Although IFNß is a safe and well-tolerated drug, dermatological side effects are common. The most common dermatological adverse effect is a local reaction at the injection site. The formation of a scar represents the extension of regenerative wound repair mechanisms. On the one hand, scar tissue helps in wound closure by supporting wound contracture and reepithelization. There is no doubt that the scar formation developed in response to an injury facilitates the organism’s survival process directly and effectively. On the other hand, the effect of the pathologically developed scarring on the global disease burden has still not been accurately defined. There are three basic subtypes of scars, namely simple, atrophic, and hypertrophic/pathological (hypertrophic scars and keloids), and they have different histopathological features, development, dispersion patterns, and remission phases. There are important consequences when scars’ global and individual effects are considered. Patients with scars face long-term functional and psychological problems. In interpreting scars for both individuals and society, individual, cultural, psychological, and social effects and prejudices play a crucial role. Both the psychosocial and financial aspects of the scars, as well as the effects on the patient and society, should be considered. © 2019 by Nova Science Publishers, Inc.

WOS: 000502985200004 OBJECTIVE: Some melanocytic lesions can be difficult to diagnose because of ambiguous histological and immunohistochemical features. Morphometric features of melanocytes in the upper and lower parts of a challenging lesion may help to facilitate an accurate diagnosis. STUDY DESIGN: We studied 32 cases of invasive cutaneous melanoma, 35 cases of mild or moderate dysplastic nevus, and 31 cases of benign melanocytic nevus. All cases were immunostained with Sox10. The nuclear areas of 30 melanocytes were measured on each papillary (upper al1) dermis and reticular dermis/subcutaneous tissue (lower ILI) in all cases by using the Image J analysis program. Then, a maturation index (U/L) was calculated for each case. Also, cutaneous melanomas were categorized into 2 groups that displayed pseudo-maturation or the lack of it. RESULTS: Mean maturation index was 1.04 +/- 0.29 in melanomas, 1.23 +/- 0.28 in dysplastic nevi, and 1.52 +/- 0.33 in benign melanocytic nevi, respectively. There were statistically significant differences between melanoma and dysplastic nevus (p = 0.012) and between melanoma and benign melanocytic nevus (p < 0.001) for the maturation index. Although pseudo-maturation was associated with low mitotic activity and thin Breslow thickness, there was no significant difference between survival distributions of 2 melanoma groups. CONCLUSION: Calculation of the maturation index can be used as a supporting tool for the differential diagnosis of challenging cases. However, it may possess limitation for evaluation of nevoid melanoma, melanoma in situ, or clonal nevus.

But due to its terrible appearance, it makes the parents and doctors uncomfortable, which leads to unnecessary laboratory tests and overtreatment. We aimed to identify the clinical course and prognosis of acute infantile hemorrhagic edema with our cases.

Amaç: Deri prick testi (DPT) başlıca atopik dermatit (AD), kronik ürtiker (KÜ), alerjik astma (AA), ve alerjik rinit (AR) gibi hastalıkların tanı ve takibinde kullanılmaktadır. Bu çalışmanın amacı, Dermatoloji Kliniği Alerji Ünitesi (DKAÜ)'nde yapılan DPT sonuçlarının geriye dönük olarak incelenerek, endikasyonları ve pozitiflik oranlarının araştırılmasıdır.Yöntem: DKAÜ arşivi kullanılarak 2014-2016 yıllarında yapılan DPT sonuçları incelenmiştir. DPT sonuçları, her hasta için ayrıca arşivlenen DPT formları okunarak yapılmıştır.Bulgular: DKAÜ'nde 2014-2016 yılları arasında 1916 hastaya DPT yapılmıştır. Bu hastaların 941'inde AA, 133'ünde AR, 842'sinde dermatolojik hastalık olduğu görülmüştür. En az bir ve birden fazla alerjen madde ile DPT pozitiflik oranı, sırasıyla AA, AR ve dermatolojik hastalıklarda; %92.1, %71.4 ve %50 olarak saptanmıştır. Dermatolojik hastalıklar incelendiğinde, 69 kronik ürtiker (KÜ), 55 atopik dermatit (AD) hastası haricinde, geriye kalan 718 hastada, başlıca dermatit, idiopatik generalize pruritus (İGP) olmak üzere farklı dermatolojik hastalıkların olduğu görülmüştür. DPT pozitifliği KÜ’de %55.1, AD’te %52.7 ve diğer dermatolojik hastalıklarda %48.6 olarak saptanmıştır.Sonuç: Bu çalışmanın sonuçlarına göre DPT pozitiflik oranı, AR ve AA hastalarında hem KÜ ve AD hem de diğer dermatolojik hastalıklara göre daha yüksektir. Bunun nedeni, KÜ ve AD etyopatogenezinde tip 1 aşırı duyarlılık reaksiyonlarının rolünün daha az olması ve/veya farklı dermatolojik tanılarla yönlendirilen hastalarda doğru olmayan DPT endikasyonları olabilir.

