10. The earthquake disaster in Türkiye: a perspective on newborn evacuation and an ophthalmological approach.

This article discusses the devastating earthquake disaster that occurred in Turkey in February 2023. The earthquakes, with magnitudes of 7.7 and 7.6, caused widespread damage and resulted in hundreds of thousands of injuries and over 45,000 reported deaths. The article focuses on the perspective of a hospital in Adana, which had to evacuate due to aftershocks and transfer newborns to other hospitals. The hospital treated earthquake victims with eye injuries and conducted screenings for premature retinopathy in newborns. The article emphasizes the need for earthquake-resistant buildings and disaster plans in hospitals, as well as the importance of identifying and protecting pediatric patients during disasters. 12. Candida Enfeksiyonlarında Probiyotik: Tek Merkez Yenidoğan Yoğun Bakım Ünitesinde On Yıllık Tecrübe

Objectives: Candida species are one of the most important causes of morbidity and mortality in the neonatal population. The probiotic use is not well documented in candida infections in neonates. The aim of this study was to evaluate candida infections in neonatal intensive care unit.Methods: A retrospective single-center, matched, case-control study was designed to identify individual risk factors for Candida infection in the NICU. Patients enrolled in the analysis were all neonates who were in the NICU during the study period for at least 72 h. For each case, 2 control cases with negative blood culture were matched on 3 factors: gestational age, birth weight, hospitalization time (within 1week compared to case patients).Results: 7696 newborns were admitted to and hospitalized >72 h in the NICU for eleven years and 51 cases (0.66 %) were followed with candida infection. Probiotic usage was higher in control group. Surgical interventions, central venous lines and parenteral nutrition duration, multiple antibiotic usages were found to be risk factors for candida infections. Conclusion: In NICU we should effectively focus on increasing using of probiotics, besides decreasing the overall use of multiple antibiotics, improving catheter care to reduce the incidence of candida infections. Probiotics might have protective effect against candida infections., Amaç: Candida türleri, yenidoğan popülasyonunda en önemli morbidite ve mortalite nedenlerinden biridir. Yenidoğanlarda kandida enfeksiyonlarında probiyotik kullanımı tartışmalıdır. Bu çalışmanın amacı yenidoğan yoğun bakım ünitesinde kandida enfeksiyonlarını değerlendirmektir.Metod: YYBÜ'de Candida enfeksiyonu için risk faktörlerini belirlemek için retrospektif tek merkezli, karşılaştırmalı bir vaka kontrol çalışması tasarlanmıştır. Çalışmaya alınan hastaların tümü, çalışma süresi boyunca en az 72 saat YYBÜ'de bulunan yenidoğanlardı. Her vaka için negatif kan kültürü olan 2 kontrol vakası 3 faktör açısından eşleştirildi: gebelik yaşı, doğum ağırlığı, hastanede kalış süresi (1 hafta içinde).Sonuç: 7696 yenidoğan on bir yıl boyunca YYBÜ'de 72 saatten fazla yatırıldı ve 51 vaka (% 0,66) kandida enfeksiyonu ile takip edildi. Kontrol grubunda probiyotik kullanımı daha yüksekti. Cerrahi müdahaleler, santral venöz hatlar ve parenteral beslenme süresi, çoklu antibiyotik kullanımları kandida enfeksiyonları için risk faktörleri olarak bulundu.Sonuç: YYBÜ'de kandida enfeksiyonlarının insidansını azaltmak için çoklu antibiyotiklerin genel kullanımını azaltmanın yanı sıra probiyotik kullanımını artırmaya, kateter bakımını iyileştirmeye etkin bir şekilde odaklanmalıyız. Probiyotiklerin Candida enfeksiyonlarına karşı koruyucu etkisi olabilir.

Here, a newborn baby with perforated appendicitis in Amyand's hernia is presented because of its different clinical follow-up.

