- Abstract
A beam dump experiment has been performed at CERN in Gargamelle using the neutrino facility to look for penetrating particles produced either directly in the beam interaction or by prompt decay of new particles. A total of 32 interactions with a visible energy greater than 10 GeV has been found, classified, aftercorrections, into 18 charged current νμor νμ, 5.1 neutral current and 8.9 νeor νecharged current events. An excess of νeevents remains after all subtractions from any established sources. Results are presented in terms of the product of the cross section and the leptonic decay branching ratio of the possible source.

BACKGOUND AND DESIGN: An association between essential mixed cryoglobulinemia and hepatitis C virus (HCV) infection has been reported. Dermatologic manifestations are a classic presenting complaint in essential mixed cryoglobulinemia. The aim of this study was to compare the frequency and the nature of dermatologic manifestations in essential mixed cryoglobulinemia according to the presence of anti-HCV antibodies. Sixty-two consecutive patients with essential mixed cryoglobulinemia were tested for anti-HCV antibodies. Dermatologic manifestations were systematically assessed. RESULTS: Anti-HCV antibodies were detected in 35 patients. Palpable purpura corresponding histologically to leukocytoclastic vasculitis was the more frequently observed dermatologic manifestation and occurred mainly in HCV-antibody-positive patients. The patients with purpura had significantly higher serum cryoglobulin levels than patients without purpura. CONCLUSIONS: The frequency of palpable purpura is higher in HCV-antibody-positive patients and is related to the serum cryoglobulin levels.(Arch Dermatol. 1995;131:1124-1127)

- Abstract
Monoclonal IgG-containing type II cryoglobulins (CG) from 25 patients, most affected with autoimmune or inflammatory diseases, were characterized using a Western blotting procedure. Rheumatoid factors were measured by ELISA. Out of the 25 type II CG, 14 contained a single monoclonal IgG and 11 contained several distinct monoclonal Ig, as assessed by the electrophoretic mobility and isotypy of the narrow bands. Comparison of blots revealed by anti-IgG subclass antibodies in the patients' sera and CG showed a clear-cut enrichment of CG in both polyclonal and monoclonal IgG1 and IgG3. All monoclonal IgG in the CG belonged to the IgG1 or IgG3 subclasses. CG also contained more detectable RF of the IgM class than did the corresponding sera. Using conventional methods, the present CG either could not be typed or were all (immunoelectrophoresis) or often (immunofixation) classified as type III. The results challenge the classical classification of CG and show the necessity of the use of sensitive techniques for immunochemical characterization of mixed CG. Copyright 1994, 1999 Academic Press

365. Role of CT in Chronic Pulmonary Embolism

- Abstract
To assess the value of CT in chronic pulmonary embolism (CPE), CT scans and pulmonary angiograms of 21 consecutive patients were reviewed. Computed tomography was better than angiography in assessing proximal clots (three thrombi not seen by angiography, three angiographic false-positives confirmed by surgery). Furthermore, CT was able to analyze pulmonary arteries distal to angiographic amputations. Computed tomography was less sensitive than angiography for vascular distortions (38 vs. 50) and stenosis (35 vs. 71.8). Pulmonary infarctions were better detected and characterized by CT than by angiography. Isolated parenchymal ground-glass opacities were seen by CT in 18 patients, especially in those with right cardiomegaly. High resolution CT delineated them better than did standard CT. Computed tomography may be a useful adjunct to angiography in the assessment of CPE.

Vindesine (VDS) has been submitted to a phase-II trial, the results of which were assessed in terms of regression induction. VDS was given weekly IV in doses of 2 mg/m
2 on two consecutive days to 59 patients, 55 of whom were evaluable. A high proportion of complete (36%) and over 50% partial regressions were obtained in acute lymphoid leukemias (ALL) (overall response 63%) whatever the perceptible phase, in blastic crisis of chronic myeloid leukemia (55%), and some responses were recorded in lymphosarcoma (40%). No effect has so far been seen in acute myeloid leukemia or in Hodgkin's disease. Malignant neoplasms of the immunoblastic type seem to be particularly sensitive to VDS. Continuous 48 h IV infusion can induce a remission where an IV push administration of the same dose has failed. One remarkable characteristic of VDS is the apparent absence of cross-resistance with VCR: in acute leukemic forms, 55% of patients who failed to obtain remission induction after three weekly injections of VCR (used in combination chemotherapy) achieved a complete or partial remission with VDS. The toxicity was mainly neurologic (paralytic ileus, constipation, paresthesias, loss of reflexes) and hematologic (leukopenia and thrombopenia), and was not more significant than with the other agents: four patients died of infection or hemorrhage.- Published

373. Long-term prognostic and predictive factors in 107 stage II/III breast cancer patients treated with anthracycline-based neoadjuvant chemotherapy

