Introduction: Athletes can exhibit abnormal electrocardiogram (ECG) phenotypes that require further evaluation prior to competition. These are apparently more prevalent in high-intensity endurance sports. The purpose of this study was to assess the association between ECG findings in athletes and intensity of sport and level of competition., Methods: A cohort of 3423 competitive athletes had their ECGs assessed according to the Seattle criteria (SC). The presence of abnormal ECGs was correlated with: (1) intensity of sport (low/moderate vs. at least one high static or dynamic component); (2) competitive level (regional vs. national/international); (3) training volume (≤20 vs. >20 hours/week); (4) type of sport (high dynamic vs. high static component). The same endpoints were studied according to the 'Refined Criteria' (RC)., Results: Abnormal ECGs according to the SC were present in 225 (6.6%) athletes, more frequently in those involved in high-intensity sports (8.0% vs. 5.4%; p=0.002), particularly in dynamic sports, and competing at national/international level (7.1% vs. 4.9%; p=0.028). Training volume was not significantly associated with abnormal ECGs. By multivariate analysis, high-intensity sport (OR 1.55, 1.18-2.03; p=0.002) and national/international level (OR 1.50, 95% CI 1.04-2.14; p=0.027) were independent predictors of abnormal ECGs, and these variables, when combined, doubled the prevalence of this finding. According to the RC, abnormal ECGs decreased to 103 (3.0%), but were also more frequent in high-intensity sports (4.2% vs. 2.0%; p<0.001)., Conclusions: There is a positive correlation between higher intensity of sports and increased prevalence of ECG abnormalities. Objectives: To investigate the sociodemographic and behavioural factors associated with incidence, persistence or remission of obesity in a longitudinal sample of Australian children aged 4-10 years., Setting: Nationally representative Longitudinal Study of Australian Children (LSAC)., Participants: The sample for this analysis included all children in the Kinder cohort (aged 4-5 years at wave 1) who participated in all four waves of LSAC (wave 1, 2004, aged 4-5 years; wave 2, 2006, aged 6-7 years; wave 3, 2008, aged 8-9 years and wave 4, 2010, aged 10-11 years). Of the 4983 children who participated in the baseline (wave 1) survey, 4169 (83.7%) children completed all four waves of data collection., Primary and Secondary Outcome Measures: Movement of children between weight status categories over time and individual-level predictors of weight status change (sociodemographic characteristics, selected dietary and activity behaviours)., Results: The study found tracking of weight status across this period of childhood. There was an inverse association observed between socioeconomic position and persistence of overweight/obesity. Sugar-sweetened beverages and fruit and vegetable intake and screen time appeared to be important predictors of stronger tracking., Conclusions: Overweight and obesity established early in childhood tracks strongly to the middle childhood years in Australia, particularly among children of lower socioeconomic position and children participating in some unhealthy behaviour patterns., (Published by the BMJ Publishing Group Limited. Objectives: This paper aimed to systematically evaluate the mental health and well-being outcomes observed in previous community-based obesity prevention interventions in adolescent populations., Setting: Systematic review of literature from database inception to October 2014. Articles were sourced from CINAHL, Global Health, Health Source: Nursing and Academic Edition, MEDLINE, PsycARTICLES and PsycINFO, all of which were accessed through EBSCOhost. The Cochrane Database was also searched to identify all eligible articles. PRISMA guidelines were followed and search terms and search strategy ensured all possible studies were identified for review., Participants: Intervention studies were eligible for inclusion if they were: focused on overweight or obesity prevention, community-based, targeted adolescents (aged 10-19 years), reported a mental health or well-being measure, and included a comparison or control group. Studies that focused on specific adolescent groups or were treatment interventions were excluded from review. Quality of evidence was assessed using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) guidelines., Primary and Secondary Outcome Measures: Primary outcomes were measures of mental health and well-being, including diagnostic and symptomatic measures. Secondary outcomes included adiposity or weight-related measures., Results: Seven studies met the inclusion criteria; one reported anxiety/depressive outcomes, two reported on self-perception well-being measures such as self-esteem and self-efficacy, and four studies reported outcomes of quality of life. Positive mental health outcomes demonstrated that following obesity prevention, interventions included a decrease in anxiety and improved health-related quality of life. Quality of evidence was graded as very low., Conclusions: Although positive outcomes for mental health and well-being do exist, controlled evaluations of community-based obesity prevention interventions have not often included mental health measures (n=7). It is recommended that future interventions incorporate mental health and well-being measures to identify any potential mechanisms influencing adolescent weight-related outcomes, and equally to ensure interventions are not causing harm to adolescent mental health., (Published by the BMJ Publishing Group Limited. Objective: Longitudinal evidence of relationships between unhealthy diets and BMI in children is crucial for appropriately targeting obesity prevention activities. The objective was to determine the relationship between frequency of consumption of sugar sweetened beverages (SSBs) and high fat foods (HFFs) and body weight in Australian children aged from 4 to 10 years., Methods: Data from 4,164 children participating in four waves (wave 1, 2004; wave 2, 2006; wave 3, 2008; and wave 4, 2010) of the Longitudinal Study of Australian Children were analyzed. A multi-level growth model tested relationships between consumption of SSB and HFF and BMI z-scores., Results: BMI z-scores were associated with daily consumption of HFF, SSB and maternal BMI independent of BMI z-scores at wave 1 (baseline); with each additional occurrence of SSB and HFF consumption intake per day, BMI z-score increased by 0.015 U (P < 0.01) and 0.014 U (P < 0.001), respectively. With each additional maternal BMI unit, BMI z-score increased by 0.032 (P < 0.001)., Conclusions: Higher BMI z-scores were strongly associated with the consumption of SSBs and HFFs. Body mass index and waist circumference are widely used tools to identify risk of non-communicable