The relationship between family size and child mental disorders was assessed through a prevalence study conducted in an urban neighborhood of Salvador, Brazil. From a representative sample of 828 children aged between 5 and 14 years, 23.3% were diagnosed as exhibiting varying degrees of mental disorders. Diagnoses were based on psychiatric interviews, following screening using the QMPI (a Portuguese-language screening questionnaire). Results supported the hypotheses that: (1) children from smaller families would exhibit higher prevalence of mental disorders than those from larger families; and (2) prevalence of neurotic and psychosomatic disorders would be lower for children from larger families than from smaller ones. Significant associations were also found between family size and diagnosis and severity of child mental disorders. Controlled analyses indicated that these relationships were not confounded by age or sex of the child, family social class, or family type.

7,12-Dimethylbenz[a]anthracene (DMBA) is a potent leukaemogenic agent in Long-Evans rats when it is administered i.v. in vivo. The prior oral administration of Sudan III [1(p-phenylazo-phenylazo)-2-naphthol] protects animals from the leukaemogenic effects of this compound. Sudan-III-treatment resulted in an increased rate of removal of DMBA from the livers and blood of treated animals and resulted in a reduced level of accumulation of the hydrocarbon in its presumed target tissue, the bone marrow. The regio-selective hepatic metabolism of DMBA was altered in Sudan-III-treated animals, thus only trans-3,4-dihydro-3,4-dihydroxy-DMBA was recovered from the livers of control animals whereas only trans-8,9-dihydro-8,9-dihydroxy-DMBA was recovered from the livers of Sudan-III-treated animals. The distribution of DMBA dihydrodiols in the blood and in the bone marrow was found to reflect the findings in the liver. DMBA, when incubated with rat bone marrow cells in vitro, was metabolized to probable phenolic compounds but no evidence whatsoever was found for dihydrodiol formation in these cells. Taken together these results are consistent with a model in which Sudan-III-induced alterations in the regio-selective hepatic metabolism of DMBA are responsible for the protection against leukaemogenesis afforded by the administration of that compound. The probable mechanism of protection is the induction in liver of a cytochrome P450, probably P450c.

A theoretical model has been developed to study the propagation and amplification of hypershort (< 200 fs) pulses in excimer laser amplifiers, dealing specifically with coherent propagation effects and giving a detailed description of the dissociation of the ground state for a linear potential. The model equations are presented and the physical significance of the terms discussed.

A novel spectroscopy is proposed in which an atomic or ion beam in collision with a surface is excited electronically by a high intensity pulsed laser beam illuminating the surface. By monitoring ions or excited atoms in the reflected beam information about the shift and broadening of the atomic energy levels near the surface is obtained. We work out the theory of the spectroscopy in a suitable model framework and demonstrate its theoretical feasibility.

Eighteen patients with small cell carcinoma of the lung received high dose cyclophosphamide (180-200 mg/kg) intensification following five pulses of 'CHOP' chemotherapy (cyclophosphamide 750 mg/m2 i.v., adriamycin 50 mg/m2 i.v., vincristine 1.4 mg/m2 i.v., prednisolone 40 mg orally for 5 d). They received infusions of autologous bone marrow which had been stored at 4 degrees C for 34 h. Pancytopenia was predictable in onset and its duration acceptable. Recovery of neutrophils to greater than 1.0 x 10(9)/l was achieved in 17.5 +/-0.9 d (mean +/- SEM) and platelets to greater than 100 x 10(9)/l in 17.5 +/- 0.8 d. Four patients with acute myeloid leukaemia in complete remission received intensification with the supralethal combination of cyclophosphamide and total body irradiation followed by infusion of autologous marrow which had been stored at 4 degrees C for 54 h. Haematological reconstitution in these patients was acceptable but slower (greater than 1.0 x 10(9)/l neutrophils between days 26 and 40; greater than 20 x 10(9)/l platelets between days 23 and 77). Except in one case, normal peripheral counts were attained in all patients. It is concluded that bone marrow stored at 4 degrees C for up to 54 h is a simple and practical source of viable stem cells which have the capacity for acceptable haematological reconstitution.

The author presents a simple model of a two-electron bound state coupled to a continuum by a laser field. This model is a close relative of the Anderson model for impurity states in a metal. The author presents a solution for the model that is not based on the Hartree-Fock approximation. It displays the transition from correlated, high Coulomb repulsion, to independent electron behaviour as the bound-continuum coupling is increased. In this way the author shows some of the phenomena that may be observable in ultra-intense field photoionisation. Furthermore, one can see how the Hartree-Fock approximation may give absurd results for moderate fields.

