Purpose: To describe some of the benefits of service learning (SL), considerations in course development and construction, and implementation and outcomes of an SL course in the undergraduate communication sciences and disorders (CSD) program at a small, public university in northwest Washington., Method: A review of the literature on SL and a description of the author's experience in course development are provided on the basis of a computerized database search, library search, and discussions with the Western Washington University Center for Service Learning., Conclusions: Teaching an SL course can present challenges to both faculty and students; nonetheless, incorporating SL into the undergraduate CSD curriculum is an excellent way of enriching the academic experience and improving critical-thinking skills of young students. SL provides hands-on opportunities for students to apply what they are learning in their CSD classes to real-world contexts, gain a better understanding of course content through engagement in real situations, and integrate information from a variety of courses in and outside of their major.

Objective: The purpose of the present study was to examine the relationship between objectively measurable acoustic changes in speech production and subjective speech production accuracy and perceived intelligibility immediately following a disruption in auditory feedback normally provided to subjects from a cochlear implant., Methods: Six children with profound sensorineural hearing loss participated in the study. Their task was to produce speech samples in two conditions: (1) with auditory feedback from their cochlear implants, and (2) without auditory feedback from their cochlear implants. Samples were subjected to both objective and subjective analyses. Objectively, measures were made of duration, fundamental frequency, and the first and second formants of the vowels. Subjectively, two groups of listeners, one familiar with the speech of children with hearing loss and the other unfamiliar, transcribed the productions and provided ratings of intelligibility., Results: All the children in this study exhibited significant differences from the cochlear implant-on to the cochlear implant-off condition, although these changes were not always in the predicted direction, nor were they always perceptually salient., Conclusions: Consistent with previous studies, children in this investigation demonstrated variable acoustic voice and speech changes following deactivation of their cochlear implant device. Few of these acoustic changes affected speech intelligibility. The results of this study overall suggest that during the initial years following implantation children who are deaf rely to some extent on the auditory feedback provided by a cochlear implant to control and modify F0, duration, and vowel formant production.

Clinical signs and symptoms of acute disseminated intravascular coagulopathy (DIC) include bleeding from body orifices, such as the nose, mouth, or ear, bleeding from an intravenous (IV) site, areas of ecchymosis, or blood in the urine or stool. The underlying disease triggering DIC usually determines the clinical presentation. However, patients with chronic DIC (compensated DIC) may possess subclinical signs and symptoms, and the bleeding disorder may only be identified through laboratory findings. In this compensated form, the triggering factor is exposed slowly and in small amounts (seen in malignancies and vasculitis), allowing replenishing of the augmented factors by the liver, adequate reticuloendothelial clearance of fibrin degradation products, and increased production of platelets, which prevent secondary fibrinolysis and the signs of bleeding. 1,4 We report a case of an 82-year-old male who presented to the emergency room 24 hours after a routine dental extraction with bleeding from the tooth socket, severe hypotension, and presence of ecchymosis on his chest. Clinical and radiographic exam revealed multiple thoracic and abdominal aortic aneurysms, as well as infrarenal and iliac aneurysms, continuous oral hemorrhage, and a unique presentation rarely documented in the literature: a bleeding tooth socket as the initial clinical sign and presentation of DIC.

Pregnancy complicated by type 1 diabetes mellitus is associated with an increased risk of complications in the mother and infant. Normal or near normal glycemic control prior to and during pregnancy reduces many of these risks to levels observed in the general population. This degree of glycemic control is generally achievable only with intensive insulin therapy: multiple daily injections (MDI) or continuous subcutaneous insulin infusion (CSII) via an insulin pump. These therapeutic regimens have been found to result in comparable glycemic control, although CSII provides increased flexibility in terms of patient lifestyle, and may reduce the incidence of hypoglycemia. Frequent home blood glucose monitoring is imperative during pregnancy in order to optimize glycemic control and reduce the risk of hypoglycemia. Furthermore, insulin requirements change significantly over the course of pregnancy. The new short-acting insulin analogs, insulin lispro and insulin aspart, have pharmacodynamic properties which make them ideal for use during pregnancy. Although the number of published studies evaluating the use of insulin lispro during pregnancy is limited, the majority support its safety. No studies of insulin aspart in pregnancy have been published in full. In addition to optimization of glycemic control, frequent assessment for development and/or progression of microvascular complications is necessary during pregnancy.

The present study was designed to examine relationships between the life values of 399 U.S. college students and their nuclear war-related thoughts, feelings, and behaviors. The students completed four scales from the Life Values Inventory: (i.e. Conventionally Defined Success [CDS]; Religious Faith and Devotion [RFD]; Activist Pursuit of Social Causes [APSC]; Materialistic Orientation [MO]), the Satisfaction With Life Scale, four scales from the Nuclear War Inventory--Nuclear Distress; Salience; Weapons Opposition; Personal Efficacy--and a single behavioral measure of approach toward information concerning nuclear weapons. Consistent with theory regarding the influence of values and commitments on attitudes and behavior, APSC was found to be positively associated with all five nuclear war measures. Additionally, MO was negatively related to Personal Efficacy and Information Approach, and CDS was positively associated with Nuclear Distress. The only value dimension which covaried significantly with general life satisfaction was RFD. Results are discussed with respect to the recent rise in conservative and materialistically-oriented values among American college students.

The heat-stable protein inhibitor (Walsh, D. A., et al. (1971), J. Biol. Chem. 246, 1977--1985) of the cyclic adenosine 3',5'-monophosphate dependent protein kinase has been isolated in pure form from rabbit skeletal muscle after a 430 000-fold purification with a 47% yield. The four-step procedure involves sequentially a heat treatment, batchwise anion and cation exchange, and affinity chromatography on protein kinase catalytic subunit covalently coupled to Sepharose 4B. The inhibitor is an acidic protein (pI = 4.24) of molecular weight 11 300. It contains 98 amino acid residues none of which contains sulfur and only 2 (phenylalanine and tyrosine) are aromatic. The NH2-terminus is blocked. The muscle content is ca. 0.6 mg of inhibitor per L of intracellular water. The inhibitor is tightly bound to the catalytic subunit of protein kinase (Ki congruent to 2 X 10(-9) M) and acts competitively with respect to the protein substrates. Protein kinase recognizes a short stretch of the inhibitor sequence, in which arginyl side chains play a crucial role. A study of various competitive inhibitors of the kinase confirms the importance of guanidino groups and hydrophobic side chains in the specific interaction with the substrate binding site.
