It has been reported that many antibiotics used today, including the carbapenem group, fail to treat Klebsiella pneumoniae infections effectively. Despite many studies in recent years, the definitive treatment for carbapenem-resistant Klebsiella pneumoniae (CRKP) infections is still uncertain. In this study, it was aimed to investigate in vitro activities of colistin (COL) and meropenem (MEM), which are frequently used in the treatment of CRKP infections, and ceftazidime-avibactam (CZA), which is recently used in our country, alone or in combination against different CRKP isolates having different carbapenem resistance mechanisms andto analyze whether the presence of colistin resistance, which is an important problem in CRKP strains, influences the drug interaction results. This study was carried out in 42 K.pneumoniae isolates, which were isolated from various clinical samples as an infectious agent in Silleyman Demirel University Faculty of Medicine, Department of Medical Microbiology, Bacteriology Laboratory and whose carbapenem resistance was confirmed by carbapenemase inactivation test. The carbapenemase genes of the isolates were determined by the polymerase chain reaction (PCR) method. Antimicrobial susceptibilities of CRKP strains to CZA, MEM, and COL were determined by the broth microdilution method and in vitro synergy activities of dual combinations of these drugs were evaluated by checkerboard and time-kill methods. Statistical evaluation of categorical data was performed using Fisher's exact test, and p-value of less than 0.05 was considered statistically significant in terms of difference between the groups. Of the 42 CRKP isolates 34 (81%) were only OXA-48 positive, 5 (11.9%) were OXA-48+NDM and 3 (7.1%) were OXA-48+KPC positive. In the checkerboard test, synergy was detected against 97.6% of the isolates both with CZA+MEM and CZA+COL combinations, whereas this rate was 50% with MEM+COL. In the time-kill test, synergy was detected with CZA+MEM and CZA+COL combinations in the OXA-48 positive isolate and OXA-48+KPC positive isolate, while synergy was detected with CZA+COL and MEM+COL combinations in the OXA-48+NDM positive isolate. There was no significant relationship between whether the isolates were resistant to colistin or not and the checkerboard test results of antibiotic combi nations (p(CZA+MEM)= 0.33, PCZA+COL= 0.11, p(MEM+COL) = 0.61). Results of our study revealed that the most common carbapenemase type in CRKP isolates was OXA-48 in our hospital, and the combinations of CZA with MEM and COL had high potential for synergism against these isolates.

Aim: The aim of this study is to improve the antibacterial properties and investigate the physical properties of modified dental luting cements (polycarboxylate cement, glass ionomer cement, dual-cure cement, composite resin cement) with oleuropein addition.Materials and Methods: The dental luting cements were modified by adding Oleuropein 1% by weight. Unmodified luting cements were referred as the control group. In order to determine the antibacterial effect of each group Modified Direct Contact Test method and E. faecalis was used. The microhardness values of all samples was measured with Vickers microhardness tester. All samples were evaluated by Scanning Electron Microscopy (SEM). Results: Fewer bacterial colonies were detected in the media of the experimental group compared to the control group. While the Vickers microhardness values of polymer-containing cements decreased with the addition of oleuropein, no statistical difference was found in polycarboxylate and glass ionomer cement. Conclusion: According to the results of this study, the addition of oleuropein improved the antibacterial properties of the luting cements but physical properties of resin based cements are negatively affected. The results obtained may shed light on the studies to be carried out on the development of luting cements.Key Words: Antibacterial, luting cements, oleuropein

