OBJECTIVE: Coronavirus disease 2019 (COVID-19) has affected the whole world and caused the death of more than 6 million people. The disease has been observed to have a more severe course in patients with chronic lung diseases. There are limited data regarding COVID-19 in patients with bronchiectasis. The aim of this article is to investigate the course of COVID-19 and factors affecting the clinical outcome in patients with bronchiectasis. MATERIAL AND METHODS: This study was conducted using the Turkish Adult Bronchiectasis Database (TEBVEB) to which 25 centers in Türkiye contributed between March 2019 and January 2022. The database consisted of 1035 patients, and COVID-19-related data were recorded for 606 patients. RESULTS: One hundred nineteen (19.6%) of the bronchiectasis patients (64 female, mean age 57.3 ± 13.9) had COVID-19. Patients with bronchiectasis who developed COVID-19 more frequently had other comorbidities (P = .034). They also more frequently had cystic bronchiectasis (P = .009) and their Bronchiectasis Severity Index was significantly higher (P = .019). Eighty-two (68.9%) of the patients who had COVID-19 were followed up in the outpatient clinic, 27 (22.7%) in the inpatient ward and 10 (8.4%) patients in the intensive care unit. There tended to be a higher percentage of males among patients admitted to the hospital (P = .073); similarly, the mean age of the patients admitted to the hospital was also higher (60.8 vs 55.8 years for the outpatients), but these differences did not reach statistical significance (P = .071). CONCLUSION: In conclusion, this study showed that severe bronchiectasis, presence of cystic bronchiectasis and worse Bronchiectasis Severity Index are associated with the development of COVID-19, but not with the severity of infection. Background: Bronchiectasis is a chronic lung disease characterized by permanent bronchial wall dilatation. Although it has been known as an orphan disease, it has recently gained attention because of registry-based studies and drug research. Aims: We aimed to use a multicenter database to analyze and compare data regarding the etiology, associated comorbidities, microbiological characteristics, and preventive strategies of bronchiectasis in Türkiye to those of other countries. Study Design: A multicenter prospective cohort study. Methods: The multicenter, prospective cohort study was conducted between March 2019 and January 2022 using the Turkish Adult Bronchiectasis Database, in which 25 centers in Türkiye participated. Patients aged > 18 years who presented with respiratory symptoms such as cough, sputum, and dyspnea and were diagnosed with non-cystic fibrosis bronchiectasis using computed tomography were included in the study. Demographic information, etiologies, comorbidities, pulmonary functions, and microbiological, radiological, and clinical data were collected from the patients. Results: Of the 1,035 study participants, 518 (50%) were females. The mean age of the patients was 56.1 ± 16.1 years. The underlying etiology was detected in 565 (54.6%) patients. While postinfectious origin was the most common cause of bronchiectasis (39.5%), tuberculosis was identified in 11.3% of the patients. An additional comorbidity was detected in 688 (66.5%) patients. The most common comorbidity was cardiovascular disease, and chronic obstructive pulmonary disease (COPD) and bronchiectasis was identified in 19.5% of the patients. The most commonly detected microbiological agent was Pseudomonas aeruginosa (29.4%). Inhaled corticosteroids (ICS) were used in 70.1% of the patients, and the frequency of exacerbations in the last year was significantly higher in patients using ICS than in nonusers (p < 0.0001). Amaç: Bu çalışmada Covid-19 sebebi ile hastanede yatarak tedavi gören ve şifa ile taburcu olan hastaların duygu durumlarının ve post-travmatik stres bozukluğunun covid-19 ile ilişkisinin araştırılması amaçlanmıştır. Materyal-Metotlar: Çalışmaya 15 Mart 2020-30 Ekim 2020 tarihleri arasında Akdeniz Üniversitesi Tıp Fakültesi Hastanesi’ne Covid-19 tanısı ile yatıp şifa ile taburcu olan hastalar alındı. Hastalara taburcu olduktan en az 30 gün sonra telefon ile ulaşılıp, postravmatik stres bozukluğu (PTSB) soru listesi-sivil versiyonu (PCL-5), Beck anksiyete ve depresyon ölçekleri uygulandı. Telefon ile cevap vermek istemeyen ancak taburculuk sonrası poliklinik kontrolüne