- Abstract
The heterogeneity of therapeutic modalities and eligibility criteria and the lack of long-term follow-up in most reports of neoadjuvant chemotherapy for breast cancer preclude us from drawing conclusions about its value in clinically relevant patient subgroups. The present study aims to identify predictive and prognostic factors in 107 non-inflammatory stage II/III breast cancer patients treated between November 1980 and October 1991 with an anthracycline-based induction regimen before locoregional surgery. Preoperative chemotherapy comprised 3-6 cycles of doxorubicin (pirarubicin after 1986), vindesine, cyclophosphamide and 5-fluorouracil. Type of subsequent surgery and adjuvant treatment were decided individually. In analysis of outcome, univariate comparisons of end points were made using the log-rank test, and significant (P < or = 0.05) pre- and post-therapeutic factors were incorporated in a Cox multivariate analysis. With a median follow-up of 81 months (range 32-164+ months), the median disease-free survival (DFS) is 90.5 months while median overall survival has not yet been reached. Cytoprognostic grade and histopathological response in both the primary and lymph nodes were independent covariates associated with locoregional relapse with or without DFS and overall survival. Eleven patients with pathological complete response remain free of disease with a 68-month median follow-up, while the 18 with residual microscopic disease on the specimen showed a 60% cumulative incidence of locoregional recurrence. Despite encouraging response rates based on clinical or radiological evaluation (87% or 70%), neither method showed any significant correlation with pathological response and failed to contribute prognostic information on patients' outcome. Pathological evaluation of antitumoral activity of primary chemotherapy remains a major source of prognostic information and might be used to select patients in need of additional adjuvant treatment.

Sonographic examinations of 17 patients revealed 21 kidneys with anechoic sonolucent areas within the renal sinus. Computerized tomography was obtained in 3 patients and the mass effects were proven to be multilocular parapelvic cysts. These parapelvic cysts simulated renal sinus lipomatosis.

377. Almitrine Decreases the Distensibility of the Large Pulmonary Arteries in Man

- Abstract
Almitrine improves blood gas values in patients with COPD, primarily through better ventilation-perfusion matching. The improvement comes largely from a vasomotor effect of the drug. The vasomotor mechanism is unknown. We suspected that decreased distensibility of the large pulmonary arteries was one probable effect of the drug. If present, reduced distensibility would explain many of the hemodynamic alterations produced by almitrine. To test this idea, we measured the diameter of the right pulmonary artery during systole and diastole both before and after almitrine administration in nine patients who were undergoing pulmonary cineangiography. Our calculations of the distensibility of the right pulmonary artery showed that almitrine caused a 60 percent decrease. This significant alteration in the stiffness of the large pulmonary arteries can account for the rise in systolic pressure, a change that would be expected to redistribute pulmonary blood flow upwards and thereby improve ventilation-perfusion balance.

Purpose:To evaluate, in an animal model, the efficacy and safety of a 7 Fr percutaneous vena cava filter for temporary or permanent use. Methods:The Dibie-Musset (DM) filter is a wire preformed into a double-spiral shape with a spring effect. The experiment was performed in 15 adult sheep and consisted of the insertion of 45 filters, clot trapping tests, and filter retrieval. Follow-up in all sheep consisted of radiologic (abdominal radiograph, inferior vena cavogram, abdominal computed tomography), macroscopic, and microscopic examinations between days 8 and 386 after filter placement. Results:Of the 45 filters implanted in 15 sheep, 29 were retrieved between day 0 and day 15. Filtration efficiency was 100% for clots 4 × 4 × 20 mm. No long-term (1 year) side-effects were detectable. Microscopic examination of the vein wall showed only a dense intimal fibrosis without signs of acute inflammation at 1 year. Conclusion:These results support the efficiency and safety of the DM filter in an animal model.

From 199 η O → 3 π O → 6 γ , 526 η O → π + π − π O and 401 η O → γγ events with all γ-rays reconstructed in a heavy liquid bubble chamber, we obtain direct and independent measurements of the branching ratios: R 1 = Γ ( η O → 3 π O )/ Γ ( η O → π + π - π O ) = 1.50 -0.29 +0.15 and R 2 = Γ ( η O → γγ )/ Γ ( η O → π + π - π O ) = 1.72 ± 0.25. We also study the Dalitz-plots for the modes η O → 3π O and η O → π + π - π O , and find no significant variation of the amplitude of the decay η O → 3 π O .

Three different targets have been adapted and used successfully for experiments with 8 GeV/c ?- (240,000 photographs), 16 GeV/c ?- (150,000 photographs) and 2 GeV/c ? + (230,000 photographs). The last experiment is in progress.

