The purpose of this study was to assess the performance of our Fracture Liaison Service (FLS) over a period of 2 years. Osteoporosis medication was prescribed for 243 patients, and zoledronic acid was the main drug prescribed (60.2%).A Fracture Liaison Service (FLS) was implemented at Lille University Hospital in 2016. Capnocytophaga canimorsus is a Gram‐negative rod commensal of oral cavity of dogs and cats. It can cause sepsis, septic shock, endocarditis, and meningitis associated with bites or licking wounds, especially in immunocompromised patients. Summary Fatty acid esters of hydroxylated fatty acids (FAHFAs) were discovered as a novel class of endogenous mammalian lipids whose profound effects on metabolism have been shown. In the current study, in vitro and in vivo the metabolic effects of two of these FAHFAs, namely palmitic acid-5- (or -9) -hydroxy-stearic acid (5- or 9-PAHSA, respectively) were profiled. In DIO mice fed with differentially composed low- or high-fat diets, acute and subchronic treatment with 5-PAHSA and 9-PAHSA alone, or in combination, did not significantly improve the deranged metabolic status. Neither racemic 5- or 9-PAHSA, nor the enantiomers were able to: (1) increase basal or insulin-stimulated glucose uptake in vitro , (2) stimulate GLP-1 release from GLUTag cells, or (3) induce GSIS in rat, mouse, or human islets or in a human pancreatic β cell line. Proposer une liste de variables à collecter dès le diagnostic et tout au long du suivi des patients atteints de spondyloarthrite axiale (SpA axiale) pour optimiser leur prise en charge en routine. Le processus s'est déroulé en plusieurs étapes : (1) estimation de l'intérêt de 51 variables proposées pour l'évaluation de la SpA axiale via une recherche systématique dans la littérature ; (2) consensus de 78 rhumatologues hospitaliers ou de ville notant chaque variable selon un barème de 4 points de « Peu utile/Inutile » à « Indispensable » ; (3) consensus sur la fréquence minimale d'évaluation des variables sélectionnées selon un barème de 5 points allant de « À chaque consultation » à « Jamais systématiquement ». La recherche systématique a identifié 14 133 résumés dont 213 ont été inclus dans la synthèse qualitative finale. Les données à collecter lors du bilan systématique initial comprenaient cinq questionnaires patient auto-administrés, trois variables de l'entretien avec le médecin, deux variables de l'examen physique, deux variables de l'imagerie spécifique à la SpA axiale et deux autres examens. La collecte systématique a été recommandée à chaque visite pour deux variables, deux fois par an pour une variable, une fois par an pour six variables et tous les deux ans pour une variable. Cette initiative a utilisé des approches fondées sur des preuves et sur un consensus d'experts pour définir un ensemble de base de variables à collecter et à consigner immédiatement après le diagnostic et pendant le suivi de routine des patients atteints de SpA axiale. Objective: To propose a list of variables to be collected right after the diagnosis has been made and during the follow-up of patients with axial spondyloarthritis (ax-SpA) for an optimal management in daily practice.Methods: The process comprised (1) the evaluation of the interest of 51 variables proposed for the assessment of ax-SpA by means of a systematic literature research; (2) a consensus process involving 78 hospital-based or office-based rheumatologists, considering the collection of each variable in a 4 grade scale from "not very useful/useless" to "mandatory"; (3) a consensus on the minimum interval of time for periodic assessment of the selected variables on a 5 grade scale from "at each visit" to "never to be re-collected".Results: The systematic literature research retrieved a total of 14,133 abstracts, of which 213 were included in the final qualitative synthesis. Data to be collected at the initial systematic review comprised 5 patient's self-administered questionnaires, 3 variables of the physician's interview, 2 variables of the physical examination, 2 variables of the specific ax-SpA imaging and 2 other investigations. Two variables were recommended to be systematically collected at each visit, 1 variable twice a year, 6 variables yearly and 1 variable every 2 years.Conclusions: Using an evidence-based and an expert consensus approaches, this initiative defined a core set of variables to be collected and reported right after the diagnosis and during follow-up of patients with ax-SpA in daily practice. Abstract Background Obstructive sleep apnea (OSA) is characterized by repetitive episodes of complete or partial obstruction of the upper airways during sleep. Conscious sedation for flexible bronchoscopy (FB) places patients in a sleep-like condition. We hypothesize that oxygen desaturation during flexible bronchoscopy may help to detect undiagnosed sleep apnea. Patients completed the Epworth Sleepiness Scale (ESS), Lausanne NoSAS