The treatment of tibial plafond fractures requires careful management of the soft tissue envelope, reconstruction of the articular surface and stable fixation with minimal additional damage. Thirty cases of AO type 43 C tibial fractures were treated by transosseous osteosynthesis (Ilizarov technique). The external fixator constructs used were Ilizarov (Transosseous osteosynthesis: theoretical and clinical aspects of the regeneration and growth of tissue, Springer, Berlin, 1992) and Sheffield (Classification AO des fractures, Springer, Berlin, 1987) circular fixator systems. All tibial plafond fractures healed. Using radiological criteria for assessment of reduction of the articular fragments and the clinical scoring system described by Teeny and Wiss, there were excellent and good restoration of articular structure in 27 cases and good clinical results in 14. This treatment method compares well with previous published series and is to be recommended for this group of difficult fractures.

The nuclear magnetic resonance (NMR) technique was used as analytical tool to determine the complete metabolic profiling of sea bass extracts: water-soluble metabolites belonging to different classes such as sugars, amino acids, dipeptides and organic acids as well as metabolites soluble in organic solvent such as lipids, sterols and fatty acids were identified. The metabolite profiling together with a suitable statistical analysis were used to discriminate between wild and cultured sea bass samples. Preliminary results show that discrimination between wild and cultured sea bass was obtained not only using fatty acid composition but also cholesterol and phosphatidylethanolamine and some water-soluble metabolites such as choline, trimethylamine oxide, glutamine, fumaric and malic acids.

A Dutch map from the eighteenth century was multi-analytically analysed making use of energy dispersive X-ray fluorescence (EDXRF), Fourier transform infrared spectroscopy (FTIR), nuclear magnetic resonance (NMR), Raman and scanning electron microscopy coupled to energy dispersive spectrometry (SEM-EDS). The cellulosic support was characterised and its state of conservation was evaluated. Besides, paramagnetic impurities were detected together with copper metallic chips. The colours present in some areas of the map were also analysed. Vermilion, carbon black and organic pigments were found. Surprisingly, in the green areas, the rare presence of the mineral moolooite (copper oxalate) was detected. A possible biological attack is discussed in order to explain the presence of such compound. Almost all of the techniques used in the analysis were portable, non-destructive and non-invasive, which is very desirable when analysing objects belonging to Cultural Heritage. The need for a multi-analytical approach using portable instrumentation is also discussed.

Two new HA derivatives bearing carborane rings were synthesized by click chemistry. The optimal conditions were assessed for the preparation of biocompatible boron carriers, potentially suitable for application in BNCT and capable of targeting the CD44 antigen. The new polymeric samples were characterized by means of NMR-spectroscopy techniques that gave degrees of 17 and 8% for HAAACB and HapACB, respectively. Both HAAACB and HApACB turned out to be nontoxic for colorectal, ovarian and bladder tumor cell lines, to disclose a specific interaction with the CD44 antigen as the native hyaluronan moiety, and to deliver boron-atom concentrations largely sufficient for BNCT therapy when accumulated in cancer cells.

pi-Conjugated low molecular weight polymers characterized by regio- and stereoregular alternation of phenylene and ( E)-1-en-3-yne moieties have been synthesized by polyaddition of 1,4-diethynylbenzene or of 2,5-diethynyl-1,4-alkoxybenzene monomers, employing the commercially available di-micro-chlorobis[( p-cymene)chlororuthenium(II)] complex as the metal catalyst source, under homogeneous, atom-economical, amine- and phosphine-free conditions. Bulk materials of poly( p-phenyleneethynylenevinylene) derivatives are obtained with yields larger than 80%, from which polymers readily soluble in chlorinated solvents and in tetrahydrofuran are extracted in 60-75% yields. The polymers with average degrees of polymerization in the range n AV = 4-8 display optical properties in solution similar to those of the higher molecular weights analogues.

A complete multianalytical study of a hand-coloured map from the seventeenth century is presented. The pigments atacamite, massicot, minium, gypsum, carbon black and vermilion were determined by means of XRF and Raman spectroscopy. The state of conservation of the cellulosic support was monitored by means of unilateral NMR. The analysis was nondestructive and noninvasive, and thus several spectra were collected from the same areas, yielding more reliable results without damaging the artwork. The role of copper pigments in the oxidation processes observed in the cellulosic support is discussed, as well as the possible provenance of atacamite as a raw material instead of as a degradation product of malachite.

