A 4-month-old girl presented with poor head control. She was born at term without complications. Parents were nonconsanguineous but from the same village. Her neurological examination revealed normal deep tendon reflexes with flexor plantar responses. She had microcephaly (head circumference 36 cm, -3.5 SD) and also dysmorphic features including a high arched palate, long philtrum, anteverted auricles. Metabolic studies including ammonia, serum amino acids, urine organic acids, acylcarnitine profile, lactate, pyruvate levels, and thyroid function studies were normal. Magnetic resonance imaging (MRI) of the brain showed delayed myelination (hyperintensities in the posterior limb of internal capsule) in the T2- weighted image and restricted diffusion in the posterior limb of the internal capsule, optic radiation, cerebral peduncles, substantia nigra, the pyramidal tracts in the midbrain, and cerebellar white matter [Figure 1]. After one month of follow-up, the patient exhibited intractable tonic seizures. Interictal electroencephalogram showed focal spike and slow waves in the left occipital region. She was treated with phenobarbital, levetiracetam, and topiramate. Whole–exome sequencing (WES) analysis revealed a homozygous pathogenic splice-site variant c. 124 + 1G > A located in intron 2 of ITPA gene (PVS1, PM2, PP3, PP5). This variant had been reported previously (rs376142053).{Figure 1}

OBJECTIVE: In this study, it was aimed to assess the neurological evaluation of children diagnosed with sydenham chorea.MATERIAL AND METHODS: Children diagnosed with Sydenham's chorea were retrospectively screened and evaluated for demographic, clinical, laboratory, neuroimaging, echocardiographic findings (ECHO), presenting complaints, and response to the treatment.RESULTS: Ten patients with Sydenham chorea were included in the study. Eight of the patients were female (80%), two were male (20%), the female / male ratio was 4 and the mean age at diagnosis was 12.05 ± 2.84 years. Carditis was detected in all cases (100%) at the admission. ECHO examination revealed mitral insufficiency in six cases (60%), mitral insufficiency and aortic insufficiency in four cases (40%). When the cases were evaluated in terms of presenting complaints, seven cases (70%) had choreiform movements; one case choreiform movements and gait disturbance; one patient presented with hemiparesis, dysarthria, and choreiform movements and one patient presented with emotional instability and gait disturbance. Generalized chorea was observed in four cases (40%) and hemicorea in six cases (60%). Four (66.6%) of the cases hadright hemicorea and two had left hemicorea (33.3%). Valproate treatment (10-15 mg / kg / day) was administered to all patients. The duration of the disappearance of choreiform movements ranged from 20 to 90 days (mean 42.50 ± 22.39 days). No side effects were observed in any of the patients. Only one patient had a positive ANA. When the neuroimaging was examined, brain magnetic resonance imaging (MRI) was normal in eight cases (80%) and pathology was observed in two cases (20%). Brain MRI revealed glioticlesions in the periventricular white matter in one case and in the bilateral frontal lobe deep white matter in the other case.CONCLUSIONS: Although the neurological examination is important in the diagnosis of Sydenham's chorea, it should be kept in mind that patients may present with behavioral symptoms before choreiform movements. It can be suggested that valproate is an effective and safe treatment option and non-specific hyperintense lesions seen on brain MRI are the result of inflammation and vasculitis mechanisms., AMAÇ: Bu çalışmada Sydenham koresi tanısı alan çocukların nörolojik