The alarming raise of multi-drug resistance among human microbial pathogens makes the development of novel therapeutics a priority task. In contrast to conventional antibiotics, antimicrobial peptides (AMPs), besides evoking a broad spectrum of activity against microorganisms, could offer additional benefits, such as the ability to neutralize toxins, modulate inflammatory response, eradicate bacterial and fungal biofilms or prevent their development. The latter properties are of special interest, as most antibiotics available on the market have limited ability to diffuse through rigid structures of biofilms. Lipidation of AMPs is considered as an effective approach for enhancement of their antimicrobial potential and in vivo stability, however, it could also have undesired impact on selectivity, solubility or the aggregation state of the modified peptides. In the present work, we describe the results of structural modifications of compounds designed based on cationic antimicrobial peptides DK5 and CAR-PEG-DK5, derivatized at their N-terminal part with fatty acids with different lengths of carbon chain. The proposed modifications substantially improved antimicrobial properties of the final compounds and their effectiveness in inhibition of biofilm development as well as eradication of pre-formed 24 h old biofilms of Candida albicans and Staphylococcus aureus. The most active compounds (C5-DK5, C12-DK5 and C12-CAR-PEG-DK5) were also potent against multi-drug resistant Staphylococcus aureus USA300 strain and clinical isolates of Pseudomonas aeruginosa. Both experimental and in silico methods revealed strong correlation between the length of fatty acid attached to the peptides and their final membranolytic properties, tendency to self-assemble and cytotoxicity.