The Kramers-Henneberger transformation is used to treat potential scattering of electrons in the presence of a strong laser field. As a first step, the one-dimensional scattering by a polarization potential is considered. It is shown that the static part of the effective potential, often called the dressed potential, may support more bound states than the original potential depending on the intensity and frequency of the field. Exact results for the transmission and reflection coefficients are presented and two approximations are discussed: A perturbative approach based on the above transformation and the adiabatic approximation.

We present a simple model for two-electron excitation and ionization of an atom in the presence of an intense laser field. We show, in particular, how the Coulomb interaction, via configuration interaction, prevents the excitation and ionization from being collective in all but the most intense fields. By collective we mean that each electron has the same spatial wavefunction: this implies that the electron pair may be described by a restricted time-dependent Hartree-Fock wavefunction. The configuration interaction prevents collective behaviour not only through the process of auto-ionization but also through the mixing of doubly excited independent electron states into the ground state. We are able to give criteria for establishing true collective excitation and ionization of our model two-electron system in an intense field.

The authors have measured the polarisation of collisionally redistributed light as a function of detuning from line centre for the Hg 3P1-1S0 transition in the presence of Kr. These are the first such results for a J=0 to 1 system for which the interatomic potentials are well documented. They present a polarisation profile for the red wing between 5 and 40 cm-1 and polarisation measurements for two individual detunings in the blue wing. The data have been corrected for various mechanisms which affect the polarisation but which do not depend on the original scattering event. The application of some of these corrections has been discussed elsewhere for the CW case. Since they report the first pulsed collisional redistribution experiments involving depolarisation measurements they discuss the differences between the pulsed and CW cases in detail.

A recombinant plasmid library representing polyadenylated RNAs in the leucocytes of a Ph1-positive chronic granulocytic leukaemia (CGL) has been constructed. One recombinant (designated pCG14) isolated from this library contains a DNA sequence complementary to a small polyadenylated RNA that is abundant in RNA from CGL leucocytes. The relative concentrations of pCG14 RNA in the RNAs from a variety of normal and leukaemic leucocytes and human haemopoietic cell lines have been measured with a molecular hybridisation assay. This has shown that pCG14 RNA is 10 to 50 times more abundant in RNA from CGL leucocytes than in the RNAs from these other cells. The data indicate that the occurrence of pCG14 RNA in high abundance is sufficiently characteristic of a CGL leucocyte population to distinguish it from other populations of leucocytes. They suggest that the measurement of the concentrations of specific RNA species in leucocyte RNA by means of molecular hybridisation with cloned complementary DNAs may provide additional markers for the objective classification of human leukaemias which could be particularly useful since the method exploits a criterion different from any currently in use.

This document constitutes a segment of a feasibility study investigating the ramifications of constructing a nuclear energy center (NEC) in an arid western region. In this phase of the study, the impacts on socioeconomic conditions in the surrounding communities and possible ways of financing and mitigating these impacts were examined. The general conclusion reached is that the socioeconomic impacts of a nuclear energy center in the Green River area of Southeastern Utah would not impose an absolute bar to NEC development. The economy of the NEC impact area would be substantially transformed by the NEC. In particular, Green River city itself would change from its current status as a relatively stable rural economy with an agricultural, mining, and recreation base to a major city with over 20,000 permanent relatively high income residents. The NEC, by itself, would provide a tax base more than adequate to finance required expansion of public facilities and public human service provisions.

Multiphoton processes with significant continuum contributions are enhanced by exploiting autoionizing resonances embedded in the continuum by configuration interaction (1). Tunable "embedding" can be induced by radiative coupling from an initially empty discrete state; such "pseudoautoionizing” states can be shifted and broadened at will.

Carbimazole and Propylthiouracil (PTU) are widely used in the treatment of Graves' disease, although controversy still exists as to whether or not they have any direct effect on the immune system. While many previous studies have investigated the possible effects of one or other of these drugs on the immune system only a few have directly compared the effects of equivalent doses of each drug. This study aimed to examine the effects of both drugs, during the initial 8 weeks of treatment, on various biochemical and immunological parameters. The results obtained showed that while thyroid hormone levels fell at similar rates in both treatment groups, TRAb levels and T cell subset abnormalities returned towards normal more rapidly in patients receiving Carbimazole compared to PTU. These results indicate that the effects of Carbimazole on TRAb levels and T cell subset abnormalities are not due solely to its action in controlling the biochemical features of Graves' disease and provide indirect evidence of an action on the immune system in vivo.

