İnfertilite, her yedi çiftten birisini etkileyen bir sorun olup 1 yıl boyunca korunmasız cinsel ilişkiye rağmen gebelik elde edilememesi olarak tanımlanır. İnfertil hastaların en önemli düşmanlarından birisi zamandır. İnfertilitenin olası nedenlerini ortaya çıkarmak için yapılacak değerlendirmenin sistematik, hızlı ve maliyet-etkin olmasına önem gösterilmeli ve değerlendirmeye en az invaziv olan tetkiklerle başlanmalıdır. İnfertil bir kadının değerlendirilmesinde, infertilitenin anatomik ve fizyolojik nedenlerini ortaya çıkarmak için ayrıntılı bir tıbbi öykü, üreme öyküsü, aile öyküsü ve fizik muayene gereklidir. Bu değerlendirme süreci aynı zamanda gebelik öncesi danışmanlık verilmesi, koruyucu hekimlik uygulamaları için gerekli danışmanlığın verilmesi, gerektiğinde genetik araştırma ve konsültasyonların yapılması için de en uygun dönemdir. İnfertilitenin değerlendirilmesinde ovülasyonun varlığı, kadın genital sisteminin yapısı ve işlevi ve aynı zamanda erkek partnerde semen analizi de değerlendirilmelidir. Tüm bu değerlendirme süreçleri boyunca çiftlerde çocuk sahibi olamamanın neden olduğu psikolojik morbidite de göz önüne alınmalı ve aynı zamanda çiftler bu açıdan da desteklenmelidir. Bu derlemede, infertil bir kadının değerlendirilmesinde izlenecek olan adımların güncel literatür bilgileri ışığında sunulması amaçlanmıştır., Infertility is a problem that affects one out of seven couples and is defined as the inability to achieve pregnancy despite unprotected intercourse for 1 year. One of the most important enemies of infertile patients is time. In order to reveal the possible causes of infertility, importance should be given to the systematic, rapid, and cost-effective evaluation, and the evaluation should be started with the least invasive examinations. In the evaluation of an infertile woman, a detailed medical history, reproductive history, family history, and physical examination are required to reveal the anatomical and physiological causes of infertility. This evaluation process is also the most suitable period for giving pre-pregnancy counseling, providing necessary counseling for preventive medicine practices, and conducting genetic research and consultations if necessary. In the evaluation of infertility, the presence of ovulation, the structure and function of the female genital system, and semen analysis in the male partner should also be evaluated. During all these evaluation processes, the psychological morbidity caused by the inability to have children should also be taken into consideration and the couples should also be supported in this respect. In this review, it was aimed to present the steps to be followed in the evaluation of an infertile woman in the light of current literature.

Boko Haram Nijerya ve komşuları Nijer, Çad ve Kamerun'da eylemler yapan bölgesel bir terör örgütüdür. Eylemleri sonucunda Nijerya ve komşularında etkiler oluşturmaktadır. Bu çalışmanın amacı Boko Haram terörünün bölgede oluşturduğu etkileri ortaya koymaktır. Çalışma meydana getirilirken kitaplardan, dergilerden, gazetelerden, raporlardan, resmi basın bildirilerinden ve internet kaynaklarından yararlanıldı. Tezde Boko Haram terör örgütünün tarihsel gelişimi ortaya konmuştur. Boko Haram'ın bölge ülkelerinde güvenlik ve terör sorunu dışında başka sorunlar yaratıp yaratmadığı, ilk lideri Muhammed Yusuf zamanında silahlı eyleme başvurup vurmadıkları, devletin etkisi sonucunda terör eylemlerine başlayıp başlamadıkları, liderleri Yusuf ölmesinden sonra radikalleşip terör örgütü haline gelip gelmedikleri, şu anki liderleri Şekau'nun radikalleşmelerine etkileri sorularına cevap bulunmaya çalışılmıştır.Boko Haram başlangıçta Nijerya'da sosyo-ekonomik ve etnik eşitsizlikler sonucunda ortaya çıkmıştır. 2009 yılına kadar birkaç küçük eylem dışında teröre bulaşmayan örgüt liderlerin öldürülmesi sonucunda şiddete başvurmaya başlamıştır. Devletin ülkede gerçek şeriatı ilan etmesini istemektedir. Bunu gerçekleştirmek için terör eylemlerine hız vermiştir. 2014 yılından sonra bölge ülkeleri Kamerun, Nijer ve Çad'da eylemler gerçekleştirmeye başlamıştır. Eylemlerin etkisi ile bölge ülkeleri Boko Haram ile mücadeleye grişmişlerdir. Bölge ülkelerinde eylemleri sonucunda bölgenin güvenliği sorunu dışında ayrıca yiyecek kıtlığı, açlık sorunu, düzensiz göç, yatırımların azalması ve bunun sonucunda iş imkânlarının azalması, bölgesel ticaretin azalması gibi sosyo ekonomik etkiler doğurmuştur. PSO with the ease at application and speed of finding a solution comes to the forefront among the other optimization techniques. In this study, the optimal solution of Unequal Preparation Time Single Machine Total Weighted Delay using Particle Swarm Optimization (PSO) Algorithm is explained. The results show that the total weighted delay will be reduced by using the PSO algorithm.