Background The chick embryo chorioallantoic membrane (CAM) model is well described in human medicine as a cost-effective, easy to perform preclinical oncological model for observing pro- and antiangiogenic response, tumor biology and metastasis. The main objective of this article was to present the modification of the CAM assay in order to evaluate tumor growth from two feline fibrosarcoma cell lines (FFS1, FFS3) and describe their morphological and histopathological features. Results The authors described morphological and histopathological features of two feline fibrosarcoma cell lines (FFS1 and FFS3) grown on the CAM. Tumors from the FFS1 cell line showed high malignancy (grade III), while tumors from the FFS3 cell line were grade II. Introduction: CNS relapse of DLBCL is associated with poor prognosis. Estimated incidence varies between 1.9 and 8.4%1. The CNS-International prognostic index (IPI)2 help risk stratify and estimate the 2-year risk of CNS relapse in DLBCL patients treated with R-CHOP chemotherapy. CNS prophylaxis is indicated in patients with a high risk of CNS relapse (a score of ≥4 equated to a 10.2% risk). High-risk DLBCL patients outside the CNS-IPI system include double/triple-hit (MYC/BCL-2/BCL-6 translocations) lymphoma, HIV lymphoma, testicular lymphoma, primary cutaneous lymphoma-leg type, stage IE breast lymphoma3. IT methotrexate or cytarabine administered during the course of systemic chemotherapy has been the most widely employed method of CNS prophylaxis but there is paucity of data validating its efficacy. Aim: The primary aim of the study was to evaluate the CNS relapse rates in DLBCL patients who received CNS prophylaxis. Patients and Methods: This was a single-centre retrospective observational study conducted in a district general hospital. Data was extracted from the regional (Dorset Cancer Network) DLBCL database and laboratory reports for CSF analysis at the time of the first intrathecal chemotherapy. Medical records of patients with DLBCL who received CNS prophylaxis were evaluated for the following patient-related and disease-related demographics: age at diagnosis, gender, stage, systemic treatment, CNS prophylaxis, treatment response, remission duration, systemic relapse rates, CNS relapse rates and survival. CNS-IPI scores were retrospectively calculated and additional indications evaluated for patients who received CNS prophylaxis. Results: Between 2013 and 2018, 178 patients were diagnosed with DLBCL. All patients were treated with RCHOP chemo-immunotherapy. CNS prophylaxis was administered in 47 (26%) patients. Median age was 69 years (range 20-86 years) and 62% were males. All 47 patients (100%) received IT methotrexate as CNS prophylaxis, with 43 (91%) receiving all of the planned 4 doses of IT methotrexate 12.5 mg each. A CNS-IPI score of ³4 was present in 31 (66%) patients, and a score of 2-3 in 9 (19%) patients. Additional risk factors identified included testicular lymphoma in 3 patients, breast lymphoma in 2 patients and oropharyngeal lymphoma in 2 patients. Ten (21%) patients received their treatment at the outset with courses 1-4 of R-CHOP. Of the 47 patients who received CNS prophylaxis, 5 (10%) relapsed; all had isolated CNS lymphoma at relapse. Median time to CNS relapse was 25 months (range 12-36 months) from initial diagnosis of DLBCL. Median survival after CNS relapse was 5 months (range 2-9 months). Of the remaining 141 patients, 2 patients relapsed with isolated CNS lymphoma. Conclusion: Although the overall incidence was low (4%), CNS relapse was observed in 10% of high-risk patients all of whom received CNS prophylaxis with IT methotrexate. The efficacy of CNS prophylaxis with IT chemotherapy remains unproven. There is no randomised study to show that IT prophylaxis alone is effective. Current British guidelines recommend high-dose intravenous methotrexate over IT methotrexate if patient's physiological fitness and renal function are acceptable4. The median age in our cohort was 69 years which makes it challenging to deliver dose-intensive systemic therapy concurrently with intravenous high-dose methotrexate. The role of CNS prophylaxis in high-risk patients including its efficacy and safety in older patients need further evaluation in prospective randomised studies. Background/AimsObesity is associated with infertility, decreased ovarian performance and lipotoxicity. However, little is known about the aetiology of these reproductive impairments. Here, we hypothesise that the majority of changes in ovarian physiology in diet-induced obesity (DIO) are a consequence of transcriptional changes downstream of altered leptin signalling. Therefore, we investigated the extent to which leptin signalling is altered in the ovary upon obesity with particular emphasis on effects on cumulus cells (CCs), the intimate functional companions of the oocyte. Furthermore, we used the pharmacological hyperleptinemic (LEPT) mouse model to compare transcriptional profiles to DIO.MethodsMice were subjected to DIO for 4 and 16 weeks (wk) and leptin treatment for 16 days, to study effects in the ovary in components of leptin signalling at the transcript and protein levels, using Western blot, Real-time PCR and immunostaining. Furthermore, we used low-cell RNA sequencing to characterise changes in the transcriptome of CCs in these models.ResultsIn the DIO model, obesity led to establishment of ovarian leptin resistance after 16 wk high fat diet (HFD), as evidenced by increases in the feedback regulator suppressor of cytokine signalling 3 (SOCS3) and decreases in the positive effectors phosphorylation of tyrosine 985 of leptin receptor (ObRb-pTyr985) and Janus kinase 2 (pJAK2). Transcriptome analysis of the CCs revealed a complex response to DIO, with large numbers and distinct sets of genes deregulated at early and late stages of obesity; in addition, there was a striking correlation between body weight and global transcriptome profile of CCs. Further analysis indicated that the transcriptome profile in 4 wk HFD CCs resembled that of LEPT CCs, in the upregulation of cellular trafficking and impairment in cytoskeleton organisation. Conversely, after 16 wk HFD CCs showed expression changes indicative of augmented inflammatory responses, cell morphogenesis, and decreased metabolism and transport, mainly as a consequence of the physiological changes of obesity.ConclusionObesity leads to ovarian leptin resistance and major time-dependent changes in gene expression in CCs, which in early obesity may be caused by increased leptin signalling in the ovary, whereas in late obesity are likely to be a consequence of metabolic changes taking place in the obese mother.

Volumetric Modulated Radiotherapy (VMAT) implementation requires additional Quality Assurance (QA) tests to assure stable machine performance especially in terms of dynamic parameters synchronization. The lack of a twin machine for TrueBeam led us to the investigation of backup workflow with Clinacs 2300C/D. These Clinacs were upgraded to make them VMAT-enabled. This study aimed to compare long-term VMAT performance QA on 3 accelerators: TrueBeam (TB) and 2 Clinacs (V4 and V5). All VMAT test plans were provided by Varian. The test set consisted of initial and advanced tests. Initial tests were intended to check the gravity effect on Multileaf Collimator (MLC) and dosimetry system. As the results of these tests were correct and there was visual inspection applied for MLC positioning accuracy analysis, they were not presented in the paper. We focused on 2 advanced tests: dose rate vs gantry speed and dose rate vs MLC speed. The idea of the advanced test was to compare segments irradiated with the same fluence but different dose rate, gantry speed or MLC speed. Test sets were irradiated weekly on average for 12 months. These tests were analysed following Varian procedures and criteria using in-house-developed software. Apart from that we calculated correlation between all segments pairs and performed profile analysis. According to Varian criteria, all tests for TrueBeam were very well within the tolerances. Dose rate vs gantry speed tests for Clinacs were within allowed limits while as many as 28% and 6% of dose rate vs MLC speed tests failed for Clinacs V4 and V5, respectively. The profile analysis revealed tests for which the difference between measured and planned dose was over 3% and still met the criteria. The correlation analysis showed that VMAT plans on TrueBeam were irradiated repeatably because all segments were strongly correlated. There was no correlation between the segment with the highest MLC speed and the other segments on Clinacs in dose rate vs MLC speed test. This segment was irradiated randomly. TrueBeam is more reliable than upgraded Clinacs 2300C/D for VMAT performance. That is why in our centre the one of upgraded Clinac that performed tests better served only as a backup machine for VMAT technique, and the second one was excluded from clinical use for this technique.

