Abstract

The objective was to analyze the impact of COVID-19 in Basic and high school education: Urban educational units of Suscal - Ecuador. It was carried out in educational institutions located in the urban sector of Cantón Suscal in the province of Cañar-Ecuador. It is of descriptive non-experimental methodology. It is stated that certain teachers did not have adequate training in the technological tools to face the health emergency of COVID-19, since they did not frequently use information and communication technology to teach classes in their corresponding assistants.

Abstract

The objective of the research is to analyze the challenges and perspectives that are generated in the use of virtual platforms by teachers of educational institutions in the Canton El Tambo - Ecuador. Methodologically, it was of a descriptive, non-experimental cross-sectional type, the population sample was made up of 62 teachers from the “Mushuk Kawsay” Educational Unit of Canton el Tambo - Ecuador. It is concluded that in educational institutions there must be a technical team that supports the two in relation to the management of virtual platforms, so that when manipulating or being in constant use of them they generate doubts or problems and it is necessary that the Institution has a technical support department so that discomfort and frustration are not generated on the part of teachers and those who are managing virtual platforms in the academic process.

Abstract

Abstract SARS-CoV-2 is a rapidly evolving pandemic causing great morbimortality. Medical therapy with hydroxicloroquine, azitromycin and protease inhibitors is being empirically used, with reported data of QTc interval prolongation. Our aim is to assess QT interval behaviour in a not critically ill and not monitored cohort of patients. We evaluated admitted and ambulatory patients with COVID-19 patients with 12 lead electrocardiogram at 48 h after treatment initiation. Other clinical and analytical variables were collected. Statistical analysis was performed to assess the magnitude of the QT interval prolongation under treatment and to identify clinical, analytical and electrocardiographic risk markers of QT prolongation independent predictors. We included 219 patients (mean age of 63.6 ± 17.4 years, 48.9% were women and 16.4% were outpatients. The median baseline QTc was 416 ms (IQR 404–433), and after treatment QTc was prolonged to 423 ms (405–438) (P 460 ms, and just one case with QTc > 500 ms. Advanced age, longer QTc basal at the basal ECG and lower potassium levels were independent predictors of QTc interval prolongation. Ambulatory and not critically ill patients with COVID-19 treated with hydroxychloroquine, azithromycin and/or antiretrovirals develop a significant, but not relevant, QT interval prolongation.

Abstract

La puesta en valor del universo de los lenguajes estéticos con relación a las necesidades de la sociedad contemporánea y el permanente cuestionamiento de la vigencia de las estructuras educativas tradicionales desemboca en la búsqueda de nuevas alternativas pedagógicas. En este sentido, se reflexiona sobre la necesidad de entretejer los ámbitos artísticos, simbólicos y estéticos con las diversas áreas de aprendizaje, a la luz de perspectivas que tengan la intención de resignificar las dimensiones que configuran los procesos educativos. Además, se vislumbra la importancia de generar oportunidades formativas que atiendan a la figura docente con la intención de que se establezca como el vector fundamental de transformación y mejora de la calidad educativa, a partir de las experiencias cognitivas y emocionales que se generan desde la expresión estética.

Abstract

BackgroundCorticosteroids are the cornerstone of the treatment of patients with COVID-19 admitted to hospital. However, whether corticosteroids can prevent respiratory worsening in hospitalized COVID-19 patients without oxygen requirements is currently unknown.AimsTo assess the efficacy of methylprednisolone pulses (MPP) in hospitalized COVID-19 patients with increased levels of inflammatory markers not requiring oxygen at baseline.MethodsMulticenter, parallel, randomized, double-blind, placebo-controlled trial conducted in Spain. Patients admitted for confirmed SARS-CoV-2 pneumonia with raised inflammatory markers (C-reactive protein >60 mg/L, interleukin-6 >40 pg/ml, or ferritin >1,000 μg/L) but without respiratory failure after the first week of symptom onset were randomized to receive a 3-day course of intravenous MPP (120 mg/day) or placebo. The primary outcome was treatment failure at 14 days, a composite variable including mortality, the need for ICU admission or mechanical ventilation, and clinical worsening, this last parameter defined as a PaO2/FiO2 ratio below 300; or a 15% decrease in the PaO2 from baseline, together with an increase in inflammatory markers or radiological progression. If clinical worsening occurred, patients received tocilizumab and unmasked corticosteroids. The secondary outcomes were 28-day mortality, adverse events, need for ICU admission or high-flow oxygen, length of hospital stay, SARS-CoV-2 clearance, and changes in laboratory parameters.ResultsA total of 72 patients were randomized and 71 patients were analyzed (34 in the MPP group and 37 in the placebo group). Twenty patients presented with treatment failure (29.4 in the MPP group vs. 27.0% in the placebo group, p = 0.82), with no differences regarding the time to treatment failure between groups. There were no cases of death or mechanical ventilation requirements at 14 days post-randomization. The secondary outcomes were similar in MPP and placebo groups.ConclusionsA 3-day course of MPP after the first week of disease onset did not prevent respiratory deterioration in hospitalized COVID-19 patients with an inflammatory phenotype who did not require oxygen.

