3. Electrostatic Time Dilation and Redshift

We first present the salient features of the gravitational time dilation and redshift effects in two ways; by considering the oscillation frequencies/rates of clocks at different heights/potentials and by considering the photons emitted by these clocks such as atoms/nuclei. We then point out to the extension of these gravitational effects to static electricity along with two experiments performed in the '30s with null results of the electrostatic redshift. We show that the absence of this redshift is a consequence of the conservation of electric charge. We discuss the electrical time dilation and redshift effects in detail and argue that the electrostatic time dilation in an electric field must be a fact of Nature. We then present a general relativistic scheme that explains this effect. We also introduce an electrical equivalence principle analogous to the gravitational one and demonstrate how to obtain the electrostatic time dilation by this principle. We emphasize the importance of ionic atomic clocks to measure this effect whose confirmation would support the general relativistic scheme presented. We finally go over an attempt in the literature to explain the impossibility of the experimental observation of the electrostatic redshift due to its smallness by employing the Reissner - Nordstr\"om metric in general relativity. We argue that the $Q^{\,2}$ - term in this metric is due to the minuscule contribution of the energy of the electric field of the central body to its gravitational field. 4. Experimental Evidences Supporting the Extension of the Equivalence Principle to Electromagnetic Fields

The principle of equivalence postulating that an acceleration is indistinguishable from gravity by any experiment, is valid within families of particles having the same passive gravitational to inertial mass ratio $m_p/m_i$. Presently experimental observations indicate that we live in a universe with one single family for which $m_p/m_i=1$, but if we consider the imaginary case of a universe with several particle families having different $m_p/m_i$, the principle of equivalence would still apply to each one of them. On the basis of this generalized formulation of the equivalence principle, which becomes relative to sets of particles, and that we designate as the \textit{single-particle equivalence principle}, one demonstrates that inertial frames can also be implemented for sets of electrically charged particles, with the same charge-to-mass ratio $q/m_i$, accelerating in homogeneous electric and / or magnetic fields (by analogy with the case of particles in free fall in a homogeneous gravitational field). Experimental evidences in support of the proposed extension of the principle of equivalence to electric fields are presented. These consist in the Witteborn-Fairbank experiment which revealed that electrons do not fall in the Earth gravitational field, and the well know fact that electric charges do not radiate either when in free fall in a homogeneous gravitational field, or when being accelerated by a homogeneous electric field. The London moment in rotating superconductors is also supporting the proposed extension of the equivalence principle. Prospects for the future exploration of the consequences of the proposed theoretical scheme to unify electromagnetism with gravitation in the paradigm of curved space-time, are also briefly suggested., Comment: 8 pages

5. Assessment of autoantibodies associated with intravenous immunoglobulin replacement therapy in children with primary immunodeficiency.

While it is known that immunoglobulin replacement therapy (IgRT) used in the treatment of primary immunodeficiency disorders (PIDs) can lead to the passive transfer of autoantibodies, there is no data indicating that these antibodies can cause clinical symptoms in patients. This study aimed to investigate the presence of autoantibodies and their clinical correlation in patients diagnosed with PIDs receiving IgRT. Paediatric patients who were diagnosed with PIDs, and administered IgRT at our immunology clinic between 1 January 2012 and 31 December 2021, were included in the study. The medical records of these patients were retrospectively analysed, and autoantibodies were screened. Autoantibody screening was conducted at least once in 48 cases. Among these cases, 29 cases (60.4%) demonstrated positivity for at least one of the autoantibodies screened in the study. Among these cases, 23 tested positive for anti‐TPO, 9 for anti‐TG and 2 for both anti‐TPO and anti‐TG. Only two of these patients were confirmed to have Hashimoto's thyroiditis. In 30 cases, autoantibodies related to Celiac disease (CD) were screened, with at least one being positive in five different cases; CD was not confirmed. The results of our study suggest that passive transfer of autoantibodies to patients with IgRT does not cause any significant clinical findings. In addition, in cases of PID, autoantibodies detected in the blood passed to patients with IgRT can lead to misdiagnosis. Screening for autoantibodies in patients with PID undergoing IgRT may not yield accurate results in terms of detecting autoimmune diseases. [ABSTRACT FROM AUTHOR]

