402. Neoadjuvant therapy for melanoma: past, present, and future

Abstract

Modern systemic therapy has dramatically improved outcomes for many patients with advanced metastatic melanoma. The success of these therapies has attracted much scientific interest while these therapies have made their way into the treatment of earlier stages of disease. Randomized trials have led to the approval of adjuvant immunotherapy and targeted therapy for resected stage III melanoma. However, most recently, these therapies have gained traction in the neoadjuvant setting. Promising early results led to randomized controlled trials that have now established neoadjuvant therapy as standard of care in advanced melanoma patients. Questions remain regarding the optimal choice of therapy, duration and timing of neoadjuvant therapy, extent of surgery, and the need for additional adjuvant therapy for patients who received neoadjuvant therapy. Herein we provide an overview of neoadjuvant therapy for melanoma and dilemmas to its broader applications.

Abstract

Stop-transfer hypothesis and conservative sorting are proposed as two models for the protein sorting mechanism of mitochondria. The first model suggests the primary translocation of the terminal region of the presequence into the matrix and the dissociation of the import channels in both membranes that enables the entry of the mature protein into the inner membrane. The second model involves the transport of the whole proteins into the matrix and into intermembrane space using the intermembrane space targeting domins as signals. Results on studies of cytochrome b2 and c1 suggests that the mitochondria decodes targeting information through the stop-transfer pathway.

Abstract

1. The attendees will be able to describe factors that impact availability of hospital palliative care. 2. The attendees will be able to describe environmental factors and hospital characteristics that are common and different across hospitals with and without palliative care. There are variations in hospital palliative care growth based on geographic location and hospital type. This mixed methods case study assessed palliative care in Alabama based on in-depth exploration of four different hospitals. Clinician champions, supportive administration, and mission fit are major factors influencing palliative care development across four hospitals. Hospital PC has expanded in the United States, though growth has varied in different geographic locations and hospital types. Given the Southern U.S.'s overall low ratings in palliative care access, the purpose of this study was to develop a thorough description of palliative care in Alabama hospitals. This was a mixed methods multiple case study of four Alabama hospitals (2 with and 2 without hospital PC). Quantitative data included assessment of environmental factors and a descriptive hospital survey, and were analyzed using descriptive statistics. Qualitative data were collected through semi-structured interviews with staff and administrators and analysis of serious illness care documentation, and analyzed using a thematic, within-case and cross-case approaches. Hospitals with PC were large, in an urban setting, nonprofit, and served multiple rural counties, while hospitals without palliative care were for-profit and served a smaller catchment area, with a focus of care on surgical and maternal/child services. The factors reported for having hospital PC was clinician champions, supportive hospital administration, and mission fit despite limited funding. Serious illness care at hospitals without PC was focused on end-of-life care and goals of care conversations. All hospitals used critical care rounding; however, the disciplines and discussions included in the rounds differed. Serious illness and palliative care training was offered at hospitals with palliative care. Hospitals without palliative care offered few voluntary training opportunities related to serious illness care. Environmental factors, such as location, and hospital characteristics, such as ownership, support previous literature on the existence of PC. Hospitals with palliative care utilized palliative care champions, multidisciplinary rounding, and focused palliative care training as core factors of the palliative care program and these should be considered in practice, warrant further research, and policy change to impact the care of patients living with serious illness. Abstract

Dietary sodium recommendations are expressed as absolute amounts (mg/day) rather than as sodium density (mg/kcal). Our objective was to determine if the strength of the relationship of sodium intake with blood pressure varied with energy intake. The Dietary Approaches to Stop Hypertension-Sodium (DASH) trial was a randomized feeding trial comparing two diets (DASH and control) and three levels of sodium density. Participants with pre- or stage 1 hypertension consumed diets for 30d in random order; energy intake was controlled to maintain body weight. This secondary analysis of 379 non-Hispanic Black and White participants used mixed effects models to assess the association of sodium and energy intakes with blood pressure. The relationships between absolute sodium and both systolic and diastolic blood pressure varied with energy intake. Blood pressure rose more steeply with increasing sodium at lower energy intake than at higher energy intake (p-interaction < 0.001). On the control diet with 2300 mg sodium, both systolic and diastolic blood pressure were higher (3.0 mm Hg, 95% confidence interval 0.2, 5.8; and 2.7 mm Hg, 95% CI 1.0, 4.5, respectively) among those with lower energy intake (higher sodium density) than among those with higher energy intake (lower sodium density).
