The objective of this study was to evaluate the influence of mesenchymal stem cells on the healing of experimental carotid artery anastomoses histopathologically. Twenty-four female Sprague-Dawley rats were used in this study. After random separation of the subjects into two groups, in both groups carotid arteries were transected and anastomosed in end-to-end fashion. Anastomoses were locally treated with 1 ml 0.09% NaCl, and 1 ml mesenchymal stem cell suspension (1×10(6) cells) in control and trial groups, respectively. Anastomoses were wrapped with an 8 mm sheet of surgicel and soaked with BioGlue in order to sequestrate the stem cells. After patencies were confirmed via Doppler USG, surgical site was closed with 2/0 silk sutures. Histopathological evaluation was carried out after 4 weeks. In respect to endothelial continuity, vessel patency (along with presence or absence of restenosis), integrities of internal and external elastic laminae, muscularis and adventitia; no statistically significant differences were present between the trial and control groups. In Trial and Control Groups, luminal thrombus was present in 8 (66.6%) and 3 (25%) of the 12 subjects, respectively. The difference was statistically significant (P < 0.05). Recanalization was present in 6 subjects in trial group; 1 subjects in Control Group, respectively. Our results suggest that local administration of mesenchyme stem cell does not have a positive influence on success of an anastomosis.

Intramedullary ependymomas are rare tumours but comprise the majority of intramedullary glial neoplasms in the adult. In this case report we introduced a 51 year old male patient with one month history of pain in the right arm and numbness in the right hand fingers. At neurological examination,there was a slight loss of sensory modalities along the right C6,C7,C8 dermatoms . Motor examination showed grade IV/V power in the right elbow flexion.Biceps and triceps tendon reflexes were diminished . In the sagittal Magnetic Resonance Images (MRI) there were a santrally localized mass lesion extending from C5 to T1 vertebra level and expanding the cord. There were also a milimetric syringomyelia cavity extending between T2-T4 vertebra .The tumour was removed totally . Pathological diagnosis was compatible with grade II ependymoma (World Health Organization Central Nervous System Tumour Classification 2007).No radiation therapy was advised to the patient. These tumours are benign, slow-growing lesions which are optimally treated with gross-total surgical resection without adjuvant therapy. Postoperative functional outcome is related to preoperative functional status. Hence, early diagnosis prior to symptomatic progressionis critical to the successful treatment of these tumours.

The aim of this experimental study was to evaluate the effect of omega-3 fatty acids solution in the prevention of the forming of peridural fibrotic tissue and adhesion in a rabbit hemilaminotomy model. This new experimental model was previously developed by the first author of this study. Twenty-one New Zealand white rabbits were used for this experiment. Seven of them were randomly selected as Control group and was named as Group I rabbits. The rabbits in this group were not operated and treated. The second group was Group II including 7 rabbits. This group was named as hemilaminotomy and untreated rabbits. Group III included 7 rabbits and named as Omega-3 treated group after hemilaminotomy procedure. In this group (Group III), 250 mg omega-3 solution was put in the hemilaminotomy area before closing the skin and fascia. Four weeks later from the operation rabbits were sacrificied by perfusion with 10% neutral buffered formalin solution. The lumbar spines were removed and immersed in 10% neutral buffered formalin for approximately 24 hours. Then each specimen was decalcified in 5% formic acid for approximately 3 weeks. Specimens were cut coronally for gross inspection. Peridural scar tissue on gross inspection in coronal sectioned specimens was graded into three grades. Grade I epidural scar tissue was scored with 1 point, Grade II with 2 point and Grade III with 3 point. The average score of hemilaminotomy sites without treatment was found as 2, 6 ± 0, 51. In the other hand the average score of hemilaminotomy sites with omega-3 fatty acids treatment was found as 1, 9 ± 0, 56. The difference between the treatment and non-treatment group was statistically significant. Omega-3 fatty acids solution seems an effective treatment modality in the prevention of peridural excessive scar tissue formation in the rabbit hemilaminotomy model.