OBJECTIVE: Studies have implicated Toxoplasma gondii in the etiology of mental disorders because of its neurotropic nature and its ability to modulate neurotransmitter pathways. This study aims to investigate T. gondii seroprevalence in patients with bipolar disorder and in healthy controls living in the Isparta Region of Turkey and to assess the probable relationship between T. gondii and bipolar disorder. METHODS: Fourty-eight patients with bipolar disorder and 50 healthy controls were included in the study. Sociodemographic data, possible risk factors for T. gondii infection and clinical characteristics were analyzed. Serum anti-T. gondii IgM and IgG antibody levels were measured by using chemiluminescence immunoassay method (Roche Cobas e601 analyzer, Roche Diagnostics, Mannheim, Germany). RESULTS: Anti-T. gondii IgG seropositivity rates were determined as 18.8% and 20% in the patient group and the control group, respectively. No statistically significant relationship was observed between T. gondii IgG seropositivity and bipolar disorder (p=0.876). Aralık 2019’da Çin’den başlayarak çok kısa bir süre içerisinde tüm dünyayı etkisi altına alan Coronavirus Disease 2019 (COVID-19) pandemisi, bugüne kadar görülen en ciddi küresel sağlık sorunlarından biri haline gelmiştir. Bu süreçte, zamanında ve doğru uygulanan mikrobiyolojik tanı testleri, salgının ve vaka yönetiminin önemli bir parçası olmuştur. Pandeminin başlangıcından itibaren solunum yolu örneklerinde severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA’sını saptamaya yönelik olarak geliştirilen gerçek zamanlı ters transkriptaz-polimeraz zincir reaksiyonu (RT-PCR), en yaygın kullanılan tanı yöntemidir. RT-PCR ile virüs RNA’sının gösterilmesi COVID-19 hastalığının kesin tanısını sağlamakla birlikte negatif test sonucu enfeksiyon olasılığını dışlamaz. Klinik şüpheli olgularda 24-48 saat arayla tekrarlayan örneklerin alınması ve ciddi akciğer bulguları olan hastalarda tanı için alt solunum yolu örneklerinin tercih edilmesi gereklidir. Düşük maliyetli, uygulanması kolay ve hızlı sonuç alınabilen antijen testlerinin kullanımı, viral yükün yüksek olduğu hastalığın akut evresi ile sınırlıdır. Duyarlılık ve özgüllük sorunlarından dolayı antijen testleriyle elde edilen sonuçların RT-PCR testleri ile doğrulanması gerekir. Antikor testleri, virüs ile karşılaşılmış olduğunu gösterir. Hastaların virüse karşı antikor yanıtının değişken ve geç olması nedeniyle antikor testleri, akut dönemde hastalığın tanısında tek başına kullanılmamalıdır. Semptomların başlangıcından 2-3 hafta sonra alınan kan örneklerinde çalışılan antikor testleri moleküler tanıya yardımcı ek test olarak kullanılabilir. Bu derlemede, günümüze kadar elde edilen veriler doğrultusunda SARS-CoV-2 enfeksiyonunun mikrobiyolojik tanısında kullanılan yöntemlerin özellikleri, klinik açıdan sağladıkları avantajlar ve kısıtlı yönleri tartışılmıştır., The Coronavirus Disease 2019 (COVID-19) pandemic, which began in China in December 2019 and affected the whole world in a very short time, has become one of the most serious global health problems to date. In this process, timely and correctly applied microbiological diagnostic tests have been an important part of the epidemic and case management. Since the beginning of the pandemic, real-time reverse transcriptase-polymerase chain reaction (RT-PCR), which has been developed to detect severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA in respiratory tract samples, is the most widely used diagnostic method. The detection of the virus RNA by RT-PCR provides the definitive diagnosis of COVID-19 disease, but a negative test result does not exclude the possibility of infection. In clinically suspicious cases, repeated samples should be taken within 24-48 hours and lower respiratory tract samples should be preferred for diagnosis in patients with severe lung findings. The use of low-cost and easy-to-apply rapid antigen tests is limited to the acute phase of the disease when viral load is high. Due to sensitivity and specificity problems, the antigen test results should be confirmed by RT-PCR tests. Antibody tests indicate the exposure to the virus. Antibody tests should not be used alone in the diagnosis of the disease in acute phase, because the antibody response of the patients against the virus is variable and late. Antibody tests, which are performed on the blood samples taken 2-3 weeks after the onset of symptoms, can be used as an additional test to aid molecular diagnosis. In this review, the features, clinical advantages and limitations of the methods used in the microbiological diagnosis of SARS-CoV-2 infection are discussed in line with the obtained data to date.

