Aim: Severe Acute Respiratory Syndrome Coronavirus 2 (SARS‐CoV‐2) infection may have a more severe course in patients with underlying disease or who have had immunosuppression. In this study, it was aimed to determine the frequency of coronavirus disease 2019 (COVID‐19) and the mortality rates related to COVID‐19 among patients with rheumatic disease. Methods: The patients who were followed up with rheumatic disease in the rheumatology outpatient clinic in a tertiary hospital were retrospectively assessed if they had COVID‐19 infection or not between March 2020 and January 2022. Results: A total of 10 682 patients were evaluated. There were 2928 (27.4%) COVID‐19‐positive and 7754 (72.6%) COVID‐19‐negative patients. The mean age of COVID‐19‐positive patients was 46.2 ± 14.6 years, and 65.8% were female. Forty‐two (1.4%) patients died due to COVID‐19. Among COVID‐19‐negative patients, 192 patients died. The most common rheumatic disease among patients with COVID‐19 was spondyloarthritis (SpA) (30.4%). Corticosteroids were the most common treatment agent in COVID‐19‐positive patients regardless of mortality. Thirty‐one (73.8%) patients were receiving corticosteroids, and 35 (83.3%) patients were receiving immunosuppressive agents among patients with mortality. According to the logistic regression analysis, older age, male gender, and receiving corticosteroid, hydroxychloroquine, mycophenolate mofetil, tofacitinib, rituximab, and cyclophosphamide were found to be related to increased mortality. Conclusion: COVID‐19 is a serious infection and the current study emphasized that patients with rheumatic diseases had increased mortality rates, particularly in patients who were old, male, and on immunosuppressive treatments. Objectives: Behçet's disease (BD) is a multisystem disease and frequently occurs during the second–fourth decades of life, although disease onset may be seen at any age. This study aimed to analyze the influence of the age of onset on clinical manifestations of BD. Materials and methods: This retrospective study analyzed two cohorts (paediatric and adult) to determine the association between the age of onset and clinical features in BD. Patients were classified into four groups to analyze the clinical characteristics according to the age of fulfilling the BD diagnostic criteria as follows: childhood onset (<12 years), adolescent onset (13–17 years), adult onset (18–39 years), and late onset (>40 years). Results: The study included 801 patients with BD. Male predominance, pathergy test positivity, aphthosis (oral or genital), and skin and ocular involvements were higher among adult patients than paediatric patients. The presence of positive family history for BD, neuro-BD, and epididymitis were observed significantly common in the paediatric group. Conclusion: There may be differences in clinical manifestations with regard to the age of disease onset. Disease presentations may differ from adult patients, and clinicians should be aware of the high familial aggregation rate of BD, especially in countries where the disease is endemic. Elektrik güç kalitesi hem şebeke yönetimi hem de son kullanıcılar için son derece önemli bir konudur. Bu çalışmada, TÜBİTAK MAM tarafından TEİAŞ için yürütülmekte olan bir AR-GE projesi kapsamında geliştirilmesine devam edilen TEİAŞ Elektrik Güç Kalitesi ve Şebeke İzleme Sistemi (TEKİS) tanıtılmaktadır. TEKİS; daha öncesinde Milli Güç Kalitesi Projesi kapsamında geliştirilen Güç Kalitesi İzleme Sistemi’nin yerini almak üzere geliştirilen ve yeni yazılım ve yapay zekâ teknolojileriyle yetenekleri büyük ölçüde artırılmış ve çeşitlendirilmiş tamamen yeni ve geniş-ölçekli bir sistemdir. TEKİS; geniş alan izleme, fazör ölçüm birimi, güncel ve tarihsel veri sorgulama, ilgili çeşitli standartlara göre otomatik raporlar oluşturma, detaylı arıza analizleri, gerilim çukuru yönü belirleme ve yapay zekâ tabanlı olay sınıflandırma gibi önemli yeteneklere sahiptir. Çalışmamızda sistemin güncel sürümünün mimarisi ve örnek ekran görüntüleri de sunulmaktadır.