The purpose of this study was to compare human fibrinogen-thrombin-based liquid dural graft; Beriplast® (Behring, Malburg, Germany) and collagen-based dural graft; Tissudura® (Baxter, Heidelberg, Germany) in terms of efficiency, side effects and complications. Thirty Spraque Dawley rats were used in this experimental study. A burrhole was opened on the left parietal bone of each subject and experimental dural defect was created. While 10 subjects were in sham group without any dural defect repair, dural defect was repaired by Beriplast in 10 subjects, by Tissudura in 10 subjects. After twenty-one day follow-up, edema, gliosis and inflammatory cell infiltration in the parenchyma, foreign body reaction in the bone, fibrosis in the epidural space and dura were evaluated histopathologically. Beriplast caused much more severe inflammation on cortex. When we compared Tissudura group with the sham group in terms of parenchymal edema and gliosis, the difference was not significant. On the other hand, we have found a significant increase in cortical parenchymal edema in Beriplast group. The last generation dural grafts result in different degrees of the tissue reaction. Severe inflammatory reaction can provide more satisfactory results in terms of watertight dural closure but on the other hand, the same reaction can be a disadvantage to the surrounding tissue.

Meme kanserinin en sık metastaz yaptığı yer kemiktir. Kemik metastazlarının da en sık yerleşim yeri vertebradır. Sıklıkla torakal ve lomber vertebralara metastaz yapar. Üst servikal metastazlar nadir görülürler fakat önemli nörolojik yapılara komşuluğu nedeniyle ölümcül komplikasyonlara yol açabilirler. Erken tanı ve tedavi hastanın sağkalım süresini ve yaşam kalitesini arttırması açısından önemlidir. Üst servikal metastazların nadir görülmesi ve ölümcül komplikasyonlara neden olabilmesi nedeniyle bu olgunun sunulması amaçlanmıştır.

Lomber disk hernisi, bel ağrısı ve radiküler ağrının yaygın bir nedenidir. Hastaların çoğu konservatif tedaviden fayda görür ve günlük yaşantısına dönebilir. Görüntüleme yöntemlerindeki gelişmeler ve kullanım sıklığının artması nedeniyle, herniye diskin gerilediği literatürde bildirilmiştir. Fakat hangi özellikteki disklerin ve ne kadar süre içerisinde gerilediği hakkında kesin elde edilmiş bulgular yoktur. Dört ay gibi bir sürede sekestre disk hernisinin regrese olması nedeniyle olgumuzu literatürler eşliğinde sunmayı uygun bulduk.

The primary treatment of malignant glial tumors is surgical intervention for maximally reducing and/or total removing of tumors. The purpose of this report was to describe pial sinangiosis or indirect nonanastomotic bypass surgery for diversion of blood brain barrier for enhancing of systemic blood-brain tissue penetration in the enhancing of the effect of chemotherapy. The other purposes are the increasing of oxygen and metabolic product providing, enhancing of radiation sensitivity, and stabilization of brain tissue around the tumor bed for reducing of malignant transformation. We used the technique of indirect non anastamotic by-pass surgery technique during the surgical treatment of malignant glial tumors. Preoperative and postoperative magnetic resonance images were obtained to evaluate peri-lesional edema and residual complications. This technique was used in three cases of malignant glial tumors. There were no operative mortality and morbidity related to this technique. This technique may be used during the surgical treatment of malignant glial tumors to enhance penetration of chemotherapeutic agents, increasing of radiation sensitivity and providing of tissue stability. VI. ÖZET SAK'tan sonra bazal serebral arterlerdeki fokal, segmental veya difîuz daralma serebral vazospazm olarak tanımlanabilir. Bazal serebral arterlerdeki