Objective: Fertility desire is one of the important factors influencing population growth and women's fertility-related health outcomes. This study aimed to assess women's fertility desire and factors associated with excess fertility desire in two populations with different socio-demographic and cultural characteristics. Methods: A hospital-based cross-sectional study was conducted at a tertiary hospital in Karabuk and a health center in N'Djamena. The sample size was calculated as 611 women (311 for Karabuk, 300 for N'Djamena). The study's dependent variable is the desire to have children, which was determined by asking women about their ideal number of children. Independent variables included women's sociodemographic and reproductive characteristics, and some perceptions about women's status. The chi-square test was used to compare the sociodemographic and reproductive characteristics of women. Crude and adjusted odds ratios (ORs) and corresponding 95% confidence intervals were calculated via binary logistic regression analysis to explore the relationship between dependent and independent variables. Results: A total of 615 women were reached. The average ideal number of children for women was 2.7 in Karabuk and 5.6 in N'Djamena. In the whole study group multivariable logistic regression analysis, unofficial marriages (OR = 12.1), the belief that a woman's fertility is determined by her husband or God (OR = 2.5), lower education level (primary school and below) (OR = 2.4) and the husband's increasing ideal number of children (OR = 2.3) were associated with women's excess fertility desire. Conclusion: The findings of the study reveal significant differences between the fertility patterns and preferences of women in the two populations and the influence of social structure on women's fertility behaviors. Policies aimed at empowering women and eliminating gender inequalities should be pursued to protect them from the adverse effects of excess fertility. [ABSTRACT FROM AUTHOR]

Introduction: This study aimed to estimate the prevalence of physical intimate partner violence among older women, identify factors associated with victimization, and gather information on the intergenerational transmission of violence. Materials and Method: This population-based cross-sectional study was conducted in Karabuk Province and included 399 ever-married women aged 65 years and older. The dependent variable was exposure to physical violence by a current or former spouse. Independent variables included women's sociodemographic and reproductive characteristics, their experience of violence in childhood, and some characteristics of their husbands and parents. The crude and adjusted prevalence ratios were estimated to explore the relationship between the dependent and independent variables using robust Poisson regression analysis. Results: The prevalence of physical intimate partner violence was 62.9% for the lifetime and 7.6% for the past year. Lifetime prevalence increased 1.2-fold with low household income, 1.4-fold with seven or more pregnancies, 1.3-fold with daily or weekly alcohol consumption by the husband, 1.3-fold with witnessing father-to-mother violence in childhood, and 1.5-fold with experiencing physical violence by parents in childhood. Women were more likely to use violence against their children if they had experienced violence in childhood and adulthood. Conclusion: This study's finding of high lifetime and past-year prevalence of exposure to intimate partner violence highlights the need for more efforts to address intimate partner violence among older women. More research is needed to better understand older women's experiences of intimate partner violence and identify health and social policy approaches to meet their support and assistance needs. Amaç: Bu çalışmanın amacı, Karabük’te bir özel hastanede görev yapan sağlık çalışanlarının üretkenlik düzeylerinin ve üretkenlik düzeyi ile ilişkili faktörlerin belirlenmesidir.Gereç ve Yöntem: Kesitsel araştırmanın çalışma grubunu, hastanede sağlık ve yardımcı sağlık hizmetleri sınıflarında görev yapan toplam 180 sağlık çalışanının 164’ü (%91.11) oluşturdu. Çalışmanın bağımlı değişkeni, sağlık çalışanlarının üretkenlik düzeyidir. Bağımsız değişkenler ise çalışma koşulları ve özlük hakları ile ilgili özellikler, iş stresi, iş güvencesizliği ve örgütsel bağlılık düzeyleridir. Veri toplama aracı olarak 1) tanımlayıcı soru formu, 2) Endicott İşte Üretkenlik Ölçeği, 3) İş Stresi Ölçeği, 4) İş Güvencesizliği Ölçeği ve 5) Örgütsel Bağlılık Ölçeği kullanıldı. Verilerin analizinde ki-kare, Pearson korelasyon ve Student’s t testleri kullanıldı. Bulgular: Sağlık çalışanlarının üretkenlik puanı ortalaması 21.41±13.90 idi. Üretkenlik düzeyi 30 yaşın üstünde (%61.11), erkeklerde (%67.35) ve hekimlerde (%74.07) yüksekti. Özlük haklarının kullanımı ve çalışma koşulları ile ilgili pek çok değişken, üretkenlik düzeyi ile ilişkili idi (p, Aim: The aim of this study is to determine the productivity levels and related factors of healthcare workers working in a private hospital in Karabuk.Materials and Methods: The study group of the cross-sectional study consisted of 164 (91.11%) of 180 healthcare workers working in the health and auxiliary health services classes in the hospital. The dependent variable is the productivity level of healthcare workers. The independent variables are the characteristics of working conditions and personal rights, the levels of job stress, job insecurity and organizational commitment. Aim: The utilization and scope of postnatal care (PNC) services play a fundamental role to improve maternal and infant health, decrease mortality and morbidity. The aim of this study is to assess the factors affecting the utilization and scope of PNC services in Safranbolu district of Karabuk province.Material and Methods: In this cross-sectional study, 169 mothers who completed the 42-day postpartum period are included. In order to reach women, family physicians' records were used. The data were collected through a questionnaire based on the Ministry of Health's Postnatal Care Management Guide. In the questionnaire, variables related to sociodemographic characteristics, health status, reproductive health and variables related to number, quality and place of pregnant follow ups were questioned. Chi-square test was used for analyses.Results: The level of women followed up at the hospital after delivery was 99.4%. 