Bu çalışmada hastanemizde erişkin hastalarda Gram negatif bakteriyemi etkenleri ve bakteriyemi gelişimi açısından hastalarda ki risk faktörleri ve mortaliteyi etkileyen faktörlerin değerlendirilmesi amaçlanmıştır. Bu çalışmaya 1 Ocak 2012- 31 Aralık 2013 tarihleri arasında Dr. Lütfi Kırdar Kartal Eğitim ve Araştırma Hastanesi Mikrobiyoloji laboratuvarında kan kültüründe Gram negatif bakteri izole edilen 229 erişkin hasta dahil edilmiştir. Hemokültür örnekleri Bact-Alert 3D otomatize kan kültür sistemlerinde değerlendirilmiştir. Hastanemiz mikrobiyoloji laboratuvarı kayıtları, bilgi işlem sistemleri kullanılarak ve ilgili birimlerdeki hasta dosyaları taranarak hastalara ait verilere ulaşılmıştır. Her hasta için Pitt bakteriyemi skoru hesaplanmıştır. Gram negatif bakteriyemide en sık etken A.baumannii, olarak saptanmıştır. A.baumannii ve P.aeruginosa YBÜ’nde hakim olan bakteriler iken, E.coli ve K.pneumoniae’nın YBÜ dışı kliniklerde ki en sık etkenler olduğu bulunmuştur. Hastaların 123’ü sağ, 91’ide mortalite ile sonlanmış ve mortalite ilişkili risk faktörleri tek değişkenli analizle değerlendirildiğinde ileri yaş, yoğun bakım ünitesinde yatış, malignensi, konjestif kalp yetmezliği, diabetes mellitus, idrar sondası, nazogastrik sonda, mekanik ventilasyon, santral venöz kateter, total parenteral beslenme, enteral beslenme, kemoterapi, önceden antibiyotik kullanımı ve Pitt bakteriyemi skorunun 4’den büyük olması (p, In this study, we aimed to find agents of Gram negative bacteremia in our hospital and to evaluate risk factors for bacteremia occurence and death related factors. 229 adult patients who had Gram negative bacteria growth in blood cultures in the microbiology laboratory of r Lutfi Kırdar Research and Aducation hospital, were included in this study. Blood cultures were done by using Bact-Alert 3D automated blood culture system. Patients data were collected by a reviewing charts of patinets and records of microbiology laboratory. A Pitt score was calculated for each patient. It was found that the most common cause of Gram negative bacteremia was A.baumannii. A.baumannii and P.aeruginosa were predominant bacteria of intensive care unit whereas E.coli and K.pneumoniae were predomiant bacteria of clinics other than intensive care unit. Fatal outcome ocurrured in 91 patients while 123 patients survived. Introduction. Treatment for bilateral vocal fold paralysis (BVFP) has evolved from external irreversible procedures to endolaryngeal laser surgery with greater focus on anatomic and functional preservation. Since the introduction of endolaryngeal laser arytenoidectomy, certain modifications have been described, such as partial resection procedures and mucosa sparing techniques as opposed to total arytenoidectomy. Discussion. The primary outcome measure in studies on BVFP treatment using total or partial arytenoidectomy is avoidance of tracheotomy or decannulation and reported success ranges between 90 and 100% in this regard. Phonation is invariably affected and arytenoidectomy worsens both aerodynamic and acoustic vocal properties. Recent reports indicate that partial and total arytenoidectomies have similar outcome in respect to phonation and swallowing. We use CO2 laser assisted partial arytenoidectomy with a posteromedially based mucosal flap for primary cases and reserve total arytenoidectomy for revision. Lateral suturing of preserved mucosa provides tension on the vocal fold leading to better voice and leaves no raw surgical field to unpredictable scarring or granulation. Conclusion. Arytenoidectomy as a permanent static procedure remains a traditional yet sound choice in the treatment of BVFP. Laser dissection provides a precise dissection in a narrow surgical field and the possibility to perform partial arytenoidectomy.