We propose an experiment to deflect electrons by a small charged sphere to determine if the standard general relativity or this modified one corresponds to reality. We first review the cosmological constant problem, and then mention a conjecture of Feynman according to which the general relativistic theory of gravity should be reformulated in such a way that energy does not couple to gravity. We point out that our recent unification of gravitation and electromagnetism through a symmetric tensor has the property that the free electromagnetic energy and the vacuum energy do not contribute explicitly to the curvature of spacetime just like the free gravitational energy. It is shown that in the cosmological models based on a vacuum energy decaying as a^{-2}, where a is the scale factor of the universe, the fate of the universe in regard to whether it will collapse in future or expand forever is determined not by the curvature constant k but by an effective curvature constant k_{eff}. It is argued that a closed universe with k=1 may expand forever, in other words simulate the expansion dynamics of a flat or an open universe because of the possibility that k_{eff}=0 or -1, respectively. Two such models, in one of which the vacuum does not interact with matter and in another of which it does, are studied. It is shown that the vacuum equation of state p_{vac}= -\rho_{vac} may be realized in a decaying vacuum cosmology provided the vacuum interacts wuth matter. The optical depths for gravitational lensing as a function of the matter density and other parameters in the models are calculated at a source redshift of 2. The age of the universe is discussed and shown to be compatible with the new Hipparcos lower limit of 11Gyr. 15. Is There an Age of the Universe Problem After the Hipparcos Data?

We have reanalyzed the age of the universe problem under the assumption that the lower limit on the age of the globular clusters is 11Gyr, as predicted by the recent Hipparcos data. We find that the globular cluster and the expansion ages in a \lambda=0 universe are consistent only if the present value H_0 of the Hubble constant is \le 60kms^{-1}Mpc^{-1}. If H_0\ge 60kms^{-1}Mpc^{-1} some kind of modification of the standard \lambda=0 model is required. Invoking a (time-independent) cosmological term \lambda in the Einstein field equations, as has been done frequently before, we have found that due to the gravitational lensing restrictions a fla universe with the present matter density parameter \Omega_M <0.5 is not problem free. 18. On the Equivalence Principle and a Unified Description of Gravitation and Electromagnetism

We first investigate the form the General Relativity Theory would have taken had the gravitational mass and the inertial mass of material objects been different. We then extend this analysis to electromagnetism and postulate an equivalence principle for the electromagnetic field. We argue that to each particle with a different electric charge-to-mass ratio in superimposed gravitational and electromagnetic fields there corresponds a spacetime manifold whose metric tensor $g_{\mu\nu}$ describes the dynamical actions of gravitation and electromagnetism. The electric field outside a charged sphere asserts itself independently rather than contributing to the gravitational field. The contribution of the electric field to the spacetime metric outside the charged sphere is shown to be similar to the gravitational one in the Schwartzschild metric but with a charge-to-mass ratio dependence of the test particle instead of the Reissner - Nordstr\"om metric, resulting in a unified description of gravitation and electromagnetism. We point out that there are existing experiments whose results can be explained by the equivalence principle for the electromagnetic field presented here. Additional experimental predictions of the theory are mentioned., Comment: A new argument that produces the $g_{00}$ of the metric outside a charged sphere is presented. We consider the low-energy effective string action in four dimensions including the leading order-$\alpha'$ terms. An exact homogeneous solution is obtained. It represents a non-singular expanding cosmological model in which the tensor