Konjenital batın ön duvarı defektleri nadir görülen yapısal anomalilerdir. Bu olguların ve eşlik eden anomalilerin erken saptanması, bebeklerin doğumdan sonra yatırılarak izleminin ve bakımlarının erken başlanması prognozu etkilemektedir. Bu çalışma 4 yıllık süre içinde hastanemizde doğmuş ve yenidoğan yoğun bakım ünitemizde takip edilmiş batın ön duvarı defekti tanılı olguların canlı doğumlar içindeki sıklığını, prenatal tanı oranlarını, demografik ve klinik verilerini incelemek amacıyla yapılmıştır. Haziran-2015 ile Ekim 2019 tarihleri arasında batın ön duvarı defekti tanısı ile yatırılarak izlenen 23 hastanın demografik ve klinik verileri retrospektif olarak incelendi, föy oluşturulduktan sonra hasta dosyaları tarandı. Çalışmamızda hastanemize başvuran batın ön duvarı defektlerinin çoğu doğum sırasında teşhis edilmişti. Antenatal takip edilen sadece 5 olgu prenatal tanı ile yatırılmıştı. Çalışmadaki olguların çoğunluğunu takipsiz gebeliklerden olan bebekler oluşturmuştu. Olguların çoğunluğunu omfaloselli bebekler (%56.5) idi. Olguların toplam mortalite oranı %26.1 idi. Mortalite ve morbiditeyi etkileyen en önemli faktörün eşlik eden anomaliler olduğu görüldü. Batın ön duvarı defekti olan olguların çoğunluğunu omafaloselli hastalar oluşturuyordu. Bu olgularda prenatal tanı oranı oldukça düşüktü. Antenatal takip ve 3.-4. düzey yenidoğan yoğun bakım ünitelerinin olduğu merkezlerde doğumun yaptırılmasının, mortalite ve morbiditeyi azaltmada etkili olacağı düşünüldü., Congenital anterior abdominal wall defects are rare structural anomalies. Early detection of these cases and associated anomalies, if any and early hospitalization and early care of babies after birth affect the prognosis. This study was conducted to examine the frequency of live births, prenatal diagnosis rates, demographic and clinical data of patients diagnosed with anterior abdominal wall defects who were born in our hospital and followed in our neonatal intensive care unit within a 4-year period. The demographic and clinical data of 23 patients who were hospitalized with the diagnosis of anterior abdominal wall defect between June-2015 and October 2019 were retrospectively analyzed, and patient files were scanned. In this study, most of the anterior abdominal wall defects admitted to our hospital were diagnosed during delivery. Only 5 cases followed antenatally were hospitalized with prenatal diagnosis. Most of the cases in the study consisted of babies from pregnancies without follow-up. The majority of cases were babies with omphalocele (56.5%). The total mortality rate of the cases was 26.1%. Concomitant anomalies were the most important factor affecting mortality and morbidity. The majority of patients with anterior abdominal wall defects were patients with omphalocele. In these cases, the prenatal diagnosis rate was quite low. Close follow up pregnant women in antenatal period and to provide giving births in centers with III-IV level NICU would be effective in reducing mortality and morbidity.

