Abstract

In this work we present the construction of a device for determining electrical impedance using embedded systems. Specifically we use two Arduinos, both of Nano Arduino, and an Digital Analog Converter (DAC). The first Arduino, together with the DAC, has the function of generating a harmonic signal that allows the frequency varies in the range of 2 Hz up to 120 Hz. The second Arduino is responsible for performing measurements of electrical voltage, electrical current and electrical impedance of the system in interest. In addition, the experimental activities had a greater emphasis on applying at this platform all learnings that the students reach during the course until the now, that were taught in theory classes. Using this knowledge, the experiment was built. The applied methods proved to be efficient for the construction and acquisition of data obtained during the impedance measurements. Abstract

El propósito de esta investigación es desarrollar un modelo explicativo de la competitividad empresarial a partir de factores internos a la empresa. Para ello se identifican los factores de competitividad más importantes en el caso de las empresas manufactureras vascas, así como las variables que permiten caracterizarlos, utilizando técnicas cualitativas basadas en entrevistas con directivos. Los factores resultantes son las capacidades directivas, las capacidades de innovación, las capacidades de marketing y las capacidades de calidad. Tras analizar estos factores bajo la Visión de la empresa Basada en Recursos (VBR), y definir sus escalas de medida, se plantea un modelo causal que relaciona cada uno de ellos con la competividad de la empresa. Este modelo otorga una importancia especial a las capacidades directivas al plantear este factor como precedente del resto de capacidades de la empresa. El modelo es contrastado a través de la técnica de modelado con ecuaciones estructurales utilizando una muestra representativa de la población de empresas manufactureras vascas formada por 861 empresas.

Abstract

The existence of a metabolic disease in rheumatoid arthritis in distant zones of the active synovitis areas, remains controversial. Indeed, the variations found, by different authors, in PTH, in alkaline phosphatase and in serum and/or in the calcium urinary values, as well as in phosphate and hydroxyproline, have not clarified this problem, despite the demonstration by histomorphometry and by densitometrical methods, of a greater loss of the bone mass in rheumatoid arthritis. At the same time, metabolic changes in sexual hormones, primary or secondary, can modulate the immune response and interfere in the clinic expression of rheumatoid arthritis and also in the bone turnover. Therefore, the purpose of this study was to compare some parameters of the bone metabolism and of the hormonal condition, in women with rheumatoid arthritis, with and without corticotherapy and in an age related control group. In Group RA (patients), we found relatively higher levels of phosphates, alkaline phosphatase and osteocalcine, just as in the nephrogenic c'AMP and the hydroxyproline/creatinine quotient. The blood levels of calcitonin, PTH, T3, T4, cortisol and estradiol did not show significant differences between the 2 groups, although they were lower in Group RA. On the contrary, the plasma levels of testosterone, of 4-androstenodione and DHEA.S were significantly reduced in patients with rheumatoid arthritis. These results are compatible with the existence of bone metabolic hyperactivity in rheumatoid arthritis, which can be related, directly or indirectly to the reduced androgens plasma levels, since these seem to play a protective role in auto-immune diseases and also on the bone metabolism. The presence of lower calcitonin levels in R.A. (although statistically not significant) will, eventually, reinforce even more this increased bone turnover.

Abstract

The elderly population is one of the most rapidly increasing populations in the world. Physiological alterations induced by the aging process make these individuals more susceptible to chronic diseases and, consequently, to increased drug utilization.Objective: To describe the profile of drug utilization in the elderly living in Porto Alegre, RS, Brazil.Methods: An observational and cross-sectional population-based study to investigate the characteristics of the population, sources of information and types of drugs used by the elderly was performed. Four hundred and eighty patients were recruited from data supplied by the City Hall of Porto Alegre. The elderly were interviewed individually during the period from January to May 2006 by trained interviewers. A validated pharmacotherapy questionnaire was used for data collection and data were tabulated and analyzed by the SPSS 11.5 computer program.Results: Of the 480 patients interviewed, 13.8% did not use any medication. Cardiovascular system drugs represented the pharmacological class most used by the elderly (64.0%). When ill, 71.9% of these individuals visited the doctor, while 36.9% self-medicated. For the majority (50.2%), drugs were identified by their labels. Only 41.2% of the elderly understood medical prescriptions and 68.3% of the patients studied obtained the necessary information for the appropriate use of therapy from their doctors. Conclusions: The present study suggests that a pharmaceutical care program for the treatment, prevention, and use of medications may provide a higher efficiency to elderly drug therapy.

Abstract

The authors present the revised version of the Portuguese Society of Rheumatology (SPR) guidelines for the treatment of Rheumatoid Arthritis (RA) with biological therapies. In these guidelines the criteria for introduction and maintenance of biological agents are discussed as well as the contraindications and procedures in the case of non-responders. Biological treatment (with a tumour necrosis factor antagonist, abatacept or tocilizumab) should be considered in RA patients with a disease activity score 28 (DAS 28) equal to or greater than 3.2 des pite treatment with at least 20mg-weekly-dose of methotrexate (MTX) for at least 3 months or, if such treatment is not possible, after 3 months of other conventional disease modifying drug or combination therapy. A DAS 28 score between 2.6 and 3.2 with a significant functional or radiological deterioration under treatment with conventional regi -mens could also constitute an indication for biological treatment. The treatment goal should be remission or, if that is not achievable, at least a low disease activity, defined by a DAS28 lower than 3.2,without significative functional or radiological worsening. The response criteria, at the end of the first 3 months of treatment, are a decrease of at least 0.6 in the DAS28 score. After 6 months of treatment res ponse criteria is defined as a decrease greater than 1.2 in the DAS28 score. Non-responders, in accordance to the Rheumatologist's clinical opi -nion, should try a switch to another biological agent (tumour necrosis factor antagonist, abatacept, rituxi mab or tocilizumab). publishersversion published