13% of the women after being discharged from the hospital have not received PNC. The prevalence of women with completed postnatal follow-up was 26%. Applications the women after being discharged from the hospital were made in order to get PNC services in 42.2% private hospitals, 41.1% family health centers and 16.8% public hospitals respectively. No women were visited at home during the postpartum period.Conclusions: Study showed that the utilization of PNC services is low and while the role of primary health care for PNC is decreasing, the role of the private sector is increasing. The fact that PNC services are community-based and comprehensive, is a fundamental requirement for the improvement of maternal and infant health., Amaç: Anne ve bebek sağlığının geliştirilmesi, mortalite ve morbiditenin azaltılmasında doğum sonu bakım (DSB) hizmet kullanımı ve kapsamı önemli rol oynamaktadır. Bu çalışmanın amacı, Karabük ili Safranbolu ilçesinde DSB hizmetlerinin hizmet kullanımını ve kapsamını etkileyen faktörleri değerlendirmektir.Gereç ve Yöntem: Kesitsel tipte planlanan bu çalışmada doğum sonu 42 günlük sürecini yeni tamamlayan 169 anne yer almaktadır. Kadınlara ulaşmak için aile hekimlerinin kayıtlarından yararlanılmıştır. Veriler, Sağlık Bakanlığı‟nın Doğum Sonu Bakım Yönetim Rehberi temel alınarak hazırlanan bir soru formu aracılığı ile toplanmıştır. Anket formunda sosyodemografik özellikler, sağlık durumu, üreme sağlığıyla ilgili değişkenler, gebelik izlemlerinin sayısı ve niteliğiyle ilgili değişkenler sorgulanmıştır. Analizlerde ki-kare testi kullanılmıştır.Bulgular: Doğum sonu hastanede izlenen kadınların düzeyi %99,4‟tür. Hastaneden taburcu olduktan sonra kadınların %13‟ü hiç izlenmemiştir. Tam izlem sıklığı %26‟dır. Doğum sonu hastaneden çıktıktan sonra DSB hizmeti almak için yapılan başvurular sırasıyla %42,2 özel hastaneye, %41,1 aile sağlığı merkezine, %16,8 kamu hastanesine yapılmıştır. Doğum sonrası dönemde hiçbir kadın evde ziyaret edilmemiştir.Sonuç: Çalışma DSB hizmet kullanımının düşük olduğunu ve DSB hizmetlerinde birinci basamağın rolü azalırken özel sektörün rolünün arttığını göstermektedir. DSB hizmetlerinin toplum tabanlı ve kapsayıcı olması anne ve bebek sağlığının geliştirilmesinin temel koşuludur

Objective: To evaluate the access of married women of reproductive age to family planning (FP)services and associated factors in Karabuk. Method: This cross-sectional study was conducted inKarabuk in 2015-2016. The population of the study was married women living in Karabuk. Thesample size was calculated by the Epi-Info-7 StatCalc software as 284, within a universe of34,207 married women of 15-49 age group, with a confidence interval of 95%, a margin of errorof 5% and a prevalence of 75.2% (prevalence of women in the Western Black Sea Region ofTurkey using any contraceptive method - 2013 Demographic and Health Survey of Turkey), andit was planned to reach 300 women. The sample was distributed proportionally by thepopulations of residential areas. Data was collected through face-to-face interviews. In theanalysis of data, Chi-Square Test and binary logistic regression were applied. Results: Of all thewomen, 80.3% were found to use some form of FP method. More specifically, 48.6% were foundto use modern methods, while 31.7% were found to use traditional methods. The three mostcommon methods were coitus-interruptus (31.0%), condoms (18.3%) and tubal-ligation(16.0%). The unmet need for FP was found to be at 9.7%. Conclusions: This study shows thattraditional FP methods are widely used to prevent pregnancy and that the FP needs are not metfor one in every ten women in Karabuk. To remove the barriers in accessing FP services,comprehensive primary care which focuses on the needs of the community is needed., Amaç: Karabük ilinde doğurganlık çağındaki evli kadınların aile planlaması (AP) hizmetlerine erişimini ve etkileyen faktörleri değerlendirilmektir. Yöntem: Kesitsel tipte planlanan çalışma,2015-2016 yıllarında gerçekleştirilmiştir. Örnek büyüklüğü, Epi-Info-7 StatCalc Programı'nda 15-49 yaş evli kadınlardan oluşan evren 34207, güven aralığı %95, hata payı %5 ve prevalans %75.2 (Türkiye Nüfus ve Sağlık Araştırması 2013'e göre Batı Karadeniz Bölgesi'nde herhangi bir AP yöntemi kullanan kadınların sıklığı) değerleriyle yapılan hesaplamada 284 olarak saptanmış ve çalışmada 300 kadına ulaşılması planlanmıştır. Çalışma grubu, yerleşim yerlerinin nüfuslarına orantılı olarak dağıtılmıştır. Veriler yüz yüze görüşme yoluyla toplanmıştır. Verilerin analizde Ki-Kare testi ve binary lojistik regresyon analizi uygulanmıştır. Bulgular: Kadınların %48.6'sı modern ve %31.7'si geleneksel olmak üzere, %80.3'ü herhangi bir AP yöntemikullanmaktadır. En yaygın kullanılan üç yöntem geri çekme (%31.0), kondom (%18.3) ve tüp ligasyonudur (%16.0). Karşılanmayan AP gereksinimi düzeyi %9.7 olarak saptanmıştır. Sonuç:Çalışma, Karabük’te gebelikten korunmak için büyük ölçüde geleneksel AP yöntemlerin kullanıldığını ve her on kadından birisinin AP gereksiniminin karşılanmadığını göstermektedir. AP hizmetlerine erişiminin önündeki engellerin kaldırılması için toplumun gereksinimlerine odaklanan yaygın birinci basamak sağlık hizmeti temel gerekliliktir.

