In this paper, end-to-end performance of transmit antenna selection (TAS) and generalized selection combining (GSC) is studied in a dual-hop amplify-and-forward relay network over flat Rayleigh fading channels. In the system, source and destination equipped with multiple antennas, communicate by the help of single relay equipped with single antenna. Source-destination link is not available. TAS is used for transmission at the source, and GSC is used for reception at the destination. By considering the relay location and the presence of error in feedback channel from the relay to the source, we derive closed-form outage probability, moment generating function and moments of end-to-end signal-to-noise ratio, and closed-form symbol error probability (SEP) expressions for channel state information (CSI)-based and fixed relay gains. The diversity order and array gain of the network are obtained for both CSI-based and fixed relay gains by deriving asymptotical outage probability and SEP expressions. Background There are no definite data about the atrial electromechanical coupling times ( AEMCT) in patients with end stage renal failure ( ESRF). The aim of this study was to investigate the AEMCT in ESRF patients without hypertension ( HT) and diabetes mellitus. Methods The study population consisted of 47 normotensive, nondiabetic ESRF patients and 41 healthy age/gender-matched control subjects. The time intervals from the onset of P-wave on the surface electrocardiogram to the beginning of late diastolic A-wave ( PA) were obtained from the lateral mitral annulus ( PA-lateral, maximum AEMCT), septal annulus ( PA-septal), and tricuspid lateral annulus ( PA-tricuspid). Time intervals were corrected according to the heart rate. The difference between PA-septal and PA-tricuspid (right AEMCT), PA-lateral and PA-septal (left AEMCT), and PA-lateral and PA-tricuspid (inter AEMCT) were calculated. Corrected time intervals were used for calculations. Results Groups were similar for age (52 ± 12.3 vs. 49.9 ± 6 years, P > 0.05) and gender. Maximum (61 ± 20 vs. 47 ± 13 ms; P < 0.001) AEMCT was significantly higher in the patients compared with the control group, but septal and tricuspid EMCT were not different (P > 0.05). Both inter-atrial (37 ± 21 vs. 24 ± 16 ms, P = 0.002) and left atrial (25 ± 18 vs. 12 ± 9 ms; P < 0.001) EMCT were significantly higher in patients when compared with the controls but intra-right atrial EMCT was not different. Conclusions Atrial conduction parameters such as maximal EMCT, left atrial, and inter-atrial EMCTs were prolonged in ESRF patients. The aim of this study was to determine whether conventional hemostasis (CH) or the harmonic scalpel (HS) results in shorter operative times for thyroidectomy and to evaluate the incidence of postoperative complications with each approach. In this study, 85 consecutive patients undergone open thyroidectomy were randomized into two groups: group CH (conventional hemostasis with classic technique of tying and knots, resorbable ligature, bipolar diathermy) and group HS (harmonic scalpel). Demographics, pathological characteristics, thyroid size, operative time, blood loss, and complications using the Student's t-test and χ-test. The two groups were similar regarding age and sex. There were no intraoperative complications. There was no difference between the two techniques regarding the amount of blood loss for different procedures. No significant differences were found between the two groups concerning mean thyroid weight and mean hospital stay (2.2 days in HS vs. 3.7 in CH; P > 0.05). The mean operative time was significantly shorter in the HS group (47.2 min vs. 79.2 min; P < 0.001). Two (4.7 %) transient recurrent laryngeal nerve palsies were observed in the CH group and no one (0 %) in the HS group. No patient developed permanent palsy. Postoperative transient hypocalcemia occurred more frequently in the CH group (21/43, 48 % vs. 7/42, 16 %). In patients undergoing thyroidectomy, HS is a reliable and safe tool. Comparing with CH techniques, its use reduces operative times, postoperative pain, drainage volume, and transient hypocalcemia. [ABSTRACT FROM AUTHOR]

Objective: This study was performed to determine of contraception knowledge levels and planning contraception methods after marriage that couples have applied to family planning center in the Van city. Methods: Study has planned as descriptive manner. This study was performed on 255 couples who have applied for official wedding procedure to Van maternal and child health and family planning center between September 2010 - January 2011. Participants were determined on a voluntary basis. Questionnaires were filled out by family medicine face to face with couples. Results: Totally, 21.3% percent of women, 23.5% percent of men have primary school degree. 18% of women, 93.4% of men have a profession. 12.1% percent of women and 12.9% percent of men does not have any knowledge about contraceptive and 43% of couples do not consider use any contraceptive method after marriage. The results has shown that when evaluate the couples, in terms of the distribution of contraception information, the couples have more information about condoms, pills, coil, injection and withdraw methods then tube ligation, implants, spermicides, vasectomy and the calendar methods Conclusion: Van region has a low socio-economic status. The couples should be informed about family planning before marriage, to talk about advantages of collaboration between the couples about birth control method choice. [ABSTRACT FROM AUTHOR]

