As vasculites sistémicas constituem um grupo de doenças, pouco frequentes na infância, caracterizadas por inflamação e necrose vascular. A sua tradução clínica é heterogénea, condicionada pelo tipo de vaso e orgão afectados. Recentemente foi proposta uma classificação das vasculites em idade pediátrica, que tem em conta a dimensão dos vasos envolvidos, sendo também validados critérios de diagnóstico para os tipos mais frequentes em crianças. A presente revisão tem como objectivo abordar os tipos mais frequentes de vasculites na infância, suas ma- nifestações clínicas, diagnóstico e terapêutica.PALAVRAS-CHAVE – Vasculites; Criança.

O acometimento neurológico no curso da granulomatose de Wegener atinge 15-50% dos pacientes, porém como sintomatologia inicial não é comentado na literatura. Relatamos o caso de uma paciente com mononeurite múltipla secundária a granulomatose de Wegener, enfocando os aspectos de antecipação do quadro neurológico em relação aos sintomas sistêmicos, a contribuição diagnóstica da biópsia do nervo sural e a boa evolução do quadro neurológico.

RESUMO Examinamos 60 pacientes com Síndrome de Imunodeficiência Adquirida (Aids), 54 do sexo masculina e 4 do sexo feminino. O grupo de risco mais freqüente era o de homossexuais ou bissexuais masculinos (70%) e mais de 50% apresentavam queixas quanto à visão, como embaçamento visual, escotomas e diminuição da acuidade. As manifestações ocUlares foram mais freqüentes em indivíduos do Grupo IV (71,6%) da classificação do "Centers for Disease Control". No exame externo, o achado mais freqüente foi o engurgitamento dos vasos conjuntivais, presente em 21 pacientes (35%), sendo que 13 desses indivíduos pertenciam ao Grupo IV. Nos pacientes com diminuição acuidade visual, encontramos alterações retínicas como exsudatos algodonosos, hemorragias superficiais, vasculites, retinites por citomegalovfrus e hemorragias vítreo retínicas. A maioria desses pacientes pertencia ao Grupo IV. Nosso achado mais freqüente foi o engurgitamento venoso retínico, presente em 65% dos olhos examinados. Tal alteração parecenos merecer ser incluída como mais um elemento de suspeição no diagnóstico precoce da Aids. SUMMARY We have examined 60 patients with Acquired Immune Deficiency Syndrome (Aids), 56 males and 4 females. The most frequent risk was the homossexual or bissexual males group (70%). Over than 50% of the patients complained about blurred vision, scotomas and decreased visual acuity. Visual signs were more frequent in patients of group IV (71,6%) of the Centers for Disease Control classification. The most frequent finding, on the eye external examination, was dilation of conjunctival blood vessels, which was present in 21 patients (35%). Of those, 13 patients belonged to group IV. In patients with decreased visual acuity, we found retinal changes like cotton wool spots, superficial haemorrhages, vasculitis, citomegalovirus retinitis and vitreo-retinal haemorrhees. The majority of those patients belonged lo Group IV. Our most frequent finding was the dilatation of retinal venous vessels, which was present in 65% of the examined eyes. ln our opinion this finding should be considered a valious element in the early Aids diagnosis.