Postovulatory ageing of mammalian oocytes occurs between their ovulation and fertilization and has been shown to decrease their developmental capabilities. Aged oocytes display numerous abnormalities, including altered Ca2+ signalling. Fertilization-induced Ca2+ oscillations are essential for activation of the embryonic development, therefore maintaining proper Ca2+ homeostasis is crucial for the oocyte quality. In the present paper, we show that the mechanism underlying age-dependent alterations in the pattern of sperm-triggered Ca2+ oscillations is more complex and multifaceted than previously believed. Using time-lapse imaging accompanied by immunostaining and molecular analyses, we found that postovulatory ageing affects the amount of Ca2+ stored in the cell, expression of Ca2+ pump SERCA2, amount of available ATP and distribution of endoplasmic reticulum and mitochondria in a manner often strongly depending on ageing conditions (in vitro vs. in vivo). Importantly, those changes do not have to be caused by oxidative stress, usually linked with the ageing process, as they occur even if the amount of reactive oxygen species remains low. Instead, our results suggest that aberrations in Ca2+ signalling may be a synergistic result of ageing-related alterations of the cell cycle, cytoskeleton, and mitochondrial functionality.

Despite advances in chronic lymphocytic leukaemia (CLL) treatment, globally chemotherapy remains a central treatment modality, with chemotherapy trials representing an invaluable resource to explore disease-related/genetic features contributing to long-term outcomes. In 499 LRF CLL4 cases, a trial with >12 years follow-up, we employed targeted resequencing of 22 genes, identifying 623 mutations. After background mutation rate correction, 11/22 genes were recurrently mutated at frequencies between 3.6% (NFKBIE) and 24% (SF3B1). Mutations beyond Sanger resolution (KRAS mutations principally composed of these low VAF variants. Firstly, employing orthogonal approaches to confirm TP53 mutations, we assessed the clinical impact of TP53 clonal architecture. Whilst ≥ 12% VAF TP53mut cases were associated with reduced PFS and OS, we could not demonstrate a difference between TP53 mutations and either wild type or ≥12% VAF TP53mut cases. Secondly, we identified biallelic BIRC3 lesions (mutation and deletion) as an independent marker of inferior PFS and OS. Finally, we observed that mutated MAPK-ERK genes were independent markers of poor OS in multivariate survival analysis. In conclusion, our study supports using targeted resequencing of expanded gene panels to elucidate the prognostic impact of gene mutations.

Cel pracyCelem niniejszej pracy jest poznanie opinii młodzieży w wieku 16-19 lat, uczęszczającej do warszawskich publicznych liceów ogólnokształcących, na temat efektywności profilaktyki samobójstw.MetodaPopulacja badana objęła młodzież, która w roku 2015 była w wieku 16-19 lat (M = 17,29; SD = 0,94). Respondenci uczęszczali do ośmiu warszawskich publicznych liceów ogólnokształcących z ośmiu różnych dzielnic. Badanie było reprezentatywne. W badaniu uczestniczyło 1439 respondentów – 821 dziewcząt (57,1%) i 592 chłopców (41,1%). Badanie przeprowadzono metodą PAPI. Kwestionariusz zawierał 34 pytania.WynikiRespondenci najczęściej nie wiedzieli czy w Polsce istnieje zorganizowany system zapobiegania samobójstwom (42,0%), ale duża część respondentów (39,7%) miała świadomość, że taki system w Polsce nie istnieje. Respondenci uważali działalność szkoły (46,7%), Kościoła (38,3%) i policji (55,9%) za nieskuteczną, jednostek opieki zdrowotnej (47,7%) i mediów (46,6%) za mało skuteczne, natomiast rodzinę (70,9%) za skuteczną w zakresie zapobiegania samobójstwom. Osoby, które upatrują znaczącą rolę kościoła równocześnie wskazują na znaczący udział rodziny w zapobieganiu samobójstwom, a w znacznie mniejszym stopniu innych z wymienionych instytucji.Wnioski1. Ponad 1/3 badanej młodzieży zdaje sobie sprawę, iż w Polsce nie istnieje zintegrowany system zapobiegania samobójstwom. 2. Najważniejszą rolę w prewencji zachowań autodestrukcyjnych, w opinii licealistów, pełni rodzina. 3. Istnieje potrzeba większego zaangażowania szkół, Kościoła, policji, jednostek służby zdrowia oraz mediów w edukację i uświadamianie młodzieży w kwestii profilaktyki zachowań autodestrukcyjnych.

