Aim: This study aimed to examine the effect of 12 sessions of abdominal massage on symptoms and quality of life of mothers in children with functional constipation. Materials- Methods: Thirty 1- to 6-year-old children diagnosed with functional constipation and their mothers participated in the study. Children were randomly divided into 2 groups, and Group 1 received only pharmacological treatment, while Group 2 received 12 sessions of abdominal massage with pharmacological treatment. Changes in stool consistency and symptoms of constipation were evaluated with the Bristol Stool Scale, Constipation Assessment Scale and mothers' quality of life the Health-Related Quality of Life scale developed by the World Health Organization. Results: The mean age of the children was 4.66 ± 1.68 years, and the mothers was 36.54 ± 1.71 years. As a result treatment, the stool consistency of both groups was softened, while the symptoms of constipation in the massage group were reduced. In addition, while no change was observed in the quality of life of the mothers in Group 1, a statistically significant improvement was obtained in the quality of life of the mothers in Group 2. Conclusion: As a result, 12 sessions of abdominal massage application in preschool children with functional constipation were effective in softening the consistency of the stool, reducing the symptoms of constipation, and also increasing the quality of life of the mothers. Periferik venöz kateterizasyon (PVK) hastaneyeyatan çocukların %80’inden fazlasına en sık yapılan invaziv girişimdir. Masumgibi görünen PVK’nin çocuklarda flebit, enfeksiyon, infiltrasyon,ekstravazasyon gibi çok sayıda komplikasyona, hatta mortaliteye neden olduğubilinmektedir. Bu çalışmada, eklem lokalizasyonunda PVK takılan çocukhastalarda kullanılan yeni tespit tahtasının PVK ömrü ve komplikasyon oranınaetkisinin araştırılması amaçlanmıştır. Haziran-Temmuz 2018 tarihleri arasındaçocuk cerrahisi servisinde tedavi edilen ve eklem bölgesinde PVK takılan 49hasta prospektif olarak takip edildi. Hastalar grup 1 tespit tahtası kullanılanve grup 2 hipoalerjenik elastik fiksasyon bandı kullanılan kontrol grubu olarakikiye ayrıldı. İki grup arasında yeni tespit tahtasının, intraket çapının veyaşın PVK yaşam süresi ve komplikasyon oranına etkisi incelendi. Although the complication rate in this study was significantly reduced,no significant result was obtained regarding PIVC lifespan.

PURPOSE: Peripheral intravenous catheter (PIVC) insertion is the most frequently used invasive intervention that is performed in more than 80% of hospitalized children. It is a known that seemingly innocent PIVC may result in various morbidities such as phlebitis, infection, extravasations, and may even lead to mortality. In this study, we aimed to investigate the effect of a new fixation board (wrist-ankle fixation board) on PIVC lifespan and complications rates in pediatric patients undergoing PIVC insertion in the joint area. Material and methods: A total of 49 patients who were treated in the pediatric surgery ward between June and July 2018 and who underwent PIVC insertion were prospectively followed. The patients were divided into the following two groups: ‘Group 1’ in which a wrist-ankle fixation board was used and ‘Group 2’, which was allocated as the control group using Hypoallergenic Elastic Fixation Tape. PVC life and complications (infiltration, purification, displacement) were recorded. Results: Forty-nine patients were included in the present study and they were divided into the following two groups: 25 patients in Group 1 and 24 patients in Group 2. There were no statistically significant differences between the groups in terms of age and gender (p>0.05). There was no statistically