Abstract

Introduction: For early diagnosis of TB, it is essential to ensure proper identification. Smear microscopy is a simple, economical, less time-consuming technique is a good alternative. The stud was conducted to compare direct smear, concentrated smear and fluroscent microscopy of sputum of patient with tuberculosis; also to assess the sensitivity and specificity of direct and concentrated smear by ZN stain and fluorescent microscopy. Methodology: The 400 samples of suspected to be a case of pulmonary tuberculosis as per RNTCP guide­lines are included. Direct smear and concentrated smear were made and stained by Carbolfuchsin methods which include the Ziehl-Neelsen (ZN) and Kenyon methods ( Light /bright field microscope) and Fluorochrome procedure using auramine-O or auramine-rhodamine dyes (Fluorescent microscope -FM). Result: On direct smear, out of 400 samples 138 samples were positive by ZN stain method and 150 samples were positive by FM stain method. While it was 154 samples and 156 sample by respective stains on concentrated smear. The sensitivity of direct and concentrated smear microscopy is comparable to FM stain and Zn stain. 352. Atypical Afta Major Healing after Photodynamic Therapy

Abstract

The aim of this study is to report a case of atypical Afta Major healing in a patient with recurrent aphthous stomatitis (SAR) with a type of photodynamic therapy. A female patient with SAR affected for about 2 years reported a history of hypothyroidism treated with Levothyroxine. The oral cavity clinical examination showed several major symptomatic ulcers, previously treated with topical and systemic therapies without any benefit. The largest of them is present for more than 40 days, in spite of topical cortisone applications, with significant pain symptoms reported by the patient. It was decided to perform a session of photodynamic therapy with a device that emits a LED light used in combination with a photosensitive reagent (Toluidine blue). The dye was applied on the entire surface of the lesion beyond the margins and even encroaching on healthy tissue. The light diode was turned on with a wavelength of 630 nm with cycles from 30 seconds, 10 consecutive times above it. After a few days, a curious phenomenon happened: healing of Afta Major starting from the center, which was almost completely healed towards the borders of the lesion. No previous literature reports this type of healing. Photodynamic therapy could be a successful treatment for SAR.

Abstract

Background Increasing the dose intensity of cytotoxic therapy by shortening the intervals between cycles, or by giving individual drugs sequentially at full dose rather than in lower-dose concurrent treatment schedules, might enhance efficacy. Methods To clarify the relative benefits and risks of dose-intense and standard-schedule chemotherapy in early breast cancer, we did an individual patient-level meta-analysis of trials comparing 2-weekly versus standard 3-weekly schedules, and of trials comparing sequential versus concurrent administration of anthracycline and taxane chemotherapy. The primary outcomes were recurrence and breast cancer mortality. Standard intention-to-treat log-rank analyses, stratified by age, nodal status, and trial, yielded dose-intense versus standard-schedule first-event rate ratios (RRs). Findings Individual patient data were provided for 26 of 33 relevant trials identified, comprising 37 298 (93%) of 40 070 women randomised. Most women were aged younger than 70 years and had node-positive disease. Total cytotoxic drug usage was broadly comparable in the two treatment arms; colony-stimulating factor was generally used in the more dose-intense arm. Combining data from all 26 trials, fewer breast cancer recurrences were seen with dose-intense than with standard-schedule chemotherapy (10-year recurrence risk 28·0% vs 31·4%; RR 0·86, 95% CI 0·82–0·89; p Interpretation Increasing the dose intensity of adjuvant chemotherapy by shortening the interval between treatment cycles, or by giving individual drugs sequentially rather than giving the same drugs concurrently, moderately reduces the 10-year risk of recurrence and death from breast cancer without increasing mortality from other causes.