Aim: Our purpose was to evaluate the decline in cancer incidence rates in the elderly in Turkey. Methods: Age specific cancer incidence rates for Izmir and Antalya in 1998-2002, which have been published in Cancer Incidence in Five Continents, IX were evaluated. Results: In both Izmir and Antalya, for both sexes, age specific incidence rates are decreasing with age significantly after the age of 64. Conclusion: Despite the fact that the incidences of most of the epithelial cancers increase with age, there are obvious decreases in cancer incidence rates in the oldest age groups in most sites in Turkey. Three main resons may be suggested for these decreases: 1, Underdiagnosis due to a lack of access to appropriate health services for the elderly; 2, A Cohort Effect and 3, Survival bias. The recent developments in cancer registration in Turkey are promising to provide more clear answers to these questions as well as to many others., Amaç: Çalışmanın amacı Türkiye’de yaşlı nüfusta gözlenen kanser insidans hızlarındaki düşüşü irdelemektir. Yöntem: Bu çalışmada Beş Kıtada Kanser Görülüşü, IX (Cancer Incidence in Five Continents, IX)’da yayımlanmış olan İzmir ve Antalya illerinde 1998-2002 yıllarında saptanan yaşa özel kanser insidans hızları değerlendirildi.Bulgular:İzmir ve Antalya illerinde tüm kanserler için ve hemen her kanser için 64 yaş üzerindeki gruplarda her iki cinsiyette de yaşa özel insidans hızlarında belirgin düşüş eğimi görülmektedir. Sonuç: Türkiye’de yaşlı grupların insidans hızlarında, pek çok kanser için belirgin düşüşler söz konusudur. Oysa gerçekte pek çok epitelyal kanserde insidans hızı yaşla birlikte artar. İzmir ve Antalya’da gelişmekte olan diğer ülkelerdekine benzer şekilde gözlenen bu örüntüyü açıklamak için üç varsayımdan sözedebiliriz: 1. Yaşlıların uygun sağlık hizmetlerine ulaşmalarındaki eksiklik (tanı konmasındaki azlık), 2. Kohort etkisi, 3. Sağkalım yanılgısı. Türkiye’de kanser kayıtçılığındaki gelişmeler bu soruyu ilerde daha açık yanıtlayabilecek olanakları sağlayacaktır.

Amaç: Bu çalışmada, uluslararası kuruluşlar tarafından önerilen sürveyans sistemlerini değerlendirme kriterlerine dayanarak Türkiye'de 2009 yılında uygulamaya konulan yeni ölüm bildirim sisteminin İzmir İli örneğinde değerlendirilmesi amaçlanmıştır.Yöntem: İki ölümden oluşan çalışmanın kesitsel tipte tasarlanan niceliksel bölümünde, ölüm bildirimlerinin veri kalitesi değerlendirilmiştir. Veri kalitesi, bildirimlerin tamlığı ve doğruluğu olarak tanımlanmıştır. Tamlık dört kriter (1. 2001-2010 yılları arasında beş yaş ve üzeri ölüm bildirimlerinin tamlık düzeyi, 2. 2000-2010 yılları arasında ölüm sayılarının ve kaba ölüm hızlarının tutarlılığı, 3. Mezarlıklar Müdürlüğü ve Sağlık Müdürlüğü kayıtlarının tutarlılığı, 4. Ölüm bildirimlerinin zamanındalığı) ve doğruluk üç kriter (1. Ölüm belgelerinin doldurulma düzeyi, 2. İyi tanımlanmayan temel ölüm nedenlerinin sıklığı, 3. Yaşa ve cinsiyete göre ölüm nedenlerinin tutarlılığı) çerçevesinde değerlendirilmiştir. Veri kalitesi, birinci tamlık kriteri ve ikinci doğruluk kriterine göre iyi, orta ve yetersiz olarak üç kategoride sınıflandırılmıştır.Niteliksel bölümde sistemin işleyiş sorunları belirlenmiş, İzmir'de ölümlerin belgelenme sürecinde görev alan yetkililerle derinlemesine görüşmeler yapılmıştır (n=19). Görüşmelerde kabul edilebilirlik, basitlik ve faydalılık kriterlerini sorgulayan 12 soruluk bir rehber kullanılmıştır. Yarım saat süren görüşmeler kişilerin çalıştıkları kuruluşlarda gerçekleştirilmiştir. Ses kayıtları her bir görüşmenin ardından çözümlenmiş, veriler içerik analizi uygulanarak değerlendirilmiştir.Bulgular: Tamlığa ilişkin değerlendirmelerde, 2001-2010 arasında İzmir'in kentsel alanlarında gerçekleşen beş yaş ve üzeri ölüm bildirimlerinin tamlık düzeyinin erkeklerde % 91.8, kadınlarda % 87.1 olduğu; 2000-2010 arasında ölüm sayılarının ve kaba ölüm hızlarının güvenilir olmadığı; Mezarlıklar Müdürlüğü'nün 2010 yılı kayıtlarındaki ölümlerin % 8.9'unun Sağlık Müdürlüğü'ne bildirilmediği; ölümlerin % 65.4'ünün öngörülen sürede bildirilmediği belirlenmiştir. Doğruluğa ilişkin değerlendirmelerde ise ölüm belgelerindeki bilgilerin eksiksiz doldurulmasına önem verilmediği; temel ölüm nedenlerinin % 29.7'sinin iyi tanımlanmadığı saptanmıştır. Tamlığı ve doğruluğu temsil eden iki özet kritere göre bildirimlerin veri kalitesi yetersiz olarak sınıflandırılmıştır.Niteliksel bölümde, ölümlerle ilgili mevzuatın günümüz koşullarını karşılamadığı, yeni sistemdeki düzenlemelerin mevzuatla uyumlu olmadığı, ölüm kayıtları ile ilgili yapılanmanın bütüncül ele alınmadığı, görev tanımlarının belirsizliğine bağlı olarak uygulama aşamasında sorunlar yaşandığı, yeni ölüm belgesinde ve geçiş aşamasında verilen eğitimlerde bazı eksiklikler olduğu ve verilerin herhangi bir alanda kullanılmadığı ortaya çıkmıştır.Sonuç: Yeni ölüm bildirim sistemi veri kalitesi, kabul edilebilirlik, basitlik ve faydalılık kriterleri açısından yetersiz bulunmuştur.Anahtar Kelimeler: Ölüm istatistikleri, yeni ölüm bildirim sistemi, sürveyans sistemlerini değerlendirme kriterleri Aim: The new death registration system which has been put into practice in Turkey since 2009 was aimed to be evaluated in the case of Izmir city based on surveillance systems evaluation criteria proposed by international organizations.Methods: The methodology used in the study was composed of quantitative and qualitative parts. Data quality of death registration was assessed in the quantitative part which was designed as a cross-sectional study. Data quality were defined as the completeness and accuracy of the death reportings. The completeness was evaluated within the scope of four criteria (1. Completeness of death reportings for five-year olds and over five years of age between 2001-2010, 2. Consistency of number of deaths and crude death rates between 2000-2010, 3. Registration consistency of the Directorate of Cemeteries and the Provincial Health Directorate of Izmir, 4. Timeliness of death registration) and accuracy (1. Death certificate filling level, 2. Frequency of ill-defined underlying cause of death, 3. Consistency of causes for death dependent on age and sex). Data quality was classifed into three categories as good, moderate and poor based upon initial completeness criterion and collateral accuracy criterion.Operation