score, STOP-BANG questionnaire and the Berlin questionnaire and underwent polygraphy within 7 days of FB. FB was performed under conscious sedation with propofol. Oxygen desaturation during bronchoscopy was measured with continuous monitoring of peripheral oxygen saturation with ixTrend (ixellence GmbH, Germany). Results 145 patients were included in the study, 62% were male, and the average age was 65.8 ± 1.1 years. The vast majority of patients (n = 131, 90%) proved to fulfill OSA criteria based on polygraphy results: 52/131 patients (40%) had mild sleep apnea, 49/131 patients (37%) moderate sleep apnea and 30/131 patients (23%) severe sleep apnea. Patients with no oxygen desaturation had a significantly lower apnea–hypopnea index than patients with oxygen desaturation during bronchoscopy (AHI 11.94/h vs 21.02/h, p = 0.011). This association remained significant when adjusting for the duration of bronchoscopy and propofol dose (p = 0.023; 95% CI 1.382; 18.243) but did not hold when also adjusting for age and BMI. Conclusion The severity of sleep apnea was associated to oxygen desaturation during flexible bronchoscopy under conscious sedation. Patients with oxygen desaturation during bronchoscopy might be considered for sleep apnea screening. Trial registration: The Study was approved by the Ethics Committee northwest/central Switzerland, EKNZ (EK 16/13) and was carried out according to the Declaration of Helsinki and Good Clinical Practice guidelines. Dans les lombosciatiques par conflit disco-radiculaire non compliquées, plusieurs essais randomisés contrôlés n'ont pas mis en évidence de différence significative sur la douleur et le handicap fonctionnel à 1 an entre prise en charge médicale et chirurgicale [1]. C'est pourquoi un geste radical n'est proposé, en dehors des situations nécessitant une intervention en urgence, qu'aux patients en échec du traitement médico-infiltratif. C'est dans ce contexte que nous avons voulu évaluer le taux de chirurgie chez les patients hospitalisés en rhumatologie pour une lombosciatique par hernie discale et identifier les facteurs prédictifs de recours à la chirurgie. Il s'agit d'un travail rétrospectif et monocentrique réalisé entre janvier 2014 et décembre 2018 dans un centre tertiaire. Sur les 450 patients hospitalisés pour une lombosciatique par hernie discale, 405 ont été inclus, après exclusion des patients justifiant d'une chirurgie en urgence. L'objectif principal était de déterminer la fréquence du recours à la chirurgie à 1 an de l'hospitalisation. L'objectif secondaire était d'identifier les facteurs prédictifs du recours à la chirurgie. Le recours à la chirurgie a été évalué à l'aide de la méthode de Kaplan–Meier et les facteurs associés en utilisant des modèles à risques proportionnels de Cox univariés, puis multivariés. Parmi les patients, 52,8 % étaient des femmes et l'âge moyen était de 47,5 ans (18–91 ans), avec un IMC moyen à 27,4 kg/m2. Le trajet radiculaire était réparti de manière équilibrée avec 44,7 % de trajets L5, 45,2 % de trajets S1 et 4,2 % de trajets mixtes L5 et S1 (Fig. 1). La fréquence du recours à la chirurgie à 1 an de l'hospitalisation était de 34,8 %, avec un délai médian de recours à une chirurgie de 31 jours (interquartile range : 3–112). En analyse multivariée, le fait d'être actif professionnellement (OR : 2,3 [1,5 ; 3,6]), l'impulsivité aux efforts physiologiques (OR : 2,0 [1,3 ; 3,1]), un déficit moteur à 4/5 (OR : 1,7 [1,2 ; 2,4]), la durée d'évolution de la symptomatologie avant l'hospitalisation supérieure à 3 mois (OR : 1,7 [1,1 ; 2,5]) et le nombre d'infiltrations (OR : 1,4 [1,2 ; 1,7]) étaient prédictifs d'une chirurgie. Dans notre étude, plus d'un tiers des patients sont opérés dans l'année, ce qui est comparable aux études antérieures [2]. Ce taux concerne une population hospitalisée dans un centre tertiaire, ce qui ne reflète probablement pas l'ensemble des lombosciatiques discales, et notamment ambulatoires. Des facteurs prédictifs de recours à la chirurgie ont été identifiés, notamment le fait d'être actif ou de présenter un déficit à 4/5. Le recours à un nombre plus important d'infiltrations est un facteur confondant, qui reflète peut-être l'intensité de la symptomatologie. Skeletal muscle myosin has potent procoagulant activity that is based on its ability to enhance thrombin generation due to binding coagulation factors Xa and Va and accelerating prothrombin activation. A well-studied myosin inhibitor that binds to myosin's neck region inhibits myosin-dependent prothrombin activation. Hence, to identify a potential binding site(s) on skeletal muscle myosin for factor Xa, 19 peptides (25-40 residues) representing the neck region, which consists of a regulatory light chain, an essential light chain, and a heavy chain (HC), were screened for inhibition of myosin-supported prothrombin activation. Peptide HC796-835 comprising residues 796-835 of the heavy chain strongly inhibited myosin-enhanced