fields tend to vanish as $t\to \infty$. The scale factor $a(t)$ of the very early universe in this model has the time dependence $a(t)^2=a_0^2+t^2$. The violation of the strong energy condition of classical General Relativity to avoid the initial singularity requires that the central charge deficit of the theory be larger than a certain value. Hızlı tek akı kuvantumu (rapid single flux quantum - RSFQ) teknolojisi, sinyallerin anahtarlanmasında süperiletken malzemelerdeki kuvantum etkilere dayanan bir dijital ve analog elektronik devre teknolojisidir. RSFQ devreler, süperiletken malzemelere dayalı bir teknoloji olduğu için kriyojenik çalışma ortamlarında kullanılmaları gerekmektedir. CMOS eşleniklerine göre temel avantajları, yaklaşık 1 THz frekanslarına kadar anahtarlama yapabilmeleri ve çok düşük güç tüketimlerine sahip olmalarıdır. Ayrıca, RSFQ teknolojisi standart CMOS tasarım ve üretim sistemleri ile uyumludur ve üretilen devrelerin CMOS ve optik tabanlı devrelerle entegrasyonları da mümkündür. Hızlı tek akı kuvantumu tabanlı (RSFQ) VLSI devre tasarım ve üretim teknolojileri, son yıllarda büyük aşama kaydetmiş ve karmaşık devrelerin yapılabilmesi mümkün olmuştur.Bu proje kapsamında, state of the art kabul edilebilecek, SFQ analog devreler, sayısal devreler ve bellek devreleri için bir optimizasyon aracı geliştirilmiştir. Geliştirilen programların birbiri ile entegrasyonları ve grubumuz dışından araştırmacıların da kullanabileceği bir arayüz üzerinde çalışılmalar devam etmektedir. Analog optimizasyon aracı ile geliştirilen karşılaştırma devreleri, literatürde rapor edilen en düşük gri bölge genişliklerine sahiptir. Sayısal optimizasyon aracı ile geliştirilen kapıların tamamı düşük bit hata oranları ile çalışmıştır. Tasarlanan VTM tabanlı bellek modülü, rastgele devre parametreleri ile başlayarak literatürdeki rapor edilen ve tecrübeli bir araştırmacının uzun sürelerde tasarlayabildiği kutuplama marjinleri seviyesinde çalışmıştır. Tasarlanan ALU’nun alt modüllerinin tamamı çalışmıştır. Deney düzeneğimiz kapalı devre analog ve sayısal devrelerin hassas ölçümlerine uygun hale getirilmiştir. Alışılagelmiş tıp anlayışında hastalıklar üzerine odaklanılmış, aynı hastalığa sahip farklı bireylerde aynı tedavi yöntemleri uygulanmıştır ancak gelişen tıp bilimi sürecinde aynı hastalığın farklı kişilerde aynı seyretmeyeceği ve dolayısıyla da tedavinin aynı olamayacağı gerçeği doğmuştur. Bu nedenle hastalıktan ziyade bireyler daha ön plana çıkmaktadır ki bu bağlamda gelişen teknolojik altyapı ile beraber hassas tıp (precision medicine) kavramı hayatımıza girmiştir. Hassas tıp terimi nispeten yeni olsa da kavram olarak uzun yıllardır sağlık hizmetlerinin bir parçası olarak yer almaktadır. İmmünolojik hastalıkların tedavisinde hassas tıp tedavi stratejileri temel olarak üç farklı tedavi yaklaşımını içermektedir. Eksik moleküllerin yerine konulması; aşırı aktif hücre içi sinyalizasyonun inhibisyonu ve sitokin blokajı. Hassas tıp İVİG, PEG-ADA ve IFN-γ gibi immünoloji kliniklerinde uzun yıllardır kullanılan tedavi yaklaşımları yanında pleriksafor, sirolimus ve vedolizumab gibi son senelerde bulunmuş daha nadir kullanılan tedavileri de kapsamaktadır. Günümüzde artan teknolojik alt yapı ile yeni moleküler yolakların tanımlanması şüphesiz bir çok hastalığın tedavisinde hassas tıp kavramının daha etkin olarak kullanılacağı günleri getirecektir. In the conventional medical understanding, the focus has been on diseases, the same treatment methods have been applied to different individuals with the same disease, but the fact that the same disease will not be the same in different people and therefore the treatment cannot be the same has emerged. For this reason, individuals are more prominent than disease, and in this context, precision medicine concept has entered our lives with the developing technological infrastructure. Although the term precision medicine is relatively new, the concept has been a part of healthcare for many years. In the treatment of immunological diseases, sensitive medicine treatment strategies basically include three different treatment approaches. Replacement of “missing molecules”, inhibition of overactive intracellular signaling and cytokine blockade. Along with the treatment approaches which have been most frequently used in immunology clinics for many years such as IVIG, PEG-ADA and IFN-y, precision medicine also includes relatively rarer treatments that have been discovered lately such as pleriksafor, sirolimus and vedolizumab. Defining new molecular pathways with today’s increasing technological infrastructure will undoubtedly bring the days when the concept of precision medicine will be used more effectively in the treatment of many diseases.