Many integral membrane enzymes require for their activity interactions with the polar headgroups of phospholipids, in addition to the hydrophobic interactions within the lipid bilayer. The interactions with the polar headgroups may have preferential or absolute specificity. To study such interactions, phospholipids have been synthesized which carry photoactivable moieties in their headgroups. Three types of phospholipids, PL-I, PL-II and PL-III, were synthesized. The synthetic phospholipids, PL-I and PL-II were able to reconstitute enzymatic activity of the membrane proteins which were studied. Covalent crosslinking between these phospholipids and the membrane proteins was demonstrated after photolysis of the reconstituted phospholipid-protein complexes.

Several studies have shown that the majority of patients receiving digoxin can be successfully withdrawn. A medical record review was conducted to determine whether, in practice, patients were being withdrawn from digoxin. Original indications for digoxin therapy in 163 outpatients were as follows: congestive heart failure (CHF), 50%; supraventricular tachycardia (SVT), 23%; CHF and SVT, 10%; and unknown/unclear, 17%. One third of these patients were withdrawn during the 3.5-year study, and 79% remained stable, off digoxin. The most significant predictor of withdrawal was chart indication of reassessment of the need for digoxin. The majority of the patients (68%) were reassessed, and of these, almost half were withdrawn. Physicians appear to be reassessing the need for digoxin therapy, resulting in higher withdrawal rates than previously reported. Results suggest that patients with unclear original indications, a onetime indication, or without clinical evidence of CHF or SVT can be successfully withdrawn.

We should like to present a brief progress report on multiphoton transitions occurring during collisions. The aim of the work is the development of techniques for studying collision dynamics using off-resonant excitation of atomic transitions in the presence of collisions [1]. We shall start with the now comparatively well understood subject of collisional redistribution, dealing with single-photon absorption during collisions [1,2]. The natural extension of this work to multiphoton effects is what we are pursuing. The first type of multiphoton effect we’ll look at involves saturation of a two-level system: the Rabi problem of collision physics. This is an enormous topic that we shall discuss briefly and very naively [1,2]. We shall turn lastly to multilevel systems where much less is known and much more remains to be done. This report will use qualitative arguments. For more rigorous discussions the reader should consult the references given in the text.

A case of septicaemia due to Neisseria catarrhalis occurring in an immunosuppressed individual is described. The clinical syndrome closely resembled that of N. meningitidis septicaemia. The potential pathogenicity in the immunosuppressed patient, of organisms usually considered harmless is stressed.

Progress in achieving initial remission of disease in acute myeloblastic leukaemia (AML) has taken place in recent years, with rates of 70%–80% being reported [1–3]. This has been effected by different scheduling of drugs rather than by the introduction of new chemotherapeutic agents. The ability of chemotherapy protocols to maintain remission is less easy to demonstrate. Unlike remission maintenance chemotherapy in acute lymphoblastic leukaemia (ALL), there is little evidence to suggest that continuous chemotherapy has prevented subsequent relapse for the majority of patients. Some improvements have been claimed for very intensive protocols, with better results being achieved in children [4, 5]. However, since the majority of patients with the disease are adults, alternative approaches are required.

The aim of the study was to examine tri-iodothyronine, thyroxine, thyroid antibody and thyrotropin receptor antibody levels in thyroid and peripheral venous blood at the time of surgery in patients with Graves' disease. T cell subset patterns in peripheral and thyroidal venous blood were compared to the distribution of T cell subset patterns within the thyroid gland itself. The results showed that at the time of surgery there were no significant differences in any of the parameters measured between thyroidal and peripheral venous samples. T cell subset patterns within the thyroid gland were subjectively similar to those in the venous samples.

Sudan III treatment of Long-Evans rats results in increased hepatic monooxygenase activity using ethoxycoumarin and aniline as substrates. Monooxygenase activity towards amino-pyrine and nitrosodimethylamine is not affected. Sudan III treatment results in increased microsomal cytochrome P448 and increased amounts of a protein band which comigrates with purified cytochrome P448 during SDS polyacrylamide gel electrophoresis. The proportions of the different dihydrodiols formed during the incubation of 7,12-dimethylbenz[a]anthracene with microsomes vary between untreated and treated animals. Thus, extracts of microsomes from untreated rats were found to contain materials with chromatographic properties identical to those of the 3,4-dihydrodiol and the 5,6-dihydrodiol when examined on two different h.p.l.c. systems. Extracts of microsomes from Sudan III treated animals were found to contain materials with chromatographic properties identical to those of the 5,6-dihydrodiol and the 8,9-dihydrodiol when similarly examined. These findings suggest that the protective effect of Sudan III against DMBA induced leukaemia is mediated by an alteration in monooxygenase activity.