problems were identified in the qualitative part and in-depth interviews were carried out with authorities taking charge in certification process of deaths in Izmir (n=19). A guideline involving 12 questions was used in the interviews examining acceptibility, simplicity and usefullness criteria. The interviews lasting half an hour were performed at the institutions in which individuals work. Audio recordings were analysed right after each interview and data were evaluated by implementing content analysis.Results: It was found out that the completeness of death registration for five-year olds and over executed in the urban parts of Izmir between 2001-2010 were 91.8% in males and 87.1% for females; mortality and crude death rate between 2000-2010 were not reliable; 8.9% of mortality rate involved in the registrations of the Directorate of Cemeteries was not notified to the Provincial Healt Directorate of Izmir; 65.4% of deaths was not declared in the anticipated period. When accuracy was evaluted it was determined that gap-filling parts in the death certificates was not considered important and 29.7% of the underlying cause of death was ill-defined. The data quality of death registration in view of two brief criteria representing completeness and accuracy was categorized as poor.It came to light in qualitative part that regulations regarding mortality did not meet current conditions, adjustments in the new system were not compatible regulations, current registration systems which vital and civil in Turkey were not dealt integrally by decision-makers, problems were experienced during application level due to indefinite job definitions, some shortcomings were identified in the new death certificate and trainings given in the transitional stage and data were not used in any field.Conclusion: The new death registration system was found insufficient in terms of data quality, acceptibility, simplicity and usefulness criteria.Keywords: Death statistics, new death registration system, surveillance systems evaluation criteria. 306

25. Reliable mortality statistics for Turkey: are we there yet?

BACKGROUND: The Turkish government has implemented several reforms to improve the Turkish Statistical Institute Death Reporting System (TURKSTAT-DRS) since 2009. However, there has been no assessment to evaluate the impact of these reforms on causes of death statistics. This study attempted to analyse the impact of these reforms on the TURKSTAT-DRS for Turkey, and in the case of Izmir, one of the most developed provinces in Turkey. METHODS: The evaluation framework comprised three main components each with specific criteria. Firstly, data from TURKSTAT for Turkey and Izmir for the periods 2001–2008 and 2009–2013 were assessed in terms of the following dimensions that represent quality of mortality statistics (a. completeness of death registration, b. trends in proportions of deaths with ill-defined causes). Secondly, the quality of information recorded on individual death certificates from Izmir in 2010 was analysed for a. missing information, b. timeliness of death notifications and c. characteristics of deaths with ill-defined causes. Finally, TURKSTAT data were analysed to estimate life tables and summary mortality indicators for Turkey and Izmir, as well as the leading causes-of-death in Turkey in 2013. RESULTSL Registration of adult deaths in Izmir as well as at the national level for Turkey has considerably improved since the introduction of reforms in 2009, along with marked decline in the proportions of deaths assigned ill-defined causes. Death certificates from Izmir indicated significant gaps in recorded information for demographic as well as epidemiological variables, particularly for infant deaths, and in the detailed recording of causes of death. Life expectancy at birth estimated from local data is 3–4 years higher than similar estimates for Turkey from international studies, and this requires further investigation and confirmation. The study was carried out among all pregnant who gavebirth between 01.01.2003 and 31.12.2003 by examining the antenatal postnatal follow-upforms.5.9 % of the pregnant women were below 20 years of age and 8.4 % were over 34years of age. The majority of women were graduated from primary school (52.2 %) and 2.1% were illiterade. The arithmetical mean of married age was found to be 20.83±3.48 and thearithmetical mean of total pregnancy number was found to be 2.11±1.26 and thearithmetical mean of abortion was found to be 0.30±0.65. It was found that 53.2 % of themdidn?t use any family planning method before their pregnancies. The percentage ofchildbirds less than two year period was 24.4 %. When the women?s age increased, theirpregnancies increased.The prevalence of anemia has been found as 25.5 % accorting to the hemoglobinlevels. The percentage of low birt weight babies was 6.1 %.The pregnants? first detection pregnancy week arithmetical mean was 15.89±7.02 andthe arithmetical mean of visits was 5.04±2.18. The percentage of the pregnancies detected atfirst 12th week was 22.9 %. The vaccination against tetanus was adequate in 80.1 % of allpregnant.There is doctor?s signature at 58.1 % of the forms. The midwives cheking thewomen?s weight, blood pressure, the existence of eudema, the existence of varicose, theamount of protein in the urine, hemoglobine values and total visit number arithmetical meanswere statistically meaningfull high on the forms which had doctor?s signature.It has been detected that 0.9 % of the births were carried out by the help of doctors,8.2 % by the midwives, 89.8 % in the hospital and 1.0 % without skilled help duringchildbirth. The percentage of cesarean deliveries was 37.8 %. The newly given birthwomen?s first detection day aricmetical mean was 8.98±9.08 and the aricmetical mean of thevisits made by the midwives was 2.34±0.91.It has been found that 68.7 % of women had antenatal postnatal follow-up forms. Kesitsel araştırmada, Karabük ilinde özel bir hastanede görev yapan sağlık çalışanlarının üretkenlik düzeylerinin ve bu düzeyi etkileyen faktörlerin belirlenmesi amaçlandı. Araştırma grubunu, Karabük Özel MEDİKAR Hastanesi'nde sağlık ve yardımcı sağlık hizmetleri sınıflarında görev yapan toplam 180 sağlık çalışanının 164'ü (%91,1) oluşturdu. Araştırmanın bağımlı değişkeni, sağlık