daralma sonucu bölgesel veya global serebral kan akımı normal seviyelerin altında kalmaktadır. Anjiografik vazospazm saptanan olguların yaklaşık olarak yarısında geç iskemik nörolojik defisit görülmesiyle vazospazm manifest hale geçer, bu iskemik defisit ya düzelmekte veya kalıcı serebral infarktüse progresyon göstermektedir (2). Serebral vazospazm teşhis ve tedavideki ilerlemelere rağmen özellikle anevrizmal SAK'lı hastalarda morbidite ve mortalitenin en büyük sebebi olarak durmaktadır (2). Serebral vazospazmın etyopatogenezi multifaktöriyel orijinlidir (2). Temel patojenik ajan oksihemoglobindir (13). Bu madde serebral vazospazmda önemi olan pek çok değişik olaym mekanizmasında rol oynamaktadır (14). SAK'tan sonra prostaglandin metabolizmasında değişme, permeabilitenin artması, endotel bağımlı relaksasyon faktörlerinin salınımının azalması ve güçlü vazokonstrüksiyon yapıcı etkilerinin olduğu bilinen endotelinlerin sentezlenerek BOS içerisine salınması başlıca vazospazm nedenleridir (13). Etyopatogenezi tam olarak aydınlatılamayan serebral vazospazmın tedavi side palyatif sınırlar içerisinde kalmıştır. Henüz serebral vazospazmı tam olarak önleyen tedavi modaliteleri geliştirilememiştir. Bu çalışmada, tavşan deneysel subaraknoid kanama modelinde, vazospazmın önlenmesi amacıyla sentetik bir somatostatin analoğu olan oktreotid asetat kullanıldı. Somatostatin etkisini, beynin değişik bölgeleri, araknoid, piamater, ön hipofiz, pankreas, gastrointestinal sistem mukozası ve immün sistem hücrelerinde bulunan spesifik membran reseptörleri aracılığı ile oluşturmaktadır (9). Somatostatin growth hormon, growth faktörler ve insulin sekresyonunu inhibe eder (9). Gastrointestinal sistem hormonlarının üretim ve salınımını azaltmasının yanında güçlü immünomodülatör etkisi de bulunmaktadır (9, 10). Deneysel çalışmalarda değişik derecelerde tümör büyümesini inhibe ettiği bildirilmiştir. Somatostatin^ bu etkisi direkt olarak growth faktörlerin inhibisyonuna bağlanmaktadır (9, 10). Oktreotid asetat somatostatinden daha güçlü ve 48daha uzun etkili, sentetik somatostatin analoğudur. Somatostatin gibi sadece infuzyon yoluyla değil, subkutan yolla da uygulanabilmektedir (9). Bizim çalışmamızda somatostatin analoğu olan oktreotid asetat'ın deneysel çalışmalarda bildirilen iki etkisinden yararlanılarak vazospazmın şiddetinin azaltılması amaçlanmıştır. Bunlardan birincisi endotel hücrelerince endotelinlerin sentezinin growth hormon ve growth faktörlerin inhibisyonu yoluyla azaltmak, diğeri ise immünomodülatör etkisinden yararlanarak immün kompleksler (immünglobulin ve kompleman) ve sitokinlerin (2, 1 1) neden olduğu immünolojik reaksiyonların kontrol altma alınmasını sağlamaktır. Bu çalışmada oktreotid asetat tedavisi, tavşanlarda vazospazmın şiddetini azaltıcı etki göstermiştir. Düşük doz (150 ug/gün) oktreotid asetat ile tedavi edilen tavşanlar, tedavi uygulanmayan SAK'lı tavşanlarla karşılaştırıldığında serebral vazospazmın şiddetinde %39.9 iyileşme gözlenmiştir (p=0,022). Yüksek doz (600 ug/gün) oktreotid asetat ile tedavi edilen tavşanlarda ise vazospazmın şiddetinde %56.7'lik iyileşme gözlenmiştir (p=0,035). ÖZETAmaç. Bu çalışmada ratlarda deneysel subaraknoid kanama (SAK) modelinde endojenpolipeptit hormon olan leptinin topikal (göz damlası) uygulanmasının serebral vazospazmüzerine etkisinin incelenmesi amaçlanmıştır.Gereç ve Yöntem. Çalışmamızda toplam 40 adet Spraque-Dawley cinsi dişi rat kullanıldı.Ratlar randomizasyon ile 5 gruba ayrıldı. 1.Grup hariç 2, 3, 4 ve 5. Grup ratlara genelanestezi altında oksipitoservikal bileşkeye düz bir insizyon yapılıp oksipital kasları ekarteedilip atlantooksipital membranları ortaya kondu. 