In-hospital cardiac arrest remains a major problem but new technologies allowing fully automatic external defibrillation are available. These technologies allow the concept of "external therapeutic monitoring" of lethal arrhythmias. Since early defibrillation improves outcome by decreasing morbidity and mortality, the use of this device should improve the outcome of in-hospital cardiac arrest victims. Furthermore, the use of these devices could allow safe monitoring and treatment of patients at risk of cardiac arrest who not necessarily must be in conventional monitoring units (Intensive or Coronary Care Units) saving costs with a more meaningful use of resources. The capability to provide early defibrillation within any patient-care areas should be considered as an obligation ("standard of care") of the modern hospital.

Background: After abnormal head-up tilt test (HUT), several trials have evaluated treatment strategies for vasovagal syncope (VVS). However, few unequivocal results have been obtained. The aim of the study was to prospectively analyze the natural history of patients with VVS who did not undergo specific treatment but received education for avoiding syncope after an abnormal HUT., Methods: From 1996, 334 consecutive patients with VVS and an abnormal HUT result were followed. All of them received education for avoiding syncope as first-line therapy., Results: During 30.4 +/- 21 months, there were no cardiac deaths. However, 101 patients (30.2%) had recurrences (1 recurrence, n = 64; > or =2, n = 37), which were not influenced by the type of response during HUT or by age. Time to first recurrence was correlated with the number of recurrences (r: -0.34, P =.0001). Mean recurrence-free time was 50.1 months (95% CI 46-54) and the cumulative probability of no recurrence was 69.8%. Receiver operator character curve analysis, demonstrated significant differences in the recurrence rate between patients with <5 or > or =5 previous episodes of syncope (25.1% vs 44%; P =.001). In addition, the mean recurrence-free time of patients with <5 episodes was significantly longer (54.1 months) than in patients with > or =5 episodes (39.6 months; P =.0005). Multivariate logistic regression identified the previous number of episodes as an independent risk marker of recurrences (odds ratio 2.34, 95% CI 1.4-3.8, P =.001). Age (at HUT performance) as a continuous variable was not a predictor of outcome., Conclusions: Survival of patients with VVS presenting after an abnormal HUT is excellent. Although the broad majority of those patients do not suffer recurrences after education, the number of previous syncopal episodes critically influences the recurrence rate.

Background and Objective: The prevalence and morbidity of the vaso-vagal syncope are well-known. With the intention of measuring the Quality of Life (QoL) of patients with vaso-vagal syncope, as well as age and gender influence, we have used the Spanish version of Short form 36 (SF-36) questionnaire in those patients and have compared it with the general population and with patients with heart failure., Patients and Method: All consecutive patients with vaso-vagal syncope submitted for head-up tilt test performance from January 2001 to December 2002 were included. SF-36 was self-administered prior to the head-up tilt test., Results: Two hundred and seventy one patients were included (50.5% females). In these patients, QoL scores were lower than those of the Spanish general population and similar to those in patients with heart failure. Women's scores were lower in eight dimensions, and only four were lower in men's. Women QoL was worst than men's (p < 0.05). Age had a negative influence on the eight dimensions of SF-36, especially in women. The number of syncopes was the most influential clinic parameter on the QoL of such patients., Conclusions: In our series, patients suffering from vaso-vagal syncope had a poor QoL when compared with heart failure or control populations. Women had lower QoL than men, and there was an age-related worsening in both men and women. Our data show that the number of syncopes is the clinic parameter having the best correlation with QoL.

Because of the absence of a uniform protocol for the head-up tilt table test (HUT), we compared 1,661 consecutive patients with syncope referred for HUT. The influence of age and gender on the results (positive response rate and patterns) obtained with three different protocols, Westminster, isoprenaline and nitroglycerin (groups A, B and C) was analyzed. The proportion of women was larger in the youngest age group. A positive response to HUT was observed in 592 patients. The positive response rate to the HUT was higher in groups B and C than in group A, and the rate diminished with age in groups A and C, because of the decrease in mixed-positive responses, but not in group B. The rate of positive responses was similar in groups A and C, but different in group B; no influence of gender on these results was observed. The results with the Westminster and nitroglycerin protocols were similar, but the rate of positive responses was higher in the latter.

Background and Objective: The frequency and circumstances associated with drugs-induced bradiarrhythmia (DB) causing hospital admission are little known., Patients and Method: Retrospective analyses of DB in Cardiology (CD) and Internal Medicine (IMD) Departments of a 3rd level hospital over two consecutive years., Results: We detected 83 cases (mean age: 72.68, 75.9% women). Most frequent DB was slow atrial fibrillation (55.4%). DB were due to the use of one drug in 41 cases and to an association in 42 cases. Most frequently involved drug was digoxine (62.7%). Most frequent pharmacological associations were negative chronotropic drugs (57.1%) followed by their association with a diuretic and/or an angiotensin converting enzyme inhibitor (42.8%), causing renal failure and/or potassium imbalance., Conclusions: Drugs-induced bradiarrhythmia is a frequent cause of hospital admission in aged people, especially women, and is frequently due to drugs associations and/or renal failure.

Objective: To determine the diagnostic yield of transtelephonic event monitors for identifying the reason for palpitations in patients with no structural heart disease., Patients and Method: For 20 months we enrolled all patients reporting palpitations in whom heart disease had been ruled out by medical history, physical examination, ECG and transthoracic echocardiography. All patients underwent 24 h Holter monitoring, which did not provide diagnostic information. Later, a Cardiotest 4DM transtelephonic event monitor was provided to each patient for a mean of 15 3 days. We used SPSS V 10 for statistical analysis., Results: Two hundred twenty-seven consecutive patients were enrolled. Mean age was 45 3 years (range 12-85); 167 were females (74%). Two hundred twelve of the 227 patients (93.3%) experienced palpitations while wearing the device, and 210 (92.5%) used the monitor correctly, recording the cardiac rhythm during palpitations. Fifteen patients (6.6%) had no palpitations during the days of study. In 125 (55%) the Cardiotest 4DM correctly recorded and transmitted arrhythmia that justified the patients' reference to palpitations. In 35 (15.4%) significant arrhythmia was detected: paroxysmal supraventricular tachycardia in 21, atrial fibrillation in 9, atrial flutter in 5, runs of ventricular extrasystoles in 4 and right outflow tract ventricular tachycardia in 1. Sinus rhythm was recorded during palpitations in 85 patients (37%), and arrhythmia as the cause could be ruled out., Conclusions: Cardiotest 4DM identifies arrhythmia in a very high proportion of patients with palpitations and no structural heart disease.