Abstract

Thin film solar cells based on Cu2ZnSn(S,Se)4 are very promising, because they contain earth abundant elements and show high absorptive behavior. However, the performance of these solar cells are limited and needs to be improved in order to reach efficiencies as high as that reported for Cu(In,Ga)Se2 and CdTe based solar cells. To obtain enhanced performance, the present study investigates the output effect of fixed bilayer CZTS/CZTSe structure with band gap 1.5 and 0.9 eV respectively and the effect of MoS2 interfacial layer between CZTS and CZTSe layer to avoid unintentional elemental diffusion. The idea of bilayer CZTS/CZTSe and CZTS/MoS2/CZTSe structure was exploited to achieve higher open circuit voltage and efficiency by band engineering mechanism compared to single layer. In this numerical study the focus is given to fixed band gap model as the structure is practically feasible to realize with the associated materials. The thin interfacial layer introduced between the two layers ultimately diminishes the recombination process due to the back induced electric field. Finally, the fixed bandgap CZTS/MoS2/CZTSe exhibits better performance with open circuit voltage of 0.88 V and efficiency of 21.1%.

Abstract

Background: Trastuzumab targets the extracellular domain of the HER2 protein. Adding trastuzumab to chemotherapy for patients with early-stage, HER2-positive breast cancer reduces the risk of recurrence and death, but is associated with cardiac toxicity. We investigated the long-term benefits and risks of adjuvant trastuzumab on breast cancer recurrence and cause-specific mortality. Methods: We did a collaborative meta-analysis of individual patient data from randomised trials assessing chemotherapy plus trastuzumab versus the same chemotherapy alone. Randomised trials that enrolled women with node-negative or node-positive, operable breast cancer were included. We collected individual patient-level data on baseline characteristics, dates and sites of first distant breast cancer recurrence and any previous local recurrence or second primary cancer, and the date and underlying cause of death. Primary outcomes were breast cancer recurrence, breast cancer mortality, death without recurrence, and all-cause mortality. Standard intention-to-treat log-rank analyses, stratified by age, nodal status, oestrogen receptor (ER) status, and trial yielded first-event rate ratios (RRs). Findings: Seven randomised trials met the inclusion criteria, and included 13 864 patients enrolled between February, 2000, and December, 2005. Mean scheduled treatment duration was 14·4 months and median follow-up was 10·7 years (IQR 9·5 to 11·9). The risks of breast cancer recurrence (RR 0·66, 95% CI 0·62 to 0·71; p Interpretation: Adding trastuzumab to chemotherapy for early-stage, HER2-positive breast cancer reduces recurrence of, and mortality from, breast cancer by a third, with worthwhile proportional reductions irrespective of recorded patient and tumour characteristics.

Abstract

The importance of solar energy has increased tremendously in the past decade due to its many salient features. The Kesterite based solar cell (CZTS) that rely on earth abundant, non-toxic elements is used in this work. In the present simulative study, optimization of different layers like Al doped ZnO (front layer), i-ZnO, CdS (buffer layer), CZTS (absorber layer) have been analyzed by numerical simulation using wxAMPS. The basic objective of this study is to achieve the optimized layers parameters such as concentration (donor and acceptor), thickness, defect density of the solar cell which comprises of Al:ZnO/i-ZnO/CdS/CZTS/Mo from top to bottom. This paper explicitly deals with the idea to optimize output parameters which are represented as VOC = 0.754 Volt, JSC = 22.10 mA/cm2 and efficiency of 14.17% of the cell.

Abstract

Many researchers have been working on recurrence relation which is an important topic not only in mathematics but also in physics, economics and various applications in computer science. There are many useful results on recurrence relation sequence but there main problem to find any term of recurrence relation sequence we need to find all previous terms of recurrence relation sequence. There were many important theorems obtained on recurrence relations. In this paper we have given special identity for generalized kth order recurrence relation. These identities are very useful for finding any term of any order of recurrence relation sequence. Authors define a special formula in this paper by this we can find direct any term of a recurrence relation sequence. In this recurrence relation sequence to find any terms we need to find all previous terms so this result is very important. There is important property of a relation between coefficients of recurrence relation terms and roots of a polynomial for second order relation but in this paper, we gave this same property of recurrence relation of all higher order recurrence relation. So finally, we can say that this theorem is valid all order of recurrence relation only condition that roots are distinct. So, we can say that this paper is generalization of property of a relation between coefficients of recurrence relation terms and roots of a polynomial. Abstract

