Many groups currently use two methods for the separation of motile spermatozoa, swim-up (S-up) and centrifugation on discontinuous Percoll gradient (PGC), and comparison of results indicates that PGC is superior. In this study we have attempted to identify the factors explaining this difference. This laboratory has long-standing expertise in seminology, thus the parameters of sperm morphology were the obvious first choice for detailed study. First, the respective effects of S-up and PGC on sperm morphology were analysed in different types of ejaculates: 62 semen samples with normal parameters and 41 with poor parameters. Both separation techniques resulted in improved morphology in the final preparation but only the increase of morphologically normal spermatozoa in the final Percoll suspension was significant. Second, application of these techniques in our in-vitro fertilization (IVF) programme revealed that, together with the improvement of sperm morphology, a higher pregnancy rate was obtained after PGC. The ongoing pregnancy rates per oocyte retrieval were 21.1% for the S-up technique and 33.3% for the PGC technique. These data show that spermatozoa selected by PGC present an improved morphology which we believe to be linked to improvement of the quality of the in-vitro fertilized embryos and ultimately the percentage of successful IVF results.

Objective: Borderline tumor of the ovary is commonly described in reproductive age women. Therefore, fertility preserving treatments should be considered in these patients. The most of patients treated conservatively is obtained spontaneous pregnancy. In case of persistent infertility, ovulation induction and in vitro fertilization procedures could be proposed. Case report: We present case of 24 years old women with affected stage 3c serous borderline ovarian tumor. She underwent conservative debunking surgery without surgery followed by adjuvant chemotherapy. IVF procedure was planned because of women did not obtain pregnancy after 2 years postsurgery. Now, she have pregnancy of 24 gestational age and women have not problem of pregnancy. Conclusion: Conservative treatment remains a therapeutic option in patient with borderline ovarian tumor. After the treatment, pregnancy could be obtained by spontaneous or in vitro fertilization procedure. But careful follow up is essential. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this study was to compare the sizes of the placenta and umbilical cord in women with natural pregnancy versus those undergoing in vitro fertilization (IVF).Methods: Overall, 1610 cases of uncomplicated single pregnancies with vaginal delivery at ≥ 37 weeks of gestation were included in this study. The patients were divided into two groups: natural pregnancy group (n = 1453) and IVF pregnancy not including intracytoplasmic sperm injection (ICSI) treatment (n = 157). The groups were compared in terms of gestational week, maternal age, parity, maternal weight gain, prepregnancy maternal BMI, infant weight at birth, infant head circumference, placental weight, cross section of the placenta, cross section of the umbilical cord, insertion site of the umbilical cord, and umbilical cord length. Stepwise selection and multivariate logistic regression were used for statistical analysis to correct the result as an independent factor.Results: There was no difference in the size of the placenta and umbilical cord between women with natural pregnancy and with IVF, but the incidence of velamentous insertion of the cord was significantly increased in women with IVF pregnancy (adjusted odd ratio [AOR] 1.72, 95% confidence interval [CI] 1.08-2.72, p = 0.026).Conclusions: Although there is no difference in placental weight and cord size, velamentous insertion of the umbilical cord increases in IVF pregnancy and needs careful observation during the delivery process. Introduction: This retrospective study aimed to evaluate the association between elevated serum estradiol (E 2) levels on the human chorionic gonadotrophin (hCG) administration day and in vitro fertilization (IVF) pregnancy and birth outcomes in the long GnRH-agonist protocol.Methods: This study analyzed the data of 3393 infertile women who underwent initial fresh IVF. The patients were categorized into high and low E 2 groups based on their serum E 2 levels on the hCG day. Pregnancy and birth outcomes were compared.Results: The implantation rate, clinical pregnancy rate, and live birth rate were all significantly higher in the high E 2 group than in the low E 2 group (p < 0.05). The good-quality embryo rate and abortion rate did not significantly differ between the two groups. There were no significant differences in the mode of delivery, gestational age, birth weight, and fetal gender between the two groups. Furthermore, there were no differences in the risk of preterm birth, low birth weight, and fetal malformation between the two groups in 860 single live births. Subgroup analysis of singleton pregnancies in the high E 2 (E 2 ≥ 3757 pg/mL) group revealed a significant increase in abortion rate in the age group of ≥37 years.Conclusions: Elevated serum E 2 levels associated with controlled ovarian stimulation did not increase the risks of preterm birth, low birth weight, and fetal malformation. High E 2 on the hCG day had no detrimental effect on the implantation rate, clinical pregnancy rate, and live birth rate. [ABSTRACT FROM AUTHOR]

Heterotopic cornual pregnancy refers to the co-existence of intrauterine and extrauterine cornual pregnancies. This is rare, but the incidence increases in pregnancies associated with in vitro fertilization and embryo transfer (IVF-ET). A 36 year old gravida 2 para 0 woman with a history of bilateral salphingectomy underwent IVF-ET and was diagnosed with a heterotopic cornual pregnancy. She underwent diagnostic laparoscopy and wedge resection of the right cornual ectopic pregnancy. The intrauterine pregnancy progressed uneventfully and was delivered via caesarean section at 36 + 6 weeks. This case report illustrates the importance of early diagnosis of the condition, and how meticulous surgical technique is effective in removing the cornual ectopic pregnancy while preserving the intrauterine pregnancy.