- Abstract
Production and consumption of genetically modified organisms (GMOs) has tended to increase in the last two decades around the world. Although the use of GMOs in the food industry has become increasingly common, their effects on human health have not yet been clarified. The attitudes and approaches of pediatricians towards GMOs are very important and have the power of influencing the nutritional habits of not only children but also the whole society through parents. In this study, it was aimed to determine the knowledge, attitudes and behaviors of pediatricians and pediatric residents about GMOs. This descriptive cross-sectional study was conducted between 1-31 December 2021 with 102 pediatricians and pediatric residents working at Ankara Dr. Sami Ulus Training and Research Hospital for Maternity and Children's Health and Diseases. The data were analyzed using the SPSS package program. 67.6% of the participants stated that GMO foods are harmful to health, 37.3% stated that physicians have sufficient knowledge about GMO foods in general and 63.7% stated that he/she advise his/her patients not to consume GMO foods. While 78 (76.4%) of the participants stated that they heard the term GMO for the first time from media tools such as internet/ television/newspaper/radio; only 13 (12.7%) participants stated that they heard it for the first-time during training activities. In our study, it was shown that the risk perception of the research group towards GMOs was high, and their level of knowledge was low. In order to eliminate the suspicions of physicians about GMO foods, giving more space to this subject in educational activities will increase the awareness of physicians and the society. Araştırma sahasını oluşturan Ayvacık İlçesi, Çanakkale İli yönetim bölgesinde bulunmaktadır. Ayvacık İlçesi Biga Yarımadası'nın, güney-güneybatı bölümünde 892 km2'lik alan kaplar. İlçeyi batı ve güneyden Ege Denizi, kuzeyden Ezine, kuzeydoğudan Bayramiç ve doğudan Edremit (Balıkesir) ilçeleri çevrelemektedir. Ayvacık İlçesi, Marmara Bölgesi'nin Güney Marmara ve Ege Bölgesi'nin Ege Bölümü sınırları içerisinde kalır. Ayvacık İlçesi'nde zeytin tarımının coğrafi dağılışı, coğrafi çevre ile üretim ilişkisi incelenmiştir. Araştırma sahasını oluşturan Ayvacık İlçesi'nde zeytin yetiştiriciliği yöre halkının temel ekonomik faaliyeti durumundadır. Araştırma alanında yükseltinin arttığı merkezi ve kuzey kesimlerinde zeytin tarımı yapılamamaktadır. Zeytin tarımı 600 metre yükseltiden sonra yapılsa bile verim çok büyük oranda düşmektedir. Yörede zeytin tarımı genellikle 250 -325 metrelerde orta eğimli arazilerde toplanmıştır. Araştırma alanımızda en yüksek zeytin verimine sahip alanlar Adatepe, Sazlı, Ahmetçe, Paşaköy, Gülpınar, Büyükhusun, Küçükçetni, Arıklı, Kösedere, Kocaköy, Bademli ve Nusratlı'dır. Ayvacık'ta işlenebilir tarım arazisinin %34.2'si (11.373 hektar) zeytin üretim alanlarından oluşmaktadır. Bu alan Çanakkale'deki zeytin tarım alanlarının da %35.2'sini meydana getirmektedir. Ayvalık İlçesi'nde 2018 yılı itibarıyla toplam 113.730 dekar zeytin üretim alanının 108.300 dekarı yağlık (%95.2), 5.430 dekarı sofralık (%4.8) zeytin üretimine ayrılmıştır. Ayvacık İlçesi'nde 2018 yılı verilerine göre, yağ üretiminde değerlendirilen toplam 1.735.517 zeytin ağacı mevcuttur. Bu ağaçlardan %98.9'u meyve veren yaşta (1.716.206), %1.1'i ise (19.311) meyve vermeyen ağaçlardan oluşmaktadır. Ayvacık İlçesi'nde 2018 yılı itibarıyla 58.277 ton yağlık, 1.500 ton sofralık olmak üzere toplam 59.777 ton zeytin üretilmiştir. Geniş üretim alanı ve yüksek zeytin üretim miktarlarına rağmen, araştırma sahasında fide temini, ilaçlama, gübreleme, hasat, kooperatifleşme, pazarlama ve zeytinyağı üretiminde sorunlar mevcuttur. Bu sorunların çözülmesi halinde zeytin üretiminden elde edilen gelirin artması mümkündür. Ayvacık District, which forms the research area, is located in the administrative region of Çanakkale Province. Ayvacık District occupies an area of 892 km2 in the south-southwest part of