gelen hastalarda ise bu anketler yüz yüze uygulama şeklinde yapıldı. Anket uygulamaları araştırmacılar tarafından yapıldı. Çalışmaya oral ya da nazal sürüntü sonucu covid PCR pozitif olan hastalar dahil edildi. Bulgular: Akdeniz Üniversitesi Hastanesi Pandemi Kliniğine 15 Mart 2020-30 Ekim 2020 tarihleri arasında 385 hastanın yatarak tedavi gördüğü tespit edildi. Bu hastalardan %55,8 (215)’ine telefon ile ulaşıldı. Çalışmaya ortalama yaşı 54,8±14,5 olan 131 (%60,9) erkek, 84 (%29,1) kadın hasta dahil edildi. Çalışmaya dahil edilen hastalar taburcu olduktan ortalama 87,9±43,4 gün sonra çalışmaya alınmıştır. Hastaların ilk tanı anında en sık şikayetleri, ateş (%26,5), dispne (%18,6) ve öksürük (%13,5) iken, çalışmaya alındığı dönem hastaların %38,1 (82)’inde rezidü semptomlar vardı. Hastaların hastanede yatış süresi ortalama 10,8 gün iken, yoğun bakım ihtiyacı olanların yoğun bakımda ortalama yatış süresi 9,7 gün olarak saptandı. Beck depresyon ve anksiyete skorları için cutt –off değer olarak 16 puan alındığında sırasıyla depresyon görülme oranı %6,6, anksiyete görülme oranı %13,1 tespit edilmiştir. PCL-5 için 39 cutoff olarak alındığında PTSB %7 oranında saptanmıştır. Çalışmaya alındığı dönem (taburculuktan ortalama 87,9 gün sonra) rezidü şikayeti olan hastaların hem depresyon skoru (p, Aim: The present study aimed to examine mood disorders in patients discharged from the hospital due to Coronavirus Disease-19(COVID-19). Material-Methods: The study included patients who were admitted to Akdeniz University Medical Faculty Hospital with the diagnosis of COVID-19 and discharged between March 15, 2020 and October 30, 2020. Patients were contacted by phone at least 30 days after discharge, and posttraumatic stress disorder (PTSD) questionnairecivil version (PCL-5), Beck anxiety and depression scales were administered. In patients who did not want to answer by phone but came to the outpatient clinic after discharge, these questionnaires were administered face-to-face. Survey applications were made by the researchers. Patients with positive covid PCR as a result of oral or nasal swab were included in the study. Results: There were 385 individuals treated as inpatients in the Akdeniz University Hospital COVID-19 Referral Clinic between March 15, 2020 and October, 30 2020. We were able to contact 215 (55.8%) of these patients. The median age was 56 years among the patients reached and 60.9% (131) were male. The study patients were included in the study after a mean duration of 87.9±43.4 days following discharge. The most common complaints of the patients reported at COVID-19 diagnosis were fever (26.5%), dyspnea (18.6%), and cough (13.5%), while 38.1% (82) of the patients had residual symptoms at the time of enrollment. The length of hospital stay of the patients was 8 (1–90) days, and the mean length of stay in the intensive care unit for those requiring intensive care was 9.7 days. The newly diagnosed patients were not receiving any treatment before the recruitment. Their general characteristics, the combined GOLD 2011 COPD categories and exacerbation histories were noted. The patients were followed up with 3 voluntary visits for 1 year. Their adherence to the inhaled treatment according to GOLD 2011 was evaluated during follow-up visits. Results: The study included 776 COPD patients. Their mean age was 59.4 +/- 9.1 years, and 11.9% of the patients were female. 35.1% of the patientswere in the GOLD 2011 C and D category. 12.6% are frequent exacerbators, and 52.8% had at least one comorbid condition. 71.8% overtreatment rate was detected. Their attendance rates for three follow-up visits became 55.9%, 32.9% and 18.7%, respectively. The adherence rate to the treatment was measured as 81.9%. Conclusion: Although these patients were diagnosed for the first time, the GOLD C and D categories and frequent exacerbator phenotype were found at a high rate. They were usually prescribed an overtreatment regimen. We think that newly diagnosed COPD patients should be evaluated carefully, and best effort should be made to treat these patients in accordance with the recommendations of the major COPD guidelines. Background/aim: Phase III trials have demonstrated a significant efficacy and an acceptable safety for pirfenidone in patients having mild to moderate idiopathic pulmonary fibrosis (IPF). Real-life data on the use of pirfenidone 200 mg