Background: Nasogastric tubes are frequently used in the clinical setting for the management of patients who require decompression of the gastrointestinal tract, diagnosis and assessment, nutritional support and medication administration. A variety of bedside methods have been used either individually or in combination to assess NG tube placement., Objectives: The objective of this review is to present the best available evidence to support decisions pertaining to methods for determining the correct nasogastric tube placement after insertion., Search Strategy: A literature search was performed using the following databases MEDLINE (1950 to Sept 2008), CINAHL (1982 to Sept 2008), EMBASE (1980 to Sept 2008) and All EBM Reviews (up to 3rd Quarter 2008). Chinese databases searched included WanFang Data (1998 to Sept 2008), China Journal Net (1994 to Sept 2008), Chinese Medical Current Contents (1994 to Sept 2008), Index to Chinese Periodical Literature (1970 to Sept 2008), and the Chinese Biomedical Literature Database (1980 to Sept 2008). In addition the reference lists of relevant trials and conference proceedings were also scrutinised. Investigators were contacted to elicit further information., Selection Criteria: Clinical trials, prospective studies, observational studies, case reports, and evaluation studies assessing the diagnostic accuracy of methods for determining the correct placement of nasogastric tube after insertion were considered for inclusion in the meta-analysis., Data Collection and Analysis: Eligibility of the trials for inclusion in the review, details of eligible trials and the methodological quality of the trials were assessed independently by two reviewers. Since not all trials reported true positive, true negative, false positive and false negative rates, these rates were calculated based on the data in the published papers. The two by two tables were reconstructed for each study, and sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio and negative likelihood ratio were calculated for each study. Diagnostic studies presenting insufficient data for construction of a two by two table were excluded from statistical analysis, but were included in the narrative part of the review., Main Results: Twenty-six trials were included in this meta-analysis: three trials investigated the effectiveness of colorimetric capnometry in detecting inadvertent airway intubation; three trials reported the use of capnography to verify tube placement in airway and to differentiate between respiratory and GI tube placement; one trial compared the use of a portable capnograph with a disposable colorimetric CO2 indicator in detecting inadvertent respiratory intubation. There were nine trials made use of a variety of cut-off points based on the biochemical measurement parameters of feeding tube aspirates (pH, bilirubin, pepsin and trypsin) in differentiating gastric from respiratory and intestinal placement of feeding tubes; one trial evaluated the accuracy of six pH test-strips. There were nine trials investigated the effectiveness of using various methods to differentiate between respiratory and GI feeding tube placement which included the use of: spring gauge pressure manometer, magnetic detection, sonography, auscultation and visual inspection of aspirates., Authors' Conclusion: The limited evidence obtained from the meta-analysis does not provide a substantial base for the development of practice guidelines. However, based on the trials undertaken to date, there is strong evidence to support the use of capnography or colorimetric capnometry for identification of feeding tube placement in mechanically ventilated patients. Until stronger evidence becomes available practice related to the diagnostic tests used within the clinical setting will continue to be dictated by local preferences and cost factors.

Aims: The purpose of this study was to describe the demographic and employment characteristics of Australian practice nurses and explore the relationship between these characteristics and the nurses' role., Background: Nursing in general practice is an integral component of primary care and chronic disease management in the United Kingdom and New Zealand, but in Australia it is an emerging specialty and there is limited data on the workforce and role., Design: National postal survey embedded in a sequential mixed method design., Methods: 284 practice nurses completed a postal survey during 2003-2004. Descriptive statistics and factor analysis were utilized to analyse the data., Results: Most participants were female (99%), Registered Nurses (86%), employed part-time in a group practice, with a mean age of 45.8 years, and had a hospital nursing certificate as their highest qualification (63%). The tasks currently undertaken by participants and those requiring further education were inversely related (R2 = -0.779). Conversely, tasks perceived to be appropriate for a practice nurse and those currently undertaken by participants were positively related (R2 = 0.8996). There was a mismatch between the number of participants who perceived that a particular task was appropriate and those who undertook the task. This disparity was not completely explained by demographic or employment characteristics. Extrinsic factors such as legal and funding issues, lack of space and general practitioner attitudes were identified as barriers to role expansion., Conclusion: Practice nurses are a clinically experienced workforce whose skills are not optimally harnessed to improve the care of the growing number of people with chronic and complex conditions. Relevance to clinical practice. Study data reveal a need to overcome the funding, regulatory and interprofessional barriers that currently constrain the practice nurse role. Expansion of the practice nurse role is clearly a useful adjunct to specialist management of chronic and complex disease, particularly within the context of contemporary policy initiatives.

