Chronic shoulder pain has a variety of causes, and treatment is multifaceted. Physical therapy typically is the first step; therapy for osteoarthritis (OA) should strike a balance between regaining and maintaining motion and strength and avoiding aggravation of the patient's condition. Oral NSAIDs typically are considered first-line treatment for patients with OA and often are included in the treatment plan for all types of chronic shoulder pain. Acetaminophen may be used as an alternative to NSAIDs for pain relief. Corticosteroid injections are used frequently; they may be combined with physical therapy as a first line of treatment. Surgical treatment may be considered when chronic shoulder pain is not managed adequately with nonsurgical options. Shoulder pain is a complex clinical condition, making the diagnosis difficult for even experienced clinicians. The most common causes of chronic shoulder pain are rotator cuff disorders and osteoarthritis of the glenohumeral joint. The shoulder is composed of 4 separate articulations, all of which may be sources of pain. Taking a thorough history is necessary to arrive at an accurate diagnosis. Important assessments include the patient's level of pain and functional limitations pertaining to the shoulder. The physical examination begins with inspection and palpation; comparing the shoulders is important. Several provocative tests may be used to determine rotator cuff pathology. Anteroposterior, scapular 'Y,' and axillary lateral are the standard radiographic views used to assess shoulder pain. CT, ultrasonography, and MRI also may be used. Shoulder instability is a common condition encountered by sports medicine and shoulder surgeons. Management can be challenging, both with regards to the underlying pathology and the high expectations associated with the very active patient population that this condition typically affects. High-performance athletes who are used to performing at activity levels which typically challenge the normal physiologic limits of the glenohumeral joint may be particularly difficult to treat. As physicians increasingly evaluate patient reported outcomes (PROs), a growing body of literature has emerged related to patient expectations for a variety of orthopaedic conditions, including shoulder instability. Physicians may face difficulty when using the literature to counsel patients due to the difference between a successful PRO and true patient satisfaction. Having a better understanding of patient expectations may improve preoperative evaluation, patient counseling, postoperative PROs, and patient satisfaction with shoulder instability surgery. The purpose of this paper was to review the literature regarding patient understanding of shoulder instability injuries, expectations following surgical repair of shoulder instability pathology, and satisfaction with operative intervention. The medial patellofemoral complex, composed predominantly of the medial patellofemoral ligament, plays an important role in patellar tracking and stability. Medial patellofemoral ligament reconstruction is accordingly one of the most broadly applied surgical techniques for treating patellar instability. Orthopaedic research has demonstrated that surgeries that restore native anatomy are often more effective. The medial patellotibial ligament clearly serves an important supporting role in patellar tracking and stability, particularly in early flexion, and its inclusion in medial soft-tissue reconstructions more closely restores native patella tracking. Shoulder instability encompasses a spectrum of disease ranging from subluxation to dislocation, and is typically associated with collision athletes such as wrestlers and football players. Instability, however, also can be the result of repetitive microtrauma, as seen in overhead athletes (baseball, tennis, volleyball, swimming). The presentation of instability can be subtle, and difficult to diagnose in the absence of an acute traumatic event without the proper suspicion, physical examination, and diagnostic evaluation. Overhead athletes present the unique challenge of requiring the glenohumeral joint to exceed its physiologic limits during competition; therefore, injury in this population can be devastating. Additionally, athletes who experience instability, regardless of treatment, require rehabilitation (including periscapular strengthening) to maximize strength of the surrounding musculature. Specifically they will require coordinated throwing programs, and gradual return to play protocols dependent on their sport. Orthopaedic specialization has increased substantially over the past several decades, partly due to the desire of residents to improve their clinical expertise and to increase the likelihood that they will obtain a position with better compensation and a more balanced lifestyle. The American Academy of Orthopaedic Surgeons (AAOS) census data support this trend, demonstrating a substantial rise in the percentage of practicing orthopaedic surgeons who identify themselves as specialists rather than as general orthopaedic surgeons. There is a perception that a more narrowed scope of practice may prevent the current orthopaedic