Published for the first time in 1940 the André Breton’s Anthology of black humor inaugurates the great season of surrealist anthologies, which will last until late 60s. The use of the traditional form of the modern anthology by the Surrealists, does not involve into a complete acceptance of its rules, already codified since the end of the 19th century, but rather a deformation of its textual structure and of its objectives, producing a literary genre with particular characteristics. The surrealist anthology, such as those realized by André Breton, Paul Éluard, Louis Aragon and Benjamin Péret, represent an hybrid literary object with structural elements in common with the dictionary, the glossary, the anthology and the catalogue. The surrealist literary collections represent both a different approach to the history of literature and an expression of surrealist poetics.

To compare the findings of demographic characteristics, clinical features and haematological profile between RSV and Non –RSV bronchiolitis. Materials and Methods: This is a prospective study, conducted in a teritiary care center for 1 year period from Jan 2015 to Dec 2015. The demographic characteristics, clinical features and haematological profile of children aged between 1 month to 3 years who fulfilled the inclusion criteria were noted in predesigned proforma, nasopharyngeal swab was sent for RSV analysis and then the findings of the parameters were compared between the two groups of RSV bronchiolitis and Non RSV bronchiolitis. Results: Among 80 cases with 40 in each group, children below the age of 1year were affected more in RSV group, with male preponderance. Among the clinical features except that 89.7% of RSV cases had wheeze that was statistically significant with no difference in other features. Investigations showed no much difference in both the groups. Percentage of Non RSV subjects who received nebulisation with bronchodilators, steroid and antibiotic therapy were higher than RSV subjects. The hospital stay was significantly higher in RSV cases and none of the study participants met with mortality. Conclusion: Children with RSV bronchiolitis had prolonged hospital stay compared to Non RSV group. Need for nebulisation with bronchodilators, steroids and antibiotic therapy was more in Non RSV group.

Abstract

Mutations in the P53 pathway are a hallmark of human cancer. The identification of pathways upon which p53-deficient cells depend could reveal therapeutic targets that may spare normal cells with intact p53. In contrast to P53 point mutations in other cancer, complete loss of P53 is a frequent event in osteosarcoma (OS), the most common cancer of bone. The consequences of p53 loss for osteoblastic cells and OS development are poorly understood. Here we use murine OS models to demonstrate that elevated Pthlh (Pthrp), cAMP levels and signalling via CREB1 are characteristic of both p53-deficient osteoblasts and OS. Normal osteoblasts survive depletion of both PTHrP and CREB1. In contrast, p53-deficient osteoblasts and OS depend upon continuous activation of this pathway and undergo proliferation arrest and apoptosis in the absence of PTHrP or CREB1. Our results identify the PTHrP-cAMP-CREB1 axis as an attractive pathway for therapeutic inhibition in OS.

Here, we analyse the coupled control of viscosity and sedimentation on forearc high growth during advanced stages of subduction accretion. We use 2D thermo‐mechanical finite element models constrained with data of the south Anatolian margin. Our simulations show that forearc highs grow as a thermally‐activated viscosity drop in the lower crust induces ductile deformation and viscous flow. Initial viscosity and the amount of sediments in the forearc basin control non‐linearly the occurrence and timing of the thermally‐activated viscosity drop, and thus of the growth of the forearc high. High sedimentation rates result in thicker forearc basins that stabilise the subduction wedge and delay the onset of uplift in the forearc high. Low viscosities promote earlier onset of forearc high uplift and lead to larger morphological variability along the subduction margin. Increasing either the sedimentation rate or viscosity may prevent forearc high formation entirely. The thermo‐viscous forearc highs grow at an age set by wedge thermal state as a function of accretionary flux, wedge viscosity, and synorogenic sedimentation. Abstract