- Abstract
[Objectives] Behçet's disease tends to be more severe in men than women. This study was undertaken to investigate sex-specific genetic effects in Behçet's disease., [Methods] A total of 1762 male and 1216 female patients with Behçet's disease from six diverse populations were studied, with the majority of patients of Turkish origin. Genotyping was performed using an Infinium ImmunoArray-24 BeadChip, or extracted from available genotyping data. Following imputation and extensive quality control measures, genome-wide association analysis was performed comparing male to female patients in the Turkish cohort, followed by a meta-analysis of significant results in all six populations. In addition, a weighted genetic risk score for Behçet's disease was calculated and compared between male and female patients., [Results] Genetic association analysis comparing male to female patients with Behçet's disease from Turkey revealed an association with male sex in HLA-B/MICA within the HLA region with a GWAS level of significance (rs2848712, OR = 1.46, P = 1.22 × 10−8). Meta-analysis of the effect in rs2848712 across six populations confirmed these results. Genetic risk score for Behçet's disease was significantly higher in male compared to female patients from Turkey. Higher genetic risk for Behçet's disease was observed in male patients in HLA-B/MICA (rs116799036, OR = 1.45, P = 1.95 × 10−8), HLA-C (rs12525170, OR = 1.46, P = 5.66 × 10−7), and KLRC4 (rs2617170, OR = 1.20, P = 0.019). In contrast, IFNGR1 (rs4896243, OR = 0.86, P = 0.011) was shown to confer higher genetic risk in female patients., [Conclusions] Male patients with Behçet's disease are characterized by higher genetic risk compared to female patients. This genetic difference, primarily derived from our Turkish cohort, is largely explained by risk within the HLA region. These data suggest that genetic factors might contribute to differences in disease presentation between men and women with Behçet's disease.

Objective: The coronavirus disease 2019 pandemic has been resulting in increased hospital occupancy rates. Rheumatic patients cannot still reach to hospitals, or they hesitate about going to a hospital even they are able to reach. We aimed to show the effect of the first wave of coronavirus disease 2019 pandemic on the treatment of biological disease-modifying anti-rheumatic drugs in patients with rheumatoid arthritis or spondyloarthritis. Methods: Patients were divided into three groups as follows: pre-pandemic (Pre-p: starting on biological disease-modifying anti-rheumatic drug therapy for the first time within 6 months before March 11, 2020); post-pandemic A (Post-p A: starting on biological disease-modifying anti-rheumatic drug therapy for the first time within the first 6 months after March 11, 2020); post-pandemic B (Post-p B: starting on biological disease-modifying anti-rheumatic drug therapy for the first time within the second 6 months). Results: The number of rheumatoid arthritis patients in the Post-p A and B groups decreased by 51% and 48%, respectively, as compared to the Pre-p group similar rates of reduction were also determined in the number of spondyloarthritis patients. The rates of tofacitinib and abatacept use increased in rheumatoid arthritis patients in Post-p period. Conclusion: The number of rheumatoid arthritis and spondyloarthritis patients starting on biological disease-modifying anti-rheumatic drugs for the first time decreased during the first year of the coronavirus disease 2019 pandemic. Objective: some studies have shown that the use of complementary and alternative medicine (CAM) is common in patients with chronic painful conditions, such as ankylosing spondylitis (AS). This study grimed to determine the prevalence and types of CAM usage in patients with AS and to evaluate the impact of treatment adherence and beliefs about medicines on CAM usage. Methods: this study has a descriptive design. A total of 140 patients with AS were included. The treatment adherence of the patients was evaluated using the Morisky Green Levine Medication Adherence Scale. The Beliefs about Medicines Questionnaire (BMQ-T) was used to assess patients' beliefs about medicines. Results: previous or current CAM usage was stated by 40% of the patients. It has been found that CAM usage was significantly high (p<0.05) in patients who were married, older, and diagnosed at older ages. The difference between patients'beliefs about medicines and CAM usage was not statistically significant (p>0.05). The BMQ-T scores were significantly different in terms of the patients'treatment adherence (p<0.05). Conclusion: this study showed that approximately half of the patients with AS were using 1 CAM method. Furthermore, medication adherence and patients'beliefs about medicines did not have any impact on CAM usage, but the patients beliefs about medicines affected treatment adherence., NA

