) groups. Tg gene polymorphism prevalences were also similar between the two groups. Estrogen levels were significantly higher in patients with ATD (89.75±84.4 pg/mL) when compared to those without (54.17±73.89 pg/mL). Conclusion: In conclusion, the prevalences of ATD and Tg polymorphism were similar between breast cancer cases and controls. As a significant finding, patients with ATD among all the cohort (with and without breast cancer) had higher serum estrogen levels when compared to those without. [ABSTRACT FROM AUTHOR]

Purpose: To diagnose and describe the appendicular mucocele and its importance and possible complications, and also to show the laparoscopic approach in this disease, against the known, open surgical procedure. Methods: The appendicular mucocele was diagnosed in a patient by abdominal ultrasonography and abdominal MR imaging techniques in our clinic. Afterwards it was decided to perform a surgical operation and laparoscopic approach was planned. With the use of laparoscopic endobag and sufficient surgical technique, mucocele of the appendix was taken out of the abdomen without causing any perforation. Results: By the laparoscopic approach in this patient, using a laparoscopic endobag, the appendicular mucocele was not ruptured. And the mucous material inside the appendicular mucocele did not contaminate the abdominal cavity, which would have caused pseudomyxoma peritonei. Conclusion: Altough there is a risk of pseudomyxoma peritonei in appendicular mucocele, the surgical procedure may be achieved by laparoscopic approach with the use of appropriate devices and sufficient surgical experience. - Abstract
Background: The minichromosome maintenance protein-2 (MCM-2) is a more sensitive proliferation marker than Ki-67. This study aimed to evaluate the relationship between MCM-2 and Oncotype DX recurrence score (ODX-RS) and determine an MCM-2 cutoff value in high-risk patients according to TAILORx risk categorization., Methods: Hormone receptor (HR) positive HER-2 negative early-stage breast cancer patients (pT1-2, pN0-N1, M0) who had ODX-RS were included in the study. According to the TAILORx trial, patients were divided into two groups with high (ODX-RS ≥26) and low risk (ODX-RS <26) in terms of ODX-RS. Formalin-fixed-paraffin-embedded tissues of patients were re-evaluated, and 3 µm sections were prepared for MCM-2 immuno-histochemical staining. The relationship between ODX-RS and the percentage of MCM-2 staining was evaluated in two groups. The ROC curve analysis was performed to determine the MCM-2 cut-off value for the TAILORx high-risk group (ODX-RS ≥26)., Results: The mean MCM-2 value was significantly higher in the high-risk group [(60.2 ± 11.2 vs 34.4 ± 13.8, p < 0.001)]. In the multivariate analysis, MCM-2 (OR: 1.27, 95% CI: 1.08-1.49, p = 0.003) and progesterone receptor (PR) levels ≤10% (OR: 60.9, 95% CI: 4.1-89.7, p = 0.003) were found to be independent factors indicating a high-risk group. A one-unit increase in MCM-2 level increased the likelihood of being in the high-risk group by 1.27 times. In the ROC curve analysis, the optimal MCM-2 cut-off level was 50 (AUC: 0.921, sensitivity: 86.7%, specificity: 96.0%, p < 0.001)., Conclusion: Our study is the first study in the literature to investigate the relationship between ODX-RS and MCM-2 levels in HR-positive HER-2 negative early breast-cancer patients. In this study, MCM-2 was an independent risk factor in identifying high-risk patients according to TAILORx risk classification. MCM 2 cut-off value (50) may help the decision on adjuvant chemotherapy in patients where the Oncotype DX test cannot be performed., Competing Interests: The authors declare that they have no competing interests., (© 2023 Ünal et al.)