the Biga Peninsula. The district surrounds the Aegean Sea from the west and south, Ezine from the north, Bayramiç from the northeast and Edremit (Balıkesir) from the east. Ayvacık District is within the boundaries of the Marmara Region's South Marmara and Aegean Region's Aegean Region. In Ayvacık District, the geographical distribution of olive farming, the relationship between the geographical environment and the production were examined. Olive growing in Ayvacık District, which constitutes the research area, is the main economic activity of the local people. Olive cultivation cannot be done in the central and northern parts, where the altitude increases in the research area. Even if olive cultivation is made after 600 meters of altitude, the yield drops significantly. Olive cultivation in the region is generally gathered on medium slope lands at 250-325 meters. The areas with the highest olive yield in our research area are Adatepe, Sazlı, Ahmetçe, Paşaköy, Gülpınar, Büyükhusun, Küçükçetni, Arıklı, Kösedere, Kocaköy, Bademli and Nusratlı. 34.2% (11.373 hectares) of cultivable agricultural land in Ayvacık consists of olive production areas. This area also constitutes 35.2% of the olive farming areas in Çanakkale. In Ayvalık District, as of 2018, 108,300 decares of total olive cultivation area of 113,730 decares was devoted to olive oil production (95.2%) and 5,430 decares for table (4.8%). According to 2018 data in Ayvacık District, there are 1,735,517 olive trees evaluated in oil production. 98.9% of these trees are age-bearing (1.716.206) and 1.1% (19.311) are trees that do not bear fruit. As of 2018, 59,777 tons of olives were produced in Ayvacık District, 58,277 tons of oil and 1,500 tons of table. Despite the large production area and high olive production quantities, there are problems in seedling supply, disinfection, fertilization, harvesting, cooperative marketing, and olive oil production in the research area. If these problems are solved, it is possible to increase the income from olive production. 128

- Abstract
In this study, it was aimed to investigate refractive errors in patients who applied to the outpatient clinic with the complaint of headache, in whom no pathology was found in routine examinations and tests that can explain the headache. This study was carried out by retrospective analysis of the cases who applied to the outpatient clinic between 01.10.2019 and 30.06.2020. Cases in whom no pathology was detected in the routine examination of headache in the pediatric outpatient clinic were included in the study. Among these cases, those who were found to have eye diseases such as strabismus, glaucoma and optic neuritis, which may cause headache, were excluded from the study. Autorefractometry values were obtained in all cases after cyclopentolate drop application and visual acuity was determined with the Snellen chart. Complaints of patients with refractive error after appropriate treatment were questioned. Data were analyzed with SPSS 22.0, frequency, mean, Chi-square, One-Way ANOVA and paired samples t test were used. A total number of 68 cases (39M, 29F) with a mean age of 13.01 (7.00-17.00) were included in the study. 41 (60.3%) cases had any eye complaints. No refractive error was detected in 21 (30.9%) cases. Hyperopia, myopia and astigmatism were detected respectively in 20 (29.4%), 16 (23.5%) and 11 (16.2%) cases. While refractive error was detected in 47 of 68 cases in this study, it was determined that the headache complaint of 72.3% of these patients disappeared after appropriate treatment of the refractive error. Among the groups with and without eye complaints; a significant difference was detected in terms of refractive error (P<0.001) and presence of headache after treatment (P<0.001). This study shows us the importance of referral to an ophthalmologist in pediatric and adolescent patients who apply to the hospital with headache complaint, even if they do not have any eye complaints. Thus, both referral to higher-level