Abstract

To the Editor: We are writing in response to the report by Boyden et al. (Feb. 18 issue)(1) regarding a genetic variant associated with hereditary vibratory angioedema and urticaria. Hereditary vibratory angioedema(2) was reported by Dr. Roy Patterson and colleagues of Northwestern University in 1972. The description of a family and studies of histamine release by Boyden et al.(1) are virtually identical to the reports by Patterson and colleagues, which described the stimulation of swelling with a laboratory vortex mixer and ascribed the swelling to histamine release.(3) Nearly 40 years passed before the revelation(1) of the likely genetic cause of . . . Abstract

• Pyruvate dehydrogenase deficiency is among the most common causes of congenital lactic acidosis. We describe siblings with congenital lactic acidosis due to a deficiency of pyruvate dehydrogenase complex. The findings of computed tomography and pathologic studies suggest that central nervous system damage had occurred in utero. These observations have implications for treatment and outcome in patients with enzymatic defects causing congenital lactic acidosis.

Abstract

• Candida pericarditis and tamponade developed in a patient with sterile purulent pericarditis secondary to systemic lupus erythematosus. Therapy with amphotericin B and properly timed surgical intervention led to a clinical and microbiological cure. This article emphasizes the importance of differentiating an infected pericardial effusion from the sterile pericarditis of systemic lupus erythematosus and provides suggested guidelines for the management of that complication.(Arch Intern Med 1988;148:715-717)

448. Occult carbon monoxide poisoning in an infant

Abstract

The diagnosis of occult carbon monoxide poisoning in children is a difficult problem and may be frequently overlooked. An infant with recurrent episodes of unrecognized CO exposure and subsequent neurologic deterioration is presented. A clue to the diagnosis in this case was the finding of basal ganglia abnormalities on computed tomography. Clinical findings, etiology, and treatment of CO poisoning are reviewed. We suggest that CO toxicity should be suspected in any child with acute or recurrent neurologic deterioration.

Abstract

• We studied familial porencephaly consistent with autosomal dominant or recessive inheritance in two families. The pathogenesis of the encephaloclastic and schizencephalic types was reviewed. Use of noninvasive neuroradiologic techniques, including computed tomography and ultrasonography, may help to delineate groups of children with smaller porencephalic cysts and to define further familial or genetic forms of this process.

450. Alpha-Pattern Coma in Childhood and Adolescence

Abstract

• Two children showed evidence of alpha-pattern coma (APC). One case followed a phenobarbital sodium overdose with complete recovery. The second case occurred subsequent to cardiopulmonary arrest with unfavorable outcome. These cases and others that we reviewed emphasize that the prognosis of APC in children, as in adults, is variable and appears to be related to the cause.