significant difference in PIVC lifespan between Groups 1 (31.8 hours) and 2 (29.5 hours) (p = 0.151). The complication rate was noted to be 4% in Group 1 and 29% in Group 2. The difference in complication rates between the two groups was statistically significant (p = 0.02). Conclusions: The rate of complications was lower in pediatric patients with PIVC compared to the other method with the use of a new fixation board (ankle-ankle fixation fixation method). Gruplar arasında yaş ve cinsiyet açısından istatistiksel olarak anlamlı fark bulunmadı (p> 0,05). PVK ömrü yönünden değerlendirildiğinde Grup 1 (31,8 saat) ve 2 (29,5 saat) arasında fark saptanmadı (p = 0,151). Komplikasyon oranı Grup1’de %4, Grup 2’de % 29 olarak bulundu. İki grup arasındaki komplikasyon oran farkı istatistiksel olarak anlamlıydı (p = 0,02). Amaç: Daha önce batın operasyonu geçirmemiş hastalarda Ladd bantları ve omfalomezenterik kanal artığı (OMKA) gibi konjenital bantlara bağlı intestinal obstrüksiyonlar bilinmektedir. Atipik konjenital bantlar (AKB) ise, çocuklarda bağırsak tıkanıklığının son derece nadir nedenlerindendir. Bu çalışmada, nadir görülen pediatrik konjenital abdominal bant hastalarımızın geriye dönük olarak incelenmesi ve tek merkez sonuçlarının paylaşılması amaçlanmıştır. Yöntem: Temmuz 2004 - Kasım 2018 yılları arasında ileus nedeniyle ameliyat edilen 204 hastanın dosyaları geriye dönük olarak incelendi. Çalışmaya daha önceden operasyon öyküsü olmayan ve konjenital abdominal bant nedeniyle ameliyat edilen 17 hasta dahil edildi. Bulgular: Hastaların 10’u erkek, 7’si kız idi. Yaş ortalaması 3,2 yıl (0,1-15) olarak bulundu. Hastaların %53’ünde (n=9) malrotasyona bağlı ladd bantları, %29’ında (n=5) OMKA bağlı bantlar ve %18’inde (n=3) AKB vardı. Tedavide OMKA bağlı hastaların %60’ına (n=3) bağırsak rezeksyonu gerekirken (30-40-65 cm), ladd bantlarına ve AKB bağlı hastaların hepsinde bant eksizyonu yeterli oldu, hiçbir hastaya bağırsak rezeksyonu gerekmedi. Hastaların tamamı şifa ile taburcu edildi. Sonuç: Pediatrik konjenital bantlar farklı etiyolojilere sahip hastalıkları içerir. Daha önceden batın ameliyatı olmamış ve ileus tablosuyla gelen bir hastada ayırıcı tanıda malrotasyona bağlı ladd bantları ve OMKA ile birlikte AKB’ninde akılda bulundurulması gerekmektedir., Objective: Intestinal obstruction due to congenital bands such as Ladd bands and ompholomesenteric duct remnant (OMDR) is known in patients who have not previously undergone abdominal surgery. However, atypical congenital bands (ACB) are extremely rarely seen causes of bowel obstruction. In this study, we aimed to examine retrospectively the patients with rarely seen pediatric congenital abdominal bands and to share the results of a single center experience. Methods: Medical records of 204 patients who had been operated for ileus between July 2004 and November 2018 were retrospectively reviewed. Seventeen patients without any history of abdominal surgery and operated for congenital abdominal band were included in the study. Results: Ten male and 7 female patients with a median age of 3,2 years (0.1-15) were included in the study. Patients had Ladd bands due to malrotation (n=9; 53%), OMDR (n=5; 29%), and ACB (n=3; 18%). Sixty percent (n=3) of the patients with OMDR required bowel resection (30-40-65 cm). Band excision was sufficient in all of the patients with Ladd bands and ACB, and bowel resection was not required. All patients were discharged with full recovery. Conclusion: Pediatric congenital bands are found in a spectrum of diseases with different etiologies. In the differential diagnosis for ileus patiens who had not previously undergone abdominal surgery, Ladd bands due to malrotation and ACB together with OMDR should be kept in mind.