diseases. Research has indicated that the risk relationships differ by ethnicity. In this study, data from chronic disease surveys in Fiji, Nauru, Solomon Islands and Wallis and Futuna were merged and analysed using receiver operator curves. The action points for body mass index and waist circumference with the highest specificity and sensitivity for identifying the risk of NCDs were identified. The analysis showed considerable differences between Melanesians and other Pacific Islanders, and also gender differences. Action points for non-Melanesians were higher than for Melanesians, and region-wide values are therefore inappropriate.:, (© 2014 Asian Oceanian Association for the Study of Obesity . Background: Community capacity building is a promising approach in reducing childhood obesity. The objective was to determine changes in capacity over a 3 year intervention (2005-2008) in schools and whether greater increases in capacity were associated with greater decreases in overweight/obesity., Methods: "It's your Move!" (IYM) was an obesity prevention project, in 12 Australian secondary schools (5 intervention; 7 comparison), that aimed to increase community capacity to promote healthy eating and physical activity. Capacity was assessed pre/post intervention using the 'Community Readiness to Change (RTC)' tool. Comparisons from baseline to follow-up were tested using Wilcoxon Signed-Ranks and results plotted against changes (Newcombe's paired differences) in prevalence of overweight/obesity (WHO standards)., Results: RTC increased in intervention schools (p=0.04) over time but not for comparison schools (p=0.50). The intervention group improved on 5 of 6 dimensions and the three intervention schools that increased three levels on the RTC scale each had significant reductions in overweight/obesity prevalence., Conclusion: There were marked increases in capacity in the intervention schools and those with greater increases had greater decreases in the prevalence of overweight/obesity. Objectives: To examine the relationships between physical activity, sedentary behaviour and body mass index (BMI) among a sample of Australian adolescents., Methods: Anthropometric, demographic and behavioural data were collected from students (n=3040 mean age 14.6, 44% female) from 12 secondary schools in South West Victoria, Australia (response rate=48.6%). The appropriate descriptive, univariate and regression analysis were used to examine the strength of the associations between physical activity, sedentary behaviour and odds of overweight or obese and the effect of interaction between physical activity and sedentary behaviour on odds of overweight and obese., Results: Males were more likely to be active during the school day than females and had higher median hours of screen time per school day. Physical activity during the school day was associated with higher standardized BMI (BMI-z) among males. Higher levels of activity after school were associated with lower BMI-z for males and females. For both males and females the odds of overweight or obese were higher among the least active. An interaction was observed for females whereby the prevalence of overweight and obesity among the most physically active was lowest for the least sedentary and highest for the most sedentary., Conclusions: The relationships between physical activity, sedentary behaviour and BMI-z were complex. Interventions to reduce BMI through increasing physical activity or decreasing sedentary behaviour need to consider the complex inter-relationships between these variables and moderating factors such as age, sex and socio economic status in their design and interpretation., (Copyright © 2011 Sports Medicine Australia. Objective: 'Traffic-light' nutrition labelling has been proposed as a potential tool for improving the diet of the population, yet there has been little published research on the impact of traffic-light nutrition labelling on purchases in a supermarket environment. This study examined changes to online consumer food purchases in response to the introduction of traffic-light nutrition information (TLNI)., Methods: The study consisted of a 10-week trial in a major Australian online grocery store. For the duration of the trial TLNI in the form of four colour-coded indicators representing the products' relative levels of fat, saturated fat, sugar and sodium content, was displayed on the product listing page of 53 of the retailer's own-brand products in five food categories (milk, bread, breakfast cereals, biscuits and frozen meals). The changes in sales before and after the introduction of TLNI were examined both within the intervention store and in a comparison store., Results: TLNI had no discernible impact on sales, with the change in sales in the intervention store corresponding to changes in sales in the comparison store. No relationship was observed between changes in sales and the relative healthiness of products., Conclusion and Implications: This limited, short-term study found no evidence to support the notion that TLNI is likely to influence behaviour change. Further research is needed to examine the impact of providing TLNI in different contexts, for a longer duration and on more products, with and without complementary awareness and information campaigns., (© 2011 The Authors. People with terminal diagnoses are increasingly encouraged to stay at home for the trajectory of their illness. Consequently, relatives and friends are progressively undertaking the informal carer role more frequently in conjunction with formal palliative care teams. Although there is international literature on the efficacy of and satisfaction with publicly funded palliative homecare, to date, there is scant literature reporting satisfaction with privately funded palliative homecare especially in Australia. Survey and focus group methods were used to explore carers' levels of satisfaction with a private Australian palliative homecare service and to uncover unmet carer needs. There were 300 (41.7%) responses to the survey. Three focus groups were conducted, separately comprising current carers, past carers and palliative homecare staff. While there was a high level of satisfaction with the service generally, carers identified a lack of knowledge about aspects of the illness and treatment course. A desire to have time with nursing staff independent of the patient, including continued contact after the death of the patient, was also expressed. Themes relating to service delivery concerned lack of continuity of attending nursing staff and a perceived inadequate number of service visits. These findings were similar to results reported in the literature for publicly funded services. Therefore interventions used in the public sector to meet carer needs are likely to be successful if applied in the private sector.
