The carpal localization is an infrequent condition for the osteoid osteoma which is a benign, solitary, painful osteoblastic bone tumour. In the hand the diagnosis may be delayed because of the nonspecific symptoms, and the tumor is often mistakenly treated as another disease. We present an unusual case of osteoid osteoma of the scaphoid treated previously as a de Quervain's tenosynovitis. The treatment of choice was the curettage and autogenous bone grafting in this case. The symptoms were dramatically settled down just after the operation. ©2008, Firat University, Medical Faculty, İyi huylu, soliter ve ağrılı osteoblastik bir lezyon osteoid osteomanın karpal yerleşimi oldukça nadir görülen bir durumdur. Elde görülen lezyonlarda tanı spesifik olmayan semptomlara bağlı olarak gecikebilir ve tümör yanlışlıkla başka bir hastalık olarak tedavi edilebilir. Burada daha önceden de Quervain tenosinoviti olarak tedavi edilmiş nadir bir osteoid osteoma olgusu sunulmuştur. Dorsal metacarpal island flaps are usually used for hand and finger defects. The aim of this study was reviewing of the operating techniques, the results and the complications of the reversed dorsal metacarpal flap applied cases. Thirty four patients had been operated by using reverse dorsal metacarpal island flap. The majority of cases were the skin defects due to Dupuytren's contracture release. Second or third dorsal metacarpal artery was used for the pedicle of the flaps. There were twenty-five male and nine female in the series and the mean age was 44. The mean follow-up period was 31 months. There were two flap necrosis treated with debridment and open wound treatment. Reflex sympathic distrophy was seen in a diabetic patient and resolved with appropriate treatment.Easy to apply, reverse dorsal metacarpal artery flap is a good technique of coverage on the dorsal and palmar surface of the hand. It protects the major arterial system of the hand and also permit the repair of the bone and nervous tissue., Dorsal metakarpal ada flepleri el ve parmak sırtındaki defektlerinin kapatılmasında teknik açıdan zorluk yaratmaması nedeniyle sıklıkla kullanılmaktadır. Bu çalışmada amaç ters akımlı olarak uyguladığımız olguların sonuçlarını ve komplikasyonları değerlendirmek, tekniği gözden geçirmektir. 1995-2002 yılları arasında toplam 34 olguya ters akımlı dorsal metakarpal flep uygulandı. Olguların büyük çoğunluğu dupuytren kontraktürü açılması sonrası gelişen cilt defektleri idi. 2. yada 3. dorsal metakarpal arterler flep için kullanıldı. Hastaların 9'u kadın, 25'i erkek ve ortalama yaşı 44 idi. Operasyon sonrasında hastaları takip süresi ortalama 31 ay oldu. Komplikasyon olarak iki hastada oluşan flep nekrozu debridman ve açık yara tedavisi ile iyileşti. Diabetik bir hastada refleks sempatik distrofi gözlendi, uygun tedavi ile sorun çözüldü. Ters akımlı dorsal metakarpal flep, uygulanması kolay, elin major arteriyel sistemini bozmayan, uygun cilt örtülmesi sağlayan aynı zamanda kemik doku ve sinirin de onarılmasına imkan veren, el dorsali yanında palmar bölgede de kullanabilecek iyi bir flep tekniğidir.

In this study, we reviewed 13 cases treated with cross finger flap, retrospectively. Our evaluation criteria were joint stiffness, discoloration, pain and cold intolerance at the donor site; two point discrimination and cosmetic satisfaction at the recipient site. Work accident, traffic accident, gun shot injury and osteomyelitis were faced in the ethiology. The average age was 21.6 years and average follow-up was16 months. The average two point discrimination test was 6.8 mm. Cosmetic satisfaction of the patients was 61%. We suggest reconstruction of the fingers with the cross finger flap, if there is no possibility of microsurgical reconstruction, because of its simplicity against the disadvantage of cosmetic dissatisfaction., Bu çalışmada “cross finger” flep uygulanan 13 olguda, verici sahada oluşan eklem sertliği, renk değişikliği, ağrı, soğuk intoleransı , alıcı sahada iki nokta ayrımı, olguların kozmetik görünümden memnuniyeti retrospektif olarak incelenmiştir. Etiolojide olguların dokuzunda iş kazası, ikisinde trafik kazası, birinde ateşli silah yaralanması, birinde osteomyelit bulunmaktaydı. Yaş ortalaması 21.6 ve ortalama takip süresi 16 aydır. İki nokta ayrımına bakıldığında ortalama 6.8 mm olarak bulunmuştur. Olguların kozmetik görünümden memnuniyet % 61 olarak saptandı. “Cross finger” flep uygulaması, ikinci bir operasyon gerekliliği ve kozmetik dezavantajlarına rağmen geniş doku kayıplı olgular için mikrocerrahi rekonstruksiyon imkanı bulunmayan durumlarda basit ve hızlı tekniği nedeniyle kullanılabilecek bir yöntemdir.

Giant celi tumor of bone is benign but locally aggressive tumor with a high recurrence rate. The distal end of the radius is the third most common localization. In this study we present a recurrent giant celi tumor localized at the distal end of the radius. İn that case curettage, curettage and grafting, local agents as bone cement, formol and alcohol were the initial treatments however wide excision and revision with otogenous non vascularized fibular graft has been applied in the continuing aggresivity of the lesion, Dev hücreli tümör benign fakat lokal agresif karakterli, rekürrens oranı yüksek olabilen bir lezyondur. Radius distal ucu üçüncü en sık lokalizasyondur. Burada radius distal uç yerleşimli, mültipl rekürrens gösteren bir olgu sunulmuştur. Olguda tedavi seçenekleri olarak küretaj, küretaj ve greftleme, kemik çimentosu, formol ve alkol gibi lokal adjuvan ajanlar kullanılmasına karşın agresivitesi devam eden lezyona geniş eksizyon ve otojen nonvaskülarize fıbula grefti ile revizyon uygulanmıştır.

Anterior interosseous nerve syndrome is a nerve entrapment syndrome recognized by the paralysis of pronator quadratus, flexor pollicis longus, flexor digitorum profundus of the second and in some cases the third digits. In this study a case operated for two times for carpal tunnel syndrome accompanying an anterior interosseous nerve syndrome with paralysed first and second deep flexor tendons is reported., Anterior interossöz sinir sendromu, fleksör pollisis longus, ikinci ve bazı durumlarda üçüncü parmak fleksör digitorum profundus ve pronator quadratus kaslarının felci ile karakterize bir durumdur. Bu çalışmada iki defa karpal tünel sendromu ön tanısıyla öpere edilmiş ancak daha sonra eşlik eden anterior interossöz sinir sendromu ortaya konularak paralize bir ve ikinci parmak fleksör tendonlanna transfer uygulanan bir olgu sunulmuştur.