ResumoA doença de Behçet é uma doença multissistêmica que consiste de diferentes combinações de manifestações oculares, mucocutâneas, neurológicas, cardiovasculares, gastrintestinais e outras. Seu diagnóstico se fundamenta em critérios clínicos, em que o teste da patergia positivo recebe um ponto. Apresenta‐se uma série de casos com 26 pacientes suspeitos, tendo o teste da patergia da pele sido realizado em 23 deles. Os resultados foram avaliados em 48 horas, tendo sido considerados como negativos diante da ausência de pápula e positivos na presença de pápula ou pústula. Os resultados positivos foram divididos pelo tamanho da pápula e efetuou‐se uma dermatoscopia para medir e observar seus aspectos clínicos. Após as leituras, foi realizada uma biópsia, com anotação dos aspectos histopatológicos. O teste foi negativo em 2 (8,7%) e positivo em 21 (91,3%) pacientes. Apresentam‐se os resultados e a revisão da literatura.AbstractBehçet's disease is a multisystemic disease consisting of a varying combination of ocular, mucocutaneous, neurologic, cardiovascular, gastrointestinal and other manifestations. Its diagnosis is based on clinical criteria, in which a positive pathergy test scores 1. A case series with 26 suspected patients is presented, and the skin pathergy test was performed in 23. The results were read in 48hours, and they were considered negative when without papule, and positive with a papule or pustule. Positive results were divided by papule size, and dermatoscopy was done to measure and observe its clinical aspects. After the readings, a biopsy was performed, with annotation of histopathological aspects. The test was negative in 2 (8.7%) and positive in 21 (91.3%) patients. The results and the literature review are presented.

Churg-Strauss syndrome (CSS) is a rare systemic vasculitis, that essentially affects medium-sized vessels, involves multiple organs and in the majority of cases is related to asthma and eosinophilia.The authors make a review of this disease and discuss its differential diagnosis presenting the case of a woman with asthma that at the age of 65 is diagnosed with CSS.Rev Port Pneumol 2005; XI (1): 73-83

Introduction: Each day, evidence accumulates related to the use of therapeutic plasma exchange (TPE) in patients with rheumatic diseases. San Ignacio University Hospital has recorded all of the TPE sessions performed by the institution's apheresis group. Objective: To describe the TPE experience of patients with rheumatologic diseases in a hospital setting. Methods: Descriptive, observational, retrospective analysis. This study included analyses of the TPE sessions that were performed in patients with rheumatic diseases from November 2009 to November 2013. Results: The apheresis group performed 136 sessions in 27 patients. The mean patient age was 43 years (SD 18.5), and 59.3% of the patients were female. Regarding the diagnosis, the most frequents ones where: ANCA-associated vasculitis followed by systemic lupus erythematosus and catastrophic antiphospholipid syndrome. The average number of sessions per patient was 5 (SD 1.8), and the average plasma exchange per patient was 1.3 plasma volume replacement units. The most used replacement solution was frozen fresh plasma (FFP; 63.2% of the sessions). Of all the sessions, 4.4% presented complications, and the majority of the complications were related to vascular access. Fifteen patients required renal replacement therapy (RRT) secondary to the same cause that created the need for TPE, 3 patients required RRT due to causes other than the TPE diagnostic intervention and 1 patient had undergone chronic dialysis. Conclusions: TPE is a therapeutic alternative that is needed for the management of patients with rheumatic diseases with renal involvement and those who are refractory to conventional management. Our clinical results were in agreement with the global literature. Resumo Introdução: Diariamente acumulam-se evidências relacionadas com o uso da troca plasmática terapêutica (TPT) em pacientes com doenças reumáticas. O Hospital Universitário San Ignacio registrou todas as sessões de TPT feitas pelo grupo de aférese dessa instituição. Objetivo: Descrever a experiência do Hospital Universitário San Ignacio na TPT em pacientes com doenças reumatológicas. Métodos: Análise observacional, retrospectiva, descritiva. Incluiu análises das sessões de TPT feitas em pacientes com doenças reumáticas de novembro de 2009 a novembro de 2013. Resultados: O grupo de aférese fez 136 sessões em 27 pacientes. A idade média foi de 43 anos (DP 18,5) e 59,3% eram do sexo feminino. Quanto ao diagnóstico, os mais frequentes foram: vasculite associada ao anticorpo anticitoplasma de neutrófilos (ANCA) seguida de lúpus eritematoso sistêmico e síndrome antifosfolipídica catastrófica. A quantidade média de sessões por paciente foi de cinco (DP 1,8) e a média de troca plasmática por paciente foi de 1,3 unidade de substituição do volume de plasma. A solução de substituição mais usada foi o plasma fresco congelado (PFC, 63,2% das sessões). De todas as sessões, 4,4% apresentaram complicações, a maioria delas relacionadas com o acesso vascular. Quinze pacientes necessitaram de terapia de substituição renal (TSR) secundária à mesma causa que levou à necessidade de TPT; três pacientes necessitaram de TSR em decorrência de outras causas além da intervenção diagnóstica de TPT e um paciente tinha sido submetido à diálise crônica. Conclusões: A TPT é uma opção terapêutica necessária para o manejo de pacientes com doenças reumáticas com envolvimento renal e daqueles que são refratários ao tratamento convencional. Os resultados clínicos do presente estudo estão de acordo com o que é encontrado na literatura global.