The use of chicken as food is rising day today and as byproduct 8.5 billion tons, poultry feathers are produced worldwide, whereas India contributes about 350 million tons per annum. It is a waste product of poultry industries, are considered a potential high-quality protein supplement owing to their crude protein content of more than 85%. In the present study Alternaria tenuissima a keratinophilic fungus was used for feather degradation in submerged state fermentation and soil. Total Protein releases were studied in submerged state fermentation by A. tenuissima are 122µg/ml and 238µg/ml in 15 and 25 days respectively. Lysine, Methionine, Cysteine and Valine were found 15.8, 6.8, 20.2, 7.5 µg/ml in 25 days, respectively. Chicken feathers were mixed with soil and inoculated with spore suspension for degradation of complex keratin protein into simpler organic forms. A. tenuissima degraded feathers in soil and enhances nutritional value. Five-gram feathers in 250 g soil mixtures were found better growth enhancers and increased height. This work will reduce the solid waste generated in the form of feathers from the poultry industry, and convert it into a simpler organic form that can be used by plants.

Introduction: Septicemia is a leading cause of morbidity and mortality in India. Blood culture remains the gold standard for the diagnosis of septicemia. The antimicrobial sensitivity pattern differs in different studies. Knowledge of likely causative organisms of septicemia and their antimicrobial sensitivity pattern can help to start appropriate therapy in order to minimize morbidity and mortality. Aim: To isolate the etiological organisms causing sepsis and study the antimicrobial susceptibility profile and its mechanism of resistance pattern. Materials and Methods: The observational study of 206 positive blood culture was carried out in the Department of microbiology, tertiary Care Hospital during the period from December 2015 to November 2016 and processed by standard conventional method. Antibiotic susceptibility pattern of isolates was studied by Kirby Bauer Disc diffusion technique. Observations & Results: Total 1281 samples were received during the study period of which 206 (16.08%) samples were found to be positive. Bacteria isolated include CONS (15.04%), S.aureus (14.08%), Klebsiella pneumonia (19.90%), Acinetobacter spp (10.19%), Escherichia coli (8.74%), Pseudomonas spp (7.77%), Salmonella typhi (1.46%) and Salmonella paratyphi A (0.49%). Majority of organisms Isolated were resistant to commonly used antibiotics. Imipenem showed 83% and colistin 94.69% sensitivity for gram negative organisms. Methicillin resistance was found in 2.91% Staphylococcus aureus Isolated. The Gram positive bacteria showed high resistance to Penicillin G (75%) but they were highly susceptible to Azithromycin (70%), Levofloxacin (80%) Linezolid (100%) and Vancomycin (98%) Conclusions: Klebsiella pneumoniae was the most predominant etiological agent of septicemia. Every hospital should monitor its antibiotic sensitivity pattern against the common isolates that can serve as a basis for empirical therapy in emergency conditions. Abstract

The impact of mining activities on spider (Araneae) diversity and assemblages was studied in two abandoned mine sites in Sardinia (Italy), where mining activities started in the middle of the nineteenth century and closed down over a century later. Spider community composition was analysed in dumps, undisturbed forests and scrublands which represent the natural chronosequence following the abandonment of the mining district. The identification of spiders was performed at the lowest taxonomic level possible and differences in abundance, species richness (SR), and functional diversity (FD) were analysed. Nonmetric multidimensional scaling (NMDS) and a permutated multivariate analysis of variance (PERMANOVA) routine were conducted to evaluate the relationships of spider assemblages with land cover, and the indicator species analysis was performed to identify the typifying species. During the entire sampling period, 2312 spiders were captured, and approximately 80% of the total were identified at the species level (79 spider species belonging to 28 different families). No differences in abundance and SR were found, whereas FD, which showed the highest values in forests, significantly differed among sites. Abstract

