351. Peak expiratory flow and risk of cardiovascular disease and death. A 12-year follow-up of participants in the population study of women in Gothenburg, Sweden.

Abstract

A longitudinal population study of 1,462 women, aged 38-60 years at entry, was begun in Gothenburg, Sweden, in 1968-1969. This paper deals with the first 12-year follow-up period. In univariate analysis, peak expiratory flow showed a significant negative correlation with the 12-year incidences of myocardial infarction, electrocardiographic changes suggesting ischemic heart disease, stroke, and death. The association with incidence of myocardial infarction, stroke, and death remained in multivariate analysis, and was independent of age, body height, body mass index, adipose tissue distribution, chest deformity, history of pulmonary disease, smoking habit, serum cholesterol concentration, serum triglyceride concentration, systolic blood pressure, diabetes, and physical activity. The findings indicate that measuring peak expiratory flow is a simple procedure to identify women with an increased risk of cardiovascular disease or death. It seems urgent to find out what preventive measures might be taken as a consequence of these observations.

352. Obesity in relation to morbidity and mortality from cardiovascular disease.

Abstract

Representative population samples of middle-aged women (1462 participants, a participation rate of 90.1%) were followed from 1968-1969 for 10 years with respect to morbidity and mortality from myocardial infarction, stroke and all causes of death. The same sample (1302 participants, 80.3%) was studied in 1974-1975. Data on the incidences of angina pectoris, intermittent claudication and hypertension in these women during the 6-year interval were analyzed. Obesity as a premorbid characteristic was estimated by weight index and sum of triceps and subscapsular skinfold thicknesses. The overall trends between weight index and the incidences of myocardial infarction and angina pectoris were weak, while there were excess frequencies in the top quintiles, indicating that only marked obesity constitutes an increased risk. The incidence of hypertension was positively correlated to both indices of obesity. The death rate irrespective of cause was negatively correlated to the sum of skinfolds but was not correlated to the weight index. The death rate from myocardial infarction seemed to be correlated to both obesity indices.

353. Cessation of smoking after myocardial infarction in women. Effects on mortality and reinfarctions.

Abstract

Among 262 women with a first myocardial infarction discharged alive from hospital in Göteborg, Sweden between 1968 and 1977, 161 (61%) were smokers at the time of infarction. Postinfarction smoking was established after three months. In relation to smoking status three months after the infarction, subsequent survival and reinfarction rate were calculated by comparing those who smoked before infarction and later stopped (52%) with those who continued to smoke after the infarction (48%). There were no differences in preinfarction characteristics between quitters and continuing smokers. Women who stopped smoking after the infarction had higher serum enzymes during the acute phase than those who continued to smoke. The cumulative five-year survival rate was 85% among those who stopped smoking compared to 73% among those who continued to smoke (p less than 0.05). No significant difference was found in the cumulative reinfarction rate between the two groups with different smoking habits.

In den vergangenen Jahren hat sich der bilinguale Sachfachunterricht zu einem immer beliebter werdenden Angebot besonders an deutschen Gymnasien entwickelt. Dabei gibt es große Unterschiede in der Organisation (vgl. Hallet 2005 zu Organisationsformen), in der lokalen Verbreitung und in der Verteilung auf die unterschiedlichen Schulformen, Schulstufen, Schulfächer und Sprachen (vgl. Werner 2007 mit aktuellen Zahlen). Traditionell überwiegen die englischsprachigen Angebote in Verbindung mit gesellschaftswissenschaftlichen Fächern. Das Unterrichtsfach ‚Darstellendes Spiel‘ wird bisher sowohl von Seiten der Bildungspolitik als auch in der Theorie und in der Praxis des bilingualen Unterrichts kaum berücksichtigt. Das verwundert angesichts der vielen Schnittmengen zwischen Theaterspielen und Spracherwerb sowie der vielbeschworenen Potenziale im Hinblick auf das interkulturelle Lernen durch Theaterspielen. Im folgenden Beitrag sollen die Chancen und Besonderheiten des interkulturellen Lernens im bilingualen Sachfachunterricht ‚Darstellendes Spiel‘ auf der Grundlage von Ansätzen aus dem Fremdsprachenunterricht, der Theaterpädagogik und der Dramapädagogik am Beispiel des Englischunterrichts dargestellt und diskutiert werden. 356. The Göteborg metoprolol trial. Effects on mortality and morbidity in acute myocardial infarction