Idiopathic cutaneous angiosarcoma (CA) of the head and neck is a distinct subtype of angiosarcoma most commonly presenting as a single or multiple purple, bruise-like patches that arise de novo and enlarge over several months. In clinical practice, both misdiagnosis and delayed diagnosis are frequently encountered. Here, we present a case of idiopathic CA on the scalp with invasion to the cranium in a patient with breast cancer metastatic to the brain. The patient was initially misdiagnosed and mistreated with herpes zoster and breast cancer metastatic to the skin, which led to a delayed diagnosis by two months until dermatologic evaluation. The diagnosis was then firmly established as CA based on consistent clinical and histological features. Since the tumor was inoperable, radiotherapy and chemotherapy were been considered as the appropriate adjuvant modes of therapy. Despite an initial favorable response, the disease demonstrated a rapidly progressive course and the patient succumbed to the disease within six months. This report briefly reviews the clinical and histological portrait and management options for this aggressive tumor.

ÖZETAmaç:Bu çalışmada HIFU dönüştürücüleri tarafından iki farklı kolon kanseri hücre hattı üzerine uygulanan farklı ultrasonik doz değerlerine karşılık ortaya çıkan biyolojik etkilerin incelenmesi amaçlanmıştır.Gereç ve Yöntem:Çalışmada, Sapareto ve Dewey tarafından hipertermi tedavileri için önerilen teorik model kullanılarak ultrasonik doz hesaplamaları yapılmıştır. Hesaplanan ultrasonik termal doz değerlerini kolon kanseri hücre hatlarına uygun bir şekilde verebilmek için gerekli olan sıcaklık karakterizasyonları yapılmıştır. Ayrıca, farklı marka ve model bir HIFU dönüştürücüsü ve kendi ürettiğimiz bir ultrasonik dönüştürücünün oluşturduğu sıcaklık etkilerinin karakterizasyonları da hem 24 kuyulu plaka hem de fantom dokular üzerinde gerçekleştirilmiştir. Ardından, istenilen termal doz değerlerini oluşturacak şekilde, iki farklı kolon kanseri hücre hattına HIFU uygulanmıştır. HIFU uygulanan kuyucuklarda ve bu kuyulara en yakın konumda bulunan ve doğrudan HIFU’nun odak noktasında olmayan kuyucuklarda bulunan hücrelerde oluşan biyolojik yanıtlar belirlenmiştir. Hücre canlılığı, apoptoz ve nekroz analizleri MTT, MMP ve TALI yöntemleri ile belirlenmiştir. İstatistiksel değerlendirmeler için ANOVA yöntemi kullanılmıştır.Sonuçlar ve Yorum:HIFU uygulanması sonucu kolon kanseri hücre hatları üzerinde gelişen biyolojik yanıtların dozla doğru orantılı olduğu belirlenmiştir. Yüksek dozlarda ise etki alanının genişlediği ve komşu hücrelerin de ultrasonik alandan etkilendiği gözlemlenmiştir. HIFU uygulanırken etkin bir dozimetri çalışması gereklidir ve dozimetri için “eşdeğer kümülatif termal doz” yöntemi kullanılabilir.--------------------SUMMARYObjective:The aim of this study was to investigate the biological effects of HIFU transducers on two different colon cancer cell lines in response to different ultrasonic dose values.Materials and Methods:In the study, ultrasonic dose calculations were performed using the theoretical model proposed by Sapareto and Dewey for hyperthermia treatments. The required temperature characterizations have been made in order to give the calculated ultrasonic thermal dose values to the colon cancer cell lines in an appropriate manner. In addition, the characterization of the temperature effects of a different branded HIFU transducer and our own production of an ultrasonic transducer were performed on both 24-well plate and phantom tissues. Then, to form the desired thermal dose values in the two different colon cancer cell lines HIFU was applied. Biological responses were determined in the wells directly in the focus of HIFU and in the cells adjacent to these wells (adjacent to the focal field of HIFU). Cell viability, apoptosis and necrosis were determined by MTT, MMP and TALI methods. ANOVA method was used for statistical evaluations.Results and Conclusions:It was determined that the biological responses on colon cancer cell lines as a result of HIFU application were directly proportional to absorbed dose. At high doses, it was observed that the effect area was enlarged and neighbouring cells were affected by the ultrasonic field. An effective dosimetric study is required when applying HIFU and the “equivalent cumulative thermal dose” method can be used for dosimetry.