In this paper, we study end-to-end performance of transmit antenna selection (TAS) and maximal ratio combining (MRC) in dual hop amplify-and-forward relay network in flat and asymmetric Nakagami- m fading channels. In the network, source and destination communicate by the help of single relay and source-destination link is not available. Source and destination are equipped with multiple antennas, and relay is equipped with single antenna. TAS and MRC are used for transmission at the source and reception at the destination, respectively. The relay simply amplifies and forwards the signal sent by the source to the destination by using channel state information (CSI) based gain or fixed gain. By considering relay location, for CSI based and fixed relay gains, we derive closed-form cumulative distribution function, moments and moment generating function of end-to-end signal-to-noise ratio, and closed-form symbol error probability expression. Moreover, asymptotical outage probability and symbol error probability expressions are also derived for both CSI based and fixed gains to obtain diversity order of the network. Analytical results are validated by the Monte Carlo simulations. Results show that diversity order is minimum of products of fading parameter and number of antennas at the end in each hop. In addition, for optimum performance the relay must be closer to the source when the diversity order of the first hop is smaller than or equal to that of the second hop. [ABSTRACT FROM AUTHOR]

Background: Coronary care units are sophisticated clinics established to reduce deaths due to acute cardiovascular events. Current data on coronary care unit mortality rates and predictors of mortality in Turkey are very limited. The MORtality predictors in CORonary care units in TURKey (MORCOR-TURK) trial was designed to provide information on the mortality rates and predictors in patients followed in coronary care units in Turkey. Methods: The MORCOR-TURK trial will be a national, observational, multicenter, and noninterventional study conducted in Turkey. The study population will include coronary care unit patients from 50 centers selected from all regions in Turkey. All consecutive patients admitted to coronary care units with cardiovascular diagnoses between 1 and 30 September 2022 will be prospectively enrolled. All data will be collected at one point in time, and the current clinical practice will be evaluated (ClinicalTrials.gov number NCT05296694). In the first step of the study, admission diagnoses, demographic characteristics, basic clinical and laboratory data, and in-hospital management will be assessed. At the end of the first step, the predictors and rates of in-hospital mortality will be documented. The second step will be in cohort design, and discharged patients will be followed up till 1 year. Predictors of short- and long-term mortality will be assessed. Moreover, a new coronary care unit mortality score will be generated with data acquired from this cohort. Results: The short-term outcomes of the study are planned to be shared by early 2023. Conclusion: The MORCOR-TURK trial will be the largest and most comprehensive study in Turkey evaluating the rates and predictors of in-hospital mortality of patients admitted to coronary care units. The purpose of this study is to determine the effects of the boarding primary schools on the students' psychological and social development. For this purpose, 6th-8th classes of Afyonkarahisar/Emirdağ Perihan Kemal Çuna Boarding Primary School has been used as a macrocosm in 2009-2010 academic year and by the help of "Cluster Sampling Method", 87 students from 6/A and 8/A classes are picked up from the 235 students in this sampling. The personal data section of the survey which is developed for the research, consists of 2 questions and the main section consists of 25 questions. During the analysis of the datas,arithmetic average and standart deviation are used for the first subordinate problem; t-test and one sided variance analysis (ANOVA) are used for the second subordinate problem. Throughout the all statistical process, the significance level (p) is ".05". It is understood that there isn't a significant difference between the girls' [t=2.446, p>05] and boys' [t=2.504, p>.05] conceptions about the effects of the boarding primary schools on the students' psychological and social development in respect of sexual terms. However, girls' conceptions ( X =2.446) are more negative than boys' ( X =2.504). Again, there is no significant difference (only .05) in one sided variance analysis [F=.086, p>.05] when we analyse the 6th-8th classes students' conceptions about the effects of the boarding primary schools on the students' psychological and social development in respect of their classes. As the arithmetic average of the answers of the 6th-8th classes students is ( X =2,49), it is true to say that students' conceptions about the boarding primary schools are "deficient" and the effects of the boarding primary schools on the students' psychological and social development are "negative". Objectives: Regional studies of diabetes mellitus (DM) are important to determine of effective environmental factors in the development of DM and defining strategy against these factors. This study aims to define the frequency of DM prevalence in the province of the Van city, distribution of DM according to age groups, effective socio-economical, geographical and metabolic factors over the diabetes development. Material and methods: This study included 191 male and 209 female totals 400 person aged year 16 years or over. Blood-Glucose levels of persons were measured with Accu-check glucometer and questionnaires were filled with face-to-face interviews. Results: Age mean of subjects was 35.09±13.75 years, DM prevalence was %13.4 between the females and %9.1 males, disglisemi (DG) prevalence were %9.1 between the females and %5.8 between the males. 