21. Clinical features and echocardiographic findings of isolated foramen ovale restriction in foetuses.

Isolated restrictive foramen ovale (rFO) without complex heart defects is a rare pathology. There may be difficulties in managing this situation, which can lead to right heart enlargement, tricuspid regurgitation and hydrops findings in the foetus. We conducted a retrospective analysis of 8451 foetuses. 7883 (93.2%) had a structurally normal heart or minor heart disease, 18 (0.22%) of which had a diagnosis of isolated rFO. Nine patients with neonatal echocardiographic examination were included in the study. In 8 (88.8%) patients, it was stated that a decision to give birth should be made at the time of presentation. Evaluating postpartum echocardiographic examinations, 7 (77.7%) patients had normal or minor defects. The decision of delivery made at the right time during follow-up is critical to determine the prognosis. What is already known on this subject? The data about the prenatal diagnosis of isolated rFO is limited. What the results of this study add? We conducted a retrospective analysis of 8451 foetuses. 7883 (93.2%) foetuses had a structurally normal heart or minor heart disease, 18 (0.22%) of which had a diagnosis of rFO. Nıne patients with foetal and postnatal follow-up and echocardiographic findings were examined in the study. The group's median gestational age at admission was 35.0 weeks (range: 27.0–39.0 weeks). The delivery decision was made in 8 (88.8%) patients at the time of admission. Evaluating postpartum echocardiographic examinations, 7 (77.7%) patients had normal or minor defects. Additionally, one patient was diagnosed with cardiomyopathy, and the other patient was diagnosed with functional pulmonary atresia. No death occurred in any foetus during follow-up. What the implications are of these findings for clinical practice and/or further research? Isolated rFO, a rare condition in the foetus, is generally well-tolerated in foetal life. However, the right heart enlargement, tricuspid regurgitation, or hydrops findings can be seen in patients. Amaç: Prematüre bebekler, çevresel stres faktörlerine karşı özellikle yaşamın ilk haftalarında daha savunmasızdır. Bu süre zarfında, özellikle bebeklerin etrafında kullanılan destekleyici konumlandırma, kendilerini uterus içinde olduğu gibi daha iyi hissetmelerini sağlar. Bu çalışmanın amacı, yenidoğan yoğun bakım ünitesinde destekleyici konumlandırmanın; prematüre bebeklerin kilo alımı, yaşamsal belirtileri, beslenme intoleransları, ventilasyon süreleri, hastanede kalış süreleri ve konfor ölçeği puanları üzerine etkisini değerlendirmektir.Gereç ve Yöntemler: Çalışmaya rastgele olarak seçilen, 25'i desteklenen grupta ve 25'i kontrol grubunda olmak üzere toplam 50 prematüre bebek dahil edildi. Desteklenen gruptaki bebekler pozisyon malzemesi olarak yumuşak örtüler ve yastıklarla yuvalandı. Kontrol grubunda yuvalama ya da kundaklama yoktu. During this time, supportive positioning, especially used all-around the baby, makes them feel better as if they are in utero. The aim of the study was to evaluate the effect of supportive positioning on weight gain, vital signs, feeding intolerance, duration of ventilation, duration of hospitalization and comfort scale scores of the premature babies in neonatal intensive care unit.Material and Methods: A total of 50 premature infants were recruited into the study randomly, 25 in the supported group and 25 in the control group. The babies in the supported group were nested with soft blankets and pillows as position material. There was no nesting or swaddling in the control group. Limited data are available on pregnant women with COVID-19 and their neonates. We aimed to evaluate the epidemiological and clinical characteristics of newborns born to women infected with COVID-19. A multicenter cohort study was conducted among newborns born to mothers with COVID-19 in 34 neonatal intensive care units (NICUs) in Turkey. Pregnant women (n = 125) who had a positive RT-PCR test and their newborns were enrolled. Cesarean section, prematurity, and low-birthweight infant rates were 71.2%, 26.4%, and 12.8%, respectively. Eight of 125 mothers (6.4%) were admitted to an intensive care unit for mechanical ventilation, among whom six died (4.8%). Majority of the newborns (86.4%) were followed in isolation rooms in the NICU. Four of 120 newborns (3.3%) had a positive RT-PCR test result. Although samples taken on the first day were negative, one neonate became positive on the second day and the other two on the fifth day. Sample from deep tracheal aspirate was positive on the first day in an intubated case. Conclusion: COVID-19 in pregnant women has important impacts on perinatal and neonatal outcomes. Maternal mortality, higher rates of preterm birth and cesarean section, suspected risk of vertical transmission, and low rate of breastfeeding show that family support should be a part of the care in the NICU. Purpose: Nosocomial sepsis is the most common acquired infection. Despite appropriate antibiotic treatment, mortality and morbidity of sepsis are still high. We aimed to evaluate the effect of pentoxifylline on prognosis of neonatal nosocomial sepsis in premature infants hospitalized in the neonatal intensive care unit.Materials and Methods: Eighty newborns diagnosed as nosocomial sepsis were included in this study. Forty of them received pentoxifylline treatment in addition to antibiotics, while the other 40 did not receive additional treatment and formed the control group. The sex, mode of delivery, gestational age, birth weight, Apgar scores at the 1st and 5th minutes, surfactant therapy, ventilator therapy and presence of early onset sepsis were compared between the case group and the control.Results: There was no statistical difference between groups according to gestational week, gender, birth weight, mortality, neutrophil count or procalcitonin level at the time of diagnosis. Also, there was not any statistical difference according to duration of hospitalization between the bronchopulmonary dysplasia or necrotizing enterocolitis groups.Conclusion: Pentoxifylline has no significant impact on mortality and morbidity of preterm nosocomial sepsis., Amaç: Nozokomiyal sepsis, hastane kaynaklı kazanılmış infeksiyonlar arasında en sık olanıdır. Uygun antibiyotik tedavisine rağmen sepsisin mortalitesi ve morbiditesi hala yüksektir. Bu nedenle diğer tedavi yöntemleri hala aranmaktadır. Yenidoğan yoğun bakım ünitesinde yatan prematüre bebeklerde pentoksifilinin neonatal nozokomiyal sepsisin prognozu üzerine etkisini değerlendirmeyi amaçladık.Gereç ve Yöntem: Bu çalışmaya nozokomiyal sepsis tanısı konan 80 yenidoğan dahil edildi. Bunların 40'ı antibiyotiklere ek olarak pentoksifilin tedavisi alırken, diğer 40'ı almadı ve kontrol grubunu oluşturdu. Gebelik yaşı, doğum ağırlığı, 1. ve 5. dakikadaki Apgar skorları, doğum şekli, cinsiyeti, surfaktan tedavisi, ventilatör tedavisi ve erken başlangıçlı sepsis varlığı vaka grubuyla kontrol arasında karşılaştırıldı.Bulgular: Gruplar arasında tanı anında gebelik haftası, cinsiyet, doğum ağırlığı, mortalite, nötrofil sayısı veya prokalsitonin düzeylerine göre istatistiksel bir fark yoktu. Ayrıca, hastanede kalış süresi, bronkopulmonerdisplazi veya nekrotizan enterokolit gelişmesine göre de gruplar arasında istatistiksel bir fark yoktu.Sonuç: Pentoksifilin, preterm nozokomiyal sepsisin mortalitesi ve morbiditesi üzerinde önemli bir etkiye sahip değildir.