çalışanlarının üretkenlik düzeyidir. Bağımsız değişkenler ise çalışma koşulları ve özlük hakları ile ilgili özellikler, iş stresi, iş güvencesizliği ve örgütsel bağlılık düzeyleridir. Veri toplama aracı olarak 1) tanımlayıcı soru formu, 2) Endicott İşte Üretkenlik Ölçeği, 3) İş Stresi Ölçeği, 4) İş Güvencesizliği Ölçeği ve 5) Örgütsel Bağlılık Ölçeği kullanıldı. Veriler, 02.10.2015-01.02.2016 tarihleri arasında yüz yüze görüşme yoluyla toplandı. Veriler, ki-kare ve pearson korelasyon testleri ile analiz edildi. Araştırmanın uygulanabilmesi için Karabük Üniversitesi'nden etik kurul onayı alındı. Sağlık çalışanlarının yaş ortalaması 28.3±8.5'dir. Araştırma grubunun büyük bölümü kadındır (%69,5). Çalışanların %39,0'ının öğrenim düzeyi lisans ve lisansüstüdür ve %47,0'ı ebe ve hemşiredir Araştırma grubunun üretkenlik puanı ortalaması 21,4±13,9 olup, yaklaşık yarısının (%49,7) üretkenlik puanı ortanca değerden yüksek idi. Üretkenlik düzeyi 30 yaşın üstünde (%61,1), erkeklerde (%67,3) ve hekimlerde (%74,1) yüksekti. İş stresi puanı yükseldikçe üretkenlik düzeyi düşüyordu (r= 0.702 p= 0.000). İş güvencesizliği algısı ve örgütsel bağlılık puanı yükseldikçe üretkenlik artıyordu (r= -0.398 p= 0.000, r= -0.341 p= 0.000). Araştırma, özel sağlık çalışanlarının önemli ölçüde psikososyal risk faktörüne maruz kaldığını göstermektedir. Sağlık çalışanlarının çalışma koşullarının ve özlük haklarının iyileştirilmesine gereksinim vardır. In this cross-sectional study, it was aimed to determine the productivity levels of health workers who work in a private hospital in Karabuk and the factors affecting this level. The study group consisted of 164 (91.1%) out of 180 health workers working in the health and auxiliary health classes at Karabuk Private MEDIKAR Hospital. The dependent variable of the study was the productivity level of health workers. The independent variables were working conditions and personal rights, job stress, job insecurity and organizational commitment levels. Data collected through five forms: 1) descriptive questionnaire form, 2) Endicott Work Productivity Scale, 3) Work Stress Scale, 4) Work Insecurity Scale and 5) Organizational Commitment Scale. Data were collected through face-to-face interviews between 02.10.2015 and 01.02.2016 and analyzed by chi-square and pearson correlation tests. Ethical approval was obtained from Karabuk University. The average age of health workers was 28.3±8.5. The majority of the study group was female (69,5%). 39,0% of the workers had undergraduate and graduate degrees and 47,0% was midwives and nurses. The mean of productivity score of the study group was 21.4±13.9. Nearly half of the workers (49,7%) had a higher productivity score than the median value. The productivity level was high in persons above 30 years of age (61,1%), males (67,3%) and physicians (74,1%). The productivity scores of workers decreased as the work stress scores of them increased (r= 0.702 p= 0.000). The productivity scores of workers increased as job insecurity perception and organizational commitment score of them increased (r= -0.398 p= 0.000, r= -0.341 p= 0.000 respectively). The study shows that private health sector workers are exposed to a significant psychosocial risk factor. There is a need to improve the working conditions and personal rights of health workers. 77

Bu çalışmanın amacı, Samsun'un Çarşamba ilçesinde Gökçeçakmak Aile Sağlığı Merkezi'ne (ASM) kayıtlı 15-49 yaş grubu kadınlar arasında vajinal enfeksiyon semptomlarının yaygınlığının saptanması, bakteriyel vajinozis ve kandida ile uyumlu vakaların tanımlanması ve vajinal enfeksiyn ile ilişkili etmenlerin belirlenmesidir. Kesitsel araştırmanın çalışma grubunu, Gökçeçakmak ASM'ye kayıtlı 15-49 yaş grubu toplam 313 evli kadının 163'ü oluşturdu. Veri toplama aşamasında ilk olarak, kadınlara bir soru formu uygulandı ve jinekolojik muayeneyi kabul edip etmedikleri soruldu. Vajinal enfeksiyon, muayeneyi kabul etmeyen kadınlarda (n=49) mevcut yakınmalarına göre; jinekolojik muayeneyi kabul eden kadınlarda (n=114) ise 1) inspeksiyon, 2) palpasyon, 3) bimanuel muayene, 4) spekulum muayenesi, 5) vajinal pH ölçümü, 6) whiff testi, 7) mikroskobik inceleme işlemlerine dayanarak belirlendi. Bu araştırmada, Karabük Üniversitesi'nde lisans düzeyinde öğrenim gören öğrencilerinin toplumsal cinsiyet eşitsizliğine yönelik tutumlarının ve bu tutumlarının belirleyicilerinin değerlendirilmesi amaçlanmıştır. Kesitsel tipte planlanan araştırmanın evrenini, 2014-2015 öğretim yılında Karabük Üniversitesi'nde lisans düzeyinde eğitim gören 29157 öğrenci oluşturmuştur. Örnek büyüklüğü, Epi-İnfo-7 StatCalc Programı'nda Güven Aralığı % 99, prevalans % 50 ve hata payı % 0.5 değerleri kullanılarak 649 olarak hesaplanmıştır. Örnek grup cinsiyete, fakülte/yüksekokullara ve sınıflara orantılı olarak dağıtıldıktan sonra her fakülteyi ve yüksekokulu temsilen birer bölüm seçilmiştir. Soru formları, dersliklerde hazır bulunan öğrencilerle yüz yüze görüşme yoluyla uygulanmıştır. Araştırma grubunu 323 kadın, 208 erkek olmak üzere 830 öğrenci oluşturmuştur. Tanımlayıcı istatistikler, yüzde dağılım ve ortalama (X̄) ± standart sapma (SS) biçiminde özetlenmiştir. Verilerin analizinde Bağımsız İki Grup Ortalaması, ANOVA, Kruskal Wallis ve Pearson Korelasyon testleri uygulanmıştır. Yaklaşık üçte ikisi (% 61.1) erkeklerden oluşan araştırma grubunun toplumsal cinsiyet rolleri tutum ölçeği (TCRTÖ) toplam puanı 136.1±21.8 olup, ölçeğin alt boyutlarına göre puan ortalaması eşitlikçi cinsiyet rolü için 32.8±5.7, kadın cinsiyet rolü için 23.9±6.1, evlilikte cinsiyet rolü için 32.6±5.4, geleneksel cinsiyet rolü için 24.8±6.1 ve erkek cinsiyet rolü için 22.0±4.5'dir. Araştırmada öğrencilerin TCRTÖ puanı arttıkça dini tutum ölçeği puanı azalmakta olup, dindarlık düzeyi fazla olan kişiler daha az eşitlikçi tutum sergilemektedir. Kültür ölçeği alt boyutları ve TCRTÖ arasında yapılan değerlendirmelerde, TCRTÖ puanı arttıkça yatay toplumculuk ve yatay bireycilik puanlarının da arttığı, dikey toplumculuk ve dikey bireycilik puanlarının ise azaldığı saptanmıştır. Eşitliğe önem veren öğrenciler, toplumsal cinsiyet rollerine karşı daha eşitlikçi tutumu sergiler iken, hiyerarşiye önem veren öğrenciler daha fazla geleneksel tutuma sahiptir. Bu çalışma öğrencilerin toplumsal cinsiyet rollerine karşı tutumumun