25 gauge iğne ile atlantooksipitalmembrandan sisterna magnaya ulaşıldı. Yaklaşık 0.1 ml beyin omurilik sıvısı (BOS) direneedildikten sonra ratların femoral arterinden alınan nonheparinize otolog arterial kan (0.1ml)yavaşça sisterna magna içerisine enjekte edildi. Tedavi gruplarından Grup 3' teki ratlara 3gün boyunca, Grup 5' teki ratlara ise 7 gün boyunca 24 saatte bir aynı göze 1.2mg/ml leptinsolusyonundan 10 ?l topikal (göz damlası) olarak verildi. 2 ve 4. Gruplara herhangi bir tedaviuygulanmadı. Grup 2 ve 3' teki ratlar 3. gün, Grup 4 ve 5` teki ratlar 7. gün genel anestezialtında intrakardiyak 3 ml kan alınıp perfüzyon fiksasyon işlemine tabi tutularak sakrifiyeedildi.Bulgular. Tüm grupların serum leptin ortalamaları karşılaştırıldığında kontrol grubu serumleptin düzeyi ile topikal leptin tedavi uygulanan Grup 3 ve Grup 5'in serum leptin düzeyleriarasında tedavi uygulanan gruplar lehine istatistiksel anlamlı fark görüldü (p0.05). Grup 5'in Grup 4 ile karşılaştırılmasında baziller arter lümen alanıölçümlerinde vazodilatasyon lehine anlamlı istatistiksel fark tespit edilirken (p0.05). Tüm grupların tunica mediatabakaları karşılaştırıldığında Grup 5 ile hem Grup 1 hemde Grup 2 arasında istatistikselanlamlı fark tespit edildi (p 0.05). Bu çalışmanın amacı, deneysel tam kat arter kesisi sonrası uç uca uygulanan arter anastomozunda mezenkimal kök hücrenin iyileşmeye olan etkisinin histopatolojik olarak değerlendirmektir.Çalışmada 24 adet dişi Spraque-Dawley cinsi rat kullanıldı. İki gruba rastgele ayrılan denekler aynı cerrah tarafından tam kat karotis arter kesisine anastomoz uygulandı. Kontrol Grubunda anastomozun üzerine 1ml serum fizyolojik, Deney Grubuna ise 1 ml (1x106 hücre) mezenkimal kök hücre lokal olarak uygulandı. Anastomoz hattı ortada kalacak şekilde 8mm uzunluğunda surgicel anastomoz hattını çepeçevre saracak şekilde serildi. Surgicel'in üstüne sıvama tarzında BioGlue uygulandı. Böylece mezenkimal kök hücrenin anastomoz etrafında hapis edilmesi sağlandı. Bu uygulama esnasında damarın sistol ve diyastol esnasında rahat hareket edip etmediği gözlendi. Damarın basınç altında olmadığı anlaşıldıktan sonra cerrahi alan 2/0 ipek sütür ile kapatıldı.Yaptığımız histopatolojik çalışma anastomoz iyileşme süresi göz önünde tutularak dört hafta sonra gerçekleştirilmiştir. Mezenkimal kök hücre uygulanan anastomoz hattında; endotel devamlılığı, damar lümeninin açıklığı (neointima hiperplazisine bağlı restenoz varlığı), internal ve eskternal elastik lamina, musküler tabaka ve adventisyanın iyileşme kalitesi istatiksel olarak karşılaştırıldığında kök hücre uygulanan grup ile uygulanmayan grup arasında anlamlı fark olmadığı ortaya konuldu. Her iki grup damar içinde trombüs açısından karşılaştırıldığında ise mezenkimal kök hücre uygulanan 12 denekten 8'inde (%66,6) trombüs gelişirken, kök hücre uygulanmayan 12 denekten 3'ünde (% 25) trombüs gelişmiştir. Bu çalışmanın amacı, deneysel siyatik sinir hasarında mezanşimal kök hücre kullanımının fonksiyonel ve histopatolojik etkilerini incelemektir. Çalışmada, toplam 32 adet, dişi, Spraque-Dawley cinsi rat kullanıldı. Sekiz rat, elektrofizyolojik çalışmalarda referans olması amacıyla siyatik sinirde hasar oluşturulmadan, sadece normal sinir kayıtları alınması için kullanıldı (Kontrol Grubu). Deney aşamasında ise toplam 24 adet rat kullanıldı. On iki ratta, siyatik sinir tam kat kesisi sonrası kök hücre uygulanmadan (Sham Grubu); 12 ratta ise siyatik sinir tam kat kesisi sonrası kök hücre uygulanarak (Kök Hücre Grubu) anastomoz yapıldı. Deneklerde sol femura paralel 3 cm'lik insizyon yapılarak, uyluk fleksör ve ekstansör grup kasları arasından diseksiyonla derinleşilerek siyatik sinire ulaşıldı. Sham Grubundaki 12 ratın siyatik siniri mikromakasla kesildikten sonra 8/0 prolenle uç uca anastomoz yapıldı ve anastomoz hattına surgicel ve bioglue uygulandı. Kök Hücre Grubunda da siyatik sinir kesisi sonrası uç uca anastomoz yapıldı, sonra