- Abstract
Objective: to investigate the early neonatal outcomes of very-low-birth-weight (VLBW) infants discharged home from neonatal intensive care units (NICUs) in Turkey. Material and methods: a prospective cohort study was performed between April 1, 2016 and April 30, 2017. The study included VLBW infants admitted to level III NICUs. Perinatal and neonatal data of all infants born with a birth weight of.1500 g were collected for infants who survived. Results: data from 69 NICUs were obtained. The mean birth weight and gestational age were 1137 +/- 245 g and 29 +/- 2.4 weeks, respectively. During the study period, 78% of VLBW infants survived to discharge and 48% of survived infants had no major neonatal morbidity. VLBW infants who survived were evaluated in terms of major morbidities: bronchopulmonary dysplasia was detected in 23.7% of infants, necrotizing enterocolitis in 9.1%, blood culture proven late-onset sepsis (LOS) in 21.1%, blood culture negative LOS in 21.3%, severe intraventricular hemorrhage in 5.4% and severe retinopathy of prematurity in 11.1%. Hemodynamically significant patent ductus arteriosus was diagnosed in 24.8% of infants. Antenatal steroids were administered to 42.9% of mothers. Conclusion: the present investigation is the first multicenter study to include epidemiological information on VLBW infants in Turkey. Morbidity rate in VLBW infants is a serious concern and higher than those in developed countries. Implementation of oxygen therapy with appropriate monitoring, better antenatal and neonatal care and control of sepsis may reduce the prevalence of neonatal morbidities. Therefore, monitoring standards of neonatal care and implementing quality improvement projects across the country are essential for improving neonatal outcomes in Turkish NICUs., Turkish Neonatal Society