Amaç: Yüksek morbidite ve mortaliteye yol açabilen bir enfeksiyon olan menenjitin hızlı ve doğru tanısı önemlidir. Çalışmamızın amacı beyin omurilik sıvısı (BOS) örneklerinden multipleks Real-Time PCR yöntemi ile tespit edilen bakteriyel ve viral menenjit etkenlerinin retrospektif olarak değerlendirilmesidir.Gereç ve Yöntem: Çalışmaya Ocak 2017 - Eylül 2019 tarihleri arasında menenjit şüphesiyle gönderilen 167 hastanın BOS örneği dahil edildi. Örnekler multipleks Real-Time PCR yöntemiyle BioGX bakteriyel ve viral menenjit ticari kitleri kullanılarak BD MAX (Becton Dickinson, ABD) cihazında çalışılmış, ayrıca bakteriyel etken araştırılması için klasik kültür yöntemi ile ekimleri de yapılmıştır. Viral etkenlerden Herpes Simplex Virüs 1 ve 2 (HSV 1-2), Varisella-zoster Virüs, bakteriyel etkenlerden ise Haemophilus influenzae, Neisseria meningitidis ve Streptococcus pneumoniae varlığı araştırılmıştır.Bulgular: Çalışmaya alınan 167 BOS örneğinin 95 (%56,8)’i erkek, 72 (%43,2)’si kadın hastalardan alınmış olup, hastaların 66 (%39,5)’sı 0-5 yaş, 37 (%22,2)’si 6-15 yaş, 64 (%38,3)’ü 16-85 yaş aralığındaydı (ortalama yaş 23). Örneklerin 4 (%2,4)’ünde viral, 16 (%9,5)’sında bakteriyel etken olmak üzere 20 (%12)’sinde pozitiflik saptanmıştır. Pozitif örneklerin 4’ünde HSV-1, 8’inde S. pneumoniae, 4’ünde N. meningitidis ve 4’ünde H. influenza tespit edilmiştir. Bir hastada S. pneumoniae ve H. influenza birlikte pozitif bulunmuştur. Eş zamanlı yapılan BOS kültürlerinin sadece 4’ünde S. pneumoniae ürediği görülmüştür. Pozitiflik saptanan 20 hastanın 7 (%35)’si 0-5 yaş, 5 (%25)’i 6-15 yaş, 8 (%40)’i 16-85 yaş aralığındaydı. En sık pozitiflik Ocak, Mart ve Mayıs aylarında görülmüştür.Sonuç: Çalışmanın verileri değerlendirildiğinde aynı örnekler sadece kültür yöntemi ile çalışılsaydı 4 S. pneumoniae, 4 H. influenzae, 4 N. meningitidis ve 4 HSV-1 etkeninin tanısının konulamamış olacağı görülmektedir. Bu nedenle özellikle klasik tanı yöntemlerinin yetersiz kaldığı bilinen etkenlerin tanımlanmasında multipleks Real-Time PCR yönteminin kullanımının tanısal duyarlılığa sağlayacağı katkı göz ardı edilmemelidir.Anahtar Kelimeler: BOS, Menenjit etkenleri, Tanı, Real-Time PCR
