Abstract

Nonsuicidal self-injury (NSSI) is a frequent phenomenon in adolescents, however there is a lack of studies on the prevalence of NSSI in adolescents placed in youth welfare and juvenile justice group homes. The goal of the present study is to investigate the prevalence rates of NSSI and mental disorders in adolescents living in the youth welfare system, as well as how occasional and repetitive NSSI differ with respect to mental disorders, suicidality, and gender. The sample consisted of 397 adolescents aged 12 to19 years (mean age = 15.98, SD = 1.77, 65.7% male) placed in youth welfare and juvenile justice group homes. NSSI, suicidality, and mental disorders were assessed using the Kiddie-Schedule of Affective Disorders and Schizophrenia (K-SADS-PL). Lifetime prevalence rates of occasional and repetitive NSSI were 21.9% and 18.4%, respectively and 85.6% of the sample endorsed a lifetime mental disorder. Occasional and repetitive NSSI were significantly associated with depressive, conduct, and substance use disorders (d = 0.50-0.67) among both genders. Prevalence rates of repetitive NSSI in youth welfare and juvenile justice institutions are higher than in the general population and males who engage in NSSI are at particularly high risk of suicidality. Due to the high prevalence of NSSI and its related problems, NSSI should be routinely assessed in this vulnerable population and staff should be trained in recognizing and handling NSSI as well as supporting adolescents in improving their emotion regulation skills. Abstract

Background: Approximately 50% of rare diseases have symptom onset during childhood. A high level of nursing care and an often uncertain prognosis put caregivers of the affected children at high risk for psychological distress. At the same time, their caregivers have limited access to appropriate psychological care. The aim of this study was to evaluate a web-based psychological support program for caregivers of children with chronic rare diseases (WEP-CARE). Methods: German-speaking parents (recruited between May 2016 and March 2018) caring for children aged 0–25 years with a rare disease showing clinically relevant anxiety symptoms, were assigned to either the WEP-CARE (n = 38) or treatment as usual (n = 36) condition within a randomized controlled trial. The primary outcome measure was parental anxiety, assessed with the Generalized Anxiety Disorder Questionnaire (GAD-7). Secondary outcomes were fear of disease progression, depression, coping, quality of life and user satisfaction. The group differences were tested through repeated-measures analyses of variance. The WEP-CARE group was additionally followed up three months after the treatment. Results: A significant time-group interaction was found for anxiety (F (1,35) = 6.13, p =.016), fear of disease progression (F (1,331) = 18.23, p <.001), depression (F (1,74) = 10.79, p =.002) and coping (F (1,233) = 7.02, p =.010), suggesting superiority of the WEP-CARE group. Sustainability of the treatment gains regarding anxiety, fear of disease progression and coping was confirmed at the 6-month follow-up assessment (p <.01). A significant interaction effect could not be found for quality of life (F(1,2) = 0.016; p =.899). Both participating parents and therapists were satisfied with WEP-CARE. Conclusions: Our results underline the efficacy and feasibility of WEP-CARE for parents of children with various rare diseases. [ABSTRACT FROM AUTHOR]

Abstract

This cross-sectional online survey study investigated whether certain health behaviors moderated the relationship between perceived stress and suicidal ideation in Austrian adolescents during the COVID-19 pandemic. A total of 1505 14–20-year-old (median age = 16) high school students (77.9% female) filled out an online survey from September to November 2021. Perceived stress was measured with the PSS10, suicidal ideation with item 9 of the PHQ-9. The following health behaviors were assessed: Physical activity (days/week), phone use (hours/day), problematic drinking behavior (CAGE). All three health behaviors significantly moderated the relationship between perceived stress and suicidal ideation (all p <.05), but effects were small. The moderation analyses revealed that higher physical activity and less time spent on the phone were associated with less suicidal ideation at higher stress levels. Showing signs of problematic drinking behavior was associated with higher suicidal ideation at higher stress levels. In conclusion, these results suggest that some health behaviors may be able to act as a buffer between perceived stress and suicidal ideation. Abstract

"Chemical restraint" oder "Pharmakologische Ruhigstellung" (PR) ist eine wichtige Intervention beim Management von aggressiven Verhaltensweisen von stationär behandelten Kindern und Jugendlichen. PR sollte ausschließlich im Rahmen eines standardisierten Interventionsplanes zur Anwendung kommen. Die Grundprinzipien eines solchen Zugangs sind transparente Vereinbarungen mit allen Beteiligten zur schnellen, sicheren und personenunabhängigen Reduktion des Verhaltens, ohne dass dabei Folgeschäden etwa im Sinne einer Retraumatisierung auftreten. Für die Praxis empfiehlt es sich, Medikamente anzuwenden, die sowohl für die Beruhigung als auch für die Prävention von aggressiven Eskalationen eingesetzt werden können. Die aktuelle Datenlage erlaubt es, in erster Linie Neuroleptika und Benzodiazepine zu empfehlen. Insbesondere atypische Neuroleptika sind hierbei von Bedeutung, da sie sowohl für die Notfallsedierung als auch die Prävention eingesetzt werden können. Weitere kontrollierte Untersuchungen zur medikamentösen Behandlung von aggressiven Zuständen mit Selbst- und Fremdgefährdung bei Kindern und Jugendlichen sind aber dringend notwendig. The use of CR during in-patient treatment should be based on standardized intervention strategies. The principles of such an approach include transparent agreement with all participants on fast save and staff-independent reduction of aggressive behaviour without inducing retraumatisation. In clinical practice medication that is effective both for tranquilization and prevention of aggressive escalation is recommended. Current data suggest first line treatment using neuroleptics and benzodiazepines. Due to their potential use both for rapid sedation and prevention atypical neuroleptics are of particular importance. Further controlled studies are needed to evaluate pharmacological treatment of aggressive states with potential (self) harming behaviour in children and adolescents.
