Objective: This research is aimed to create themes and codes about professional competency in midwifery, to reveal the relationship between the codes created. Methods: This qualitative study was conducted between October 2018 and March 2019 at the Obstetrics and Gynecology and Pediatric Emergency Campus of a training and research hospital. This study was conducted in a training and research hospital in Sakarya province and 18 midwives who met the inclusion criteria were interviewed. The midwives were asked semi-structured questions in the Midwife Introduction and Interview Form and audio recorded. The responses to the questions were recorded, transcribed and coded using MAXQDA2018 software program. The codes were then analyzed and grouped according to categories and themes. Results: The median age of the midwives was 35 years. Of them, eight were bachelor’s degree, and six had been working in the delivery room for ≥16 years. In addition, a total of five themes, 17 categories, 35 codes were created and the usage density of these codes is 289. Conclusion: Since the midwives were unable to sufficiently describe the concept of competency as well as their own competencies, it was determined that additional education and research in community health services, hands-on experience and technological training could help build their skills and knowledge, make them more effective in their practice, increase their self-confidence and boost their overall job satisfaction.

Aims To evaluate different cardiovascular magnetic resonance (CMR) parameters for the differentiation of light chain amyloidosis (AL) and transthyretin-related amyloidosis (ATTR). Methods and results In total, 75 patients, 53 with cardiac amyloidosis {20 patients with AL [66 ± 12 years, 14 males (70%)] and 33 patients with ATTR [78 ± 5 years, 28 males (88%)]} were retrospectively analysed regarding CMR parameters such as T1 and T2 mapping, extracellular volume (ECV), late gadolinium enhancement (LGE) distribution patterns, and myocardial strain, and compared to a control cohort with other causes of left ventricular hypertrophy {LVH; 22 patients [53 ± 16 years, 17 males (85%)]}. One-way ANOVA and receiver operating characteristic analysis were used for statistical analysis. ECV was the single best parameter to differentiate between cardiac amyloidosis and controls [area under the curve (AUC): 0.97, 95% confidence intervals (CI): 0.89–0.99, P < 0.0001, cut-off: >30%]. T2 mapping was the best single parameter to differentiate between AL and ATTR amyloidosis (AL: 63 ± 4 ms, ATTR: 58 ± 2 ms, P < 0.001, AUC: 0.86, 95% CI: 0.74–0.94, cut-off: >61 ms). Subendocardial LGE was predominantly observed in AL patients (10/20 [50%] vs. 5/33 [15%]; P = 0.002). Transmural LGE was predominantly observed in ATTR patients (23/33 [70%] vs. 2/20 [10%]; P < 0.001). The diagnostic performance of T2 mapping to differentiate between AL and ATTR amyloidosis was further increased with the inclusion of LGE patterns [AUC: 0.96, 95% CI: (0.86–0.99); P = 0.05]. Conclusion ECV differentiates cardiac amyloidosis from other causes of LVH. T2 mapping combined with LGE differentiates AL from ATTR amyloidosis with high accuracy on a patient level. Objective: This study was carried out with the aim of determining the reasons for using, and quitting the coitus interruptus method at any stage of the lives of women aged 15- 49 years who applied to family planning clinic to use effective contraceptive methods. Method: The study was carried out descriptively and cross-sectionally. The sample of the study consisted of 200 sexually active women aged 15- 49 years who used the coitus interruptus method at any stage of their lives, and applied to a Mother and Child Health Family Planning Center in Bornova, Izmir so as to use an effective family planning method. The data of the study were collected by face to face interview method using the questionnaire form prepared by the researchers in line with the literature.The data were evaluated in SPSS, 16.0 package program and the numerical and percentage distributions of the collected data were created. Results: Sixty percent of women aged 15-49 years who participated in our study were in the 20-34 age group, and 40% of them were literate / primary school graduates.It was determined that 30.5% of these women were using the coitus interruptus method because of their spouses desired and 34% of these women quit this method due to its negative aspects.Seventy-eight percent of these women were using effective family planning methods because of their positive features and 56.0% of them preferred IUD as an effective method. Background/Objectives: Previous trials reported comparable results with PASCAL and earlier MitraClip generations. Limited comparative data exist for more contemporary MitraClip generations, particularly the large MitraClip XT(R/W). We aimed to evaluate acute and 30-day outcomes in patients undergoing mitral valve transcatheter edge-to-edge repair (M-TEER) with one of the large devices, either PASCAL P10 or MitraClip XT(R/W) (3rd/4th generation). Methods: A total of 309 PASCAL-treated patients were matched by propensity score to 253 MitraClip-treated patients, resulting in 200 adequately balanced pairs. Procedural, clinical, and echocardiographic outcomes were collected for up to 30 days, including subgroup analysis for mitral regurgitation (MR) etiologies. Results: PASCAL and MitraClip patients were comparable regarding age (80 vs. 79 years), sex (female: 45.5% vs. 50.5%), and MR etiology (degenerative MR: n = 94, functional MR [FMR]: n = 96, mixed MR: n = 10 in each group). Technical success rates were comparable (96.5% vs. 96.0%; p > 0.999). At discharge, the mean gradient was higher (3.3 mmHg vs. 3.0 mmHg; p = 0.038), and the residual mitral valve orifice area was smaller in MitraClip patients (3.0 cm2 vs. 2.3 cm2; p < 0.001). At discharge, the reduction to MR ≤ 2+ was comparable (92.4% vs. 87.8%; p = 0.132). However, reduction to MR ≤ 1+ was more frequently observed in PASCAL patients (67.7% vs. 56.6%; p = 0.029), driven by the FMR subgroup (74.0% vs. 60.0%; p = 0.046). No difference was observed in 30-day mortality (p = 0.204) or reduction in NYHA-FC to ≤II (p > 0.999). Conclusions: Both M-TEER devices exhibited high and comparable rates of technical success and MR reduction to ≤2+. PASCAL may be advantageous in achieving MR reduction to ≤1+ in patients with FMR. BACKGROUND: We assessed the safety profile of tricuspid transcatheter edge-to-edge repair (TEER) in patients with right ventricular (RV) dysfunction. METHODS: We identified patients undergoing TEER to treat tricuspid regurgitation from June 2015 to October 2021 and assessed tricuspid annular plane systolic excursion (TAPSE) and RV fractional area change (RVFAC). RV dysfunction was defined as TAPSE <17 mm and RVFAC <35%. The primary end point was 30-day mortality after TEER. We also investigated the change in the RV function in the early phase and clinical outcomes at 2 years. RESULTS: The study participants (n=262) were at high surgical risk (EuroSCORE II, 6.2% [interquartile range, 4.0%–10.3%]). Among them, 44 patients met the criteria of RV dysfunction. Thirty-day mortality was 3.2% in patients with normal RV function and 2.3% in patients with RV dysfunction (P=0.99). Tricuspid regurgitation reduction to ≤2+ was consistently achieved irrespective of RV dysfunction (76.5% versus 70.5%; P=0.44). TAPSE and RVFAC declined after TEER in patients with normal RV function (TAPSE, 19.0±4.7 to 17.9±4.5 mm; P=0.001; RVFAC, 46.2%±8.1% to 40.3%±9.7%; P<0.001). In contrast, those parameters were unchanged or tended to increase in patients with RV dysfunction (TAPSE, 13.2±2.3 to 15.3±4.7 mm; P=0.011; RVFAC, 29.6%±4.1% to 31.6%±8.3%; P=0.14). Two years after TEER, compared with patients with normal RV function, patients with RV dysfunction had significantly higher mortality (27.0% versus 56.3%; P<0.001). CONCLUSIONS: TEER was safe and feasible to treat tricuspid regurgitation in patients with RV dysfunction. Aims: Prognostic impact of post‐procedural changes in right ventricular (RV) function after tricuspid transcatheter edge‐to‐edge repair (T‐TEER) is still unclear. We investigated association of RV function and its post‐procedural changes with clinical outcomes in patients undergoing T‐TEER. Methods and results: We retrospectively analysed 204 patients who underwent T‐TEER and echocardiographic follow‐up at 3 months after T‐TEER. RV function was assessed by RV fractional area change (RVFAC), and RV dysfunction was defined as RVFAC <35%. Patients with an increase in RVFAC from baseline to the follow‐up were considered as RV responders. Patients were divided into four groups according to baseline RVFAC and the RV responder. The primary outcome was a composite of mortality and hospitalization due to heart failure within 1 year. Forty‐five of 204 patients (22.1%) had RVFAC <35% at baseline, and 71 (34.8%) were RV responders. The association between the RV responder and the composite outcome had a significant interaction with RVFAC at baseline. Among patients with baseline RVFAC <35%, RV responders had a lower risk of the composite outcome than RV non‐responders, while this association was not significant in those with baseline RVFAC ≥35%. Among patients with baseline RVFAC <35%, a smaller RV diameter and a greater reduction of tricuspid regurgitation were predictors for the RV responder. Conclusion: Post‐procedural increase in RVFAC after T‐TEER is associated with improved outcomes in patients with RV dysfunction. The factors related to the increase in RVFAC may support patient selection for T‐TEER in patients with RV dysfunction. [ABSTRACT FROM AUTHOR]

Introduction: Chronic obstructive pulmonary disease (COPD) is one of the most important morbidity and mortality reasons worldwide. The purpose of our study is to evaluate physician-diagnosed COPD epidemiology according to age groups. Methods: ICD10 J44.0-9 diagnoses codes in the database of Turkish Social Security Institution between 2007 and 2016 was checked through. Distribution of COPD through age groups (40-49, 50-59, 60-69, 70-79, and 80 years and older) among male and female population still alive in 2016 was calculated. Results: 56.7% of 3,168,459 patients with average age of 62.53±15.07 were males. It was determined that 3.2% prevalence in female age group of 40-49 increased up to 23.1% in the age of 80+, 4.2% prevalence in male age group of 40-49 increased up to 35.3% in the age of 80+. In total, it increased from 3.7% to 27.7% in the same group of ages. Rate of the patients between the ages of 40-69 was calculated as 66% of the total number of patients. Discussion and Conclusion: As a result, physician-diagnosed COPD prevalence in Türkiye has an increasing trend by age both in male and female population. One out of each four individuals over the age of 80 years is diagnosed with COPD. Such an evaluation shows that COPD is more frequently in older ages. However, it is not correct to limit COPD as an advanced age disease, since the majority of the patient population is 70 years and under. [ABSTRACT FROM AUTHOR]