sosyodemografik, kültürel ve dini özelliklerinden etkilendiğini göstermektedir. Toplumsal cinsiyet eşitsizliğinin ortadan kaldırılması için, toplumdaki farklı gruplara yönelik araştırmaların sayısının artırılması ve eşitsizlik ile mücadele stratejilerinin geliştirilmesi gereklidir. In this research, evaluation is aimed at the attitudes of students studying at bachelor's degree at Karabük University on gender inequality and the determiners of these attitudes. The universe of study planned in cross-sectional type consists of 29157 students who studied at bachelor's degree at Karabük University in the 2014-2015 educational year. Sample size is calculated as 641 through Epi-Info-7 StatCalc software by considering confidence interval as 99%, prevalence as 50%, margin of error as 0.5%. After the sample group is placed proportionally according to gender, faculty and grades, one department is chosen in the name of each faculty and college. Descriptive statistics are summarized in percentile distribution and mean (X̄) ± standard deviation (SD). Question forms are applied to the students who are ready in the classrooms through face to face interview method. Independent two-sample, ANOVA, Kruskal Wallisand Pearson Correlation tests are applied during analysis of data.The total score of the Gender Role Attitude Scale (GRAS) of the research group consisting of about two thirds (61.1%) men is 136.1±21.8. The mean score according to the subscales of the scale is 32.8±5.7 for the egulitarian gender role, 23.9 ± 6.1 for the female gender role, 32.6±5.4 for the marriage gender role, 24.8±6.1 for the traditional gender role and 22.0±4.5 for the male gender role. As the gender role attitude scores of the students increase, the points of religious attitude scale decrease and those with higher religiosity also show less equalitarian attitudes. In the evaluations made between the culture scale's subscales and the GRAS, it is found that as the GRAS score has increased, the scores of horizontal individualism and horizontal collectivism have increased and vertical collectivism and vertical individualism scores have decreased. While equal-minded students exhibit a more egalitarian attitude towards gender roles, students with hierarchical emphasis have more traditional attitudes. This study shows that students' attitudes towards gender roles are influenced by the sociodemographic, cultural and religious characteristics. In order to eliminate gender inequality, it is necessary to develop strategies to combat inequality and to increase the number of researches for different groups in society. 119

Araştırmanın amacı, Karabük İl'inde kamu sağlık kuruluşlarında çalışan ebelerin mesleksel sorunlarının tanımlanmasıdır.Karma sıralı açıklayıcı araştırma metodolojisine uygun olarak tasarlanan araştırma niceliksel ve niteliksel olmak üzere iki aşamada gerçekleştirilmiştir. Niceliksel çalışmanın yürütüldüğü birinci aşamada Karabük'te kamu sağlık kuruluşlarında çalışan toplam 221 ebenin 204'üne (% 92,3) yüzyüze görüşme yoluyla soru formu uygulanmıştır. Yıllık veya ücretsiz izinde olan on ebe ve araştırmaya katılmak istemeyen yedi ebe kapsam dışında kalmıştır. Tanımlayıcı veriler ortama±standart sapma ve yüzde dağılım biçiminde özetlenmiş, analizde ki-kare testi uygulanmıştır. Niteliksel olan ikinci aşamada 17 ebe ile derinlemesine görüşme yapılmıştır. Ses kaydı alınabilen ortamda gerçekleştirilen görüşmelerden elde edilen veriler içerik analizi uygulanarak değerlendirilmiştir. Araştırmanın uygulanabilmesi için Karabük Üniversitesi'nden etik onay, Türkiye Halk Sağlığı Kurumu'ndan ve Karabük Kamu Hastaneleri Birliği Genel Sekreterliği'nden araştırma izni alınmıştır.Araştırmada, ebelerin % 53,0'ü 20-39 yaşları arasında ve (% 57,9) lisans ve lisansüstü eğitim mezunudur. Hastane ebelerinin yalnızca % 25'i ebelik ilgili birimlerde görev yapmaktadır. Ebelerin % 69,1'i toplumda ebelerin saygın konumu olmadığını, % 72'si görev tanımlarının, yetki ve sorumluluklarının belirsiz olduğunu, % 78,9'u sağlık örgütlenmesinin hekim merkezli yapısının ebeliğin gelişiminin olumsuz etkilediğini düşünmektedir. Derinlemesine görüşmelerde ebenin tanımı, ebelik eğitimi, mesleğin statüsü, çalışma koşulları, mevzuat ve özlük hakları, ebelik hizmetlerinin niteliği ve örgütlenme olmak üzere yedi ana tema ortaya çıkmıştır.Bu araştırma ebelerin, çalışma koşulları ve özlük hakkı sorunlarının yanı sıra, istihdam, eğitim, statü gibi sorunlarla karşı karşıya olduğunu göstermektedir. Bu konuda ülke genelini temsil edecek geniş kapsamlı araştırmalara gereksinim vardır. The study designed in descriptive research methodology was carried out in two stages, quantitative and qualitative. The question form was applied face to face to 204 (92,3%) midwives as the first phase of quantitative study from total number 221 midwives who work at health institutions in public sector Karabuk. Ten midwives on an annual leave and seven midviwes who didn't want to attend to this study were excluded from the study. The chi-square test was used to summarize the descriptive data as mean ± standard deviation and percent distribution. In the second phase was performed in-depth interviews with 17 midwives. The data obtained from the interviews conducted on the environment where the voice recording is possible were evaluated by applying content analysis. The ethical approval from Karabuk University and the survey permissions from Turkey Public Health Institution and general secretory of Karabuk Public Hospitals Association were taken to apply this research. In the study, 53,0% of midwives are between the ages 20 and 39, graduate and post-graduate degrees (57,9%). 25,0% of the midwives in the hospitals are positioned in units related to midwifery. 69,1% of midwives state that midwives do not have a respectable position among society, and 72,0% of midwives state that their job definitions, authorizations, and responsibilities are not clear, and 78,9% state that physician-centered structure in health organization affect the development of midwifery adversely. The in the in-depth interviews, seven main themes emerged as definition of the profession, midwifery education, occupational status, working conditions, legislation and personal rights, quality of midwifery services and organization.In this study showed midwifes faced problems such as employment, education, status as well as working conditions and the right to personal rights. Comprahensive studies represented whole Turkey are needed in this issues. 101