workforce from being able to adequately care for populations in rural areas of the United States. Additional consideration should be given to clearly defining the necessary knowledge and skills of a general orthopaedist in the twenty-first century, to understanding their role in musculoskeletal care, and to reevaluating residency educational experiences relative to their ability to prepare graduates to practice general orthopaedics independently. Background: Women are underrepresented as applicants to and trainees of orthopaedic surgery residencies. Factors that attract women to or deter them from orthopaedic surgery have been previously published; however, there has been no analysis of the programs that train high percentages of female residents and the factors that differentiate them from programs that have low percentages of women. The purpose of this study was to identify and compare these factors between programs with high and low percentages of female residents.Methods: Information on each orthopaedic surgery residency program listed in the American Medical Association (AMA) Fellowship and Residency Electronic Interactive Database (FREIDA) was collected utilizing residency program web sites, an online survey distributed to residency program coordinators, and a follow-up telephone survey. These included data on resident and faculty demographic characteristics and residency program curriculum structure. The prevalence of factors in programs with the highest percentages of female residents was compared with those with lower percentages.Results: Data were obtained from 143 (97.3%) of 147 programs, with 3,406 residents identified. Only 485 residents (14.2%) were female. Programs with more female residents had more female faculty members per program (p = 0.001), a higher percentage of faculty who were female (p < 0.001), more female associate professors (p < 0.001), more women in leadership positions (p < 0.001), and a higher prevalence of women's sports medicine programs (p = 0.03); were more commonly listed in the Top 40 for National Institutes of Health (NIH) funding (p = 0.03) and U.S. News & World Report rankings (p = 0.02); and were more likely to offer a research year (p = 0.045).Conclusions: There are greater percentages of female residents at orthopaedic residency programs with more female faculty members, more women in leadership positions, a women's sports medicine program, and the option to do a research year. Departmental and national leaders may consider these factors when efforts are undertaken to enhance the recruitment of female applicants and improve female interest in orthopaedic surgery as a specialty. Objectives: Though there are no research requirements to match into an orthopaedic sports medicine fellowship, many applicants are productive in research endeavors during residency. We hypothesize that the number of publications by Orthopaedic sports medicine applicants are increasing. Methods: A list of current and recent sports medicine fellows was compiled from publicly accessible information on sports medicine fellowship websites. Articles published while the fellow was a resident were identified via publicly available search engines. The following information was collected: year of fellowship and years of residency, fellowship program, geographic location of fellowship program, total number of publications (noting specifically first and last author publications), number of publications in high impact orthopaedic journals(AJSM, JBJS Am, JSES, or Arthroscopy). Results: Overall, 189 fellowship-matched surgeons from 2010 – 2017 were identified. There were 746 publications (average of 3.95 per fellow), with 218 (29.2%) in high impact orthopaedic journals. Surgeons who completed their fellowship during the 2016–17 academic year, published on average 5.42 publications per fellow. Fellowship applicants in the Northeast region had the highest number of total publications (359 publications, 48.1% of all publications; 6.41 publications per fellow). Applicants were listed most often as middle authors (462 publications, 61.9%). Conclusions: There has been an overall increase in the number of publications among sports medicine fellowship applicants in the last several academic years. To compare postoperative complication rates between patients who underwent medial patellofemoral ligament reconstruction (MPFLR) and those who underwent MPFLR with tibial tubercle osteotomy (TTO) in a large-scale study. A secondary goal was to investigate demographic associations with outcomes. Patients who underwent MPFLR (n = 3,480) or MPFLR-TTO (n = 615) for patellar instability were identified in the PearlDiver database. Rates of surgery for infection, procedures for knee stiffness, patellar fracture, and revision MPFLR within 2 years postoperatively were compared using multivariable logistic regression. Demographic associations with outcomes were also investigated. The MPFLR-TTO cohort exhibited a significantly lower rate of revision surgery at 2 years (0.8% vs 1.9%; odds ratio [OR], 0.33; 95% confidence interval [CI], 0.10-0.80; P =.036) when compared with the MPFLR group. Independent of index procedure, patients younger than 21 years had