Bu tez çalışmasının temel amacı, Atık Mermer Tozu (WMP) ve Uçucu Kül (FA) ikame edilmiş çimento bazlı enjeksiyon karışımlarının yüksek su / bağlayıcı (w / b) oranlarında reolojik, işlenebilirlik, taze ve sertleştirilmiş özellikleri üzerindeki etkisini araştırmaktır.Doğal taş olan mermer talebi hızlı artışa bağlı olarak mermer ile ilgili işletme ve fabrika sayısı da artmaktadır. Bu fabrikaların kapasitesine bağlı olarak çamur, toz ve mermer atıkları ortaya çıkmakta; atık miktarı %75'e kadar ulaşmaktadır. Mermer atıklarının kullanılabilir tarımsal alanlara boşaltılması, çok ince toz şeklinde olan kısmının havaya ve suya karışması çevre kirliliğine neden olur, sağlığa olumsuz etki yapar, uzun sürede tarım arazilerini kaplayarak toprağa ve ürünlere zarar verir. Uçucu Kül, termik santrallerin baca gazından kaynaklanan silika esaslı toz halinde bir maddedir. Puzolanik etkisi olduğu için çimento hidratasyonundan açığa çıkan serbest kireci bağlar. Bu özelliği nedeni ile uçucu kül, beton üretiminde yaygın olarak kullanılmaktadır, ancak Türkiye'de elektrik enerjisine olan ihtiyacın hızla artması, mevcut santralların yetersiz kalması yeni termik santralların kurulmasını zorunlu kılmaktadır, bu da ülkemizde yıllık yaklaşık 13 milyon ton olan bu atık miktarının daha da artacağı anlamına gelmektedir. Günümüzde tüm dünyada çevresel bir sorun olan katı atıklardan mermer tozu ve uçucu külün inşaat sektörünün çok önemli bir alanı olan zeminin iyileştirilmesinde kullanılması, bu atıkların ekonomik değer kazanmasını sağlayacak, katı atıkların bertaraf edilmesi ile çevre kirliliğinin önlenmesinde önemli katkıda bulunacaktır. Bu tez çalışmasında; atık sorununun çözümüne katkıda bulunmak için mermer tozu ve uçucu külün zeminlerin iyileştirilmesinde kullanılan çimento esaslı enjeksiyon harcının üretiminde kullanılması amaçlanmıştır. Bu amaçla planlanan deneysel çalışmada atık mermer tozu (WMP) ve uçucu kül (FA) çimentoya ikame edilmiş; yüksek su / bağlayıcı (w / b) oranlarındaki çimento esaslı enjeksiyon karışımlarının reolojik ve işlenebilirlik gibi taze, basınç dayanımı gibi sertleştirilmiş özellikleri deneysel çalışma ile araştırılmıştır. Bu karışımların zemine yapacağı etkilerin model deneyler ile araştırılması kapsam dışı bırakılmıştır. Çimento esaslı enjeksiyon harçlarının üretiminde dört seri karışım hazırlanmıştır. Hazırlanan ilk seride çimentoya ağırlıkça %5, 10, 15, 20, 25 oranında atık mermer tozu ilave edilmiştir. İkinci seride çimentoya ağırlıkça %25 (sabit) oranında uçucu kül, %10, 15, 20, 25, 30 oranlarında atık mermer tozu ilave edilmiştir. Üçüncü ve dördüncü seri karışımda ise ilk iki serideki oranlar sabit kalmak koşulu ile tüm karışım oranınına ağırlıkca %20 oranında kil eklenmiştir. Her seride suyun bağlayıcıya oranı 0.75, 1.00, 1.25, ve 1.5 olarak seçilmiştir. Bu malzemeler ve oranları ile toplam 48 (kırk sekiz) farklı enjeksiyon karışımı hazırlanmıştır. Karışımların her birinde taze halde iken işlenebilirlik ve reoloji testlerini içeren plaka kohezyon, marsh hunisi akış zamanı, mini slump akış çapı ve silindirik dönen rheometre gibi bir dizi test yapılmıştır. Özellikleri belirlenen seri üç ve dört karışımlar ile çapı 5.5 cm, yüksekliği 11 cm olan 432 adet silindir numune hazırlanmış; 3, 7, 28 günlerde basınç deneyleri yapılmıştır. Her seride su / bağlayıcı oranları göz önüne alınarak kontrol numunesi hazırlanmıştır. Kontrol numunelerinin içine seri bir ve ikide sadece su ve çimento, seri üç ve dörtte çimento, kil ve su katılmış herhangi bir katkı maddesi ilave edilmemiştir.Deney sonuçları kontrol numuneleri ve literatür sonuçları ile karşılaştırılmış; atık mermer tozu ve uçucu kül kullanılarak hazırlanan çimento bazlı enjeksiyonun karışımlarının zayıf zeminlerin iyileştirilmesinde enjekte edilebilir ve kullanılabilir olduğu kanaatine varılmıştır. The main objective of this thesis study is to investigate the effect of Waste Marble Powder (WMP) and Fly Ash (FA) on the rheology, workability, fresh and hardened properties of cementitious grout with respect to higher water/ binder (w/b) ratios. Due to the rapid increase in the demand for marble, which is a natural stone, the number of enterprises and factories related to marble sector is increasing. Depending on the capacity of these factories, mud, powder and pieces of marble waste are exposed; waste amount reaches up to 75%. Emptying the marble wastes to the usable agricultural land, the mixing of the very fine powdered part into the air and the water causes environmental pollution, has a negative effect on the health, covering the agricultural land in a long time and damages the soil and the products.Fly Ash is a silica-based powdery substance originating from the flue-gas of thermal power plants. Because of its pozzolanic effect, it binds