tablets are limited. This study aimed to investigate the efficacy and safety of pirfenidone 200 mg tablets for the treatment of IPF in a real-life setting. Materials and methods: A retrospective, multicenter study conducted in four university hospitals in Turkey between January 2017 and January 2019. Clinical records of patients diagnosed with mild to moderate IPF and receiving pirfenidone (200 mg tablets, total 2400 mg/day) were reviewed retrospectively and consecutively. Pulmonary function measurements including forced vital capacity (FVC%) and diffusing capacity of the lungs for carbon monoxide (DLCO%) were analyzed at baseline and after 6-month of pirfenidone treatment. Descriptive statistics were expressed as mean, standard error or median (minimum-maximum), number and percentage, where appropriate. Results: The study included 82 patients, of whom 87.8% were males (mean age, 66 years). After 6-month of treatment, 7 patients discontinued the treatment. Of the remaining 75 patients, 71 (94.6%) remained stable, 4 (5.4%) had progressive disease as evident by a decline in the FVC% of at least 10% while on treatment, and 45 (61.3%) had improved cough. At least one adverse event (AE) associated with the treatment was observed in 28 (37.3%) patients. Conclusion: Pirfenidone 200 mg was effective and well tolerated and associated with relatively mild and manageable AEs in IPF patients. [ABSTRACT FROM AUTHOR]

OBJECTIVE: To evaluate the rate of pneumococcal pneumonia (PP) among patients with community-acquired pneumonia (CAP) in Turkey and to investigate and compare features of PP and non-PP CAP patients. MATERIAL AND METHODS: This multicenter, non-interventional, prospective, observational study included adult CAP patients (age > 18 years). Diagnosis of PP was based on the presence of at least 1 positive laboratory test result for Streptococcus pneumoniae (blood culture or sputum culture or urinary antigen test [UAT]) in patients with radiographic findings of pneumonia. RESULTS: Four hundred sixty-five patients were diagnosed with CAP of whom 59 (12.7%) had PP The most common comorbidity was chronic obstructive pulmonary disease (30.1%). The mean age, smoking history, presence of chronic neurological disease, and CURB-65 score were significantly higher in PP patients, when compared to non-PP patients. In PP patients, 84.8% were diagnosed based ony on the UAT. The overall rate of PP patients among CAP was calculated as 22.8% considering the UAT sensitivity ratio of 63% (95% confidence interval: 45-81). The rate of intensive care treatment was higher in PP patients (P = .007). While no PP patients were vaccinated for pneumococcus, 3.8% of the non-PP patients were vaccinated (P = .235). Antibiotic use in the preceding 48 hours was higher in the non-PP group than in the PP group (31.8% vs. 11.1%, P = .002). The CURB-65 score and the rate of patients requiring inpatient treatment according to this score were higher in the PP group. CONCLUSION: The facts that PP patients were older and required intensive care treatment more frequently as compared to non-PP patients underline the burden of PP. [ABSTRACT FROM AUTHOR]

Background/aim: There has been growing interest in the use of serum procalcitonin (PCT) and C-reactive protein (CRP) in patients with community-acquired pneumonia (CAP). the aim of this study was to investigate whether an assessment of fever, leukocyte count, and serum CRP and PCT levels on admission and during follow-up (day 3) provides any information about the clinical outcome in hospitalized patients with CAP. Materials and methods: Patients with a diagnosis of CAP who were admitted to and followed at four university hospitals were evaluated retrospectively using the Turkish Thoracic Society Pneumonia Database. Results: A total of 103 hospitalized CAP patients (57 males, mean age: 61.5 ± 16.7 years) were enrolled in the study. Treatment failure (TF) was observed in 20 patients (19.4%). Pneumonia Severity Index scores, serum CRP levels, and PCT levels on admission were significantly higher in the TF group. There were significant decreases in CRP and PCT levels between admission day and day 3 in the treatment success group. Conclusion: in patients with CAP, the body temperature and leukocyte count on admission do not predict outcome. Monitoring levels of CRP and PCT may be useful as a predictor of treatment outcome.