Resumo Introdução: A crioglobulinemia é uma vasculite de pequenos vasos mediada por imunocomplexos que normalmente envolvem a pele, os rins e os nervos periféricos. A síndrome antifosfolipídica (SAF) é um transtorno da hipercoagulabilidade autoimune que provoca trombose dos vasos sanguíneos. Pode se manifestar como um distúrbio microtrombótico que afeta múltiplos órgãos, denominado SAF catastrófica. Objetivo: Esta série de casos objetiva descrever os desafios de diagnóstico e tratamento que surgem quando esses dois graves transtornos estão presentes simultaneamente no mesmo paciente. Métodos: Foram descritos quatro pacientes internados em nosso hospital em decorrência de danos graves a múltiplos órgãos mediados pela vasculite crioglobulinêmica com SAF concomitante. Resultados: As manifestações clínicas incluíram úlceras de perna, livedo reticular, insuficiência renal e neuropatia periférica. As etiologias sugeridas para a combinação de síndromes foram a hepatite C, o lúpus eritematoso sistêmico e a doença mieloproliferativa retal associada a linfoma de zona marginal tipo células B. Todos os pacientes foram tratados com anticoagulantes, altas doses de corticosteroides, rituximabe, gamaglobulinas intravenosas e troca de plasma. Conclusão: A rara associação entre a SAF grave ou catastrófica e a crioglobulinemia deve ser considerada por médicos que atendem pacientes com isquemia ou necrose de múltiplos órgãos. Abstract Background: Cryoglobulinemia is an immune-complex-mediated small vessel vasculitis that classically involves the skin, kidneys and peripheral nerves. Antiphospholipid syndrome (APS) is an autoimmune hypercoagulable disorder which causes blood vessel thrombosis. It can present as a multi-organ microthrombotic disorder which is called catastrophic APS. Objective: In this case series we aim to describe the diagnostic and management challenges that arise when these two severe disorders simultaneously present in the same patient. Methods: We describe four patients who were admitted to our hospital due to multi-organ life threatening damage mediated by cryoglobulinemic vasculitis with concurrent APS. Results: Clinical manifestations included leg ulcers, livedo reticularis, renal failure, and peripheral neuropathy. Suggested etiologies for the combined syndromes were hepatitis C, systemic lupus erythematosus and myeloproliferative disease rectal maltoma. All of our patients were treated with anticoagulation, high-dose corticosteroids, rituximab, intravenous gammaglobulins and plasma exchange. Conclusion The rare association of severe or catastrophic APS with cryoglobulinemia in patients should be considered by physicians who treat patients with multi-organ ischemia or necrosis.

Childhood primary angiitis of the central nervous system (cPACNS) is a rare inflammatory brain disease of unknown etiology. Of note, brain hemorrhage has been rarely reported in cPACNS patients, generally associated with a delayed clinical diagnosis, or with a diagnosis only at necropsy. We present the case of a boy with cPACNS that previously suffered an ischemic stroke. At the age of 7 years and 10 months, he presented a sudden and severe headache, vomiting and reduction in consciousness level (Glasgow coma scale 7), requiring prompt tracheal intubation. Brain computed tomography demonstrated intraparenchymal hematoma in the right parieto-occipital lobe and a small focus of bleeding in the right frontal lobe, vasogenic edema, herniation of the uncus and a 10 mm deviation to the left from the midline. C-reactive protein (9.2 mg/dL) and von Willebrand factor (vWF) antigen (202%) were elevated. Decompressive craniotomy was performed and methylprednisolone and cyclophosphamide were administered. One week later, the patient had left hemiparesis without other sequelae. Importantly, motor deficits have been improving progressively. Our case reinforces the inclusion of this vasculitis as a differential diagnosis in children and adolescents with CNS hemorrhage.