The aim of the study is evaluation of anti-hypertensive drug prescription pattern according to Joint National Committee-8, assessment of medication adherence and drug related problem in type-2 diabetes mellitus patients. The objectives of the study is to evaluate the prescribing pattern of anti-hypertensive drugs in type 2 diabetic patients according to JNC-8 guidelines, to assess medication adherence to treatment, to assess major drug-drug interactions, and to assess drug inappropriateness. A prospective and hospital based study was conducted on in-patients admitted in a tertiary care teaching hospital. Data regarding patient’s demographic details, diagnosis, complete prescription, and any other information will be collected in a predesigned pro forma. The study reveals that males (58%) were more likely to have reported with hypertension and diabetes. Among all medications the major class of drugs prescribed were calcium channel blockers (44.48%) and diuretics (32.65%). The most common anti-hypertensive drugs prescribed were amlodipine (43.67%), Lasix (27.75%). Out of 339 drug-drug interactions, 53(4.61%) were major, 906 (78.85%) were moderate and 190(16.53%) were minor. Morisky medication adherence scale analysis showed fair adherence to the treatment. The present study concluded that the prescribing pattern of antihypertensive drugs showed less adherence to current JNC-8 guideline. However, further studies are needed to identify the rationale in the prescribing pattern. The study also tried to depict the drug related problems in the patients. Hence, the study reveals a lot of potential for pharmaceutical care services and the relevance of having clinical pharmacist on the wards to access patients drug- related needs.

Data were collected immediately prior to and following the 4-week REAC-NPPO treatment cycle.Results: This study demonstrates a significant reduction in scores measuring subjective perceptions of stress for subjects treated with one cycle of REAC-NPPO. This study aimed to investigate the efficacy of noninvasive brain stimulation using a radioelectric asymmetric conveyor (REAC) for agoraphobia.Patients and methods: Twenty-three patients (3 males and 20 females) suffering from agoraphobia and without a history of panic disorder were evaluated by a psychiatrist using the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision, and the Agoraphobia Scale (AS). The patients were subjected to two 18-session cycles of noninvasive brain stimulation with the REAC, according to an established therapeutic protocol called neuro-psycho-physical optimization.Results: Analyzing the anxiety and avoidance parameters of the AS after the first and second cycles of REAC treatment revealed variation in levels of response to treatment, including weak (AS item 7), moderate (AS items 10 and 13), and good responses (AS items 1–6, 8, 9, 11, 12, and 14–20).Conclusion: These results highlight the potential of the REAC to treat complex clinical situations such as agoraphobia, which is typically resistant to pharmacologic treatments. Abstract Retinoic acid (RA) triggers physiological processes by activating heterodimeric transcription factors (TFs) comprising retinoic acid receptor (RARα, β, γ) and retinoid X receptor (RXRα, β, γ). How a single signal induces highly complex temporally controlled networks that ultimately orchestrate physiological processes is unclear. Using an RA‐inducible differentiation model, we defined the temporal changes in the genome‐wide binding patterns of RARγ and RXRα and correlated them with transcription regulation. Unexpectedly, both receptors displayed a highly dynamic binding, with different RXRα heterodimers targeting identical loci. Comparison of RARγ and RXRα co‐binding at RA‐regulated genes identified putative RXRα–RARγ target genes that were validated with subtype‐selective agonists. Gene‐regulatory decisions during differentiation were inferred from TF‐target gene information and temporal gene expression. This analysis revealed six distinct co‐expression paths of which RXRα–RARγ is associated with transcription activation, while Sox2 and Egr1 were predicted to regulate repression. Finally, RXRα–RARγ regulatory networks were reconstructed through integration of functional co‐citations. Our analysis provides a dynamic view of RA signalling during cell differentiation, reveals RAR heterodimer dynamics and promiscuity, and predicts decisions that diversify the RA signal into distinct gene‐regulatory programs.