- Abstract
Gerekçe: Mesleksel Beceriler Laboratuarında maket üzerinde arteryel kan alma eğitimi gören öğrencilerin, bu eğitim yönteminden memnuniyet derecelerini ve hasta üzerindeki uygulamalarında bu eğitimin katkısına yönelik düşüncelerini öğrenmek amaçlandı. Yöntem: Beşinci sınıftan 94 öğrenciye, mesleksel beceriler laboratuarında maket üzerinde radyal arterden kan alma becerisi öğretildi. Eğitim sonrasında öğrenciler, geribildirim formları doldurarak eğitim yöntemi, eğitim ortamı, kullanılan malzemeler ve eğiticiyi 1 ile 5 arasında puanlar vererek değerlendirdiler. Aynı öğrencilerin altıncı sınıftayken ulaşılabilen 86 tanesine anket formları doldurtularak hasta üzerindeki deneyimlerinde maket eğitiminin olumlu katkısı olup olmadığı sorgulandı. Bulgular: Eğitimden hemen sonra doldurulan geri bildirim formlarındaki genel ortalama puan 5.0 üzerinden 4,83 olup, en yüksek ortalama puan eğiticiyle (4,94), en düşük ise kullanılan malzemelerle (4,66) ilgiliydi. Altıncı sınıfta doldurulan anket formlarına göre, öğrencilerin %68,8’i önceden maket üzerinde beceriyi öğrenmenin, klinikteki deneyimlerinde olumlu katkısı olduğunu düşünüyordu. Sonuç: Öğrenciler radyal arterden kan alma becerisinin maket üzerinde öğretilmesinden memnun görünmekte ve çoğunluğu bu eğitimin klinik deneyimlerinde olumlu katkı sağladığını düşünmektedir.

Objective: Obstructive sleep apnea syndrome (OSAS) is associated with increased cardiovascular morbidity and mortality. Red cell distribution width (RDW), a measure of the variability in size of circulating erytrocytes, has been reported to be a risk marker of morbidity and mortality for cardiovascular disease. Therefore, we have investigated the possible association between OSAS and RDW. Material and Methods: We selected 30 mild, 32 moderate and 31 severe OSAS patients and 31 healthy control subjects matched for age, sex, and body mass index. RDW was measured by using an automated blood cell counter. Results: The RDW values were significantly higher in patients with severe OSAS than in the control group (14.6±2.1 vs.13.2±2.4 %; p=0,03). There were no significant differences between controls and patients with mild and moderate OSAS (13.2±2.4 vs. 13.4±0.6 %; p=0,98; 13.2±2.4 vs. 14.0±2.4 %; p = 0.43) and between patients with mild, moderate and severe OSAS (13.4±0.6 vs. 14.0±2.4 vs. 14.6±2.1 %; p=0,67 for comparison of mild and moderate OSAS, p=0.08 for comparison of mild and severe OSAS, p=0.56 for comparison of moderate and severe OSAS) in respect to RDW. Additionally, significant correlation of RDW with parameters of sleep was noted. RDW was correlated with apnea-hypopnea index (r=0.264, p=0.01) and minimal oxygen saturation (r=-0,284, p=0.006). Conclusions: RDW is a widely available diagnostic marker, routinely performed hemogram. In our study, we found that RDW was higher in the patients with OSAS compared with the control group. These results show that RDW is an important marker in terms of identifying the risk of atherosclerosis in patients with OSAS at an early stage. Çalışmamızda OUAS ile EDG arasındaki olası ilişkiyi araştırdık. EDG otomatik kan sayım cihazı kullanılarak ölçüldü. Bulgular: Ciddi dereceli OUAS hastalarında EDG, kontrol grubuna kıyasla anlamlı derecede yüksekti (14.6±2.1 vs.13.2±2.4%;p=0,03). Sonuç: EDG, hemogram içerisinde rutin olarak bakılan bir tanı aracıdır. Çalışmamızda OUAS'lu hastalarda kontrol grubuna kıyasla EDG'nin daha yüksek olduğunu bulduk. Bu sonuçlar, EDG'nin OUAS'lu hastalardaki ateroskleroz riskinin erken evrede saptanması için önemli bir tanı aracı olduğunu göstermektedir. 33. Obstrüktif Uyku Sendromu Olan Hastalarda Hipotiroidi Taraması Yapılmalı mı?