A case of secondary infertility related to prolonged retention of fetal bones in the cervix is presented. A 34-year-old nulliparous woman was admitted to the hospital with chronic pelvic pain, dyspareunia, and 12-year-long secondary infertility following an induced abortion due to fetal demise. Transvaginal ultrasonography (USG) revealed a linear echogenic area around the posterior cervical wall. An hysteroscopic examination confirmed the existence of an irregular calcified [Pass embedded in the cervix. The mass turned out to be conglomerated fetal bone fragments which probably acted as an intrauterine device. After removal of the cervical mass, the patient conceived spontaneously within four months. The present case report emphasizes the significance of a detailed history and a thorough evaluation by transvaginal USG in the case of secondary infertility following an induced or spontaneous abortion.

Fakat elenmesi gereken pek çok çözüm alternatifi varsa simülasyon metodunun yanında kullanılması gereken başka metotlarda vardır. Bu metotlardan en çok kullanılanları başta yapay sinir ağları, sonra genetik algoritmalar, tavlama benzetimi, parçacık sürü optimizasyonu gibi arama algoritmalarıdır. Bu çalışmada benzetim çalışması sonucu elde edilen imalat sistemine ait performans ölçütleri kullanılarak Yapay Sinir Ağı (YSA) eğitilmiştir. Yani YSA ya istenilen performans ölçüleri girildiğinde, YSA her bir makine merkezindeki olması gereken makine sayısını vermektedir. Bunun yanında imalat sistemlerinin performansını etkileyen en önemli faktör sistemde kullanılan öncelik kurallarıdır. Bu sebeple simülasyon işlemini yaparken makine merkezlerindeki makine sayılarının değiştirilmesinin yanı sıra farklı öncelik kuralları da kullanılarak sistemin performans ölçütleri elde edilmiştir. Kullanılan öncelik kuralları SPT, EDD, CR, FCFS olmuştur. Her bir öncelik kuralı için farklı bir YSA eğitilmiştir. However, if there are some other solution alternatives to be eliminated, there should be some other solution methods to be used with simulation method. The most used methods for this purpose are mainly artificial neural networks (ANN), then genetic algorithms, simulated annealing, particle swarm optimization. In this study, an artificial neural network has been trained by using obtained performance criterion belonging production system as a result of simulation studies. In other words, when the performance criteria are entered to ANN, ANN gives the number of machines that should be in each machine center. Additionally, the most important factor affecting the per-formance of production systems is the priority rules used in the system. Therefore, in addition to changing the number of machines during the implementation of the simulation process in the machine centers performance criteria of the system have been obtained by using various priority rules. The used priority rules have been SPT, EDD, CR, FCFS. For each priority rule different neural network has been trained. To obtain the solution, re-quired performance criteria are given to these trained four neural networks. The performance criteria are obtained by re-simulation of the obtained results. The nearest performance values to the performance criteria are taken as solution. The simulated annealing approach is used to investigate whether there is a better solution than this tak-en solution. There is error possibility of the trained neural networks as %0.5. Therefore, simulated annealing ap-proach has been used to capture better solution missed if there is. %12 of the solutions among the obtained dif-ferent solutions have been found better than the solutions obtained by using ANN.

Transfüzyonla ilişkili Graft versus Host Hastalığı, genellikle hücresel immünitesi bozulmuş bireylerde histokompatibl olmayan lenfoid hücrelerin transfüzyonu sonrası gelişen, iyi bilinen fakat nadir bir komplikasyondur. İmmünitesi sağlam olan bireylerde de nadiren görülebilir. Ateş, cilt döküntüsü, diyare, karaciğer fonksiyon bozukluğu ve kemik iliği aplazisi bu hastalığın klinik tablosunu oluşturur.