In newborns, hyperammonemia leads to encephalopathy which is usually characterized by vomiting, hypotonia, lethargy, seizures and coma. Continuous venovenous hemodiafiltration (CVVHDF) is a modality choice to treat acute decompensation in hyperammonemia. Here we report three newborn patients with hypotonia, convulsion and hyperammonaemia. In the first three days of life, their serum ammonia levels were 4609, 1023 and 1949 µg/ml. They were successfully treated with CVVHDF and serum ammonia levels subsequently decreased to 268, 164 and 65 µg/ml in the first 24 hours of treatment. The complications were mild hypothermia and anemia., Yenidoğan bebeklerde hiperamonemi, hipotoni, letarji, nöbetler ve koma ile karakterize ensefalopati tablosuna yol açabilir. Akut dekompanse olmuş hiperamonemi tedavisinde devamlı venovenöz hemodiafiltrasyon tedavi seçeneği olabilir. Burada hipotoni, konvülziyon ve hiperamonemi olan üç yenidoğan olgusu sunulmuştur.yaşamlarının ilk üç gününde amonyak değerleri 4609, 1023, 1949 microgram/ml idi. Devamlı venovenöz hemodiafiltrasyon ile başarılı bir şekilde tedavi edilerek tedavinin ilk 24 saatinde amonyak değerleri 268, 164 65 microgram/ml'ye azaltıldı.hafif hipotermi ve anemi komplikasyon olarak saptandı.

Objective: Understanding the etiologies of infant deaths and predicting risk begins with defining cases correctly. The collection and analysis of reliable statistical data are an essential part of comprehensive investigations on local and national levels. Material and Methods: This was a descriptive study. Diagnosis as a cause of infant death and other factors were examined. Data were analyzed using the SPSS 17.0 program. Frequencies and means were computed. Results: According to the data of 35 227 live births and 288 infant deaths during the year, the infant mortality rate of Adana province was 8.2 per thousand. We found that the most common cause of infant death was sepsis (26.0%). Other causes of infant death included prematurity and problems related to prematurity (12.6%), congenital heart disease and complications (12.6%), respiratory problems (11.9%), congenital anomaly (9.8%), immaturity (<750 g birth weight or 24 gestational weeks) (8.4%), and sudden death (6.0%). Conclusion: In our descriptive study, we presented the causes of infant mortality to minimize infant mortality due to preventable causes. Efforts to reduce infant mortality should be supported and improved by health management. Emphasis on prenatal care is one of the important things to reduce premature births. [ABSTRACT FROM AUTHOR]

- Abstract
Background to evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition. Methods: a prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA)<= 32 weeks and those with a BW> 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses. Results: the TR-ROP study included 6115 infants: 4964 (81%) with a GA <= 32 weeks and 1151 (19%) with a GA>32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. Of all infants, 414 needed treatment and 395 (95.4%) of the treated infants had a BW <= 1500 g. Sixty-six (16%) of the treated infants did not fulfil the Early Treatment for Retinopathy of Prematurity requirements for treatment. Conclusions: screening of infants with a GA <= 34 weeks or a BW<1700 g appears to be appropriate in Turkey. Background/aim: Systemic to pulmonary shunts (SPS) have proven to be highly effective for the palliation of neonates with cyanotic congenital heart disease. Mortality after SPS surgery in neonates has multifactorial basis. We aimed to investigate the clinical results of the SPS in relation to the underlying cardiac disease and to identify the risk factors contributing to an adverse outcome.Material and method: All neonates who underwent first shunt insertion for cyanotic congenital heart disease during the study period from 1 January 2014 to 31 December 2017 were included. A retrospective review of patient records was done. Patients were grouped into two different categories: survived with or without any reintervention and death before or after any reintervention till discharge.Result: During the study period, 47 patients underwent SPS shunt placement. Patients who survived with or without any reintervention were in Group 1 and patients who died before or after any reintervention till discharge were in Group 2. Preoperative epinephrine requirement and mechanical ventilation and postoperative erythrocyte transfusion need were statistically significant.Conclusion: Although primary cardiac pathology is the most important prognostic factor, some other preoperative and postoperative factors like preoperative epinephrine requirement, and postoperative erythrocyte transfusion might also affect the prognosis. As there are very few centers in the region that specialize in pediatric cardiac surgery, a multicenter approach will be helpful in reaching reliable conclusions. [ABSTRACT FROM AUTHOR]