When the symptoms of pain and physical problems do not respond to various therapeutic approaches such as medication, physiotherapy, and psychotherapy, they are often called medically unexplained symptoms. As such, these symptoms are reported to be a response to stressful situations or emotional states, often unknown to patients themselves. To explore the effectiveness of noninvasive radioelectric brain stimulation in the amelioration of symptoms of pain and physical problems, we administered a neuropsychophysical optimization protocol using a REAC device.Methods: The PSM, a self-administered questionnaire, was used to measure psychological stress and pain and physical problems in a group of 888 subjects. Data were collected immediately prior to and following a 4-week REAC treatment cycle.Results: There was a significant reduction in scores measuring subjective perceptions of stress for subjects treated with one cycle of neuropsychophysical optimization REAC-BS. At the end of the study, the number of treated subjects reporting symptoms of stress-related pain and physical problems on the PSM test was significantly reduced, whereas there was no difference in placebo-treated subjects.Conclusion: One cycle of neuropsychophysical optimization REAC-BS appears to reduce subjective perceptions of stress as measured by the PSM, particularly on the pain and physical problems cluster.Keywords: stress disorders, adaptation disorders, medically unexplained symptoms

GAD-MD can be a chronic and disabling condition, and patients suffering from this disorder often respond poorly to psychopharmacological treatment and experience side effects with medication. Therefore, there is a high demand for effective nonpharmacological therapy for GAD-MD patients. The current study explores the use of a radioelectric asymmetric conveyer (REAC) device in the treatment of GAD-MD.Methods: Participants were 24 patients diagnosed with GAD-MD being treated at a public psychiatric center. All patients were dissatisfied with their current pharmacological treatment. Patients were evaluated using the 21-item Hamilton Depression (HAM-D) rating scale and the Symptom Check List-90-Revised (SCL-90R) before and after REAC brain stimulation treatment cycles.Results: After REAC brain stimulation treatment, all patients experienced a significant reduction in anxiety and depression. These results were confirmed by physician examination, HAM-D scores, and SCL-90R total scores.Conclusion: These results indicate a role for REAC brain stimulation in the management of psychiatric conditions, specifically, GAD-MD comorbidity. REAC treatments are synergistic to drug therapy and appear to be helpful in reducing the side effects of medication. Patients and methods: Two subjects, a 54-year-old male with a tear bruise on the upper third of the leg and a 19-year-old female with a stab wound to the hand, were treated with 12 REAC-TO treatment sessions. Results: In both patients, the wounds showed shorter healing time compared with the time usually required for similar wounds, and good repair quality. This study analyzed the medical records of two similar groups of patients with coxarthrosis (n = 15 in each) who were treated in two different clinics for rehabilitation therapy.Methods: Patients in Group A were treated with a course of traditional physiotherapy, including sessions of local hyperthermia. Group B patients were treated with only a course of radioelectric asymmetrical brain stimulation (REAC) to improve their motor behavior.Results: Group A showed a significant decrease in symptoms of pain and stiffness, and an insignificant improvement in range of motion and muscle bulk. A single patient in this group developed worsened symptoms, and pain did not resolve completely in any patient. The patients in Group B had significantly decreased levels of pain and stiffness, and a significant improvement in range of motion and muscle bulk. No patients worsened in Group B, and the pain resolved completely in one patient.Conclusion: Both treatments were shown to be tolerable and safe. Patients who underwent REAC treatment appeared to have slightly better outcomes, with an appreciable improvement in both their physical and mental states. The present study explores the use of radio electric asymmetric brain stimulation (REAC) in patients who have had a poor response to pharmacological treatment.Patients and methods: Eight patients (five females and three males; mean [±standard deviation] age at study baseline: 69.9 ± 3.0 years) diagnosed with AD according to the DSM-IV-TR criteria (mean onset age of AD: 65.4 ± 3.5 years) were cognitively and psychometrically assessed with the Mini-Mental State Examination (MMSE), the Activity of Daily Living (ADL), the Instrumental Activity of Daily Living (IADL), and the Neuropsychiatric Inventory (NPI), prior to and after each of 2 REAC treatment cycles.Results: Scores on the MMSE and all subscales of the NPI (frequency, severity, and distress), the ADL, and the IADL were significantly improved following the initial REAC treatment. There was further significant improvement in all measurements (with a tendency for improvement in the IADL) after the second REAC treatment cycle.Conclusion: The improvement of cognitive and behavioral/psychiatric functioning following REAC treatment suggests that this innovative approach may be an effective, safe, and tolerable alternative to pharmacological treatment of AD patients, especially in the area of BPSD. Several therapeutic approaches have been employed to treat stress and anxiety disorders, ranging from typical psychopharmacological strategies to novel physical protocols, such as transcranial magnetic stimulation and radioelectric asymmetric conveyor (REAC) stimulation. The purpose of the present study was to test the efficacy of REAC brain stimulation in atypical deglutition.Methods: The position of the lingual apex (Payne method), pattern of free deglutition, and subjective and objective impression of deglutition were evaluated in 128 outpatients suffering from atypical deglutition. Deglutition testing consisted of an operator holding down the lower lip, hence counteracting the strength exerted by the orbicularis muscle. All subjects were treated using two REAC brain stimulation protocols. Patients were assessed before treatment, immediately after treatment, and three months following the last cycle of REAC therapy.Results: REAC stimulation led to an improvement in positioning of the lingual apex and a significant decrease of muscle involvement in all patients immediately after REAC treatment, and the improvement was maintained at three-month follow-up.Conclusion: In the present study, the REAC therapeutic protocols led to normalization in lingual apex positioning and significant improvement in swallowing in all participants suffering from atypical deglutition.Keywords: atypical deglutition, stress, anxiety, radioelectric asymmetric brain stimulation

