32. WHAT HAPPENS WHEN YOU TREAT SEPTIC CEREBRAL EMBOLI WITH INTRAVENOUS THROMBOLYTIC TREATMENT? A CASE REPORT

Abstract

Infective endocarditis (IE) is the infection of the endocardial surfaces of the heart and intracardiac foreign bodies, and is caused by various microorganisms such as bacteria, fungus and viruses. Most common findings of IE include cardiac murmur, splenomegaly, Osler nodes, splinter hemorrhages, and Janeway lesions. Here, we aimed to present a IE case which was misdiagnosed as thromboembolic stroke, treated with intravenous (IV) thrombolytic treatment and developed multiple cerebral and cerebellar hemorrhages. A 35-year-old man with a history of diabetes mellitus (DM) and hypertension (HT) was admitted to our clinic with multiple cerebral and cerebellar hemorrhages. It was found out that the patient was given an antibiotic treatment due to high fever (39-40 oC) and pharyngitis two weeks before admission to our clinic, and a week later he developed right side weakness. As computerised tomography (CT) and diffusion-weighted magnetic resonance imaging (MRI) were normal at the clinic the patient applied with right sided weakness, he was diagnosed with acute ischemic stroke and was given intravenous thrombolytic treatment-alteplase (IV rTPA). Patient was referred to our clinic due to persistent fever, clinical worsening and multiple cerebral and cerebellar hemorrhages in follow-up CT scan. The patient was referred to the Cardiology clinic with the preliminary diagnosis of infective endocarditis. Echocardiography (ECO) showed a thrombotic vegetation measuring 1.1 x 0.7 centimeters (cm) on the anterior mitral valve, and transoesophageal ECO showed a thrombotic vegetation measuring 1.9 x 0.9 on the mitral valve. The patient was referred to Infctious Diseases clinic for initiation of antibiotherapy, and Cardiovascular Surgery clinic regarding the need for surgery. Yet, as the surgery required high-dose heparin application, an operation was not performed. After the treatment, hemorrhage areas were seen to be regressed in follow-up brain CT scan. While evaluating patients who admit to the emergency with high fever and neurological deficits, septic embolism should also be kept in mind at the first visit and imaging methods should be used for further evaluation.

39. A Rare Case Report of Eight Syndrome Secondary to Syringomyelia Associated with Type I Chiari Malformation.

Abstract

Eight syndrome is defined as the combination of a unilateral conjugate gaze palsy and ipsilateral seventh cranial nerve palsy. It may occur as a result of demyelinating, vascular, infectious, or compressive lesions of the brainstem localized to the caudal pontine tegmentum. A 43-yearold woman was admitted to our clinic with complaints of headache, inability to look to the left, and weakness on the left side of her face. The complaints had begun abruptly about a month before her admission. Suboccipital decompression surgery for type I Chiari malformation had been performed 10 years earlier. Neuro-ophthalmological examination revealed left-sided horizontal gaze palsy and anisocoria. Cranial and cervical magnetic resonance images revealed cerebellar tonsillar herniation and syringomyelia, the latter of which was considered to be the cause of eight syndrome. No interventions were performed, and periodic follow-up was advised on neurosurgical consultation. Left gaze palsy and facial palsy recovered almost completely in three months, while the anisocoria persisted. Syringomyelia should be considered among the causes of horizontal gaze palsy plus ipsilateral seventh nerve palsy, termed as eight syndrome. Abstract

Objective: This study aims to determine the clinical features associated with the development of vestibular migraine (VM) in patients with migraine headaches. Methods: A cross-sectional, multicenter study was performed in nine tertiary neurology clinics. Patients with migraine without vestibular symptoms were classified as having migraine only (MO) and compared with patients with VM to determine any differences in clinical features, associated disorders, past medical history, and family history of migraine headaches. Moreover, we investigated the features that might predict the development of VM. Results: Two hundred forty-four patients with MO and 461 patients with VM were included. The age of onset of headache attacks was later in life for patients with VM (p<0.001). Migraine without aura (MwoA) was significantly more common than migraine with aura (MwA) in patients with VM (p=0.016). All associated features of migraine headaches were significantly more frequent in patients with MO than patients with VM (p<0.005). The same was true for all triggers, including fasting, sleep disturbances, menstruation, stress, flickering lights, and smartphones/computer games (p<0.005). A family history of migraine headaches was more common in MO patients (p=0.002). However, a previous history of motion sickness was significantly more common in patients with VM (p<0.001), as was aural fullness/tinnitus accompanying attacks (p<0.001). Logistic regression analysis indicated that aural fullness/tinnitus accompanying attacks and a previous history of motion sickness were risk factors for the development of VM. Conclusion: Patients with migraine reporting aural symptoms accompanying attacks and motion sickness in their past medical history are at increased risk of vestibular attacks fulfilling the diagnosis of VM later in life.