significantly lower rates of requiring procedures for knee stiffness (OR, 0.35; 95% CI, 0.22-0.54; P <.001) and any complication at 2 years (OR, 0.59; 95% CI, 0.45-0.78; P <.001) when compared with older patients. Male patients displayed a significantly lower rate of requiring procedures for knee stiffness at 2 years than female patients (OR, 0.46; 95% CI, 0.25-0.78; P =.007). Tobacco use was associated with a significantly higher rate of postoperative infection at 2 years (OR, 2.35; 95% CI, 1.00-5.38; P =.046). The MPFLR cohort exhibited higher rates of revision surgery at 2 years compared with the MPFLR-TTO cohort. Patient age under 21 years was associated with lower rates of any complication and requiring procedures for knee stiffness, male sex was associated with a lower rate of requiring procedures for knee stiffness, and tobacco use was associated with a higher rate of surgery for postoperative infection. Introduction/Purpose: Return to sport (RTS) commonly serves as a measure for assessment of clinical outcomes in orthopaedic sports medicine surgery. Unfortunately, while RTS is commonly utilized in research for this purpose, currently, there is no widely accepted or standardized definition for when an athlete has officially returned to his or her sport. The purpose of this study was to conduct a systematic review to evaluate and report the differences in specific definitions of return to sport (RTS) utilized in the orthopaedic surgery literature. Methods: A systematic review was performed using PubMed, Embase and Cochrane databases per PRISMA guidelines. Search terms consisted of variations of 'RTS' combined with variations of 'orthopedic surgery' and 'define' to capture as many relevant articles as possible. The definition of return to sport was recorded and analyzed. Results: 718 articles were identified in the initial search, 29 of which met eligibility criteria, providing a clear definition of RTS. Of the 29 studies included, 20 (69.0%) defined RTS as an athlete competing in a game or other competitive play, and these definitions are further broken down in Table 1. Three (10.3%) defined this as the athlete competing in a game or other competitive play, but with an explicitly stated competition-level modifier of the athlete returning to their pre-injury level of competition. Two articles (6.9%) included returning to training or practice and the remaining four articles (13.8%) used terminology other than the standard RTS. Conclusion: There is variability in the definition of RTS used in the literature. Most studies refer to the athlete competing in a game or other competitive play. Other variants include returning to practice/training and explicitly defined competition levels and objectives. Future studies should aim to standardize the definition of RTS to facilitate more precise assessment of outcome following sports medicine surgery. Using terminology that describes components of the recovery and rehabilitation process, such as 'return to participation' and 'return to performance' in addition to RTS will allow us to better understand the athlete's recovery and associated level of competition or performance. Introduction/Purpose: Chronic ankle instability is a common condition that can be treated with lateral ankle ligament repair. These procedures have a reported success rate greater than 85% in the literature, but little has been reported about the differences in postoperative outcomes between males and females. The purpose of this study was to evaluate sex-specific outcomes following lateral ankle ligament repair. Methods: In this systematic review and meta-analysis, we utilized Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria to search for articles on electronic databases. Inclusion criteria consisted of English-language, in-vivo clinical studies, mean follow-up of at least one year, sex-specific evaluation of outcomes, and study participants who underwent primary lateral ligament repair. Functional postoperative outcomes for males and females were recorded and statistically analyzed. Results: Out of 2,768 studies, eight (0.29%) met inclusion criteria and were analyzed in this review. These studies included 631 patients (409 males [65%] and 222 females [35%]) who underwent primary lateral ligament repair for ankle instability. There was no statistically significant difference between males and females in terms of Karlsson scores (93.5 +- 1.6 for males, 92.7+- 2.3 for females, P =.1582) or American Orthopaedic Foot & Ankle Society (AOFAS) scores (90.44 +- 4.82 for males, 90.19 +- 3.54 for females, P =.1586). Postoperative success was defined as a 'good' or 'excellent' Karlsson score (>81), and success rate analysis revealed no statistically significant difference between males and females (86% +- 7.1% for males, 87% +- 3.5% for females, P =.9374). Conclusion: There was no difference in postoperative mean Karlsson scores, AOFAS scores, or success rates between males and females who underwent primary lateral ankle ligament repair. This surgical procedure appears to have comparable outcomes in both sexes; however, more research is warranted to further assess the impact of patient sex on postoperative outcomes.