Wall paintings in the XVI century Serra Chapel in the "Chiesa di Nostra Signora del Sacro Cuore" Rome, have been studied using unilateral NMR. In order to map the distribution of moisture content in the wall painted, a large number of Hahn echo measurements, covering large areas of the wall painting were performed. Because the intensity of the Hahn echo is proportional to the amount of moisture in the area under study, the experimental data were transformed into 2D gradient colour maps which allowed an easy visualization of the moisture content of the wall. The state of conservation of the wall painting was monitored using T2 measurements specially with regards to outcropping salt.

We report a detailed analysis of deuteron NMR spectra of micellar, lamellar, cubic, and hexagonal mesophases in the aqueous non-ionic surfactant system C(12)E(6)/water. Samples are prepared with and without shear. Particular attention is paid to an interesting temperature-driven phase sequence that includes all of the above phases that are studied before and after shear parallel or perpendicular to the magnetic field direction. Surprising memory effects are found across mesophase transitions. These memory effects provide clues to the structure of the various phases.

Boron neutron capture therapy (BNCT) represents a promising approach for tumor therapy. A critical requirement for BNCT is tumor targeting, a goal that is currently addressed with the development of low and high molecular weight agents capable of interacting with receptors expressed by cancer cells. Here, we describe a new bioconjugate (HApCB) composed by n-propyl carborane linked to hyaluronan (HA) via an ester linkage for a degree of substitution of approximately 30%, leading to a water-soluble derivative. The structure and main physicochemical characteristics of the new HA derivative were determined by means of Fourier transform infrared, fluorescence, and 1H, 13C, and 10B NMR analysis and are herein reported in detail. As HA is recognized by the CD44 antigen, densely populating the surface of many tumor cells, HApCB is expected to deliver boron atoms from the locally released carborane cages directly to target cells for antitumor application in BNCT. In vitro biological experiments showed that HApCB was not toxic for a variety of human tumor cells of different histotypes, specifically interacted with CD44 as the native unconjugated HA, and underwent uptake by tumor cells, leading to accumulation of amounts of boron atoms largely exceeding those required for a successful BNCT approach. Thus, HApCB may be regarded as a promising new BNCT agent for specific targeting of cancer cells overexpressing the CD44 receptor.

Consolidation treatment of porous materials was performed by in situ and frontal polymerization of acrylic monomers inside a porous stone. To study the penetration of the polymer inside the stone and its consolidating effects we used water as a contrast agent, detecting its penetration using unilateral NMR and magnetic resonance imaging. All data obtained on differently treated stones were compared with corresponding ones obtained analyzing both untreated stones and stones simply painted with a well-known polymeric protective agent. In situ polymerization of acrylic monomers inside porous stones has been demonstrated to be an extremely powerful consolidating method, whereas thermally initiated frontal polymerization seems less efficient. In both cases the optimal choice of monomers is still open and requires further study. Our data indicate that unilateral NMR represents an inexpensive and simple technique for the non-invasive observation of the water uptake and of the effect of consolidation procedures in porous materials.

The aim of the present work was to develop a family of novel materials based on a combination of sodium alginate and acrylic polymers and to evaluate their potential in drug delivery applications. In the presence of sodium alginate, acrylic chains with acidic as well as basic moieties were polymerized to create an interpolymer complex based on electrostatic interactions that are able to modulate the release rate of low molecular weight drugs. The synthesized materials were used to prepare hydrophilic matrices for drug delivery and tested for their adhesion properties to glass, used as a model substrate for mucoadhesion.

A versatile atom transfer radical polymerization (ATRP) method for polysaccharide grafting in homogeneous mild conditions without using protecting group chemistry is presented. Water/DMF mixtures with different compositions were used as the solvent. The "grafting-from" approach was used in order to prepare suitable pullulan and dextran ATRP macroinitiators with a well controlled degree of functionalization. Methacrylate and acrylamide monomers were grafted obtaining good control over the number, molecular weight and polydispersity of the grafted chains without homopolymer formation and polysaccharide degradation. The versatility of this method allowed us to prepare comblike derivatives with a wide range of properties (amphiphilic, ionic, and thermoresponsive) by simply changing the solvent composition and the catalyst. This could make possible the synthesis of new interesting biomaterials starting from a wide range of polysaccharides.

Nitric oxide (NO) and hyaluronic acid (HA), two species widely different in terms of molecular complexity and biological competence, are both known to play an important role in the wound healing process. To combine the properties of HA and NO, we synthesized new NO-donors based on hyaluronic acid derivatives exhibiting a controlled NO-release under physiological conditions (in vitro tests). Since two molecules of NO can form a covalent bond with secondary amines to yield structures, named NONO-ates, able to release NO in solution, we used spermidine bound to HA as the NO-linker. The HA-spermidine derivative was obtained by controlled HA amidation in aqueous media, activating the biopolymer carboxylate groups with a water soluble carbodiimide. The resulting derivative, soluble in water, was fully characterized by high field 1H and 13C NMR spectroscopy. The amount of grafting of spermidine on HA was determined by integration of suitable 1H NMR signals. In addition, cross-linked derivatives of HA were synthesized by the Ugi's four-component reaction using formaldehyde, cyclohexylisocyanide, and spermidine. The HA-spermidine networks were characterized by 13C CP-MAS NMR spectroscopy. The degree of cross-linking of the networks was also determined. Finally, the release of NO from the swollen hydrogels freshly saturated with NO, in contact with aqueous media, was monitored by means of UV spectrophotometric measurements.