Research Areas: Endocrinology & Metabolism The regulation of corpus luteus (CL) luteolysis is a complex process involving a myriad of factors. Previously, we have shown the involvement of Nodal in functional luteolysis in mares. Presently, we ask the extent of which Nodal mediation of luteolysis is done through regulation of angioregression. We demonstrated the interaction between Nodal and hypoxia-inducible factor 1 a (HIF1a) and thrombospondin 1/thrombospondin receptor (TSP1/CD36) systems, could mediate angioregression during luteolysis. First, we demonstrated the inhibitory effect of Nodal on the vascularmarker platelet/endothelial cell adhesion molecule 1 (CD31). Also, treatment of mid CL explants with vascular endothelial growth factor A (VEGFA) showed a trend on activin-like kinase 7 (Alk7) protein inhibition. Next, Nodal was also shown to activate HIF1a and in vitro culture of mid CL explants under decreased oxygen level promoted Nodal expression and SMAD familymember 3 (Smad3) phosphorylation. In another experiment, the crosstalk between Nodal and TSP1/CD36 was investigated. Indeed, Nodal increased the expression of the anti-angiogenic TSP1 and its receptor CD36 in mid CL explants. Finally, the supportive effect of prostaglandin F2a (PGF2a) on TSP1/CD36 was blocked by SB431542 (SB), a pharmacological inhibitor of Nodal signaling. Thus, we evidenced for the first time the in vitro interaction between Nodal and both HIF1a and TSP1 systems, two conserved pathways previously shown to be involved in vascular regression during luteolysis. Resumen: El uso de apoyo nutricional ambulatorio, enteral o parenteral, ha sido un paso necesario en la opti mización del soporte nutricional en pacientes, que, por diversas patologías, no logran cumplir con sus requerimientos por vía oral (VO). En el presente artículo se presentan las recomendaciones de la Rama de Nutrición, dirigidas a los equipos de salud que atienden pacientes pediátricos, que requieran alimentación enteral por un tiempo prolongado. Su objetivo general es entregar pautas para un co rrecto manejo en estos pacientes. Se describe la conformación ideal del equipo de salud para atención y seguimiento de dichos pacientes, los criterios de ingreso al programa y su forma de evaluación en el tiempo. Además, se describen características generales de la alimentación enteral, vías de admi nistración, fórmulas enterales disponibles, complicaciones de este soporte nutricional y por último monitorización y seguimiento del paciente.

Abstract The use of home enteral or parenteral nutrition has been a necessary step in the optimization of nu tritional support in patients who, due to several diseases, fail to meet their nutritional requirements by oral feeding. This article presents the recommendations of the Chilean Pediatric Society Nutritio nal Branch, aimed at health teams that treat pediatric patients who require enteral feeding for a long time. The general objective is to provide guidelines for the proper management of these patients. It describes the ideal conformation of the health team for the care and follow-up of those patients, the program admission criteria, and its evaluation method over time. In addition, it describes general characteristics of enteral feeding, routes of administration, available enteral formulas, complications, and patient follow-up.