centers for further research can be prevented and refractive errors that may lead to amblyopia and cause permanent vision loss for life can be detected at an early stage and be treated. Üyesi İlknur YAZICILAR ÖZÇELİK Bu çalışmanın amacı, ortaokul öğrencilerinin özsaygı düzeylerini, spor yapma (lisanslı, serbest zaman değerlendirmek için) ve yapmama, cinsiyet, yaş, spor yapma amacı, spor branşı ve haftalık egzersiz sıklığı değişkenleri açısından araştırmaktır. Yöntem: Çalışmaya 2018-19 öğretim yılında Tokat ili Erbaa ilçesindeki ortaokullarda öğrenim gören 1045 (Nkadın:541, Nerkek:504) öğrenci katılmıştır. Verilerin analizinde tanımlayıcı istatistikler, bağımsız gruplarda t-testi, tek yönlü varyans analizi ve LSD Post Hoc. Testleri kullanılmıştır. Method: The study included 1045 (Nfemale: 541, Nmale: 504) students from secondary schools in Erbaa district of Tokat province in 2018-19 academic year. In order to obtain the research data, Personal data form and Coopersmıth Self-Esteem Inventory adapted to Turkish by Pişkin (1996) were used. In the analysis of the data, descriptive statistics, t-test in independent groups, one-way analysis of variance and LSD Post Hoc Tests were used. According to the results of the study, a statistically significant difference was found between the variables of doind licensed (in school teams), unlicensed (to spend leisure time)sports and "general self-esteem, social self-esteem and academic self-esteem" levels; between gender and "general self-esteem and social self-esteem levels"; between age and "social self-esteem, home-family self-esteem and academic self-esteem"; between the fact of doing sport or not generally and "the levels of general self-esteem and academic self-esteem"; between the level of sporting and "general self-esteem, social self-esteem and academic self-esteem levels". As a result, based on the findings of the study, it can be said that doing licensed sports and doing sports to spend leisure time, gender, taking part in team sports branches and weekly exercise frequency (two days or more) have an effect on increasing self-esteem levels of students.

Bu araştırmanın amacı, takım özerkliği, örgütsel destek ve takım dayanışması-nın takım başarısına etkilerinin araştırılması, ayrıca, takım dayanışmasının, öncül değiş-kenler ve proje takım başarısı arasındaki ilişkide aracılık etkisinin varlığını sorgulamaktır. Öngörülen modelde, literatüre uygun olarak takım dayanışmasının görev dayanışması ve sosyal dayanışması boyutlarının etkileri ayrı ayrı incelenmiştir. Proje takım başarısı ise proje takımı literatürüne uygun olarak etkililik ve inovasyon boyutlarıyla ele alınmıştır. Çok uluslu bir finans kurumunda 110 proje takımında (343 takım üyesi) gerçekleştirilen çalışmada Kısmi En Küçük Kareler Yapısal Eşitlik Modeli (PLS-SEM) kullanılmıştır. Araştırma bulguları, örgütsel desteğin, takım görev dayanışmasının ve takım etkililiğinin anlamlı öncülü olduğunu, takım dayanışması boyutlarından görev dayanışmasının örgüt-sel destek ve takım etkililiği arasındaki ilişkide kısmi aracılık rolüne sahip olduğunu or-taya koymaktadır. Ayrıca, takım özerkliğinin takım etkililiği ve takım inovasyonu üze-rindeki anlamlı olumlu etkisi teyit edilmekle birlikte, bu ilişkilerde takım dayanışması boyutlarından hiçbirinin aracılık rolüne sahip olmadığı görülmektedir. Literatüre uygun olarak, takım görev dayanışmasının takım etkililiğine ve takım inovasyonuna anlamlı olumlu etkisi desteklenmiş, ancak sosyal dayanışmanın anlamlı bir etkisi bulunamamıştır. The relationship between cohesion and performance in project teams has received subs-tantial attention, however limited number of studies investigated antecedents of team cohesion and the mediator role of team cohesion, between its antecedents and team suc-cess. The purpose of this research was to investigate the effects of team autonomy, or-ganizational support and team cohesion on team success and to examine the existence of mediating effect of team cohesion between its antecedents and