free lime from cement hydration. This feature caused by fly ash in concrete production are widely used, but the rapid increase in the demand for electrical energy in Turkey, inadequacy of existing plants necessitates the creation of new thermal power plants, which in our country annually about 13 million tons, which will further increase the amount of this waste it means.The use of marble dust and fly ash from the solid wastes, which is an environmental problem in the world, in the improvement of the ground, which is a very important area of the construction sector, will provide the economic value of these wastes and will contribute to the prevention of environmental pollution by the removal of solid wastes. In this thesis; In order to contribute to the solution of the waste problem, it is aimed to use marble dust and fly ash in the production of cement based grout which is used for the treatment of soils. In the experimental study planned for this purpose, waste marble powder (WMP) and fly ash (FA) were replaced with cement; in this study, the fresh and hardened properties of cement based grout mixtures were investigated by experimental study. The investigation of the effects of these mixtures on the ground by model experiments was excluded.Four series of mixtures were prepared for the production of cement based injection mortars. In the first series prepared, 5, 10, 15, 20, 25% by weight of the marble was added to the cement. In the second series 25% (fixed) by weight of fly ash, 10, 15, 20, 25, 30% of the amount of waste marble dust was added to the cement. In the third and fourth batch mix, 20% by weight of clay is added to the whole mixture ratio provided that the ratios in the first two series remain constant. The ratio of water to the binder in each series was chosen as 0.75, 1.00, 1.25, and 1.5. A total of 48 (forty-eight) different injection mixtures were prepared with these materials and ratios. In each of the mixtures, a series of tests, such as plate cohesion, marsh funnel flow time, mini slump flow diameter, and cylindrical rotating rheometer, were carried out, including freshness and rheology tests. Specifically prepared series of 432 cylindrical specimens with a diameter of 5.5 cm, 11 cm in height, with three and four series identified; unconfined compressive strength test (UCS) were performed on 3, 7, 28 days. In each series, control sample was prepared by considering water-binding ratios. Control samples ingredient included first and second series water and cement, thirth and fourth series water, cement and clay. No additives were added to the cntrol samples.The test results were compared with the control samples and the literature; It has been concluded that mixtures of cement based grout prepared using waste marble dust and fly ash can be injected and used in the treatment of weak soils. 190

Objectives: The aim of this study was to assess the demographic and clinical characteristics of patients with adult-onset Still's disease (AOSD) under biological treatment. Patients and methods: This retrospective cohort study included a total of 19 AOSD patients (13 males, 6 females; median age: 37 years; range, 28 to 52 years) who received biological drugs due to refractory disease between January 2008 and January 2020. The data of the patients were obtained from the patient files. The response to the treatment was evaluated based on clinical and laboratory assessments at third and sixth follow-up visits. Results: Interleukin (IL)-1 inhibitor was prescribed for 13 (68.4%) patients and IL-6 inhibitor prescribed for six (31.6%) patients. Seventeen (89.5%) patients experienced clinical remission. Conclusion: Biological drugs seem to be effective for AOSD patients who are resistant to conventional therapies. Due to the administration methods and the high costs of these drugs, however, tapering the treatment should be considered, after remission is achieved. [ABSTRACT FROM AUTHOR]

Social Network Analysis (SNA) is widely used in revealing meaningful information from the relationship networks in different domains. Crime organizations are also a social network which composed of members and relations between members. SNA used by security forces in the whole world in order to uncover structure of terrorist organizations and destabilization of them. Today considering that terror or cocaine organizations are not active in a region and a country and that they are accompanied by different criminal organizations in the context of different interest relations all over the world. The disclosing mathematical model of criminal networks via analyzing structure of criminal organizations, the identification and analysis of the key role players (Leaders, Reporters and Fundraisers etc.) who are interacting with the different groups and the people who is close to them provide