74.4% of impaired glucose metabolism persons were people who they do not make physical exercise and adopted sedentary lifestyle. Conclusion: In this study, age and family history from unchangeable factors were found to be effective on the development of DM but sex, educational level, social security, incomes were not found to be effective on development of DM. Exercise, blood pressure levels, obesity and diets from changeable factors are the factors that influence the frequency of DM was determined in this study. [ABSTRACT FROM AUTHOR]

Objectives: In this study, we aimed to investigate the association of cancer with vasculitis and the frequency of cancer-related paraneoplastic syndromes and their relationship by measuring blood levels of Perinuclear type Anti-Neutrophilic Cytoplasmic Antibody (p-ANCA) and Cytoplasmic type antineutrophil cytoplasmic antibodies (c-ANCA) in patients with lung cancer and asthma and in healthy individuals. Materials and methods: 20 patients diagnosed with asthma, 26 patients following up with a diagnosis of lung cancer and control group of 20 healthy individuals were enrolled in this study. ANCA levels were measured in blood samples. Results: Borderline positive c-ANCA detected in 1 of 26 patients and 1/32 percent were positive detected in one patient. Borderline positive p-ANCA detected in 1 of 26 patients and 1/32 percent were positive detected in 1 patient. p-ANCA was positive detected in 2/20 of asthmatic patient and c-ANCA was borderline positive detected in 1/20 asthma patient. In asthma and cancer patients, physical examinations and pathological findings did not support the vasculitis. Only one subject in the control group was found to be borderline positive for c-ANCA. Conclusion: As a result, in some of the patients the positivity of ANCA was detected not related with vasculitis and paraneoplastic findings. Neurofibromatosis is a group of clinically related systemic disorders characterized by autosomal dominant inheritance. Hypophosphatemia may rarely develop due to phosphorus loss in the urine in a patient with neurofibromatosis type 1. We present here a 42-year-old male patient with neurofibromatosis type 1, who has two-year history of bone pain and fatigue. Hypoproteinemia, very low blood phosphorus level, significant reduction in bone mineral density, and insufficiency fractures of the proximal femurs and left fibula were detected. It was considered that hypophosphatemic osteomalacia led to stress fractures. The fractures were sufficiently healed with bisphosphonate, vitamin D, calcium treatment and phosphorus-rich diet. The blood phosphorus level of the patient approached approximately the normal limits. Long-lasting spinal, arm and leg pain in patients with neurofibromatosis type 1 must be carefully followed. It should be kept in mind that hypophosphatemic osteomalacia may occur in these patients. In addition to systemic examination, measurement of blood phosphorus level and bone mineral density must be done in order to prevent severe morbidities. [ABSTRACT FROM AUTHOR]

Desmoid tumors are histologically benign fibrous neoplasms originating from the musculoaponeurotic structures throughout the body. Despite their aggressive local infiltration, desmoid tumors lack a metastatic potential. Main locations of the desmoid tumors are the abdominal wall, the proximal extremities and the mesenteric intestine, especially in patients with familial adenomatous poliposis. In this study, we report a 27-year-old woman, who presented with a mass in the right upper side of her abdomen and was diagnosed as abdominal desmoid tumor postoperatively. [ABSTRACT FROM AUTHOR]

Objective: Cardiac arrhythmias commonly occur in hemodialysis patients. QT dispersion (QTd=QTmax-QTmin) reflects heterogeneity of cardiac repolarization, and increased QTd is known to predispose the heart to ventricular arrhythmias and sudden cardiac death. The aim of our study was to assess the association of iron stores, reflected by transferrin saturation (TSAT) and ferritin, with the dispersion of corrected QT intervals (QTc) in patients undergoing hemodialysis. Methods: This cross-sectional, case-controlled study included 40 patients (23 men and 17 women) with renal failure undergoing hemodialysis (Patient group) and 27 subjects (10 men and 17 women) with normal renal function (Control group). In all patients and control subjects, QT intervals were measured on electrocardiogram, and QTc intervals and QTc dispersion were calculated. Electrolyte, hemoglobin and serum TSAT and ferritin levels were also determined. Results: Hemodialysis patients had significantly greater QTc dispersion compared to that of control subjects (61.7±23.0 msec vs. 46.0±15.7 msec; p=0.001). Though serum iron levels were significantly associated with greater QTc dispersion (r=0.324, p=0.042), other electrolyte levels, duration of dialysis, TSAT and serum ferritin levels were not. Conclusion: Although hemodialysis patients have greater QTc dispersion than control subjects, their levels of iron stores as reflected by TSAT and ferritin levels, does not correlate with the degree