Aim: Obstructive sleep apnea syndrome (OSAS) is common in society, and hypothyroidism is a disease that can be seen along with rates ranging from 1.2 to 11%. Symptomatology spectrum of OSAS is very wide. These symptoms suggest OSAS but these symptoms are not definitive diagnostic feature. Patients with OSAS have symptoms such as throughout the day drowsiness, obesity, snoring, apathy and lethargy, that can be seen in all of them are faced with hypothyroidism. Regarded among the etiological causes in endocrine disorders, reduction of thyroid hormones can be easily detected. Measurement of serum levels of thyroid hormone is inexpensive and it is guidance for treatment planning. We investigate thyroid hormone levels in patients who were diagnosed as OSAS with polysomnography (PSG) and determine the prevalence of hypothyroidism in patients with OSAS. Material and Method: Ninety-three patients with complaints of snoring who were diagnosed as OSAS with PSG were examined in the study between March 2009-November 2010. Thyroid function tests were performed in these patients. Results: According to the severity, 30 patients were in mild, 32 patients were in moderate and 31 patients were in severe OSAS group. Hypothyroidism was detected in 5 of those 93 OSAS patients (5,37%). Two of these patients had clinical hypothyroidism and three of these patients had subclinical hypothyroidism. Conclusion: In our study, the prevalence of hypothyroidism and OSAS was found compatible with the literature. Reduction in symptoms and apnea hypopnea index with thyroid hormone therapy in OSAS patients with hypothyroidism is considering and hypothyroidism screening tests should be made with the suspicion of OSAS. Key words: Obstructive Sleep Apnea Syndrome; Hypothyroidism; Diagnosis., Amaç: Obstrüktif uyku apne sendromu (OUAS) toplumda sık görülen ve hipotiroidi ile %1,2-11 arasında değişen oranlarda birlikte görülebilen bir hastalıktır. OUAS'ın semptomatoloji spektrumu oldukça geniştir. Bu semptomlar OUAS'a yönelmemizi sağlarlar ancak hepsi tanı koydurucu özelliğe sahip değildir. OUAS'lu hastalar gün boyu sersemlik, horlama, apati ve letarji gibi hepsi hipotiroidide de gorülebilen semptomlarla karşı karşıyadır. Etyolojik nedenler arasında sayılan endokrin bozukluklardan tiroid hormonlarının azalması kolaylıkla tespit edilebilir. Serum tiroid hormon düzeylerinin tesbitinin maliyeti düşüktür ve tedavi planlanmasında yol gostericidir. Bu noktadan hareketle polisomnografi (PSG) ile OUAS tanısı alan hastalarda tiroid hormon düzeylerine baktık ve hipotiroidinin OUAS'taki sıklığını araştırdık. Materyal ve Metod: Çalışmaya Mart 2009 – Ekim 2010 tarihleri arasında Akdeniz Üniversitesi Tıp Fakültesi Hastanesi Göğüs Hastalıkları Polikliniğine horlama şikayeti ile başvuran ve PSG ile OUAS tanısı konulan 93 hasta alındı. Hastalardan tiroid fonksiyon testleri istendi. Bulgular: Hastaların 30'unun hafif dereceli, 32'sinin orta dereceli, 31'inin ise ciddi dereceli OUAS olduğu görüldü. Hastaların 5'inde (%5,37) hipotiroidi saptandı. Hipotiroidi saptanan hastalardan 2'sinde klinik hipotiroidi, 3'ünde ise subklinik hipotiroidi olduğu görüldü. Sonuç: Çalışmamızda OUAS ile hipotiroidi sıklığı literatür