The clinical complexity of these illnesses due to the coexistence of depressive and excitative phases is correlated with the global difficulty of adequate treatment; consequently, the prognosis is not optimal. For this reason, in recent years, novel nonpharmacologic physical approaches have been tested for bipolar disorders, with encouraging results. The aim of this study was to evaluate the long-term effectiveness of a radioelectric asymmetric brain stimulation device associated with lithium (REAC-lithium) versus previous treatments in subjects with bipolar disorder I or II, evaluated as the number of recurrences compared with the period of illness preceding treatment with REAC-lithium.Methods: The charts of 56 bipolar patients attending our institute were retrospectively evaluated. Treatment with REAC-lithium was administered following the standard Rinaldi-Fontani Institute protocol. Add-on treatments were allowed in the event of manic or depressive recurrence. Eight patients (Group 1) were followed for 30.2 ± 3.0 months, 14 patients (Group 2) were followed for 25.3 ± 3.3 months, 25 patients (Group 3) were followed for 20.3 ± 1.6 months, and nine patients (Group 4) were followed for 16.2 ± 0.5 months.Results: After REAC-lithium treatment, the number of manic and depressive episodes in Group 1 decreased from 2.1 ± 0.6 and 3.0 ± 0.7 to 0.12 ± 0.0 and 0.8 ± 0.4, respectively. In Group 2, the number of manic and depressive episodes decreased from 2.4 ± 0.6 and 3.9 ± 0.7 to 0.14 ± 0.2 and 0.0 ± 0.0, respectively. In Group 3, the number of manic and depressive episodes decreased from 2.6 ± 0.8 and 3.6 ± 0.9 to 0.04 ± 0.0 and 0.0 ± 0.0, respectively. In Group 4, the number of manic and depressive episodes decreased from 2.6 ± 1.1 and 3.7 ± 1.0 to 0.1 ± 0.0 and 0.0 ± 0.0, respectively. 393. Evaluation of mandibular morphology in different facial types