açıdan değerlendirilmesi amaçlanmıştır.GEREÇ VE YÖNTEM: Tanı alan hastalar retrospektif olarak taranıp demografik, klinik, laboratuar, nörogörüntüleme, ekokardiyografi (EKO), başvuru yakınmaları ve uygulanan tedaviye yanıtları bakımından değerlendirilmiştir.BULGULAR: Çalışmaya 10 olgu dahil edildi. Hastaların sekizi kız (%80), ikisi erkek (%20), kız/erkek oranı 4, ortalama tanı yaşı 12,05±2,84 yıl olarak saptandı. Başvuru anında olguların hepsinde (%100) kardit tesbit edildi. EKO incelemesinde altı olguda (%60) sadece mitral yetmezlik, dört olguda (%40) ise mitral yetmezlik ve aort yetmezliği mevcuttu. Başvuru yakınmaları açısından değerlendirildiğinde yedi olgu (%70) koreiform hareketler; bir olgu koreiform hareketler ve yürüyüş bozukluğu; bir olgu hemiparezi, dizartri ve koreiform hareketler ve bir olgu da emosyonel instabilite ve yürüyüş bozukluğu nedeniyle başvurdu. Jeneralize kore dört olguda (%40), hemikore altı olguda (%60) izlendi. Olguların dördünde sağ (% 66,6) ve ikisinde sol (%33,3) hemikore bulgusu saptandı. Hastaların tümüne valproat tedavisi (10-15 mg/kg/gün) başlandı. Tedavi ile koreiform hareketlerin kaybolma süresi 20-90 gün (ortalama 42,50±22,39 gün) arasında değişti. Hastaların hiç birinde yan etki gözlenmedi. Yalnızca bir hastanın ANA değeri pozitif olarak saptandı. Olguların sekizinin (%80) beyin manyetik rezonans görüntüleme (MRG) tetkiki normal olup iki olguda (%20) patoloji izlendi. Beyin MRG’de bir olguda periventriküler beyaz cevherde bir olguda ise bilateral frontal lob derin beyaz cevherde gliotik lezyonlar saptandı.SONUÇ: Tanıda nörolojik muayene önemli olmakla beraber hastaların koreiform hareketlerden önce davranışsal semptomlar ile de başvurabileceği akılda tutulmalıdır. Valproatın etkin ve güvenilir bir tedavi seçeneği olduğu ve beyin MRG’de görülen spesifik olmayan hiperintens lezyonların inflamasyon ve vaskulit mekanizmalarının sonucu ortaya çıktığı ileri sürülebilir.

AMAÇ: Nörofibromatoz tip I (NF1), Tuberoskleroz kompleksi (TSC) ve Sturge-Weber sendromu (SWS) en sık görülen nörokütanöz sendromlar arasında yer almaktadır. Bu çalışma ile NF1, TSC ve SWS tanısı alan hastaların klinik ve nöroradyolojik bulgularını değerlendirmeyi amaçladık.GEREÇ VE YÖNTEM: Kliniğimizde Aralık 2017 ile Mayıs 2019 arasında NF1, TSC ve SWS tanılı 15 hastanın kayıtları retrospektif olarak incelendi. Hastaların klinik ve nöroradyolojik bulguları ayrıntılı olarak değerlendirildi.BULGULAR: Dokuz gün ile 13,83 yaş arasında (5 kız ve 10 erkek) değişen 9 NF1, 5 TSC ve 1 SWS tanısı alan 15 hasta belirlendi. Tüm NF1 hastalarında café au lait lekeleri vardı. Bir olguda (% 11.1) Lisch nodülü, bir olguda (% 11.1) optik gliom, bir olguda (% 11.1) epilepsi, iki olguda(% 22.2) makrosefali, iki olguda (% 22.2) hidrosefali ve yedi olguda aile öyküsü (% 77,7) saptandı. Tüm TSC hastalarında hipomelanotik maküller ve epilepsi vardı. Üç hastada (% 60) kardiyak rabdomiyom, bir hastada (% 20) anjiyomiyolipom ve bir hastada (% 20) polikistik böbrek mevcuttu. SWS tanılı olguda fasiyal anjiyom, glokom ve epilepsi saptandı. NF1 tanılı hastalarda 6 olguda (% 66.6) UBO (bilinmeyen parlak cisim), 2 olguda(% 22.2) hidrosefali, bir olguda (% 11.1) subependimal nodül ve bir olguda (% 11.1) optik gliom gösterildi. Tüm TSC hastalarının beyin manyetik rezonans görüntülemesinde kortikal / subkortikal tüberler ve iki hastada subependymal nodül saptandı. SWS tanılı hastanın kraniyal bilgisayarlı tomografisinde kortikal ve parankimal atrofi ve kalsifikasyon gösterildi.SONUÇ: Nörokütanöz sendromların klinik ve nöroradyolojik bulguları tanı açısından yol gösterici olup, ayrıntılı inceleme ve nöroradyolojik bakış açısı tanı ve takipte kolaylık sağlayacaktır., OBJECTIVE: Neurofibromatosis