prothrombin activation by factors Xa and Va (50% inhibition at 1.2 μM), but it did not inhibit phospholipid vesicle-enhanced prothrombin activation. Peptide inhibition studies also implicated several myosin light chain sequences located near HC796-835 as potential procoagulant sites. A peptide comprising HC796-835's C-terminal half, but not a peptide comprising its N-terminal half, inhibited myosin-enhanced prothrombin activation (50% inhibition at 1.2 μM). This inhibitory peptide (HC816-837) did not inhibit phospholipid-enhanced prothrombin activation, indicating its specificity for inhibition of myosin-dependent procoagulant mechanisms. Binding studies showed that purified factor Xa was bound to immobilized peptides HC796-835 and HC816-837 with apparent Kd values of 0.78 and 1.3 μM, respectively. In summary, these studies imply that HC residues 816-835 in the neck region of the skeletal muscle myosin directly bind factor Xa and, with contributions from light chain residues in this neck region, contribute to provision of myosin's procoagulant surface. Background: Hypoxemia is a surrogate marker for periprocedural endoscopic complications. There are no data comparing the safety of propofol sedation with another sedative regimen in medical thoracoscopy. Objective: To evaluate whether sedation with propofol is as safe and effective as sedation with midazolam. Methods: Ninety consecutive patients undergoing medical thoracoscopy were randomly allocated to receive either intravenous propofol or midazolam. Predefined periprocedural complications included hypoxemia, hypotension, bleeding, need for airway insertion, mechanical ventilation, intensive care unit transfer and death. The primary endpoint was the mean lowest oxygen saturation during the procedure. Results: Randomized groups had similar demographics (64 ± 16 years, 57% male, 91% American Society of Anesthesiologists class III–IV) and a balanced distribution of procedures. The mean lowest oxygen saturation during the procedure was significantly lower in the propofol group as compared to the midazolam group (93 ± 6 vs. 96 ± 3%, p = 0.007). Patients randomized to propofol showed more episodes of hypoxemia (27 vs. 4%, p = 0.007) and hypotension (82 vs. 40%, p < 0.0001). No procedure had to be aborted. None of the patients required an artificial airway, mechanical ventilation or intensive care unit care, and none died. Conclusions: As assessed by the surrogate marker hypoxemia, propofol should not be considered the first choice for sedation in medical thoracoscopy. [ABSTRACT FROM AUTHOR]

Objectives: Axial Spondyloarthritis (ax-SpA) is associated with increased risk of cardiovascular disease (CVD)-specific deaths. We aimed to assess the prevalence of left ventricular (LV) systolic and diastolic dysfunction and valvular heart disease (VHD) by transthoracic echocardiography (TTE) in ax-SpA patients without history of CVD.Methods: A systematic literature review was performed in PUBMED, Embase, Cochrane Library databases published before April 2020. We included all controlled studies assessing myocardial function and heart valve by TTE in ax-SpA without history of CVD. A meta-analysis was performed with random or fixed effects model estimating mean differences (MD) and odds ratio (OR).Results: Literature search selected 189 abstracts and 28 articles were included (1471 ax-SpA and 1115 controls). ax-SpA had a statistically slight alteration of LV ejection fraction (MD=0.64%, 95%CI: 0.14-1.14). ax-SpA had more frequently LV diastolic dysfunction (OR=3.43, 95%CI: 1.78-6.59) and an alteration of E/A ratio (MD=0.15, 95%CI: 0.08-0.21), deceleration time (MD=13.07ms, 95%CI: 7.75-18.40), isovolumetric relaxation time (MD=7.90ms, 95%CI: 4.50-11.30), left-ventricular end diastolic (MD=0.57mm, 95%CI: 0.19-0.95) and systolic (MD=0.77mm, 95%CI: 0.36-1.17) diameters. Three studies (15%) used a combination of TTE parameters to diagnose LV diastolic dysfunction. Prevalence of mitral regurgitation and aortic regurgitation were similar in ax-SpA patients and healthy individuals.Conclusion: ax-SpA have a non-clinically relevant alteration of LV ejection fraction and similar prevalence of VHD compared to healthy individuals. LV diastolic TTE parameters are altered in ax-SpA. However, most studies do not combine set of parameters to recognize diastolic dysfunction. The article presents a randomized, double blind, placebo controlled study on the suppression of cough during flexible bronchoscopy (FB) through combined sedation with benzodiazepines such as midazolam and hydrocodone. This provides a better antitussive effect than a benzodiazepine alone. Several patients undergoing diagnostic FB were prospectively randomized to receive either midazolam and hydrocodone or midazolam and placebo in a double blind fashion. The study revealed that there were no significant differences between the hydrocodone and placebo groups in age, sex and indication for FB.