Amaç: Bu çalışmada yabancı cisim aspirasyonu nedeniyle akut dönemde rigid bronkoskopi ile yabancı cisim çıkarılan hastaların uzun dönem akciğer durumlarının spirometri ile değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Mart 2013 – Nisan 2018 tarihleri arasında yabancı cisim aspirasyonu için bronkoskopi yapılan 142 çocuğa ait kayıtlar retrospektif olarak incelendi. Spirometri işlemine kooperasyon kurabilen 20 hastanın verileri çalışmaya dahil edildi (Grup 1). Rutin çocuk cerrahisi polikliniği’ne başvuran akciğer problemi ve ağrı şikayeti olmayan beş yaştan büyük 20 hasta kontrol grubu olarak çalışmaya dahil edildi (Grup 2). Değerlendirme için; 1. Saniyedeki zorlu ekspiratuar akım hızı (FEV1), zorlu vital kapasite (FVC), FEV1/FVC ve zorlu ekspirasyon ortası akım hızı (FEF 25-75) parametreleri kaydedildi.Bulgular: FEV1, FVC ve FEF 25-75 gibi değerlerin üzerinde yaş, BMI ve cinsiyetin anlamlı bir etkisi olmadığı görüldü. Aynı değerler üzerinde bronkoskopi işleminin etkisi incelendiğinde Grup 1 ve Grup 2 için FEV1 ve FVC değerleri arasındaki fark istatistiksel olarak anlamı, FEV1/FVC ve FEF 25-75 arasındaki fark ise anlamlı değildi.Sonuç: Erken dönemde tespit edilen ve 24 saatten önce müdahale edilen hastalarımızın spirometre ile değerlendirilen akciğer kapasitelerinde uzun dönem olumsuz bir etkilenme tespit edilememiştir., Purpose: The purpose of the present study was to evaluate the long-term lung status using spirometry in patients who underwent rigid bronchoscopy in acute period due to foreign body aspiration. Materials and Methods: Records of 142 children who underwent bronchoscopy due to foreign body aspiration between March 2013 and April 2018 were retrospectively reviewed. The data of 20 patients who cooperated with the spirometry process were included in this study (Group 1). Twenty patients who were admitted to the routine pediatric surgeon polyclinic without any lung problem and pain complaints were included in the study as a control group (Group 2). Forced expiratory flow rate (FEV1), forced vital capacity (FVC), FEV1/FVC, and forced expiratory flow at 25-75% of forced vital capacity (FEF 25–75) parameters were recorded in the first second.Results: Based on the statistical analysis results, there was no significant effect of age, sex, and BMI on FEV1, FVC, and FEF 25–75 values. The effect of bronchoscopy procedure on the same values, the difference between FEV1 and FVC values was statistically significant, whereas the difference between FEV1/FVC and FEF 25–75 values was not statistically significant for Groups 1 and 2. Conclusion: There was no long-term negative effect on lung capacities assessed using spirometry in our patients who received early-diagnosis and intervention within 24 h.

WOS: 000418201400006 Amaç: Transversus abdominis plan (TAP) blokun postoperatif analjezik etkinliği abdominal cerrahi geçirecek olan hastalarda tartışmalıdır. Bu çalışmanın amacı, inguinal herni tamiri yapılan pediyatrik hastalarda ultrasound eşliğinde yapılan TAP blok ile yara yeri infiltrasyonunun analjezik etkinliğini karşılaştırmaktır. Yöntem: Etik onam alındıktan sonra 3-8 yaş arası, tek taraflı inguinal herni onarımı yapılacak olan 60 hasta randomize olarak TAP blok (Grup TAP, n=30) veya yara yeri infiltrasyonu (Grup infiltrasyon, n=30) olarak iki gruba ayrıldı. TAPB grubuna ultrason eşliğinde 0,5 ml/kg olacak şekilde %0,25’lik bupivakain, Grup C’ye ise 0,5 ml/kg olacak şekilde 0,25’lik bupivakain cerrahi öncesi uygulandı. Ağrı skorları (Wong-Baker Yüz Skalası), aile memnuniyeti, blok komplikasyonları ve ek analjezik gereksinimi kaydedildi. Aim: The most important way for children to communicate with their surroundings is to take every substance that they keep in hand to their mouths. Foreign bodies that are ingested can be found anywhere in the gastrointestinal tract. Especially foreign bodies in esophagus may be the cause of morbidity and mortality. For this reason, the ingestion of foreign bodies in children is one of the most important health problems. Material and Methods: Between January 2014 and June 2017, 119 patients, admitted to Mustafa Kemal University [MKÜ] Faculty of Medicine, Department of Pediatric Surgery with foreign body ingestion diagnosis, were retrospectively studied. Patients were evaluated in terms of age, gender, complaints of arrival, ingested foreign body quality, location in the gastrointestinal tract and treatment approaches. Results: Of the patients, 74 were males [62%] and 45 were females [38%]. The mean age of the patients was 4.3 years [6 months-15 years]. Foreign bodies most commonly seen in the esophagus first stenosis in 73 patients [61%].Most commonly complaint at presentation was parental recognition of the ingested object and hypersalivation. The most commonly ingested foreign bodies included coins. Conclusion: Esophagoscopy must be performed foreign bodies in the esophagus should be treated conservatively, in case of clinical suspicion, the possibility of foreign body ingestion must be kept in mind. Education of the parents and taking some precautions where the children play are the most important protective factors.