project team success. In the predicted model, the effects of the task and social dimensions of team cohesion we-re examined separately in accordance with the literature. Project team success was dis-cussed in terms of effectiveness and innovation in line with project team literature. Par-tial Least Squares Structural Equation Model (PLS-SEM) was used in the study carried out in 110 project teams (343 team members) in a multinational financial institution. The findings of the research revealed that organizational support was a significant antece-dent of task cohesion and team effectiveness and one of the cohesion dimensions of task cohesion, had a partial mediating role between organizational support and team ef-fectiveness. In addition, although the significant positive effects of team autonomy on team effectiveness and team innovation were confirmed, it appeared that none of the dimensions of team cohesion had a mediating role in this relation. In accordance with the literature, a positive effect of team task cohesion on team effectiveness and team in-novation was supported, however no significant effect of social cohesion was found. 211

In the conventional medical understanding, the focus has been on diseases, the same treatment methods have been applied to different individuals with the same disease, but the fact that the same disease will not be the same in different people and therefore the treatment cannot be the same has emerged. For this reason, individuals are more prominent than disease, and in this context, precision medicine concept has entered our lives with the developing technological infrastructure. Although the term precision medicine is relatively new, the concept has been a part of healthcare for many years. In the treatment of immunological diseases, sensitive medicine treatment strategies basically include three different treatment approaches. Replacement of ""missing molecules", inhibition of overactive intracellular signaling and cytokine blockade. Along with the treatment approaches which have been most frequently used in immunology clinics for many years such as IVIG, PEG-ADA and IFN-y, precision medicine also includes relatively rarer treatments that have been discovered lately such as pleriksafor, sirolimus and vedolizumab. Defining new molecular pathways with today's increasing technological infrastructure will undoubtedly bring the days when the concept of precision medicine will be used more effectively in the treatment of many diseases. Amaç: Retina ven oklüzyonu ile ilişkili maküler ödem tedavisinde deksametazon implantın 1 yıllık sonuçlarını değerlendirmeyi amaçladık. Gereç ve Yöntem: Bu retrospektif gözlemsel çalışmada santral retina ven oklüzyonu ve retina ven dal oklüzyonu ile ilişkili maküler ödemde hastanemize başvurmuş ve deksametazon implant tedavisi uygulanmış 47 hastanın (47 göz) dosyaları tarandı. Dosyalardan en iyi düzeltilmiş görme keskinliği, optik koherans tomografi ve göz içi basıncı ölçümü kayıtları retrospektif olarak başlangıç, 4,8,12,24,36 ve 48. haftalardaki vizitlerden temin edildi. Bulgular: Ortalama görme keskinliği (logMAR) 0,230,11 seviyesinden 0,640,18 seviyesine artış göstermiş ve 48. haftada bu değer 0,55 0,17 olarak kaydedilmiştir (p0,001). Ortalama santral maküler kalınlık değeri ilk ve 48. hafta vizitinde sırası ile 507,724,9 µm, 251,8 25,5 µm olarak tespit edilmiştir (p0,001). Başlangıçtaki ortalama göz içi basıncı 14,9 2,7, 24. haftada 15,3 3,3 ve 48. haftada 15,6 3,1mmHg olarak bulunuştur. İlk vizit ile diğer vizitler arası göz içi basıncı ölçüm değerleri arasında istatistiksel anlamlı fark bulunamamıştır(p0,05) Fakik olan ve olmayan hastalar ayrı ayrı değerlendirildiğinde görme keskinliği ortalamalarında 12. haftadan sonra fakik olmayangrup lehine fark saptanmıştır (p0,001).Sonuç: Klinik pratikte deksametazon implant uygulaması santral retina ven oklüzyonu ve retina ven dal oklüzyonu ile ilişkili maküler ödemde hem görme keskinliği hem de santral maküler kalınlıkta etkili bir düzelme