anastomoz hattına mezanşimal kök hücre uygulandı ve ardından kök hücreyi ortamda tutmak için bioglue ve surgicel uygulandı. Çalışmadan 8 hafta sonra tüm deneklerde elektrofizyolojik çalışmalar yapıldı ve ardından denekler dekapite edildikten sonra siyatik sinirleri, anastomoz hattını içerecek şekilde çıkarıldı, patolojik incelemeler için formole kondu. EMG hızı ve sinir ileti hızları ile histopatolojik olarak Wallerian dejenerasyon, nöroma oluşumu ve skar oluşumu incelendi. Histopatolojik olarak Sham Grubu ve Kök Hücre Grubu arasında anlamlı fark saptanmadı. Sinir ileti hızları, Kök Hücre Grubu ile Sham Grubu arasında ANOVA testi ile karşılaştırıldı ve p=0,001 olarak saptandı. Kök Hücre Grubunda, Sham Grubuna göre sinir ileti hızlarında anlamlı fark saptandı. Çalışmamızın, mezanşimal kök hücrenin, sinir ileti hızını arttırarak fonksiyonel rejenerasyonu iyileştirdiğinin saptanması yönünden literatüre katkıda bulunacağını düşünmekteyiz. The aim of this study was to research the functional and histopathological effects of mesenchymal stem cell usage on experimental sciatic nerve injury. In this study we used totally 32, female Spraque-Dawley rats. Eight rats were used to be a reference for electrophysiological measurements and used for normal nerve records and didn?t go under sciatic nerve injury (Control Group). For experiments, 24 rats were used. Twelve rats? sciatic nerve were cut and anastomosis was applied without stem cell (Shame Group). The next 12 rats? sciatic nerve were cut and anastomosis was applied with stem cell (Stem Cell Group). Sciatic nerves of rats in shame and Stem Cell Group were exposed between flexor and extansor group muscles after a 3 cm skin insicion parallel to left femur of rats. In Shame Group, 12 rats? sciatic nerves were cut by microscissors and end to end anastomosis was applied by 8/0 prolen suture and then surgicel and bioglue were applied to anastomosis line. In Stem Cell Group, sciatic nerves were cut by microscissors and end to end anastomosis was applied by 8/0 prolen suture and then mesenchymal stem cell was applied to anastomosis line before surgicel and bioglue. Bioglue and surgicel were applied for keeping stem cell on anastomosis line. After 8 weeks from operation, all rats were used for electrophysiological measurements. After that, rats were decapitated and sciatic nerves were excised as including anastomosis area and were put in formalin for pathological researchs. EMG velocity, nerve conduction velocity and as histopathological parameters neuroma formation, Wallerian degeneration and scar formation were evaluated. There was not a significant difference between shame and Stem Cell Group for histopathological parameters. Nerve conduction velocities were compared by ANOVA test in shame and Stem Cell Groups and p value was 0,001. There was a significant increase in nerve conduction velocity in Stem Cell Group according to Shame Group. Usage of mesenchymal stem cell increased nerve conduction velocity and caused a functional regeneration; so our study can contribute to literature by this result. 80

Giriş: duramater, santral sinir sistemini dış etkilerden koruyan, BOS çıkışına izin vermeyen en kalın beyin zarıdır. Beyni kranium içinde kompartmanlara ayırıp BOS içinde dengede kalır.Duraplasti; primer olarak süture edilemeyen duramaterin greftlerle kapatılmasıdır. Yetersiz duraplasti yapılması durumlarında BOS fistülleri, enfeksiyonlar, psödomeningoseller, uzamış hospitalizasyon süresi vb. Komplikasyonlar görülebilir. Bu komplikasyonlara bağlı olarak artmış morbidite ve hatta mortalite görülebilir. Modern nöroşirurji kliniklerinde bu komplikasyonların önüne geçebilmek adına duramaterin primer süturasyonuna en yakın, su geçirmez bir şekilde duraplasti ile kapatma ihtiyacı hep önemli bir sorun olmuştur. Biz