Mikozis fungoides (MF) en sık görülen kutane T hücreli lenfoma (KTHL) formudur. Hastalık yavaş seyirliden agresif hızlı seyirli formlara kadar değişken klinik seyri vardır. MF tedavisinde birçok alternatif mevcuttur ve deriyi hedefleyen tedavilerden kemoterapiye kadar değişen farklı yöntemler tedavide kullanılır. Retinoidler hücre proliferasyonu, diferansiyasyonu ve apoptozisi düzenleyen ve immün yanıtı etkileyen A vitamini türevleridir. Retinoidler MF tedavisinde uzun yıllardır kullanılmaktadır ancak etki mekanizması belirsizliğini korumaktadır. Retinoidler uygulama kolaylığı ve nispeten düşük toksisite profili nedeniyle MF’in tüm evrelerinde tedavi seçeneklerinin önemli bir bileşenidir. Beksaroten KTHL tedavisinde Gıda ve İlaç İdaresi (FDA) tarafından onaylanan ilk retinoiddir. MF tedavisinde retinoidlerin diğer ajanlarla kombinasyonu iyi tolere edilir ve nükseden, dirençli hastalarda daha yüksek tedavi cevabı ile ilişkilidir. Bu derlemede, MF hastalarında retinoidlerin kullanımı ile ilgili litetarür bilgileri gözden geçirlmiştir.

Literatür bilgilerine dayalı olarak Türkiye’de psoriasise özel HKS olmadığı bilinmektedir. Burada psoriasise özel olarak ilk kez kullanılan HKS’nin sunulması amaçlanmıştır. Yöntemler: Psoriasis Takip Sistemi (PSORTAKSİS), Kayseri Sağlık Uygulama ve Araştırma Merkezi (KSUAM) Dermatoloji Kliniği tarafından hazırlanmış ve KSUAM Bilgi İşlem Merkezi tarafından hastane otomasyon sistemine entegre edilmiştir. PSORTAKSİS, 01 Ekim 2016 tarihinden itibaren KSUAM Dermatoloji Kliniği Kronik Deri Hastalıkları Polikliniği’nde, hastalık şiddeti ya da tedavi farklılıkları gözetilmeksizin tüm psoriasis hastalarının takibinde kullanılmaktadır. Bulgular: PSORTAKSİS hastaların demografik verileri, klinik bulguları, laboratuvar çıktıları ve tedavi bilgilerinin kaydedilmesini sağlayan ana ve alt sekmelerinden oluşmaktadır. Sonuç: HKS, tedavi seçenekleri oldukça çok olan ve ayrıntılı takip gerektiren psoriasis için önemlidir. İlaçların güvenli ve etkin kullanılmasının yanı sıra HKS epidemiyolojik ve klinik çalışmalar için bilgiler sağlar. KSUAM Dermatoloji Kliniği’nde bir yıla yakın zamandır PSORTAKSİS kullanılmaktadır. Türkiye’de ilk uygulama olan PSORTAKSİS’in geliştirilmesi ve ülke çapında yaygınlaştırılması önemli ve gerekli bir katkı sağlayacaktır. Objective: In many countries, information about patients receiving systemic and biologic agent treatment has been included in patient registration systems (PRS) to enable effective follow-up of medications. Based on literature info, it is known there is not PRS a specific to psoriasis in Turkey. In this study, it was aimed to represent the first used PRS, specifically for psoriasis. Methods: Psoriasis Follow-up System (PSORTAKSIS, abbreviation of "Psoriasis Takip Sistemi" in Turkish) was designed by Kayseri Health Training Research Center (KHTRC) Dermatology Clinic and integrated in to the hospital automation system by KHTRC Data Processing Center. PSORTAKSIS is used for follow-up of patients with psoriasis regardless of disease severity and treatment differences at Chronic Skin Disease Polyclinic of KHTRC Dermatology Clinic since 01.10.2016. Results: PSORTAKSIS consists of main and sub tabs which provide recording of patients' demographic data, clinical findings, laboratory output, and treatment informations. Conclusion: PRS is important for psoriasis which has a lot of treatment options and requires detailed follow-up. Besides the safe and effective use of drugs, PRS provides informations for epidemiological and clinical studies. PSORTAKSIS has been used in KHTRC Dermatology Clinic nearly a year. The development and nationwide dissemination of PSORTAKSIS, as a first application in Turkey, will provide a significant and necessary contribution.

PubMed ID: 29989537 Behçet disease (BD) is a systemic inflammatory disorder that affects the skin, mucosa, eyes, joints, blood vessels, brain, and gastrointestinal tract. The etiopathogenesis of BD has not yet been fully elucidated, but disorganized immune responses against the stimuli of environmental triggering factors have been considered to play a major role in the pathogenesis of the disease in individuals with genetic susceptibility. Human leukocyte antigen (HLA)-B*51 is known to be the main factor involved in genetic susceptibility to BD. Among the environmental factors, infectious agents in particular are thought to be important. Immunological abnormalities could thus be the cornerstone in the development of BD. Along with cytokines that play a role in disease pathogenesis, numerous other cytokines have been recently identified or have been the focus of recent studies. This contribution sheds light on the etiopathogenesis and immunology of BD in relation to the current literature. © 2018 Pulse Marketing & Communications, LLC.