Galactose depleted locust bean gum was selectively oxidized in C(6) position and epimerized with mannuronan C(5)-epimerases to obtain the corresponding artificial uronanes. These new pseudo-alginates were characterized by NMR spectroscopy and circular dichroism (CD). Specifically, 1D and 2D NMR techniques allowed the degree of epimerization, the distribution of mannuronic and guluronic acid residues in the polysaccharidic chain, and the average G block length to be determined. In addition, NMR diffusion experiments showed that the epimerization reaction did not significantly degrade the polysaccharidic chains. Circular dichroism was used to investigate the kinetics of the epimerization reaction and to evidence the specific interaction between the epimerized locust bean samples with Ca(II) ions in dilute solution. All of the samples considered in this study form wall to wall gels in concentrated polymer solutions.

The aim of this study is to clarify the structure of an iron-rich clay and the structural changes involved in the firing process as a preliminary step to get information on ancient ceramic technology. To this purpose, illite-rich clay samples fired at different temperatures were characterized using a multitechnique approach, i.e., by electron paramagnetic resonance, scanning electron microscopy with electron dispersion X-ray spectrometry, X-ray powder diffraction, magic angle spinning and multiple quantum magic angle spinning NMR. During firing, four main reaction processes occur: dehydration, dehydroxylation, structural breakdown, and recrystallization. When the results are combined from all characterization methods, the following conclusions could be obtained. Interlayer H2O is located close to aluminum in octahedral sites and is driven off at temperatures lower than 600 degrees C. Between 600 and 700 degrees C dehydroxylation occurs whereas, between 800 and 900 degrees C, the aluminum in octahedral sites disappears, due to the breakdown of the illite structure, and all iron present is oxidized to Fe3+. In samples fired at 1000 and 1100 degrees C iron clustering was observed as well as large single crystals of iron with the occurrence of ferro- or ferrimagnetic effects. Below 900 degrees C the aluminum in octahedral sites presents a continuous distribution of chemical shift, suggesting the presence of slightly distorted sites. Finally, over the whole temperature range, the presence of at least two tetrahedral aluminum sites was revealed, characterized by different values of the quadrupolar coupling constant.

Unilateral NMR has been used to monitor the state of conservation of frescoes in the Vasari's house in Florence. The causes of deterioration of ancient frescoes are varied, which result in the detachment and crumbling of the painted film from the supporting plaster and in the outcropping of salts. Unilateral measurements of Hahn echo performed on such frescoes have allowed a perfect identification of the detachment of the painted film from the plaster. The presence of soluble salts on the pictorial film affects the spin-spin relaxation times, T(2). It is then possible using this technique, to characterize the effect of chemical treatments, of cleansing and consolidation procedures using the distribution of T(2) spin-spin relaxation times.

High quality paper samples have been oxidized with a specific oxidant to reproduce one of the possible causes of the aging of paper. All samples have been characterized by 13C CP-MAS NMR spectroscopy. The artificial aging of paper has been monitored using a standard NMR relaxometer and the results have been compared with the corresponding data obtained using an unilateral NMR relaxometer. Experimental values obtained with both techniques are in agreement, demonstrating that unilateral NMR relaxometric measurements constitute a suitable non-invasive method for assessing the degradation process of cellulose-based materials. The sensitivity of the non-invasive NMR method allows the detection of degradation even at a very early stage. Effects due to the sample volume and to the penetration depth have been investigated.

A high-field NMR study of the polysaccharidic fraction extracted from Lentinula edodes mycelium grown on olive mill waste waters is reported. Diffusion-ordered NMR spectroscopy (DOSY) was applied to the polysaccharidic fraction. The results showed the presence of two polysaccharides of different sizes, whose structures were revealed using one- and two-dimensional NMR techniques. These two polysaccharides were identified as xylan and lentinan.

