Objective: The effect of subcutaneous rifampicin administration on postoperative early wound discharge in patients undergoing hemiarthroplasty after femoral neck fracture was investigated. Methods: Between 2013 and 2015, 58 (36 female, 22 male) patients aged 65-94 (mean 79.29±7.99 years) who underwent hemiarthroplasty for hip fracture were included in the study. Two of the patients were Garden type 2, 21 were Garden type 3 and 35 were Garden type 4. The patients were followed from the postoperative period until discharge, and the length of stay was recorded. The subcutaneous rifamycin-administered group (group 1), saline irrigation-only group (group 2), and saline solution-added rifamycin group (group 3) were examined in 3 groups. Results: The duration of discharge was statistically significant according to age (p<0.05). The mean duration of wound discharge was significantly higher in patients older than 75 years (p=0.02). The operation duration of RIF + SF irrigation was significantly higher than that of SF irrigation (p=0.037). There was no statistically significant relationship between operation duration, incision length and additional diseases, and discharge time (p>0.05). Conclusion: There was no significant difference between the groups in terms of postoperative discharge times. The operative time was longer in patients who received subcutaneous rifamycin and were older than 75 years. Additional or isolated rifamycin application for hip hemiarthroplasty irrigation has no superiority over the saline solution. [ABSTRACT FROM AUTHOR]

Introduction: Cell salvage and tranexamic acid are used in total hip arthroplasty to reduce allogeneic blood transfusions. This study aimed to determine the effect of tranexamic acid used before surgery on the volume of intraoperative and postoperative allogeneic blood transfusions, hospital costs, and postoperative complications in geriatric patients undergoing total hip arthroplasty by comparing it with that of the cell salvage and control treatment. Materials and Methods: The data of 1408 patients in who underwent primary total hip arthroplasty at a single center between January 2012 and October 2021 were retrospectively analyzed. Patients were classified into three groups according to the treatment used cell salvage (n=227), tranexamic acid (n=485), and control treatment (n=533). Blood loss, volume of allogeneic blood transfused, hospitalization, cost parameters and thromboembolic complication rates were evaluated. Results: Patients in the TXA group received the least volume of blood transfusions during the intraoperative and postoperative periods (p=0.001, p=0.004 respectively). The volume of autologous blood transfusion in the cell salvage group was similar to the volume allogeneic blood transfusions in the tranexamic acid group (p=0.92). The cost was significantly lower in the tranexamic acid group (p<0.001), whereas it was significantly higher in the cell salvage group compared to that in the other groups. Conclusion: In total hip arthroplasty, a common procedure in the geriatric population, preoperative use of tranexamic acid is associated with less intraoperative and postoperative blood loss, less allogeneic blood transfusion, and lower hospital costs, without increasing the risk of thromboembolic events [ABSTRACT FROM AUTHOR]

Objectives: This study aims to categorize and map the incidence and patterns of upper extremity fractures in children during and after novel coronavirus disease 2019 (COVID-19) quarantine and to identify changes in the demographic characteristics and mechanisms of these fractures., Patients and Methods: Between April 2020 and April 2022, a total of 3,549 upper extremity fractures occurring in 1,028 pediatric patients (682 males, 346 females; median age: 7 years; range, 0 to 18 years) were retrospectively analyzed. Those who presented between the dates of April 1 st , 2020 and April 1 st , 2021 (quarantine) were included in Group 1, whereas those who presented between April 1 st , 2021 and April 2 nd , 2022 (post-quarantine) were included in Group 2. The fracture map also showed the fracture density and location., Results: There were statistically significant differences in terms of age range between Groups 1 and 2 (p<0.01). The 6-11 age range was significantly higher in Group 1, and the 12-18 age range was significantly higher in Group 2., Conclusion: Reducing physical activity during quarantine reduces fractures, particularly in adolescents. The removal of restrictions increases fractures in children in this age range. These findings highlight the importance of considering age ranges and physical activity levels while planning safety measures to prevent injuries in children.