Abstract

BackgroundRapid molecular diagnostic (RMD) platforms may lead to better antibiotic use. Our objective was to develop analytical strategies to enhance the interpretation of RMDs for clinicians.MethodsWe compared the performance characteristics of 4 RMD platforms for detecting resistance against β-lactams in 72 highly resistant isolates of Escherichia coli and Klebsiella pneumoniae (PRIMERS I). Subsequently, 2 platforms were used in a blinded study in which a heterogeneous collection of 196 isolates of E. coli and K. pneumoniae (PRIMERS II) were examined. We evaluated the genotypic results as predictors of resistance or susceptibility against β-lactam antibiotics. We designed analytical strategies and graphical representations of platform performance, including discrimination summary plots and susceptibility and resistance predictive values, that are readily interpretable by practitioners to inform decision-making.ResultsIn PRIMERS I, the 4 RMD platforms detected β-lactamase (bla) genes and identified susceptibility or resistance in >95% of cases. In PRIMERS II, the 2 platforms identified susceptibility against extended-spectrum cephalosporins and carbapenems in >90% of cases; however, against piperacillin/tazobactam, susceptibility was identified in 95% of cases, while prediction of resistance was 69%-73% for ceftazidime and 41%-50% for imipenem.ConclusionsRMD platforms can help inform empiric β-lactam therapy in cases where bla genes are not detected and the prevalence of resistance is known. Our analysis is a first step in bridging the gap between RMDs and empiric treatment decisions.

446. Strong Chromatic Microlensing in HE0047-1756 and SDSS1155+6346

Abstract

We use spectra of the double lensed quasars HE0047-1756 and SDSS1155+6346 to study their unresolved structure through the impact of microlensing. There is no significant evidence of microlensing in the emission line profiles except for the Ly$\alpha$ line of SDSS1155+6346, which shows strong differences in the shapes for images A and B. However, the continuum of the B image spectrum in SDSS1155+6346 is strongly contaminated by the lens galaxy and these differences should be considered with caution. Using the flux ratios of the emission lines for image pairs as a baseline to remove macro-magnification and extinction, we have detected strong chromatic microlensing in the continuum measured by CASTLES (http://www.cfa.harvard.edu/castles/), in both lens systems, with amplitudes $0.09 (\lambda 16000) \lesssim |\Delta m |\lesssim 0.8 (\lambda 5439)$ for HE0047-1756, and $0.2 (\lambda 16000) \lesssim |\Delta m |\lesssim 0.8 (\lambda 5439)$ for SDSS1155+6346. Using magnification maps to simulate microlensing, and modeling the accretion disk as a Gaussian source (I $\propto$ exp(-R$^{2}$/2r$^2_s$)) of size r$_{s}$ $\propto$ $ \lambda^{p} $ we find, r$_{s}$ = 2.5$_{-1.4}^{+3.0}$ $\sqrt{M/0.3M_{\odot}}$ light days and p = 2.3 $\pm$ 0.8, at the rest frame for $\lambda$ = 2045, for HE0047-1756 (log prior), and r$_{s}$ = 5.5$_{-3.3}^{+8.2}$ $\sqrt{M/0.3M_{\odot}}$ light days and p = 1.5 $\pm$ 0.6 at the rest frame of $\lambda$ = 1398, for SDSS1155+6346 (log prior). Contrary to other studied lens systems, the chromaticity detected in HE0047-1756 and SDSS1155+6346 is large enough to fulfill the thin disk prediction. The inferred sizes, however, are very large compared to the predictions of this model, especially in the case of SDSS1155+6346., Comment: 31 pages, 7 figures, 7 tables. Abstract