We describe 3 patients who developed a severe palsy of the intrinsic ulnar supplied hand muscles after bicycle riding. Clinically and electrophysiologically all showed an isolated lesion of the deep terminal motor branch of the ulnar nerve leaving the hypothenar muscle and the distal sensory branch intact. This type of lesion at the canal of Guyon is quite unusual, caused in the majority of cases by chronic external pressure over the ulnar palm. In earlier reports describing this lesion in bicycle riders, most patients experienced this lesion after a long distance ride. Due to the change of riding position and shape of handlebars (horn handle) in recent years, however, even a single bicycle ride may be sufficient to cause a lesion of this ulnar branch. Especially in downhill riding, a large part of the body weight is supported by the hand on the corner of the handlebar leading to a high load at Guyon's canal. As no sensory fibres are affected, the patients are not aware of the ongoing nerve compression until a severe lesion develops. Individual adaptation of the handlebar and riding position seems to be crucial for prevention of this type of nerve lesion.

Ten commercial composts were evaluated on the basis of chemical, physical and biological characteristics and compared with a compost produced by a safe matrix in controlled conditions. FT-IR spectra, (13)C CP-MAS NMR spectra and molecular weights were also assayed on humic acids extracted from the composts. The results show that the origin of the starting materials affects the quality of the end products especially for their heavy metal content and genotoxic effect. The presence of a high percentage of low molecular weight fractions generally recorded in the humic acids from commercial composts could be due to uncorrected composting process. FT-IR and (13)C CP-MAS NMR spectra agree with functional group assignments, characterising the level of humification. The results show that commercial composts can display harmful properties probably due to the origin of the starting material, therefore a regulation about both compost production and characterisation should be suitable.

Magnetic multipoles of rank higher than one become active in spin systems with I > 1/2 and their contribution to relaxation depends on dynamics. The appearance of multipole terms complicates the relaxation description and supports the multiexponential behavior of relaxation. In this paper the effects of high-rank multipoles on lineshape and longitudinal relaxation of I = 3 spin systems are presented. Results obtained from both numerical simulation and experimental data show that longitudinal and transverse relaxation are strongly influenced by these multipole terms, especially at lower temperature where, due to lower molecular mobility, the extreme narrowing condition is not fulfilled., (Copyright 2001 Academic Press.)

Original data are provided demonstrating that the title condensations are simple and versatile methods for the synthesis of hydrogels based on a variety of carboxylated polysaccharides. In this work, the biopolymers considered are sodium hyaluronate and sodium alginate. Nonnatural carboxylated polysaccharides were commercial (carboxymethyl)cellulose or were obtained by carboxymethylation or selective oxidation of primary alcohol groups of scleroglucan and dextran. Hydrogels prepared via the Passerini reaction were transparent, alkali labile materials whereas the transparency of the Ugi gels depended on the polysaccharide, the cross-linker, and the degree of cross-linking. The Ugi gels were stable for several months at a pH ranging from 1.3 to 11 and up to temperatures over 90 degrees C. The structure of the networks was studied by means of 13C CP-MAS and 15N CP-MAS NMR spectroscopy. A quantitative NMR analysis and elemental analysis of the dry gels allowed us to estimate the efficiency of the reactions, i.e., the actual degree of cross-linking, which appeared to be about 80% of theoretical. The influence of added salt and pH on the swelling of several Ugi gels with different degrees of cross-linking was studied in a qualitative manner.

The authors describe a case of iliopsoas muscle injury complicated by nerve entrapment, a rare occurrence both in the literature and in clinical experience. The diagnostic procedure excluded the many differential diagnoses possible. The authors prescribed conservative treatment which achieved nearly full recovery of the athlete approximately 6 months after injury.

From September 85 to September 88, 130 patients were treated by our own technique for chronic laxity of the knee. All the patients were examined at regular intervals (6, 12 and 24 months) both clinically and by arthroscopy. Neuromorphological studies, an experimental approach with the "Pivot" and evaluation of the results, show that surgery is indicated, especially in young patients, before intraarticular lesions appear.

The ultimate outcome of fractures of the scaphoid bone is determined by the choice of initial treatment. The special characteristics of the blood supply to the bone, which often increase the risk of non-union, have prompted us to propose the indications for primary surgical treatment based on the radiographic appearances. The authors propose a classification of the radiographic characteristics of fractures of the scaphoid for determining at the outset the treatment most likely to achieve consolidation and hence a more rapid resumption of occupational and sports activities. They present the results obtained in the treatment of 101 recent fractures of the scaphoid, eighty-one of which they were able to follow up after at least one year. In cases of established pseudarthrosis of the scaphoid they propose a pedicled transplant technique as conforming better to the anatomical, morphological and functional pre-requisites of the operation. The results obtained in the treatment of twenty-nine cases of pseudarthrosis are presented. The case material relates to patients treated at the G. Pini Orthopaedic Institute of Milan and, in the case of sportsmen, at the Centre for sports injuries of the same institute.