type I (NF1), Tuberous sclerosis complex (TSC) and Sturge-Weber syndrome (SWS) are the most common neurocutaneous syndromes. The purpose of this study is to evaluate the clinical and neuroradiological manifestations of patients diagnosed with NF1, TSC, and SWS.MATERIAL AND METHODS: In our clinic, records of 15 patients with NF1, TSC, and SWS were retrospectively reviewed between December 2017 and May 2019. Clinical and neuroradiological manifestations of patients were detailed.RESULTS: 15 patients consisting of 9 NF1, 5 TSC, and 1 SWS were determined between the ages of 9 days and 13,83 years (5 females and 10 males). All NF1 patients had café-au-lait spots. One (11.1%) with lisch nodules, one (11.1%) with optic glioma, one (11.1%) with epilepsy, two (22.2%) with macrocephaly, two (22.2%) with hydrocephalus, one (11.1%) with optic glioma and seven (77.7%) with family history were established. All TSC patients had hypomelanotic macules and epilepsy. It was indicated cardiac rhabdomyoma in 3 (60%) patients, angiomyolipoma in one (20%) patient and polycystic kidney in one (20%) patient. The patient with SWS was showed available of facial angioma, glaucoma, and epilepsy. It was pointed UBOs (unknown bright objects) with 6 (66.6%) patients, 2 (22.2%) patients with hydrocephalus, one (11.1%) patient with subependymal nodule and one (11.1%) patient with optic glioma in NF1 patients. Brain magnetic resonance imaging of all TSC patients showed cortical/subcortical tubers and two patients with subependymal nodules. Cortical and parenchymal atrophy and calcification were presented in cranial computed tomography of the patient with SWS.CONCLUSIONS: Clinical and neuroradiological manifestations of neurocutaneous syndrome are guidance for diagnosis so that detailed examination and neuroradiological perspective on neurocutaneous diseases will provide convenience in diagnosis and follow-up.

Inthis study, the role of glycine on the mitotic index, chromosome aberrations, micronucleusfrequency as cytogenetic parametersand on the seed germination, radicle length, radicle number, fresh weightas physiological parameters in Allium cepa L. seeds exposed to saltnesswere studied. Salinity displayeda significant inhibitory effect on the seedling growth and seed germination of Allium cepa. Furthermore, salinityreduced significantly the mitotic index in A. cepa root tip cells and ascended number of chromosomal aberrationsand frequencyof micronucleus which is the simplestindicator, the most effective of cytological damage. On the other hand, the restricting effectsof salinity on the seed germination, mitotic activity, chromosomal aberrations and seedling growth were alleviated dramatically invarying degrees by glycine application, but glycine was ineffective in reducingof salt damage on the micronucleusfrequency., Bu çalışmada, tuzluluğa maruz bırakılan Allium cepa L. tohumlarındakifizyolojik parametreler olarak tohum çimlenmesi,radikula uzunluğu, radikula sayısı, taze ağırlık ve sitogenetik parametreler olarak mitotik aktivite, kromozomal anormallikler vemikronükleus sıklığı üzerine glisinin rolü incelenmiştir. Tuzluluk A. cepa'nın tohum çimlenmesi ve fidebüyümesinde önemli bir inhibe edici etki göstermiştir. Dahası,tuz A. cepa’nın kök ucuhücrelerindeki mitotik indeksi önemli ölçüde düşürmüş ve sitolojik hasarın enbasit ve en etkili göstergesi olan mikronükleus sıklığı ve kromozomalanormalliklerin sayısını arttırmıştır. Buna karşılık, tohum çimlenmesi,mitotik aktivite, kromozomal anormallikler ve fide büyümesi üzerine tuzuninhibe edici etkileri glisin uygulamasıyla önemli ölçüde azalmış, fakatmikronükleus sıklığı üzerinde glisin tuz hasarının azaltılmasında yetersizkalmıştır.