the authorities of the countries affected by criminal activies to make effective decisions in their struggle with criminal organizations. One of the most important goals of the metrics used in the SNA is to identify the individuals who are the key players in the network. For the long-term future of all organizations exhibiting social network structures, the identification of those who can replace the key player in the organization is one of the most important issues in terms of the continuity and effectiveness of the organization. While the identification of successors which may cause the organization to be more effective is great importance for the efficiency and development of the organization, the identification of the succesors which may cause organizations such as criminal organizations to be more ineffective, is equally important. The thesis is summarized in four main titles. In this context; i) structural analysis of network structures of some real terrorist and cocaine criminal organizations in terms of SNA metrics and their differences from non-criminal social networks; ii) the creation of a new model for the identification of criminal organizations through the use of machine learning and statistical methods; iii) the creation of a model that includes different strategies for identifying the successors of key players and successors’ new connections to others within the criminal organizations; and iv) the analysis of changes in similarities of criminal networks to new networks which will be formed according to different strategies as a result of the removal of members from criminal organizations and detection of the member that will cause the most damage to the network. Since the military guides, which played a very important role in the state and the army of ancient Turks, have been given the name of YIZEK because of their superior military service and administrative experience, the thesis and a SNA metric created in this thesis was called YIZEK which will guide the decision-making systems of the security forces to neutralize criminal organizations. Suç örgütleri de, üyeleri ve üyelerin birbirleri arasındaki ilişkilerden oluşan bir sosyal ağdır. SAA suç örgütlerinin yapısının ortaya çıkarılması ve etkisizleştirilmesi adına tüm dünyada güvenlik güçleri tarafından yaygın olarak kullanılmaktadır. Günümüzde terör grupları ya da kokain ticareti yapan gruplar gibi suç örgütleri bir bölge veya ülkede faaliyet göstermemekte, tüm dünyada farklı çıkar ilişkileri çerçevesinde farklı suç örgütleri ile hareket etmektedirler. Suç örgütlerinin ilişki yapısının incelenerek matematiksel modelinin ortaya çıkarılması, farklı gruplar arasında irtibatı sağlayan anahtar roldeki kişilerin (Liderler, Haberciler, Para Akışı Sağlayanlar vb.) ve bunlara yakın kişilerin tespiti, suç örgütlerinin eylemlerinden etkilenen ülkelerin yetkili birimlerinin bu örgütlerle mücadelelerinde etkin karar vermelerini sağlamaktadır. Sosyal ağ analizinde kullanılan metriklerin en önemli amaçlarından birisi ağ içerisindeki anahtar roldeki kişilerin tespit edilmesidir. Sosyal ağ yapısı sergileyen tüm organizasyonların uzun vadede geleceği için organizasyon içerisindeki anahtar kişilerin yerine geçebilecek varis kişilerin tespit edilmesi de organizasyonun devamlılığı ve etkinliği açısından en önemli hususlardan birisi olmaktadır. Organizasyonun daha etkin olmasına sebep olabilecek varis kişilerin tespiti organizasyonun verimliliği ve gelişmesi için büyük öneme sahipken, suç örgütleri gibi organizasyonların daha etkisiz olmasına sebep olabilecek varis kişilerin tespiti de aynı derecede öneme sahip olmaktadır. Tez çalışması dört temel başlıkta özetlenmektedir. Bu kapsamda; i) gerçek bazı terörist ve kokain suç örgütlerinin oluşturduğu ağ yapılarının SAA metrikleri açısından yapısal olarak analizlerinin gerçekleştirilmesi ve normal sosyal ağlardan farklılıklarının incelenmesi; ii) makine öğrenmesi ve istatistiksel yöntemlerin bir arada kullanılması ile suç örgütlerinin tespitine yönelik yeni bir model oluşturulması; iii) suç örgütleri içerisinde yer alan kişilerin varislerinin tespitine ve varislerinin yeni oluşturacağı bağlantılara yönelik farklı stratejileri içeren model oluşturulması ve iv) suç örgütleri içerisinden üyelerinin çıkartılması sonucu farklı stratejilere göre oluşacak yeni ağların suç örgütü yapılarına benzerliklerinin değişiminin incelenmesi ve çıkartılmasıyla ağa en fazla zararı verecek olan üyenin tespiti gerçekleştirilmiştir. Eski Türklerde devlet ve ordu içinde çok önemli rol oynayan ve üstün askerlik bilgisi ile idari tecrübesi olan askeri kılavuzlara YİZEK adı verildiğinden, güvenlik güçlerinin suç örgütlerinin etkisizleştirilmesine yönelik karar destek sistemlerine katkı sağlayarak kılavuzluk edecek bu tez çalışmasına ve çalışmada yeni oluşturulan SAA metriğine YİZEK adı verilmiştir.