sağlamıştır. Ayrıca deksametazon implant tolere edilebilir ve kabul edilebilir bir güvenlik profiline sahiptir. Objective: To evaluate one year results of dexamethasone implants in the treatment of macular edema related to retinal vein occlusion. Material and Method: This retrospective, observational study conducted in our hospital enrolled 47 patiens (47 eyes) diagnosed with macular edema following branch retinal vein occlusion or central retinal vein occlusion who were given dexamethasone implant. Patients were evaluated by the best corrected visual acuity, optical cohorens tomograph and intraocular pressure recordings at the first, 4,8,12,24,36 and 48th week visits retrospectively. Results: The mean best corrected visual acuity (logMAR) level increased from 0.23±0.11 to 0.64±0.18 in the 8th week and was enrolled 0.55±0.17 in the 48th week (p 0.05). Amaç: Yeni bir travmatik katarakt modeli oluşturmak ve nikotinamid adenin dinükleotid fosfat (indirgenmiş) oksidaz inhibitörü olan aposinin molekülünün travmatik katarakt üzerine olan etkisini incelemektir. Gereç ve Yöntemler: Çalışma için erişkin ve sağlıklı Yeni Zelanda cinsi tavşanlar kullanıldı. Yirmi bir tavşan eşit olarak üç gruba ayrıldı. 1. Grup: Kontrol grubu, 2. Grup: Santral 5 mm ön kapsülün künt spatül ile süpürülerek (polisaj) katarakt (perforasyonsuz) oluşturulup ilaçsız bırakılan grup, 3. Grup: Santral 5 mm ön kapsülün künt spatül ile polisaj yapılarak katarakt (perforasyonsuz) oluşturulup 21 gün boyunca intraperitoneal 20 mg/kg/gün aposinin verilen grup idi. Tavşanlara günlük olarak biyomikroskobik muayene yapıldı. Katarakt varlığı ve ilk oluşum zamanları kayıt edildi. Yirmi birinci gün kataraktlı bölge çapları ölçüldü. Ötanazi sonrası lens çıkarılarak kapsüler histopatolojik incelemeler yapıldı. Bulgular: Kontrol grubundaki hiçbir tavşanda katarakt oluşumu gözlenmez iken, 2 ve 3. Gruptaki bütün tav- şanlarda travmatik kataraktın 7. günde başladığı saptandı. Oluşan kataraktların 21. gündeki çapları 2. Grupta ortalama 7,60,5 mm, 3. Grupta ise ortalama 3,40,5 mm idi ve sonuç istatistiksel olarak anlamlıydı (p 0,0001). Sonuç: Günümüze kadar deneysel travmatik katarakt modelleri lens kapsülünün perfore edilmesi ile yapılıyordu. Bu çalışmada, ilk defa kapsül perforasyonu yapmadan travmatik katarakt modeli oluşturuldu. Kullanılan aposinin molekülünün katarakt gelişimini tam olarak engelleyemediği, ancak anlamlı şekilde katarakt progresyonunu azalttığı saptandı. Objective: To create a new traumatic cataract model and to evaluate the effect of Apocynin which is a reduced nicotinamide adenine dinucleotide phosphate oxidase intibitor on traumatic cataract formation. Material and Methods: Experiments were performed on healthy adult New Zealand rabbits. Twenty one animals were equally assigned to the following 3 Groups: Group 1: Control, Group 2: Central 5 mm anterior capsular area was polished to create cataract (nonperforated) by blunt spatula and monitored without apocynin. Group 3: Central 5 mm anterior capsular area was polished to create cataract (nonperforated) by blunt spatula and intraperitoneal 20 mg/kg/day apocynin was given for 21 days. The animals were examined daily. The presence of cataract and first appearance time of cataract was recorded. Cataract diameters were measured in 21. day. The animals were euthanized and lens was extracted for histopathological examinations. Results: There wasn’t any cataract formation in the control group. Cataract started in all animals in the Group 2 and Group 3 at 7. days. The mean diameters of cataract were 7.60.5 mm in the Group 2 and 3.40.5 mm in the Group 3 at 21. days. The difference was statistically significant (p 0.0001). Conclusion: Experimental models of traumatic cataract have been done by the lens capsule perforation until today. In our study, we created the model of traumatic cataract without capsule perforation for the first time. We found that apocynin couldn’t prevent cataract formation but provided a significant decrease in cataract progression.