çalışmamızda mevcut olan birçok; otolog, heterolog, sentetik vb. Duramater greftlerinden, en popüler olan son kuşak insan fibrinojen-trombin bazlı, likit dura grefti olan beriplast® ve kollajen bazlı tissudurayı® etki ve yan etki anlamında kontrol grubu ve birbiriyle karşılaştırdık.Materyal metod; otuz adet spraque dawley cinsi rat kullanıldı. Deneklerin sol parietal kemiklerine burholle açıldı. Deneysel dura defekti oluşturuldu. On adet denekte beriplast, 10 adet denekte tissudura, 10 adet denekte sham grubu olarak duraplasti uygulandı. Yirmibir günlük takibin ardından histopatolojik olarak; parankimde ödem, gliozis, enflamatuar hücre infiltrasyonu, kemikte yabancı cisim reaksiyonu, dura ve epidural bölgede fibrozis değerlendirildi.Bulgular: beriplast ve tissudura grubunda, kontrol grubuna göre enflamatuar hücre infiltrasyonu ve fibrozis yönünden olumlu yönde istatiksel anlamlı farklılık tespit edildi. Gliozis, kortikal parankimal ödem ve hücre infiltrasyonu yönünden olumsuz yönde istatiksel anlamlı fark saptandı.Sonuç: primer dural süturasyonun yapılamadığı, doku kayıplarının olduğu ya da yeterli cerrahi ekspojur sağlanamadığı durumlarda; en modern duramater grefti olan son kuşak fibrinojen ve trombin bazlı likid dura greftleri bile aynı grupta olmalarına rağmen, etki ve yan etki bakımından birbirlerinden farklı davranmaktadırlar.Anahtar kelimeler; BOS fistülü, duramater, likit dura grefti Introduction: durameter is the thickest brain membrane that protects the central nervous system and doesn?t allow CSF outlet. It separates the brain to compartments in the cranium and holds it in balance within the CSF.Duraplasty; is the closure of the durameter which couldn?t have been primarly sutured by grafts. When the duraplasty is insufficient, some complications like CSF fistules, infections, pseudomeningoceles, prolonged hospitalization can be seen. Increased morbidity, further mortality could be seen related to these complications. In the modern neurosurgery clinics, for the sake of preventing these complications; the necessity of dural closing with a water-resistant duraplasty on the nearest to the primary suturing of durameter has always been the most important problem. In our study, among the autologous, heterologous, synthetic duramater grafts; we compared the beriplast® and tissudura® that are the most popular last generation fibrinogen-thrombin and collagen based synthetic dura grafts respectively with control groups and each other.Material and method: 30 spraque dawley rats were used for this study. Burholle was opened to subjects? left parietal bones. The experimental dura defect was comprised. Whereas beriplast was used in 10 subjects, tissudura was used in 10 subjects and duraplasty was performed as shum group to 10 subjects. After a 21-day follow; the parenchymal edema, gliosis, inflammatory cell infiltration, bone foreign reaction, fibrosis in dura and epidural area were evaluated as histopathologically.Results: a possitively stastistically significant difference was determined in beriplast and tissudura group in terms of inflammatory cell infiltration and fibrosis comparing to the control group. Also a negatively statistically significant difference was determined in terms of gliosis, cortical parenchymal edema, cortical parenchymal cell infiltration.Conclusion: ın the conditions as primary dural suturation couldn?t have been performed, presence of tissue loss or the sufficient surgical exposure couldn?t have provided; although they are in the same group, even the last generation fibrinogen&thrombin based liquid dura graft and collagen based synthetic dura grafts that are the most modern durameter grafts behave differently from each other in terms of effect and side-effect.Keywords; CSF fistule, duramater, liquid duragraft 44