An analysis is presented of the distribution of cases of leukemia and allied disorders occurring in 151 administrative districts from England, Wales, and Scotland during 1984. The age-adjusted rates for certain conditions present an unusual pattern highlighting excessively high and low rates in parts of the country, some of which share contiguous boundaries. In particular, high rates for non-Hodgkin's lymphoma are found in rural Yorkshire districts, whereas leukemias and primary polycythemias are much more common in the Midland districts.

Fourteen patients who received a bone marrow transplant (BMT) as treatment for leukaemia were included in a prospective study of the histological changes in the skin. The aim of this study was to improve the early diagnosis of graft-versus-host disease (GVHD). It was found that the clinically 'normal' pre-transplant skin was in some cases histologically abnormal on H & E examination in patients who were on regular maintenance cytotoxic chemotherapy. These changes were similar to some of the features of GVHD. Immunocytochemistry, although not specific, was found to be helpful in the diagnosis of some cases of GVHD. Suggestive features included a reduction in the numbers of Langerhans cells, an increase in the number of suppressor (OKT8+) cells in the dermal infiltrate and the presence of Ia positivity of the keratinocytes in the epidermis.

Lindau disease is characterized by angioblastic lesions of the cerebellum and spinal cord. It may, rarely, be associated with pheochromocytoma. We describe the management of a patient with Lindau disease who was scheduled to undergo craniotomy for removal of a cerebellar hemangioblastoma and who developed severe hypertension following induction of anesthesia. Subsequent work up revealed a previously undiagnosed pheochromocytoma. The importance of recognizing a possible association between these two conditions is discussed.

Recently developed table-top laser sources are capable of producing intensities on target of 1017Wcm −2 in femtosecond pulses. This has stimulated interest in the possible mechanisms for laser light absorption in a plasma produced by such a source. The plasma, expanding at approximately the ion sound speed ( υ s = Z k B T e / m i ) , remains at virtually solid density during the pulse. Light incident on such an overdense plasma boundary will be largely reflected, unless there is strong absorption within the skin depth of the target (~ c/w p ), where the evanescent wave does penetrate.

Graft-versus-host disease prevention was attempted in 35 consecutive patients with hematological malignancy who received bone marrow from an HLA match sibling donor who was depleted of T cells ex vivo. Five of the first 8 patients who received cyclophosphamide 60 mg/kg on 2 consecutive days followed by fractionated total body irradiation (TBI) (6 x 2 Gy) had graft failure. The subsequent 27 patients had received an extra fraction of TBI (7 x 2 Gy), and only one failed to have stable engraftment. There were no differences in nucleated cell dose, granulocyte-macrophage colony-forming units, or T cell numbers given to the two groups. Neutrophil but not platelet regeneration of those patients who successfully grafted was slower than in a group of historical controls receiving unmanipulated marrow. Significant graft-versus-host disease was prevented with no increase in relapse rate. We suggest that engraftment can be reliably achieved by augmenting the TBI conditioning in recipients of T cell-depleted matched allogeneic bone marrow.

A case of neurenteric intraspinal cyst with vertebral anomalies is reported; the cyst contained gastric mucosa. MRI and CT provided useful information.

This lecture will take a very quick tour of how d.c. and a.c. electric fields affect atoms and ions along with atomic and molecular processes. Some of the topics I’ll discuss are the subject of great interest and speculation at the present time. I can only give the most basic of pictures of what is going on.