verileri ile benzer oranda bulundu. Yapılan çalışmalarda hipotiroidisi olan OUAS'lu hastalarda tiroid hormon tedavisi ile apne hipopne indeksinde ve semptomlarda azalma olduğu dikkate alınarak OUAS şüphesi ile tetkik edilen hastalarda hipotirodi taraması yapılmalıdır. OBJECTIVES: The aim of this study is to investigate the factors affecting cost in patients with severe community-acquired pneumonia (CAP) who were admitted to the intensive care unit (ICU). MATERIALS AND METHODS: This retrospective cohort study was conducted between January 2013 and December 2016. A total of 291 sequential patients with severe CAP were included in the study. Patients' demographic and clinical data; the need for invasive mechanical ventilation or non-invasive mechanical ventilation; intensive care severity (ICU) scores, including Acute Physiology and Chronic Health Evaluation (APACHE II), Sepsis-related Organ Failure Assessment, Quick SOFA, pneumonia severity index (PSI); and Confusion, Urea, Respiratory Rate, and Blood Pressure-65 (CURB-65) scores were obtained from medical records and recorded for all cases. RESULTS: The mean age of 291 patients was 68.4±16.8 years, and 61% were female. The median length of ICU stay was 7 days. Forty-six percent of patients had chronic obstructive pulmonary disease (COPD), and 42% had hypertension. The mean cost of each hospitalization was US$ 2722 (TL 5578). The highest cost was found in the group of patients aged 50-59 years, and the lowest cost was found in the patients aged <50 years. A statistically significant relationship was found between ICU severity scores and health cost. The cost of patients in PSI class V, APACHE II (>20 points), and CURB-65 score were higher. The presence of COPD, atrial fibrillation, congestive heart failure, hypoalbuminemia, mental state deterioration, in-hospital mortality, severe sepsis, septic shock, mechanical ventilation requirement, and haloperidol and vasopressor usage were associated with higher cost, while the use of florokinolon was associated with lower cost. CONCLUSION: The presence of certain comorbidities and high disease severity in patients with severe CAP hospitalized in ICU increase the cost of inpatient treatment. The need for mechanical ventilation during treatment and the presence of sepsis/septic shock are additional factors that increase the cost. [ABSTRACT FROM AUTHOR]

Amaç: Obstrüktif uyku apne sendromu toplumda en sık karşılaşılan uyku bozukluklarından birisidir. OUAS’lu hastaların %30-60’ında hipertansiyonun varlığı gösterilmiştir. Hipertansif OUAS’lu hastalarda kreatinin klirensinin (KK) hipertansif olmayanlara göre daha düşük olduğu saptanmıştır. Çalışmamızda hipertansiyon gelişmemiş olan OUAS’lu hastalarda KK’ni kontrol grubu ile kıyaslayarak, OUAS’da KK’nde azalma olup olmadığı ve OUAS’nun ciddiyet derecesinden etkilenip etkilenmediğini saptamayı amaçladık. Gereç ve Yöntemler: Çalışmaya Mart 2009–Ekim 2010 tarihleri arasında Akdeniz Üniversitesi Tıp Fakültesi Hastanesi Göğüs Hastalıkları Polikliniğine horlama şikayeti ile başvuran ve polisomnografi ile OUAS tanısı konulan 93 hasta alındı. Hastalardan böbrek fonksiyon testleri istendi ve KK hesaplandı. Bulgular: Hastaların 30’unun hafif, 32’sinin orta, 31’inin ise ciddi dereceli OUAS olduğu görüldü. Hasta grupları ve kontrol grubunun demografik verilerinde, sistolik ve diyastolik kan basıncı ve KK değerlerinde istatistiksel anlamlı farklılık saptanmadı. OUAS’nun