Resumo: A síndrome de Churg-Strauss (SCS) é uma vasculite sistémica necrotizante, que afeta os vasos de pequeno e médio calibre e se associa a granulomas eosinofílicos extravasculares, eosinofilia periférica e asma.É uma síndrome rara, de etiologia desconhecida e que afeta ambos os géneros e todos os grupos etários.Os doentes com SCS geralmente apresentam boa resposta à terapêutica com glucocorticoides, embora as recidivas sejam frequentes após a sua suspensão. O diagnóstico e terapêutica atempada levam geralmente a um bom prognóstico, com uma sobrevivência de 90% um ano após o diagnóstico.Neste artigo é apresentada uma breve revisão da SCS, com particular atenção ao diagnóstico, terapêutica e progressos recentes nesta área.De seguida, os autores apresentam e discutem as características clínicas, laboratoriais e imagiológicas de quatro doentes internados num Serviço de Medicina Interna com este diagnóstico. O tratamento instituído, as respostas observadas e o seguimento dos casos são também descritos. Abstract: Churg-Strauss syndrome (CSS) is a systemic necrotizing vasculitis of the small and medium vessels, associated with extravascular eosinophilic granulomas, peripheral eosinophilia and asthma.This is a rare syndrome of unknown etiology, affecting both genders and all age groups.CSS patients usually respond well to steroid treatment, although relapses are common after it ends. Timely diagnosis and treatment generally lead to a good prognosis with a 90% survival rate at one year.A brief review of CSS is presented, with particular attention to diagnosis, therapy and recent developments in this area.The authors then report and discuss the clinical, laboratory and imaging characteristics of four patients admitted to an Internal Medicine Department with this diagnosis. The treatment, response and follow-up of the cases are also described. The pulmonary manifestations of ulcerative colitis (UC) are rare and include inflammation of small and large airways, parenchymal disease and serositis among others. A substantial proportion of patients with inflammatory bowel disease, particularly those with ulcerative colitis presents positive ANCA, most p-ANCA pattern. We present a case of patient with ulcerative colitis, with positive c-ANCA, which progressed to hemoptysis associated with radiological findings consistent with pulmonary vasculitis.

CONTEXT: Erythema elevatum diutinum is a chronic and rare dermatosis that is considered to be a variant of leukocytoclastic vasculitis. It is probably mediated by immune complexes. It is generally associated with autoimmune, neoplastic and infectious processes. Recently, it has been added to the group of specific dermatoses that are associated with HIV. CASE REPORT: We report on the case of a patient who had erythema elevatum diutinum as the first clinical evidence for diagnosing HIV infection. Dapsone was used in the treatment of this patient, and partial regression of the lesions was achieved within 15 days, even before antiretroviral therapy was prescribed. CONCLUSION: When there is a diagnosis of erythema elevatum diutinum, HIV infection should be investigated, especially in atypical and exacerbated clinical manifestations. CONTEXTO: O eritema elevatum diutinum é uma dermatose crônica, rara, variante clínica da vasculite leucocitoclástica, provavelmente mediada por imunocomplexos. Está sendo incluído no grupo das dermatoses específicas associadas à infecção pelo HIV. Em geral, associa-se a outros processos infecciosos, auto-imunes e neoplásicos. RELATO DE CASO: Relatamos o caso de um paciente em que a manifestação cutânea de eritema elevatum diutinum foi a primeira evidência clínica para diagnóstico da infecção pelo HIV. O tratamento foi feito com dapsona e se obteve regressão parcial das lesões após 15 dias, mesmo antes de o esquema anti-retroviral ser prescrito. CONCLUSÃO: Frente ao diagnóstico de eritema elevatum diutinum, deve-se investigar a infecção pelo HIV, principalmente nas apresentações clínicas atípicas e exacerbadas.