This study investigated the use of late dose intensification therapy (LDIT) with cyclophosphamide (180 mg/kg) and VP 16 (1 g/m2) plus autologous bone marrow rescue in 22 patients with small cell lung cancer (SCLC). These patients were selected from a group of 95 patients who received three courses of a five-drug induction regimen comprising cyclophosphamide (750-1000 mg/m2), adriamycin (40 mg/m2), VP 16 (100 mg/m2) for 3 days, methotrexate (50 mg/m2) and vincristine (2 mg) (CAVMO). There were 16 patients with limited disease, 8 of whom were in complete remission (CR) and 8 in partial remission (PR) after the induction therapy. The other 6 patients had extensive disease; 3 of these achieved CR and 3 PR after induction therapy. Of the 11 patients in PR, 5 responded to LDIT; 3 had a further PR, and 2 CR. Subsequent to LDIT radiotherapy 4000 cGy was given to the primary site in 10 of the 22 patients. Since the start of the study, 19 of the 22 patients have relapsed and died (median survival 11 months), while 3 remain alive and in remission at 11, 11, and 24 months. Comparison of the survival of patients receiving LDIT with that of an equivalent group (with respect to staging and response to induction chemotherapy) of patients who received induction chemotherapy alone showed no significant difference. In this study, LDIT following conventional induction therapy in patients with chemosensitive tumours did not improve survival.

Seventy one patients underwent bone marrow transplantation for aplastic anaemia or haematological malignancy, 39 as allografts and 32 as autografts. All patients who were seronegative to cytomegalovirus received blood product support exclusively from seronegative community blood donors; seropositive patients received unscreened products. In no patients was there any attempt to reduce cytomegalovirus (CMV) infection by giving prophylaxis with immunoglobulin, and granulocyte transfusions were not given. The incidence of cytomegalovirus infection in the seronegative recipients (22 allograft, 15 autograft) was 0%; in the seropositive recipients 16 (63%) in allografts and 17 (18%) in autografts. These results suggest that provision of exclusively seronegative blood products is an important contribution for seronegative transplant recipients, but make little impact in autologous transplantation where the incidence of infection is low.

The rates of formation of diols of dimethylbenz[a]anthracene (DMBA-3,4-diol, DMBA-5,6-diol and DMBA-8,9-diol) have been determined in hepatic microsomes prepared from untreated rats or from animals treated with phenobarbital (PB) or Sudan III. PB treatment enhanced the formation of the proximate carcinogen, DMBA-3,4-diol, and of the 5,6-diol while treatment with Sudan III suppressed the formation of DMBA-3,4-diol but greatly increased the rates of formation of the other two diols. Metyrapone, a reagent which is specific for members of the major PB-induced cytochrome P-450 subfamily (P-450-PB3), did not alter the rate of formation of the diols other than in microsomes prepared from PB-treated animals in which formation of the 5,6-diol was inhibited. Incubation of DMBA with microsomes resulted in the formation of covalent, DMBA-microsome adducts. Treatment of the animals with PB and Sudan III increased the rate of formation of DMBA-microsome adducts to a similar extent (approximately 5-fold). The formation of adducts could be inhibited by metyrapone in microsomes from untreated and PB-treated animals but the reagent had no effect on adduct formation in microsomes from Sudan III-treated animals. These observations may indicate that adduct formation in microsomes from Sudan-treated animals involves primary epoxide metabolites while in microsomes from PB-treated animals secondary metabolites are involved and these may be formed by a P-450-PB3 isoenzyme. Specific P-450 isoenzymes involved in the regioselective formation of DMBA-diols have been identified by the use of antibodies directed against specific isoenzymes. An antibody to P-450-MC1b inhibited the formation of DMBA-5,6-diol and 8,9-diol in microsomes from Sudan III-treated animals. Western blot analysis demonstrated that P-450-MC1b was induced in microsomes of animals treated with Sudan III but was not present in the other two microsomal preparations. In accord with the observations with metyrapone, anti-P-450-PB3 inhibited formation of DMBA-5,6-diol in microsomes from PB-treated animals but was without effect on the formation of other diols. Anti-P450-PB1 inhibited the formation of DMBA-3,4-diol in microsomes from PB-treated animals. Western blot analysis of microsomes from animals treated with several xenobiotics indicated a qualitative correlation between the content of P-450-PB1 and the rate at which DMBA-3,4-diol was formed.

Peripheral blood T lymphocyte subpopulations were analysed using OKT3, OKT4 and OKT8 monoclonal antibodies in eleven patients undergoing autologous bone marrow transplantation following high dose chemotherapy +/- total body irradiation for bronchial carcinoma or as intensification therapy for acute myeloblastic leukaemia. Marked inversion of the OKT4:OKT8 ratio was noted in all patients following the procedure. This is due to both a fall in the number of OKT4 positive cells and a rise in the number of OKT8 positive cells in all but one patient. Sequential analysis of six patients shows a return to the normal ratio in two with time and a trend towards normalization in the others. We suggest that the subpopulation inversion noted following bone marrow transplantation is an effect of marrow regeneration and is not causally related to graft versus host disease.