In this study, β-alanine effects on the seedling growth (radiclenumber, radicle length and fresh weight), seed germination, chromosomalaberrations, mitotic activity and micronucleus frequency in Allium cepa L. germinated in both salt stressand normal conditions investigated. In onlyβ-alanine medium, the fresh weight, radiclenumber and radicle length of the seeds partially reduced compared to the controlseeds germinated in distilled water medium but the germination percentage indicate statistically the samevalue ​​as the control. Besides, while the micronucleus formation and mitotic index in root tip meristems of A. cepa seeds germinated in aloneβ-alanine medium showed the same values as the control seeds germinated in distilledwater medium, the chromosomal abnormalities exhibitedan increase compared to the control. On the other hand, salt stresssignificantly inhibited the seedling growth and seed germination of A. cepa.What’s more, it majorly reduced the mitotic index in meristem cells of the seedsand increased the number of chromosomal abnormalities and micronuclei formation,which is the simplest indicator and the most effective of cytological damage. Althought the harmful affects of saltness on the chromosomal aberrations, seed germination, seedling growth and mitoticactivity alleviated in varying degrees by β-alaninetreatment, this application was inefficient in damage reducing of salt on the micronucleus formation., Bu çalışmada, hem normal hem de tuz stresi koşullarında çimlendirilen Allium cepaL.’da fide büyümesi (radikula sayısı, radikula uzunluğuve taze ağırlık), tohum çimlenmesi, kromozomalanormallikler, mitotik aktivite ve mikronükleussıklığı üzerine β-alaninin etkileriincelenmiştir. Tek başına β-alanin ortamında,tohumların taze ağırlığı, radikula sayısı veradikula uzunluğu distile su ortamında çimlendirilen kontrol tohumlarınakıyasla kısmen azalmış ancak çimlenme yüzdesiistatistiki olarak kontrolle aynı değeri göstermiştir. Bununla birlikte, tekbaşına β-alanin ortamında çimlendirilen A. cepatohumlarının kök ucu meristem hücrelerindeki mitotik indeks ve mikronükleus oluşumu distile su ortamında çimlendirilenkontrol tohumlarıyla aynı değerleri gösterirken, kromozomal anormalliklerkontrole kıyasla bir artış sergilemiştir. Diğer taraftan, tuz stresi A. cepa'nın tohum çimlenmesi ve fide büyümesini önemli ölçüde engellemiştir. Dahası,tuz tohumların kök ucu hücrelerindekimitotik indeksi önemli ölçüde azaltmış vesitolojik hasarın en basit ve en etkili göstergesi olan mikronükleus oluşumu vekromozomal anormalliklerin sayısını arttırmıştır. Kromozomalanormallikler, tohum çimlenmesi, fide büyümesi ve mitotik aktivite üzerindekituzun zararlı etkileri β-alanin uygulamasıylaçeşitli derecelerde azalmasına rağmen bu uygulamamikronükleus oluşumu üzerindeki tuz hasarının azaltılmasında etkisiz olmuştur.

Tıp biliminin gelişmesiyle birlikte tıbbi uygulamalara olan güven de hızla artış göstermiştir. Tıbbi uygulamalardaki çeşitlilik hekimlerin insan yaşamındaki önemini de artırmıştır. Bireylerin hayatına ve kişilik haklarına doğrudan müdahalede bulunan hekimlerin, dikkat ve özen yükümlülüğü oldukça fazladır. Hekimlerin tıbbi uygulamaları ceza hukuku açısından önem taşımaktadır. Son zamanlarda gerçekleşen malpraktis vakıaları ile hekimler tarafından işlenebilen diğer suçlardaki artış nedeniyle çalışmamızda bu konulara açıklık getirilmiştir. Bir kısım tıbbi müdahaleler gerek yaşama hakkı gerek kişilik hakları gerekse kamu düzeni gereğince belirli kurallara bağlanmıştır. Çalışmamızda hekimlerin söz konusu uygulamalarının kuralları ve cezai boyutu üzerinde durulmuştur.--------------------Due to the development of medicine, trust to medical pratice rapidly increases recently. The diversity in medical practice has increased the importance of medical doctors in human life. Duty of care of medical doctors who toucher lives and personal rights of individuals is strong. Medical practice of medical doctors is consideres as a key element in criminal law. In this work focus is mostly on the malpractice cases and other crimes which can be commited by medical doctors, due to the increasing number of both cases. Some medical interventions require stricted rules in public order perspective in regord of the right to live and personal rights. In this work, the rules on mentioned practices by medical doctors and criminal aspect of these issues are discussed.