We present a joint estimate of the stellar/dark matter mass fraction in lens galaxies and the average size of the accretion disk of lensed quasars from microlensing measurements of 27 quasar image pairs seen through 19 lens galaxies. The Bayesian estimate for the fraction of the surface mass density in the form of stars is $\alpha=0.21\pm0.14$ near the Einstein radius of the lenses ($\sim 1 - 2$ effective radii). The estimate for the average accretion disk size is $R_{1/2}=7.9^{+3.8}_{-2.6}\sqrt{M/0.3M_\sun}$ light days. The fraction of mass in stars at these radii is significantly larger than previous estimates from microlensing studies assuming quasars were point-like. The corresponding local dark matter fraction of 79\% is in good agreement with other estimates based on strong lensing or kinematics. Abstract

Several observational studies and numerical simulations suggest that mergers must contribute to the evolution of galaxies; however, the role that they play is not yet fully understood. In this paper we study a sample of 52 double nucleus disk galaxies that are considered as candidates for a minor merger event. The luminosity of each of the nuclei and their relative separation are derived from a multi-component photometric fit of the galaxies in the Sloan Digital Sky Survey optical images. We find that the nuclei in most of the sources have projected separations $\leq$ 4 kpc. The ratio of nuclear luminosities indicates that most of the systems are likely in the coalescence stage of a major merger. This is supported by the existence of a single galaxy disk in 65% of the systems studied and the finding of a correlation between nuclear luminosity and host luminosity for the single-disk systems: those sources fitted with as single disk are in a more evolved stage of the merger and present an enhancement of the nuclear luminosity compared to the double-disk systems, as expected from simulations of galaxy mergers. Finally, we identify a sample of 19 double nucleus disk galaxies in which the two nuclei are physically separated by $\leq1$ kpc and constitute thus a sample of sub-kpc binary AGN candidates., Comment: 15 pag, 7 fig., accepted for publication in ApJ

Abstract

The QUIJOTE (Q-U-I JOint Tenerife) CMB Experiment is designed to observe the polarization of the Cosmic Microwave Background and other Galactic and extragalactic signals at medium and large angular scales in the frequency range of 10-40 GHz. The first of the two QUIJOTE telescopes and the multi-frequency (10-20 GHz) instrument have been in operation since November 2012. In 2014 a second telescope and a new instrument at 30GHz will be ready for commissioning, and an additional instrument at 40 GHz is in its final design stages. After three years of effective observations, the data obtained by these telescopes and instruments will have the required sensitivity to detect a primordial gravitational-wave component if the tensor-to-scalar ratio is larger than r = 0.05. At the moment, we have completed half of the wide Galactic survey with the multi-frequency instrument covering 18 000 square degrees of the Northern hemisphere. When we finish this survey in early 2014, we shall have reached approximately 14{\mu}K per one degree beam at 11, 13, 17 and 19 GHz, in both Q and U., Comment: Rencontres du Vietnam 2013: Cosmology in the Planck Era. Abstract

We use multi-wavelength microlensing measurements of a sample of 10 image pairs from 8 lensed quasars to study the structure of their accretion disks. By using spectroscopy or narrow band photometry we have been able to remove contamination from the weakly microlensed broad emission lines, extinction and any uncertainties in the large-scale macro magnification of the lens model. We determine a maximum likelihood estimate for the exponent of the size versus wavelength scaling ($r_s\propto \lambda^p$ corresponding to a disk temperature profile of $T\propto r^{-1/p}$) of $p=0.75^{+0.2}_{-0.2}$, and a Bayesian estimate of $p=0.8\pm0.2$, which are significantly smaller than the prediction of thin disk theory ($p=4/3$). We have also obtained a maximum likelihood estimate for the average quasar accretion disk size of $r_s=4.5^{+1.5}_{-1.2} $ lt-day at a rest frame wavelength of $\lambda = 1026~{\mathrm \AA}$ for microlenses with a mean mass of $M=1 M_\sun$, in agreement with previous results, and larger than expected from thin disk theory., Comment: 18 pags, 4 figures.