It is only recently that recognition of the serious and debilitating sequelae of trauma has prompted exploration of outcomes beyond survival, such as disability, health status and quality of life. This paper aims to review the literature describing outcomes following severe traumatic injury to provide clinicians with a greater understanding of the recovery trajectory following severe trauma and highlight the issues faced by those recovering from such injury. Electronic databases, published reference lists and the Internet were searched to identify relevant literature. The heterogeneous nature of published literature in this area prohibited a systematic approach to inclusion of papers in this review. Trauma survivors report significant sequelae that influence functional status, psychological wellbeing, quality of life and return to productivity following severe injury. Key themes that emerge from the review include: current trauma systems which provide inadequate support along the recovery trajectory; rehabilitation referral which is affected by geographical location and provider preferences; a long-term loss of productivity in both society and the workplace; a high incidence of psychological sequelae; a link between poor recovery and increased drug and alcohol consumption; and valued social support which can augment recovery. Future research to evaluate interventions which target the recovery needs of the severely injured patients is recommended. Particular emphasis is required to develop systematic, sustainable and cost-effective follow-up to augment the successes of existing acute trauma services in providing high quality acute resuscitation and definitive trauma management.

Although considerable research and development work has been undertaken on the role of General Practitioners (GPs) and pharmacists in medication management, monitoring and referral, there is limited research into a role in medication management for community nurses. One hundred and thirteen older people living at home and receiving community nursing care were assessed for their knowledge of, and ability to manage their medication regimen. From these data, a nurse-initiated intervention was developed that included nursing interventions and referral pathways to GPs for people who might benefit from a GP and/or pharmacist medication review. A subgroup of 24 participants with diminished knowledge of medications or ability to manage their regime (considered to be at risk of drug-related problems) who were followed up, demonstrated an increase in knowledge, some alteration in compliance aids and with no statistically significant change in medication regime complexity. This paper details an approach to medication review and intervention suitable for community nurses that includes referral pathways within the medication team.

Background: This systematic review updates a previous review published in 2001. Cleansing is a vital component of wound management; however; little attention has been give to the solutions and techniques used for cleansing purposes. The objective of this review was to assess the effectiveness of different solutions, pressures and techniques used for wound cleansing to prevent infection and promote wound healing., Search Strategy: Randomised and clinical controlled trials were identified using the Cochrane Central Register of Controlled Trials (CENTRAL). Additional searches of other databases and hand searches of journals and bibliographies was undertaken to identify further trials., Selection Criteria: All randomised and clinical controlled trials involving adults and/or children whose wounds were cleaned with commercial cleansers, normal saline, water, chlorhexidine, hydrogen peroxide or povidone-iodine were eligible for inclusion. Studies that utilised solutions for preoperative skin cleansing, compared solutions for burns or dental procedures, and those that compared dressings for patients with ulcers were excluded from this review. Outcomes included rate of healing, incidence of infections or levels of bacterial count. Selection of potential articles, assessment of methodological quality and data abstraction was conducted independently by two reviewers. Trials with similar patients, comparisons, and outcomes were pooled. The data were analysed using Cochrane Review Manager 4.2. Where pooling was inappropriate, trials are discussed in a narrative review., Results: Fourteen randomised controlled trials were included that compared various solutions for wound cleansing. The evidence indicates that there is no difference in the infection and healing rates in acute and chronic wounds cleansed with either tap water or normal saline. An irrigation pressure of 13 psi is effective for cleansing wounds and reducing infection without causing tissue trauma.There were no studies comparing common techniques for wound cleansing such as swabbing or scrubbing. Showering postoperative wounds did not demonstrate any difference in infection rates; however, it increased the morale of the patient. Whirlpool therapy was effective in reducing inflammation and pain in surgical wounds., Conclusions: These conclusions are based on the best available clinical evidence. Loss of peripheral intravenous (PIV) catheter patency is a common problem in the clinical setting. A survey of all private and public hospitals in a metropolitan city in Australia was conducted to identify practices related to maintaining patency of PIV catheters. Eighty-three hospitals were surveyed and responses were obtained from all of them. Normal saline was the most commonly used solution for flushing. The frequency for flushing ranged from 4 to 12 hours, and the volume of the irrigant used ranged from 2 to 10 mL in those hospitals where flushing is routine practice. Policy documents were available from only a few hospitals to validate the practice reported. Significant diversity in practices was documented among healthcare organizations.