Acne vulgaris is a chronic inflammatory disease that primarily affects the pilosebase unit. Acne vulgaris remains one of the commonest diseases to afflict humanity, with prevalence up to 90% during adolescence. Relatively little is known about its epidemiology despite the fact that acne is a universal condition in younger population. This disease is more common in developed countries than in less industrialized regions of the world. The etiopathogenesis of acne is multifactorial. Sebum production, follicular epithelial keratinization, bacterial proliferation, and inflammation play significant role in etiopathogenesis. Recent studies shown that toll like receptors and antimicrobial peptides play a role in the pathogenesis of acne vulgaris. The relationship between diet and acne is highly controversial. Strong evidence shows that high glycemic loads diet may exacerbate acne. In addition, the factors contributing to the formation of acne also include genetic predispositions, hormonal abnormalities, psychological, environmental and iatrogenic factors. The disease is mainly observed face, chest, back and shoulders. When acne is untreated, it may cause social isolation, difficulty in finding jobs, depression and suicide attempt as a result of emotional and physical scar formation. In this article, the epidemiology and etiopathogenesis of acne are overviewed., Akne vulgaris başlıca pilosebase üniteyi etkileyen kronik inflamatuvar bir hastalıktır. Akne vulgaris adolesanlarda %90'a varan prevalansıyla, insanlığı etkileyen en sık görülen hastalıklardan biridir. Hastalığın epidemiyolojisi ile ilgili bilinenler nispeten az olmasına karşın; akne genç popülasyonda görülen evrensel bir durumdur. Bu hastalık gelişmiş ülkelerde dünyanın daha az endüstrileşmiş bölgelerinden daha yaygındır. Aknenin etiyopatogenezi multifaktöriyeldir. Etiyopatogenezinde sebum üretimi, foliküler epitelyal keratinizasyon, bakteriyel proliferasyon ve inflamasyon önemli rol oynar. Güncel çalışmalar akne vulgaris etiyopatogenezinde toll benzeri reseptörler ve antimikrobiyal peptidlerin de rol oynadığını göstermektedir. Akne ve diyet arasındaki ilişki ise son derece tartışmalıdır. Yüksek glisemik indeksli gıdaların akne vulgarisi şiddetlendirdiğine yönelik güçlü kanıtlar mevcuttur. Ayrıca genetik yatkınlık, hormonal bozukluklar, psikolojik, çevresel ve iyatrojenik faktörler de akne gelişimine katkıda bulunur. Hastalık sıklıkla yüz, sırt, göğüs ve omuz bölgesini tutar. Tedavi edilmediği zaman emosyonel ve fiziksel bozukluklara neden olarak sosyal izolasyon, iş bulma güçlüğü, depresyon ve intihar girişimine yol açabilmektedir. Bu makalede akne vulgarisin epidemiyolojisi ve etiyopatogenezi gözden geçirilmiştir.

Dermatologic examination revealed an erythematous, painful, firm, fixed plaque-nodular lesion on the plantar aspect of the right foot (Figure 1A). Nothing of distinction was noted from his family history or his laboratory tests. An incisional biopsy revealed parallel, regular bundles composed of uniform, plump spindle cells. Thin collagen fibers were seen in contact with and located between the spindle cells. This mass was separated from the surrounding soft tissue by an irregular, unclear border (Figure 2A). Acne vulgaris is a chronic skin disease that develops as a result of inflammation of the pilosebaceous unit and its clinical course is accompanied by comedones, papules, pustules, and nodules. A different group of disease, which is clinically similar to acne vulgaris but with a different etiopathogenesis, is called “acneiform eruptions.” In clinical practice, acneiform eruptions are generally the answer of the question “What is it if it is not an acne?” Although there are many subgroups of acneiform eruptions, drugs are common cause of acneiform eruptions, and this clinical picture is called “drug-induced acneiform eruptions.” There are many drugs related to drug-induced acneiform eruptions. Discontinuation of the responsible drug is generally sufficient in treatment.

Pseudoxanthoma elasticum nadir görülen, deri, göz, kardiyovasküler sistem ve gastrointestinal sistemi etkileyebilen multisistem hastalığıdır. 18 yaşındaki kadın hasta boyun ve her iki antekubital bölgede cilt lezyonları şikayeti ile başvurdu. Fizik muayenesinde bu bölgelerde sarı papüler lezyonları saptandı. Boyun cildinden alınan punch biyopsi materyali pseudoxanthoma elasticum ile uyumlu olarak değerlendirildi. Pseudoxanthoma elasticumun prognozu ekstrakutanöz organ tutulumuna bağlı olduğu için hastalığın erken tanınması , sistemik komplikasyonların azaltılması ve hatta önlenmesi açısından önem kazanmaktadır.