O edema hemorrágico agudo da infância (EHAI) é uma vasculite leucocitoclástica rara, com aproximadamente 100 casos descritos na literatura de língua inglesa. As lesões cutâneas características são púrpuras palpáveis, que se localizam em face, orelhas e extremidades, e lembram a figura de um medalhão. É uma vasculite de pequenos vasos, característica de crianças menores de dois anos de idade. Na maioria das vezes, tem curso autolimitado e benigno, apesar da aparência das lesões. Relatamos o caso de uma lactente, que iniciou edema de mãos e pés, lesões purpúricas na face e febre, e comparamos a outros já descritos, de acordo com a revisão da literatura acerca do assunto. A raridade da doença pode estar associada ao subdiagnóstico ou ao diagnóstico equivocado de púrpura de Henoch-Schönlein (PHS). EHAI é precedido na maioria dos casos por infecções, imunizações ou drogas. O envolvimento de mucosas e vísceras raramente ocorre. Nenhum tratamento é recomendado atualmente. O alerta para essa vasculite tem como objetivo auxiliar o diagnóstico, tornando-o mais precoce, e evitar tratamentos e preocupações desnecessárias.

Métodos: O estudo foi realizado através da revisão dos prontuários de todos os pacientes com diagnostico de Doença de Behcet atendidos e/ou acompanhados no ambulatório de vasculites da Disciplina de Reumatologia do Hospital das Clinicas da Universidade Estadual de Campinas, São Paulo, Brasil, no período de 1988 a 2010. Resultados: Foram incluídos no estudo oitenta e sete pacientes. A proporção entre os sexos foi de M/F = 0,84 e a media de idade do inicio da doença foi de 28,03 + ou - 7,57 anos. Lesão aftosa oral foi a manifestação clinica mais comum (100%). Ulceras genitais também foram comuns (77%), seguidas pela pseudofoliculite (47,67%). Envolvimento ocular esteve presente em 80% e envolvimento neurológico em 31,03% dos pacientes. Artralgia foi relatada em 31,03% e artrite em 13,79% dos casos. O envolvimento vascular foi encontrado em 13,95% dos pacientes. Somente 1,14% apresentaram envolvimento gastrointestinal. Conclusão: Nesta serie de casos, realizada em um pais da America do Sul, foram encontrados resultados semelhantes aos previamente descritos em diferentes áreas endêmicas da doença, com uma maior frequência das manifestações oculares e neurológicas Abstract: Objective: To analyze demographic and clinical features in patients diagnosed with Behcet's disease (BD) in Brazil. Methods: We performed a retrospective review of all the patients' records with BD diagnosed from 1988 to 2010 in the Rheumatology Department at the State University of Campinas (UNICAMP). All patients had to fulfill the International Study Group for Behcet's disease diagnostic criteria. Results: Eighty-seven patients were included in the study. The male/female rate was 0,84 and the mean age at the onset of the disease was 28.03 + or - 7.57 years. Oral aphtosis was the most frequent manifestation (100%). Genital aphtosis was also frequent (77%), followed by pseudofolliculitis (47.67%). Ocular symptoms were present in 80% and neurologic manifestations in 31.03% of the patients. Arthralgia was reported in 31.03% and arthritis in 13.79% of the cases. Vascular involvement was seen in 13.95% of the patients. Only 1.14% had gastrointestinal involvement. Conclusion: This series, from a South American country, showed a similar general pattern of the BD to those found in different endemic areas in the world, with a high frequency of ocular and neurological manifestations Doutorado Clínica Médica Doutor em Clínica Médica

Livedoid Vasculopathy is a disease characterized by occlusion of the capillaries of the dermis, without inflammatory signs. It begins with purpuric papules or macules that develop into painful ulcers, mainly involving the ankles and feet. In this case report, we describe diagnosis and treatment in a young pregnant patient, with excellent clinical response.
