The historical development of vaginal reconstruction in vaginal atresia is reviewed. Two more recent indications for vaginoplasty are (i) total pelvic exenteration for carcinoma; and (ii) sex conversion in trans-sexuals. The operations available for these conditions are discussed, with particular emphasis on sex-conversion techniques. A method of vaginoplasty in inadequately treated transsexuals, using random medial thigh flaps, is presented with illustrative case reports.

Quadrilateral shaped brims made of high-density polyethylene have been used with satisfactory results on more than 120 patients who needed ischial-bearing long leg calipers. At first the fitting technique was the same as that used for thigh amputees. Subsequently it has been possible to fit most patients from a range of pre-formed brims. The properties of high-density polyethylene allow the caliper side irons to be riveted directly to the brim which, with the use of ready-made brims, allows a patient to be fitted rapidly with a comfortable and effective caliper.

The in vitro metabolic fate of beta-nicotyrine has been examined in rabbit lung and liver microsomal preparations as part of an effort to characterize the formation of potentially reactive metabolic species that may contribute to the toxic properties of tobacco products. HPLC analysis revealed the formation of an unstable metabolite which displayed HPLC-MS/MS characteristics consistent with the structure 1-methyl-5-(3-pyridyl)-3-pyrrolin-2-one. Attempted synthesis of this pyrrolinone, however, resulted in the isolation of the isomeric 1-methyl-5-(3-pyridyl)-2-pyrrolin-2-one. The HPLC, diode array UV, and mass spectral characteristics of this delta 4,5-isomer proved to be identical with those of the metabolite derived from beta-nicotyrine. Studies in D2O suggest that the 2- and 3-pyrrolinones are in equilibrium in aqueous solution. The metabolite undergoes autoxidation, possibly via radical intermediates, to yield 1-methyl-5-(3-pyridyl)-5-hydroxy-3-pyrrolin-2-one.

A new methodology for comparative bioavailability testing is described in which each drug formulation is compared with a stable isotope-labeled variant of the drug that is consumed orally in solution at the same time the tested formulation is ingested. The methodology is used to determine the comparative bioavailabilities of two commercially available brands of imipramine hydrochloride. The power of the new methodology to detect differences between drug formulations, when, in fact, such differences exist, is shown to be superior to that of conventional bioavailability tests.

A halo-body system is described which has been developed at Princess Margaret Rose Orthopaedic Hospital in Edinburgh. The system is easy to apply and consists of a standard halo, linked to malleable plates by two hollow vertical rods. These rods are connected to the ring by swivel joints which allow for the movements of flexion and extension. The malleable plates are incorporated into the plaster jacket which rests upon the iliac crests. The results of 13 patients who have had this system applied are discussed and include patients who have sustained fractures of the cervical spine or who have had bone destruction from metastatic disease or from infection. Using this type of system there have been few complications and the patients have benefitted from easy mobilisation.

During regulatory analysis of pesticides and industrial chemicals, compounds are frequently encountered which cannot be readily identified by gas chromatography alone on two or more stationary phases. Such incurred unknown analytical responses are examined by gas chromatography mass spectrometry, particularly if they have been shown to contain P, S, N or halogen via specific gas chromatography detectors. Seven case histories are presented to illustrate the analytical approach adopted to structurally elucidate such compounds at the ppm level. These examples demonstrate the potential complementary combination provided by low resolution electron impact and chemical ionization mass spectrometry in structural determinations.

A case-control study was conducted to investigate the findings of antibody to Ebola virus in the serum of a guinea pig from Tandala, Zaire. Case households, defined by the possession of one or more guinea pigs, were compared to neighboring households without guinea pigs. Seven (5.1%) of 138 samples of human sera and 36 (26%) of 138 samples of guinea pig sera had antibody to Ebola virus. There was no clustering of seropositivity among humans or guinea pigs within households, nor was there any association between the ownership of guinea pigs and seropositivity among household members. These data suggest sporadic subclinical infection of guinea pigs and humans without a dominant role for person-to-person or guinea pig-to-guinea pig transmission.

During a 5 year period from 1982 to 1986, the FDA Los Angeles District Laboratory analyzed 19,851 samples of domestic and imported food and feed commodities for pesticide residues. A single, rapid, multiresidue method was used. The resultant data have been compiled showing the commodities sampled and the identity and range of levels of pesticide residues detected, including an indication of those residue findings that did not comply with U.S. federal tolerance levels. The residue data presented should not be viewed as being representative of the U.S. food supply; rather, the results are indicative of a surveillance- and compliance-oriented sampling of various food shipments collected by the Los Angeles District.