şiddeti ile KK arasında bir ilişki bulunmadı. Sonuç: Hipertansiyon gelişmemiş olsa bile OUAS’nun patofizyolojik özelliklerinden dolayı bu hastalarda kronik böbrek hastalığı gelişebileceği bildirilmektedir. Bu yüzden OUAS’lu hastalarda hipertansiyon gelişebileceği göz önünde bulundurulmalı; tansiyon takibi,böbrek fonksiyonlarının değerlendirilmesi ve takibi ihmal edilmemelidir. Objective: Obstructive sleep apnea syndrome (OSAS) is one of the most common sleep disorders in society. The presence of hypertension is shown in 30-60% of OSAS patients. Creatinine clearance (CC) in hypertensive OSAS patients was found to be lower than non-hypertensive OSAS patients. In our study, we aimed to determine CC in non- hypertensive OSAS patients comparing with the control group and determine that CC is affected by the severity of OSAS. Material and Methods: Ninety-three patients with complaints of snoring who were diagnosed as obstructive sleep apnea syndrome with polysomnography were examined in the study between March 2009-November 2010. Renal function tests were performed in these patients and creatinine clearance was calculated. Results: According to the OSAS severity, 30 patients were in the mild, 32 in moderate and 31 in severe OSAS group.There were no statistically significant differences in the demograpic data, systolic and diastolic blood pressure and CC between OSAS and control groups There was no corelation between severity of disease and CC. Conclusion: Due to the pathophysiologic features of OSAS, chronic kidney disease can develop in these patients, although hypertension has not yet developed. Therefore it must be considered that, hypertension may develop in patients with OSAS; blood pressure monitoring, evaluation and monitoring of renal function should not be neglected.

Amaç: Obstrüktif uyku apne sendromu (OUAS) toplumda sık görülen ve hipotiroidi ile %1,2-11 arasında değişen oranlarda birlikte görülebilen bir hastalıktır. OUAS’ın semptomatoloji spektrumu oldukça geniştir. Bu semptomlar OUAS’a yönelmemizi sağlarlar ancak hepsi tanı koydurucu özelliğe sahip değildir. OUAS’lu hastalar gün boyu sersemlik, horlama, apati ve letarji gibi hepsi hipotiroidide de gorülebilen semptomlarla karşı karşıyadır. Etyolojik nedenler arasında sayılan endokrin bozukluklardan tiroid hormonlarının azalması kolaylıkla tespit edilebilir. Serum tiroid hormon düzeylerinin tesbitinin maliyeti düşüktür ve tedavi planlanmasında yol gostericidir. Bu noktadan hareketle polisomnografi (PSG) ile OUAS tanısı alan hastalarda tiroid hormon düzeylerine baktık ve hipotiroidinin OUAS’taki sıklığını araştırdık. Materyal ve Metod: Çalışmaya Mart 2009 – Ekim 2010 tarihleri arasında Akdeniz Üniversitesi Tıp Fakültesi Hastanesi Göğüs Hastalıkları Polikliniğine horlama şikayeti ile başvuran ve PSG ile OUAS tanısı konulan 93 hasta alındı. Hastalardan tiroid fonksiyon testleri istendi. Bulgular: Hastaların 30’unun hafif dereceli, 32’sinin orta dereceli, 31’inin ise ciddi dereceli OUAS olduğu görüldü. Hastaların 5’inde (%5,37) hipotiroidi saptandı. Hipotiroidi saptanan hastalardan 2’sinde klinik hipotiroidi, 3’ünde ise subklinik hipotiroidi olduğu görüldü. Sonuç: Çalışmamızda OUAS ile hipotiroidi sıklığı literatür verileri ile benzer oranda bulundu. Yapılan çalışmalarda hipotiroidisi olan OUAS’lu hastalarda tiroid hormon tedavisi ile apne hipopne indeksinde ve semptomlarda azalma olduğu dikkate alınarak OUAS şüphesi