Amaç: Safra yolları tıkanıklıklarında genellikle gram negatif mikroorganizmalara bağlı gelişen sepsis; ciddi morbidite ve mortalite sebebidir. Probiyotiklerin tıkanma sarılıklarında bakteriyel translokasyonu azalttığına dair deneysel çalısmalar vardır. Bu çalışmanın amacı kombine probiyotik mikroorganizma kullanımının bakteriyel translokasyon yanısıra karaciğer (KC) ve ileum (İL) histolojisi üzerindeki etkilerinin araştırılması idi.Yöntem: Çalışmada her biri 10'ar rat içeren 3 grup oluşturuldu. Grup 1 (Sham grubu), grup 2 (Tıkanma sarılığı), grup 3 (Tıkanma sarılığı+Probiyotik). Deneklere lactobacillus acidophilus, lactobacillus rhamnosus, bifidobacterium bifidum, enterococcus faecium ve bifidobacterium longum içeren solüsyon oral yoldan verildi. 7. günün sonunda mikrobiylojik açıdan değerlendirmek amacıyla mezenterik lenf nodu (MLN), dalak ve portal venden kan örnekleri; Histopatolojik değerlendirme için İL ve KC doku örnekleri; Biyokimyasal inceleme için de kan örnekleri alındı.Bulgular: Kan, dalak ve MLN kültürlerinde grup 2 de % 33.8-58.8 oranında üreme saptanırken grup 3'te %14.3-28.6 oranında üreme saptandı ve bu düşüş istatistiksel olarak anlamlıydı (p˂0.05). Hydatid cyst (HD), a very common disease in the world, is often transmitted to humans through dog feces. HD is especially common in countries where agriculture and animal husbandry is prevalent. HD is caused by Echinococcus granulosus. The clinic course of HD usually features settling in the liver and lungs. Lung involvement with chest pain, cough, shortness of breath can cause non specific symptoms such as fever and hemoptysis. Diagnostic interventions for HD include chest X-ray tomography, and physical examination. Although the treatment options of HD vary according to the clinical findings of the patients, the primary treatment may be considered as surgery. The most frequently applied methods of surgical treatment are cystectomy and capitonnage. Medical treatment is usually warranted for complicated cases, including the patients with multiple and unresectable lesions, patients who refuse surgery and the patients who cannot tolerate surgery. Treatment with albendazole and its derivatives are used. In this case report, we present a child with primary complaints of cough, chest pain, dyspnea, fatigue and fever who was latterly diagnosed with lung HD.(Turkiye Parazitol Derg 2012; 36: 45-7), Kist hidatik (KH) dünyada oldukça yaygın olup çoğunlukla köpek dışkısı yoluyla insana bulaşan tarım ve hayvancılığın yaygın olduğu ülkelerde sık görülen Echinococcus granulosus’un neden olduğu paraziter bir hastalıktır. KH klinik olarak sıklıkla karaciğer ve akciğere yerleşmektedir. Akciğer yerleşiminde göğüs ağrısı, öksürük, nefes darlığı, nonspesifik ateş ve hemoptizi gibi şikâyetlere neden olabilir. KH tanısında görüntüleme yöntemleri Akciğer Grafiği, Tomografi ve fizik muayeneyi içerir. Akciğer kist hidatiğinin tedavisi klinik bulgular ve cerrahi deneyime bağlı olarak değişmekle birlikte öncelikli olarak cerrahidir. Cerrahi tedavide en sık kistektomi ve kapitonaj yöntemleri uygulanmaktadır. Medikal tedavi komplike vakalarda cerrahiye ek olarak ve ya çıkarılamayacak kadar çok lezyonu olanlarda, cerrahi girişimi reddedenlerde ve tolere edemeyeceği düşünülen vakalarda uygulanır. Tedavide albendazol ve türevleri kullanılmaktadır. Bu vaka takdiminde, biz öksürük, göğüs ağrısı, dispne, halsizlik ve ateş gibi nonspesifik semptomları olan ve radyolojik olarak KH düşünülen çocuk olguda akciğer kist hidatiği sunduk.