OBJECTIVES: Patient-controlled sedation (PCS) with propofol, is well tolerated and reduces recovery time and staff required during endoscopic interventions.“Who” administers the drug proves economically crucial. With the aim of maintaining safety, medical quality, and patient satisfaction, this study investigates PCSversusnurse-administered propofol sedation (NAPS) in a cohort of consecutive patients.METHODS: One hundred and fourteen patients, aged 22–90 yr, undergoing only colonoscopy participated in this prospective randomized trial. Patients were randomly assigned to either PCS or NAPS. If patients declined randomization for different reasons of reluctance to PCS they were assigned to a standard nurse-sedated control group. All patients received pethidine presedation for analgesia. Visual analogue scales followed patient anxiety level, tolerability, pain, and satisfaction, and endoscopist's assessment of the procedure.RESULTS: Given the choice, 35% of the patients who were rather younger and more anxious declined randomization to PCS. The mean total dose of propofol needed in this group was higher, but the patients had a tendency to rate the global tolerance and the pain of the examination as less comfortable compared to the two randomized groups. Self-administration of propofol created a significantly different drug profile and higher medication costs. With regard to the safety parameters there was no difference between PCS and NAPS. In their global assessments, the patients and endoscopists tended to prefer NAPS.CONCLUSIONS: Individual patient characteristics and attitudes toward self-control are crucial for PCS. : ObjectivePropofol, a rapidly-acting hypnotic agent, is increasingly being used for endoscopic sedation. Serious adverse effects, including respiratory and cardiovascular depression, make many endoscopists reluctant to use propofol in critically ill patients. This study characterizes propofol’s safety profile in consecutive high-risk patients (American Society of Anesthesiologists [ASA] classes III and IV) compared with matched subjects (ASA classes I and II).: MethodsDuring a 19-month period, 1370 at risk-patients were sedated with propofol, of whom 47% (614 ASA III, 28 ASA IV) were age matched with 642 consecutive patients of the same gender and age assigned to ASA classes I and II and undergoing the same endoscopic procedures (395 gastroscopies, 201 colonoscopies, 14 combined). Registered nurses performed all sedations by propofol dose titration while carefully monitoring arterial oxygen saturation, heart rate, and blood pressure.: ResultsNo major complications occurred among the critically ill patients. There was, however, an increased risk for a short relevant oxygen desaturation (<90%) of 3.6% for ASA III and IV versus 1.7% for ASA I and II (p = 0.036). In four versus one case, short mask ventilation was necessary. Also, a greater proportion of patients showed a ≥5% oxygen saturation decrease. There was no pronounced influence on arterial pressure or heart rate and no perforations in 336 colonoscopies.: ConclusionsWith careful monitoring, propofol sedation during GI endoscopies is safe, even for high-risk patients. Considering their higher comorbidity and tendency toward oxygen desaturation, they need particularly careful monitoring, and the required dose is, on mean, 10–20% lower than in ASA classes I and II. Summary Aim: To characterize the safety of endoscopic procedures and propofol use as administered by nurses in a cohort of elderly patients. Methods: During 19 months all endoscopy patients > 70 years were eligible for this prospective observational study. Patients were assigned to group A (70–85 years) or group B (> 85 years). Records from all patients < 70 years treated during the same period served as controls. Specially trained nurses administered the propofol and monitored for complications (decrease in the peripheral oxygen saturation, mean arterial pressure or heart rate). Results: There were 1435 endoscopic procedures in group A and 351 in group B. There was no procedure or sedation-related mortality, nor a significantly greater need for emergency intervention than in the control group. Two patients required short mask ventilation but recovered without sequelae. Compared with younger patients, there was a significant increase in risk for a short oxygen desaturation < 90% and a decrease in oxygen saturation > 5%. Arterial hypotension occurred significantly more often in the control group than among the aged patients. Conclusions: This present study documents that gastrointestinal endoscopic procedures are safe and well tolerated even in the very elderly.