Surfactant replacement therapy has been the mainstay of treatment for preterm infants with respiratory distress syndrome for more than twenty years. In recent years, the growing interest in noninvasive ventilation has led to novel approaches of administration. Non-invasive techniques of respiratory support were developed in order to reduce the adverse effects associated with ventilation via an endotracheal tube. Noninvasive surfactant administration technique during spontaneous breathing along with nasal continous positive airway pressure support successfully reduces the need for further respiratory support and bronchopulmonary dysplasia rate in very low birth weight infants. Here we reviewed the new approches ton surfactant administration., Yenidoğan bebeklere ve özellikle respiratuvar distres sendromlu prematürelere dışarıdan endotrakeal yolla surfaktan uygulaması, günümüzde yenidoğan tedavisinin önemli bir parçası olmuştur. Son yıllarda invaziv olmayan ventilasyon uygulamaları beraberinde surfaktan uygulama metodlarını da değiştirmeye başlamıştır. Endotrakeal tüp ile ventilasyonun komplikasyonlarını azaltmak için invaziv olmayan ventilasyon teknikleri geliştirilmiştir. Dolayısıyla da endotrakeal tüp kullanmadan surfaktan uygulamaları da gündeme gelmiştir. Nazal devamlı pozitif havayolu basıncı ile birlikte kanül ile surfaktan uygulamasının bronkopulmopner displazi gibi komplikasyonları azaltabileceği çalışmalarla ortaya konmuştur. Bu yazıdasurfaktan uygulamasındaki yeni metodlar kısaca gözden geçirilmiştir.

Respiratory distress syndrome in premature babies is one of the most common and most important health problems in newborns. Respiratory distress syndrome of newborn is a syndrome in premature infants caused by developmental insufficiency of surfactant production and structural immaturity in the lungs. Respiratory distress syndrome begins shortly after birth and is manifest by tachypnea, tachycardia, chest wall retractions, expiratory grunting, nasal flaring and cyanosis during breathing efforts. Respiratory distress syndrome or complications caused by respiratory distress syndrome are the most important causes of mortality and morbidity in premature infants. This article briefly reviews respiratory distress syndrome and its complications., Respiratuar distres sendromu prematüre yenidoğanların en sık görülen ve en önemli hastalıklarından birisidir. Respiratuar distres sendromu akciğerlerin immatüritesi ve sürfaktan eksikliğinden kaynaklanan solunum sıkıntısı sendromudur. Klasik olarak, bebeklerde doğumdan sonraki ilk saatlerde başlayan taşipne, dispne, interkostal çekilmeler, inleme, burun kanadı solunumu, siyanoz ve oksijen ihtiyacının artmasıyla karakterize bir hastalıktır. Prematüre bebeklerdeki mortalite ve morbiditenin en önemli nedenini respiratuar distres sendromu ve buna bağlı olarak gelişen komplikasyonlar oluşturmaktadır. Bu yazıda respiratuar distres sendromu ve olası komplikasyonları kısaca gözden geçirilmiştir.