of QT dispersion. BACKGROUND: Sarcopenia and frailty may develop at varying rates depending on the metabolic, structural, and functional changes in the muscle structure after stroke. OBJECTIVE: To evaluate the prevalence of sarcopenia and frailty in patients with stroke and affecting clinical parameters. METHODS: This was a cross-sectional and analytical study. Twenty-six (49.1%) women and 27 men (50.9%) with stroke with an average age of 60.2 ± 10.3 (median = 62) years were included in the study. To evaluate physical performance, balance, and mobility, we used the 4-meter walk test (4-MWT), timed up and go test, chair sit and stand test, and the Berg Balance Scale (BBS). Sarcopenia was diagnosed according to the Asian Working Group for Sarcopenia 2019 criteria. The SARC-F scale was used for sarcopenia. RESULTS: The average stroke duration was 21.6 ± 18.2 months. Prefrailty was present in 14 (26.4%) patients, frailty was present in 34 (64.2%) patients, and two (3.8%) patients were normal. Sarcopenia was detected in 28 (52.8%) patients. According to handgrip strength, the prevalence of low muscle strength was 17% (n = 9). In the sarcopenic group, female sex (p = 0.005) was more frequent, 4-MWT was longer (p = 0.003), chair stand test results were lower (p < 0.001), the clinical frailty scale (CFS) was higher (p < 0.001), fall risk was higher compared with the BBS (p < 0.001), there was lower HG strength (p = 0.009), lower malnutrition scores (PNI, prognostic nutritional index, p < 0.001), and lower lymphocyte counts (p = 0.037). EAT-10 scores and dysphagia presence (p = 0.026), the presence of malnutrition (p < 0.021) and Nottingham Health Profile score (p < 0.001) were higher in the sarcopenic group. CONCLUSION: Sarcopenia and frailty frequently develop in patients with chronic stroke, with low lymphocyte counts, physical performance, muscle strength, and mobility and balance scores, and it is associated with poor prognostic factors including high disease severity, dysphagia, malnutrition, risk of falling, and poor quality of life. Background and Aim: Direct oral anticoagulants (DOAC) are widely used, safe, and effective agents for thromboprophylaxis in nonvalvular atrial fibrillation (NVAF). In this study, we investigated the efficacy and safety of DOACs and the factors affecting DOAC dose preference in Turkey. Methods: In this multicenter, cross-sectional, and national study, we included 1497 patients using DOACs for more than three months for NVAF in five different centers in Turkey. The demographic, clinical, and laboratory characteristics and medical history of the patients were recorded. Ischemic and hemorrhagic events that occurred under all types and doses of DOAC treatment were noted. Logistic regression analysis was performed to test predictors of ischemic events and hemorrhagic complications under DOAC treatment. Results:The median age of the population was 73 (67-79) and 61.2% of those were female. The most common atrial fibrillation (AF) type for DOAC indication was permanent AF (73.1%). The most commonly used DOAC was rivaroxaban 20 mg (29.4%),and the least preferred DOAC was apixaban 2.5 mg (7.6%). The rate of dabigatran 110 mg was 10.7%, dabigatran 150 mg was 15.4%, rivaroxaban 15 mg was 11.5%, and apixaban 5 mg was 25.5%. There were no differences between DOAC subgroups, including low and recommended doses, in terms of ischemic events and bleeding complications, despite CHA2DS2-VASc and HAS-BLED scores,which were found to be significantly different between groups. Conclusions: Our study showed that all DOAC types and doses are similar in preventing ischemic events in NVAF patients. Submarine fan deposits which constitute the remnants of Neo-Tethys are widely observed throughout the Bitlis–Zagros Suture Belt. Most of these deposits are located in the Arabian-Eurasian collision zone and have been intensively deformed due to ongoing tectonic activity. In this study, the depositional characteristics and preservation processes of the undeformed, Lower Miocene Görendağı Submarine Fan deposits, associated with this zone, are studied with respect to their sedimentological and structural features. These deposits display feeder or distributary channels, overbank, and lobe sediments and classical Bouma Ta–d divisions. They are subdivided into inner, middle, and outer fan, according to sedimentological and morphological properties. Sand-rich submarine fan deposits have 39 m thickness and approximately 2 km of lateral extension. The submarine fan body is bounded to the north and the south by two reverse faults, namely the Özyurt and Göktepe faults. These faults formed an accommodation space where the fan was deposited and also caused the deepening of the basin. The position and activity of these oppositely inclined reverse faults, which form a triangular zone, are the main reason for preservation of the original fan morphology in the Görendağı Submarine Fan. Our results indicate that the Görendağı Submarine Fan represents one of the last sequences deposited before the closure of the southern branch of the Neo-Tethys Ocean and also the most well-preserved. • GSF is one the last marine clastics deposited before the closure of the Neo-Tethys. • GSF deposits are preserving their original radial-fan morphology since Early Miocene. • Parts of the submarine fan are clearly distinguished by sedimentary facies. • The submarine fan body is bounded by Özyurt and Göktepe reverse faults. • Original morphology of the GSF was preserved by oppositely-inclined two reverse faults.