42. Karabük ilinde doğurganlık çağındaki kadınların aile planlaması hizmetlerine erişiminin ve hizmetlerin sürekliliğinin değerlendirilmesi

Araştırmada, Karabük ilinde doğurganlık çağındaki evli kadınların aile planlaması (AP) hizmetlerine erişiminin ve hizmetlerin sürekliliğinin değerlendirilmesi amaçlanmıştır. Kesitsel tipte planlanan araştırma, 2015-2016 yılları arasında gerçekleştirilmiştir. Araştırmanın evrenini, Karabük'te yaşayan evli kadınlar oluşturmuştur. Örnek büyüklüğü, Epi-İnfo 7 StatCalc Programı'nda evren 34207, Güven Aralığı % 95, hata payı % 5 ve prevelans % 75.2 (2013 Türkiye Nüfus ve Sağlık Araştırması'na göre, Batı Karadeniz Bölgesi'nde herhangi bir AP yöntemi kullanan kadınların sıklığı) değerleriyle yapılan hesaplamada 284 olarak saptanmış ve araştırma grubunun 300 kadından oluşmasına karar verilmiştir. Araştırma grubu öncelikle yerleşim yerlerinin nüfuslarına orantılı olarak dağıtılmıştır. Ardından kura yöntemiyle kentsel ve kırsal alanlardan ulaşılacak mahalle ve köyler seçilmiş ve son olarak görüşme yapılacak haneler sistematik örnekleme ile belirlenmiştir. Veriler yüz yüze görüşme yoluyla toplanmıştır. Verilerin analizinde Ki-kare, ANOVA ve Pearson Kolerasyon Testi uygulanmıştır. Kadınların ortalama yaşı 34.8±7.13, ilk evlenme yaşı 20.9±3.41 ve gebelik sayısı 2.5±1.32'dir. Kadınların % 48.7'si modern ve % 31.7'si geleneksel olmak üzere, % 80.4'ü herhangi bir AP yöntemi kullanmaktadır. En yaygın kullanılan üç yöntem geri çekme (% 31.0), kondom (% 18.3) ve tüpligasyondur (% 16.0). AP konusunda karşılanmamış gereksinim düzeyi % 9.7 olup, geri çekme yöntemi uygulayan kadınlar dahil edildiğinde % 40.7'ye yükselmektedir. Kadınların bağlı oldukları aile sağlığı birimince son bir yılda 15-49 yaş izlemi yapılan kadınların sıklığı yalnızca % 15'dir. Kadınların % 6.0'ı aile hekimini, % 31.7'si aile sağlığı elemanını tanımamaktadır. Bu araştırma, Karabük ilinde gebelikten korunmak isteyen kadınların büyük bölümünün geleneksel yöntem kullandığı ve karşılanmamış AP gereksinimi olduğunu göstermektedir. Karabük'te AP hizmetlerinin erişilebilirliğinin ve sürekliliğinin geliştirilmesine gereksinim vardır. The study aimed to evaluate the accessibility of family planning (FP) services of married women of reproductive age and the continuity of the services in Karabuk province. This cross-sectional study was conducted in Karabuk between 2015 and 2016. The sample-size was estimated to be 284 using Epi-Info 7 StatCalc including values with the universe of 34207 who married women of 15-49 age groups, the confidence interval of 95% and the prevalence of 75.2% which the women using any FP method in Western Black-Sea Region by Turkey Demographic and Health Survey-2013) and the study group was decided to include 300 women. The study group was distributed proportionally by populations of settlements. Then, neighbourhoods and villages to be reached from urban and rural areas were chosen by lot, and finally households to be interviewed were selected with systematic sampling. Data were gathered by face-to-face interviews and analysed by performing Chi-Square, ANOVA and Pearson Correlation Test. The average of age and first marriage age and the number of pregnancies numbers of women were 34.8±7.13 and 20.9±3.41 and 2.5±1.32 respectively. The percentage of women who used any method of FP was 80.4%, 48.7% for modern and 31.7% traditional methods. The most used methods were coitus interruptus (31.0%), condom (18.3%) and tubal-ligation (16.0%). The unmet need for FP was 9.7%. If the women who used coitus interruptus method were included, the unmet need for FP went up to 40.7%. The percentage of women that were monitored by their family healthcare unit in the last year was only 15%. 6.0% and 31.7% of women did not know their family physician and family health worker respectively.This study shows that the most of women who wish to avoid pregnancy use traditional methods and have an unmet need for FP. It is needed to improving the accessibility and continuity of services in Karabuk. 127

- Abstract
Background: The role of endocervical curettage (ECC) in the diagnosis of cervical intraepithelial neoplasia (CIN) is a controversial topic., Objectives: Investigate the role of ECC in the diagnosis of CIN in human papillomavirus (HPV) positive patients., Design: Retrospective., Setting: A tertiary training and research hospital., Patients and Methods: This study included patients who were referred for colposcopy between 2018-2022 because of abnormal screening results. ECC results, age, cytology, HPV status, and colposcopic impression of the patients were extracted from the medical records. Multinomial logistic regression analyses were performed to identify factors that could predict CIN on ECC., Main Outcome and Measures: The likelihood of high-grade squamous intraepithelial lesions (HSIL) in ECC in patients with cervical biopsy results of normal and low-grade squamous intraepithelial lesion (LSIL)., Sample Size: 2895 women., Results: In patients with normal and LSIL cervical biopsy results, HSILs were detected in 6.7% of ECC results. There was no difference in the detection rates of CIN in ECC among groups with smear results negative for intraepithelial lesions or malignancy (NILM), atypical squamous cells of undetermined significance (ASC-US), and LSIL. The likelihood of HSIL being observed in ECC was 2.2 times higher in patients with HPV16. The probability of LSIL disanois was 2.3 times higher in women aged 50-59 years and 2.8 times higher in women ≥ 60 years compared to the reference group of <30 years. The probability of LSIL was 2.3 and HSIL by ECC was 2.2 times higher in both age categories ( P <.012 and P =.032, respectively) than the reference group of <30 years., Conclusion: Regardless of colposcopic findings, ECC should be performed in patients with smear results of NILM who are positive for HPV16, in patients with smear results of ASC-US and LSIL who are positive for any oncogenic type of HPV and in patients 50 and above with any result of smear or any oncogenic HPV type., Limitations: We did not have the components of the HPV types in mixed groups.