Abstract

The purpose of this study was to evaluate mandibular morphology in different facial types using various parameters. This study was conducted on lateral cephalograms of a total of 110 subjects, which included 55 males and 55 females between the age of 18-25 years having a mean of 22.3 years for males and 21.5 years for females. The sample was divided into normodivergent, hypodivergent, and hyperdivergent subgroups based on Jarabak′s ratio. Symphysis height, depth, ratio (height/depth) and angle, antegonial notch depth, ramal height and width, mandibular depth, upper, lower, and total gonial angle, and mandibular arc angle were analyzed statistically and graphically. It was found that the mandible with the vertical growth pattern was associated with a symphysis with large height, small depth, large ratio, small angle, decreased ramus height and width, smaller mandibular depth, increased gonial angle, and decreased mandibular arc angle in contrast to mandible with a horizontal growth pattern. Sexual dichotomy was found with mean symphysis height and depth in the female sample being smaller than in the male sample, but symphysis ratio was larger in the female sample; males having greater ramus height and width, mandibular depth than females. The mandible seemed to have retained its infantile characteristics with all its processes underdeveloped in hyperdivergent group.

Pendahuluan:  ASI merupakan hadiah pertama untuk bayi baru lahir dikehidupannya. Masalah yang sering dihadapi ibu post partum adalah koostrum yang belum keluar sehingga perlu dilakukan upaya non farmakologis yaitu akupresure dan pijat oksitosin. Tujuan penelitian: mengetahui efektvfitas akupresure dan pijat oksitosin  efektif terhadap waktu pengeluaran kolostrum pada ibu postpartum. Metodologi : Quasi experimental design dengan rancangan penelitian Post-test Only Design, populasi adalah seluruh ibu postpartum hari I dan II yang melahirkan di RSUD Ratu Aji Putri Botung Penajam Paser Utara. Instrumen penelitian adalah SOP dan  lembar observasi. Teknik analisa data univariat dan analisa bivariat menggunakan uji independent t test.  Hasil : Tidak ada perbedaan efektifitas waktu pengeluaran kolostrum antara kelompok yang dilakukan akupresure dan kelompok yang dilakukan pijat oksitosin pada ibu post partum di RSUD Ratu Aji Putri Botung dengan nilai p value 0,201 dimana selisih waktu pengeluaran kolostrum 0,575 jam. Kesimpulan : akupresure dan pijat oksitosin dapat mempercepat waktu pengeluaran kolostrum pada ibu post partum. Kata Kunci : Akupresure, Pijat Oksitosin, Waktu Pengeluaran Kolostrum

Abstract

Background: For women with early-stage oestrogen receptor (ER)-positive breast cancer, adjuvant tamoxifen reduces 15-year breast cancer mortality by a third. Aromatase inhibitors are more effective than tamoxifen in postmenopausal women but are ineffective in premenopausal women when used without ovarian suppression. We aimed to investigate whether premenopausal women treated with ovarian suppression benefit from aromatase inhibitors. Methods: We did a meta-analysis of individual patient data from randomised trials comparing aromatase inhibitors (anastrozole, exemestane, or letrozole) versus tamoxifen for 3 or 5 years in premenopausal women with ER-positive breast cancer receiving ovarian suppression (goserelin or triptorelin) or ablation. We collected data on baseline characteristics, dates and sites of any breast cancer recurrence or second primary cancer, and dates and causes of death. Primary outcomes were breast cancer recurrence (distant, locoregional, or contralateral), breast cancer mortality, death without recurrence, and all-cause mortality. As distant recurrence invariably results in death from breast cancer several years after the occurrence, whereas locoregional recurrence and new contralateral breast cancer are not usually fatal, the distant recurrence analysis is shown separately. Standard intention-to-treat log-rank analyses estimated first-event rate ratios (RR) and their confidence intervals (CIs). Findings: We obtained data from all four identified trials (ABCSG XII, SOFT, TEXT, and HOBOE trials), which included 7030 women with ER-positive tumours enrolled between June 17, 1999, and Aug 4, 2015. Median follow-up was 8·0 years (IQR 6·1–9·3). The rate of breast cancer recurrence was lower for women allocated to an aromatase inhibitor than for women assigned to tamoxifen (RR 0·79, 95% CI 0·69–0·90, p=0·0005). The main benefit was seen in years 0–4 (RR 0·68, 99% CI 0·55–0·85; p<0·0001), the period when treatments differed, with a 3·2% (95% CI 1·