The relativistic increase of ionization is studied in various gaseous mixtures by the method of drop-counting in a cloud chamber. If we call?the relative ionization ofß/v1-ß2equal to 300, we find that in pure Xenon or mixtures of Xenon and Hydrogen,?is of the order of 1.7. On the other hand the value of?drops to 1.3 if Helium is mixed with Xenon. - Abstract
Special conditions associated with laboratory autoimmune testing are not well compatible with recent developments in regulatory frameworks such as EN/ISO 15189 accreditation or in vitro diagnostic medical device regulation (IVD-R). In addition, international recommendations, guidelines and disease criteria are poorly defined with respect to requirements on autoantibody testing. Laboratory specialists from Austria, Belgium, Croatia, Estonia, Finland, France, Germany, Greece, Hungary, Italy, Norway, Poland, Portugal, South Africa, Spain, Sweden, Switzerland, and The Netherlands collected information, reported national experience, and identified quality issues in relation to autoantibody testing that require consensus on interpretation of the regulatory frameworks and guidelines. This process has been organized by the European Autoimmunity Standardisation Initiative (EASI). By identifying the critical items and looking for a consensus, our objective was to define a framework for, in particular, EN/ISO accreditation purposes. Here, we present a review of current publications and guidelines in this field to unify national guidelines and deliver in this way a European handout on quality control and accreditation for laboratories involved in autoantibody testing. We focus on quality items that can be checked during accreditation visits. Despite various local varieties, we encountered an overwhelming dedication to quality assurance in all contributing countries.

The reaction nu /sub mu /e to nu /sub mu /e has been investigated in the bubble chamber Gargamelle exposed to the wide-band neutrino beam of the CERN SPS. Nine electron candidates of which 0.5 are expected to be background, were found in a total statistics available of 64000 charged current neutrino events. Results are given in terms of vector and axial vector coupling constants. The present results agree with the standard SU(2)(X)U(1) model for current values of sin/sup 2/ Theta /sub w/. A critical comparison is made with the previous result based on statistics which were smaller by a factor 2.6 and which led to a rather higher cross section. (10 refs).

Data is given for single and multiple strange particle production in neutrino reactions in both charged and neutral current channels. A total of 15000 neutrino and antineutrino events has been examined for possible evidence of semi-leptonic decay of a charmed particle. One candidate has been observed. Upper limits on charmed particle production cross-sections are given as a function of the neutrino energy and the invariant mass of the final state hadrons.

An attempt at the direct observation of the cascade decay of beauty particles, produced by π- of 350 GeV/c leading to 3 muons or 4 muons in the final state, has been made in an emulsion/counter hybrid experiment at CERN. The cross section ratio of the elastic neutral current reaction νp→νp to the quasi-elastic charged current reaction νn→μ−p has been measured in the kinematical region 0.3⩽q2⩽1.0 (GeV/c)2. The measured value is RM=0.17±0.08. Model dependent corrections are applied, especially for νn→νn contamination, and the result is compared to various models.

A search for direct evidence of the production and subsequent decay of gluinos has been performed in nuclear emulsions. The emulsion stacks were exposed, as part of a large hybrid apparatus, to a beam of 350GeV/ c π t- mesons, and events were selected on the basis of a “missing-energy” trigger. The missing-energy signature is expected to be a characteristic feature of events in which gluinos are produced and subsequently decay into hadrons and almost non-interacting photinos. The negative result of this search sets an upper limit to the production cross section of light gluinos.

A study is presented of single πo production in neutrino and antineutrino interactions in Gargamelle filled with freon CF3Br. Limits on the neutral to charged current cross-section ratios R v =σ(v N →v N ′π o )/2 σ(v N → μ - N ′π o ) and R ν = σ( ν N → ν N ′π o )/2 σ( ν N → ω + N ′π o ) are found to be 0.10 ν and 0.26 ν at 68% confidence level.

A search for the reaction nu /sub mu /+e/sup -/ to nu /sub mu /+e/sup -/ was made in the bubble chamber Gargamelle exposed to the wide- and neutrino beam of the CERN-SPS. Ten isolated e/sup -/ were found above 2 GeV energy. All of them were emitted at an angle smaller than 1.5 degrees with respect to the beam. The background is 0.2+or-0.2 events. Results on cross section are given in terms of vector and axial vector coupling constants. A comparison is made with the standard SU(2)(X)U (1) model: no agreement is found with the currently accepted value of the mixing angle. Possible sources other than the above reaction are discussed. New results are presented of an analysis of pictures taken during an exposure of Gargamelle to the CERN ν beam in 1975. In addition to three ν μ +N→ μ − +e + +V 0 + hadrons events previously reported, an excess is found of events of the type ν μ +N→ μ − +e + + hadrons without V 0 . An analysis of strange particle production in charged current events suggests a possible violation of the ΔS = ΔQ rule. Interpreted in terms of charm production, the data permit an estimate of branching ratios and production rate of charmed particles. A limit is also placed on charmed particle production by neutral currents.