44. Level and Factors Associated with Participation in Population-Based Cancer Screening in Safranbolu District of Karabuk, Turkey.

- Abstract
Background: Cervix, breast and colorectal cancers are included in the national population-based screening (PBS) program in Turkey. This study aimed to assess participation in PBSs for these cancers and to identify factors associated with participation in screenings in Safranbolu district of Karabuk, Turkey in 2016-2017., Methods: In this cross-sectional study, separate studying groups for cervix, breast and colorectal cancers were identified, taking into account the target age range specified in the national screening standards. The sample size was determined to be 374 for cervical cancer, 371 for breast cancer and 373 for colorectal cancer in the Epi-Info StatCalc program with a prevalence of 50%, a 95% Confidence Interval (CI) and a 5% error margin. The results of the data collected through face-to-face interview using questionnaires were evaluated with Chi-square tests ( P <0.05) and included in the binary logistic regression model., Results: Participation in PBS at least once between 2011 and 2016 years was 26.2% for cervical cancer, 27.6% for breast cancer and 31.6% for colorectal cancer, whereas the level of PBS or opportunistic screening at least once was 51.1%, 42.7% and 32.2%, respectively. A 2.9-fold increase in participation for the cervical cancer screening was associated with informing women about cervical cancer by the family physicians. Being married and living in the district center showed associations with a higher rate of participation for colorectal cancer screening., Conclusion: Participation in PBS was low for the 5.5-year period. - Abstract
Objectives: To evaluate the synchronous gynecologic cancers in Turkish women., Materials and Methods: A population-based longitudinal cohort study was conducted using Izmir Cancer Registry (ICR) data on gynecologic cancer patients diagnosed in the period 1993 to 2005. The registry covers the 3.7 million population of Izmir and has been collecting data on cancer incidence and survival of cancer patients' since 1992. The ICR collects data on all new cases of cancer from all the hospitals (n = 22) in the city., Results: A total of 4,185 women were identified with gynecologic cancer between 1993 and 2005, 1,526 with endometrial, 1,206 with cervical, 1,198 with ovarian, 115 with vulvar, 67 with other uterine ( sarcoma etc.), 33 with vaginal and 40 with other gynecologic cancers ( tuba uterina etc.). Fifty-five (1.3%) patients with invasive synchronous primary cancers were identified, 43 of these tumor pairs being endometrium-ovaries (81%), 66 of all lesions being endometrioid adenocarcinomas., Conclusions: Independent primary tumors of the endometrium and ovary are the most commonly encountered synchronous tumors of the female genital tractus with endometrioid adenocarcinoma as the most frequent component.

- Abstract
Purpose: The purpose of this study is to provide a detailed report on cancer incidence in Turkey, a relatively large country with a population of 72 million. We present the estimates of the cancer burden in Turkey for 2006, calculated using data from the eight population based cancer registries which have been set up in selected provinces representative of sociodemographic patterns in their regions., Methods: We calculated age specific and age adjusted incidence rates (AAIR-world standard population) for each of registries separately. We assigned a weighting coefficient for each registry proportional to the population size of the region which the registry represents., Results: We pooled a total of 24,428 cancers (14,581 males, 9,847 females). AAIRs per 100 000 were: 210.1 in men and 129.4 in women for all cancer sites excluding non-melanoma skin cancer. The AAIR per 100 000 men was highest for lung cancer (60.3) followed by prostate (22.8), bladder (19.6), stomach (16.3) and colo-rectal (15.4) cancers. Among women the rate per 100 000 was highest for breast cancer (33.7) followed by colorectal (11.5), stomach (8.8), thyroid (8.8) and lung (7.7). The most striking findings about the cancer incidence in the provinces were the high incidence rates for stomach and esophageal cancers in Erzurum and high stomach cancer incidence rates in Trabzon for both sexes., Conclusions: We are thus able to present the most accurate and realistic estimations for cancer incidence in Turkey so far. Lung, prostate, bladder, stomach, colorectal, larynx cancers in men and breast, colorectal, stomach, thyroid, lung, corpus uteri cancers in women are the leading cancers respectively. This figure shows us tobacco related cancers, lung, bladder and larynx, predominate in men. Concurrently, we analyzed the data for each province separately, giving us the opportunity to present the differences in cancer patterns among provinces. The high incidences of stomach and esophageal cancers in East and high incidence of stomach cancer in Northeast regions are remarkable.