ile tetkik edilen hastalarda hipotirodi taraması yapılmalıdır. Aim: Obstructive sleep apnea syndrome (OSAS) is common in society, and hypothyroidism is a disease that can be seen along with rates ranging from 1.2 to 11%. Symptomatology spectrum of OSAS is very wide. These symptoms suggest OSAS but these symptoms are not definitive diagnostic feature. Patients with OSAS have symptoms such as throughout the day drowsiness, obesity, snoring, apathy and lethargy, that can be seen in all of them are faced with hypothyroidism. Regarded among the etiological causes in endocrine disorders, reduction of thyroid hormones can be easily detected. Measurement of serum levels of thyroid hormone is inexpensive and it is guidance for treatment planning. We investigate thyroid hormone levels in patients who were diagnosed as OSAS with polysomnography (PSG) and determine the prevalence of hypothyroidism in patients with OSAS. Material and Method: Ninety-three patients with complaints of snoring who were diagnosed as OSAS with PSG were examined in the study between March 2009-November 2010. Thyroid function tests were performed in these patients. Results: According to the severity, 30 patients were in mild, 32 patients were in moderate and 31 patients were in severe OSAS group. Hypothyroidism was detected in 5 of those 93 OSAS patients (5,37%). Two of these patients had clinical hypothyroidism and three of these patients had subclinical hypothyroidism. Conclusion: In our study, the prevalence of hypothyroidism and OSAS was found compatible with the literature. Reduction in symptoms and apnea hypopnea index with thyroid hormone therapy in OSAS patients with hypothyroidism is considering and hypothyroidism screening tests should be made with the suspicion of OSAS.

50. Ağır Alerjik Astımlı Hastalarda Omalizumab Tedavisi.

- Abstract
Introduction: Omalizumab is a monoclonal antibody used for treating patients with severe allergic asthma. It has has been shown to be effective and is being increasingly used. This study aimed to evaluate the long-term efficacy and safety of omalizumab treatment in patients with severe allergic asthma. Methods: The efficacy and safety of omalizumab treatment was assessed by analyzing the medical records of the patients. While patients were on omalizumab treatment, they were clinically evaluated monthly. The level of control of asthma symptoms as determined in the Global Initiative for Asthma (GINA) guideline was used to evaluate the efficacy of treatment. Results: Fifty-four patients were included. All patients were sensitive to at least one perennial allergen and had uncontrolled allergic asthma despite providing standard treatments. The mean±standard deviation (SD) age, body weight, and IgE level were 53.6±13.9 years, 75.4±14.3 kg, and 294.7±227.5 IU/mL, respectively. The mean±SD omalizumab treatment duration was 34.8±20.2 (range,10-84) months. Patients did not experience major adverse reactions that were considered related to omalizumab treatment. Two patients had local adverse reactions, and one had myalgia that was considered to be drug-related. After omalizumab treatment, asthma symptoms were well controlled in 25 patients (46.3%), partly controlled in 23 patients (42.6%), and uncontrolled in 6 patients (11.1%). Conclusion: Our study showed that omalizumab treatment is well tolerated and effective for patients with uncontrolled allergic asthma. [ABSTRACT FROM AUTHOR]