Abstract

In the Göteborg Metoprolol Trial, 1395 patients with suspected acute myocardial infarction were, on admission, randomly allocated to double-blind treatment, 697 to placebo and 698 to metoprolol (15 mg i.v. + 200 mg/day) for 90 days. During this period, there were 62 deaths in the placebo group (8.9%) and 40 in the metoprolol group (5.7%), a mortality reduction of 36% (p less than 0.03). This effect persisted regardless of age, previous infarction or previous chronic beta blockade. All deaths were classified as cardiovascular. After 3 months, all patients were recommended open treatment with metoprolol, and the difference in mortality between the two groups was maintained after 1 year. Early institution of metoprolol (within 12 hours) influenced infarct development during the first 3 days (infarct diagnosis and indirect measures of infarct size). Metoprolol also reduced the incidence on fatal and nonfatal infarction during the next 4-90 days by 35%. Furthermore, fewer episodes of ventricular fibrillation were recorded in the metoprolol than in the placebo group (six vs 17 patients). The tolerance was judged to be very good. The same percentage of patients (19%) was withdrawn from the blind treatment in the two groups. Fewer patients in the metoprolol group used lidocaine, furosemide and analgesics. We conclude that metoprolol therapy instituted on admission in patients with suspected acute myocardial infarction reduced 3-month mortality and exerted beneficial clinical effects.

Abstract

The Göteborg Metoprolol Trial was a double-blind, placebo-controlled, stratified trial aimed at evaluating the effect of the beta 1-selective blocker, metoprolol, in suspected acute myocardial infarction and during 2 years of follow-up. The primary end-point was 3-month mortality (blind treatment period). Secondary end-points were 2-year mortality, indirect signs of infarct size, chest pain, arrhythmias and tolerability. The entry criteria were fulfilled in 2,802 patients, 1,395 of whom were included in the trial. Treatment started as soon as possible after arrival in hospital with intravenous administration followed by oral treatment for 3 months. All patients were randomized 48 hours or less after estimated onset of infarction and 69% were randomized at 12 hours or less. The blind treatment had to be withdrawn in 19% of all randomized patients before the end of the 3-month follow-up.

Abstract

In completion to electrophysiological studies in man auditory evoked brain stem potentials of the guinea pig were recorded by way of the far-field technique. Two age groups of albino and pigmented guinea pigs were compared with regard to several parameters of the response. The four groups showed significant differences in auditory threshold. Albinos exhibited lower thresholds and a minor hearing loss in age. Changes in response pattern could not be found as components of the aging process. Beside higher thresholds old guinea pigs showed reduced amplitudes to high-frequency stimuli. The effect of age at the peripheral stations of the auditory pathway is discussed in the lights of these results. [ABSTRACT FROM AUTHOR]

Abstract

Heart rate, systolic blood pressure and rate-pressure product were analyzed during the first 18 hours and 4 days after intravenous metoprolol or placebo. On injection of metoprolol there was an immediate decrease in mean heart rate from 72.9 +/- 0.6 to 62.7 +/- 0.4 beats/min, but no change was found in the placebo group. The difference in heart rate remained during the first 4 days. Systolic blood pressure was reduced from 144.1 +/- 0.9 to 134.6 +/- 0.9 mm Hg after intravenous metoprolol and was lower than that in the placebo group during 4 days of follow-up. Indirect signs of congestive heart failure tended to be less severe in patients given metoprolol within 12 hours of the onset of symptoms than in those given placebo. The duration of hospitalization also tended to be shorter in patients given early metoprolol treatment than in those given placebo early.