Venetoclax is a BCL2 inhibitor with activity in relapsed/refractory (R/R) chronic lymphocytic leukaemia (CLL). We conducted a multi-centre retrospective analysis of 105 R/R CLL patients who received venetoclax pre-National Health Service commissioning. The median age was 67 years and median prior lines was 3 (range: 1–15). 48% had TP53 disruption. At ≥2 lines, 60% received a Bruton Tyrosine Kinase inhibitor (BTKi) and no prior phosphoinositide 3-kinase inhibitor (Pi3Ki), 25% received a Pi3Ki and no prior BTKi, and 10% received both. Patients discontinued B cell receptor inhibitor (BCRi) because of toxicity in 44% and progression in 54%. Tumour lysis syndrome risk was low, intermediate or high in 27%, 25%, and 48% respectively. Overall response was 88% (30% complete response [CR]). The overall response rate was 85% (CR 23%) in BTKi-exposed patients, 92% (CR 38%) in Pi3Ki-exposed patients and 80% (CR 20%) in both (P = 0·59). With a median follow-up of 15·6 months, 1-year progression-free survival was 65·0% and 1-year overall survival was 75·1%. Dose reduction or temporary interruption did not result in an inferior progression-free or discontinuation-free survival. Risk of progression or death after stopping a prior BCRi for progression was double compared to those stopping for other reasons (predominantly toxicity) (Hazard Ratio 2·01 P = 0·05). Venetoclax is active and well tolerated in R/R CLL post ≥1 BCRi. Reason(s) for stopping BCRi influences venetoclax outcomes.

Wstęp. Główne czynniki ryzyka wielu chorób wynikają z nieprawidłowych zachowań zdrowotnych i są przyczyną m. in. rozwoju nowotworów złośliwych, w tym raka nerki. Jednym z elementów zachowań zdrowotnych jest określenie stanu odżywienia, celem wczesnego wykrycia zaburzeń i podjęcia interwencji. Cel pracy. Określenie i porównanie zachowań zdrowotnych ze stanem odżywienia pacjentów z rozpoznanym rakiem nerki. Materiał i metody. Badanie przeprowadzono wśród 100 osób, u których wykonany był zabieg operacyjny z powodu raka nerki. W badaniach zastosowano metodę analizy dokumentacji medycznej oraz sondaż diagnostyczny z wykorzystaniem narzędzia standaryzowanego – Inwentarza Zachowań Zdrowotnych. Wyniki. Wskaźnik zachowań zdrowotnych dla ogółu badanych wyniósł średnio 75,56 pkt. Najwyższe wyniki uzyskano w kategorii zachowań profi laktycznych (średnia 3,89), następnie w kategorii pozytywnych nastawień psychicznych (średnia 3,34) oraz w kategorii praktyk zdrowotnych (średnia 2,91). Najniższy wynik uzyskano w kategorii prawidłowych nawyków żywieniowych (średnia 2,49). Stwierdzono zależność pomiędzy wiekiem, BMI, a kategorią prawidłowe nawyki żywieniowe (p ≤ 0,001) oraz pomiędzy wiekiem, BMI, a kategorią praktyki zdrowotne (p < 0,001). Wnioski. W całej badanej grupie najniższe wyniki uzyskano w kategorii prawidłowych nawyków żywieniowych. Pomimo stwierdzenia w całej grupie najniższych wyników w kategorii prawidłowych nawyków żywieniowych, pacjenci w starszym wieku i z wyższym BMI uzyskali wyższe wyniki, ale w zakresie praktyk zdrowotnych uzyskali wyniki niskie. Introduction. The main risk factors of numerous diseases result from abnormal health behaviors and among others are causing development of malignant tumors, including kidney cancer. One of the elements of health behaviors is to determine the state of nutrition, in order to early detect disorders and to undertake interventions. Aim of the study. Determination and comparison of health behaviors with the nutritional status of patients with diagnosed kidney cancer. Material and methods. The study was conducted among 100 people who underwent surgery because of kidney cancer. The research used the method of analysis of medical documentation and a diagnostic survey with a tool – Health Behavior Inventory. Results. The average index of health behaviors for all respondents was 75.56 points. The highest results were obtained in the category of preventive behaviors (mean 3.89), then in the category of positive mental attitudes (mean 3.34) and in the category of health practices (mean 2.91). The lowest result was obtained in the category of proper eating habits (mean 2.49). The relationship between age, BMI, and the category of normal eating habits (p ≤.001) was observed as well as between age, BMI, and the category of a health practice (p < 0.001). Conclusions. The lowest results were obtained in the category of proper eating habits. Despite obtaining the lowest results in the category of normal eating habits in the whole group, among patients with a higher BMI and in the elderly patients, normal eating habits were found at a higher level, but their health practices remain at a low level.
