101. Total Synthesis of (+)-Aplykurodinone-1

Abstract

Starting from (R)-citronellic acid and (R)-seudenol, the total synthesis of (+)-aplykurodinone-1, a highly degraded marine steroid, has been achieved in 11 steps and in 19% overall yield with excellent stereochemical control. In addition to the features such as an Ireland–Claisen rearrangement, an intramolecular carbonyl–ene cyclization, and an intramolecular Michael addition, the present synthetic strategy is accomplished without the use of protecting groups.

Abstract

Objectives The excessive activation of sympathetic nerve can lead to heart failure which causes heart remodelling, including both electrical and structural remodelling. Whether the endoplasmic reticulum (ER) stress mechanism on the electrical remodelling of heart failure induced by sympathetic storm is still poorly understood. This study investigated whether excessive activation of sympathetic nerve causes aberrant ER stress and electrical remodelling in heart failure. Methods Heart failure was induced in SD rats by different dose of isoproterenol (ISO) (85 mg/kg and 170 mg/kg) with the same amount of physiological saline as control. At the end of two weeks, hemodynamic and plasma BNP was performed and measured to evaluate the heart function. Real-time quantitative PCR and western blot were used to detect the expression of ER stress associated elements and Kv4.2 both at the mRNA level and the protein level. Moreover, the primary cultured neonatal rat ventricular myocytes after quiescence for 24 h were exposed to different concentration of ISO (0, 10, 20 and 100 μmol/l). Immunofluorescence, Real-time quantitative PCR and western blot were also used to investigate the of ER stress and the location of Kv4.2 on cell membrane of neonatal rat ventricular myocytes. Abstract

Objectives Previous studies have suggested that Angiotensin-receptor blockers (ARBs) can reduce the incidence and recurrence of atrial fibrillation (AF). Recent studies started to focus on the effect of ARBs on prognosis of AF patients. In this study, we collected the existing evidence to conduct a comprehensive analysis on whether ARBs can reduce cardiovascular events in AF patients. Methods We performed a meta-analysis, searching EMBASE, PubMed and Cochrane Library from 1970 to 2012, for randomised controlled trials (RCTs) on ARBs in AF patients. Primary outcomes were cardiovascular death, stroke, and major adverse cardiovascular events (MACE); secondary outcomes were myocardial infarction (MI), hospitalisation for heart failure, and all-cause mortality. Results We identified 4 RCTs (LIFE, CHARM, J-RHYTHM II and ACTIVE I) enrolling 10,824 patients who fulfilled our inclusion criteria. when compared with control, ARBs was associated with no significant improvement in cardiovascular death (risk ratio [RR], 0.98; 95% confidence intervals [CI], 0.90–1.07), stroke (RR, 0.90; 95% CI, 0.79–1.02), MACE (0.87; 95% CI, 0.71–1.07), MI (RR, 1.08; 95% CI, 0.87–1.36), all-cause mortality (RR 0.98; 95% CI, 0.91–1.06), but a significant reduce on hospitalisation for heart failure (RR 0.87; 95% CI, 0.79–0.96). Conclusions ARBs do not reduce cardiovascular events of AF patients expect hospitalization for heart failure. For the limited evidence, it is still uncertain to use ARBs in AF patients beyond blood pressure lowering and heart failure treatment.

125. Prognostic value of neutrophil to lymphocyte ratio in heart failure patients.

Abstract

Background Neutrophil to lymphocyte ratio (NLR) has been indicated to be an independent predictor for all-cause mortality or adverse events in a variety of diseases. However, no consistent conclusions regarding it's relevance to patients with heart failure have been made. This meta-analysis was conducted to assess the significance of NLR in patients with heart failure. Methods Pubmed and Embase databases were searched for eligible studies that reported the association between NLR and heart failure through September 2017. The overall hazard ratio (HR) and corresponding 95% confidence interval (CI) were used to assess the associations. Results Ten studies met the eligibility criteria and a total of 5979 heart failure patients were included in the meta-analysis. The overall hazard ratio (HR) for all-cause mortality was 1.28 (95% CI 1.14–1.43) and the HR of renal dysfunction was 1.23 (95% CI 1.07–1.41) comparing the highest with the lowest category of NLR. However, the total pooled adjusted HR for in-hospital mortality (HR = 1.18, 95% CI 0.88–1.59) and rehospitalization (HR = 2.19, 95% CI 0.94–5.09) were not statistically significant. A subgroup analysis showed that sample size with moderate heterogeneity may be the origin of heterogeneity in all-cause mortality. Sensitivity analysis proved the stability of results of our meta-analysis. Conclusions The meta-analysis demonstrates that NLR is a predictor of all-cause mortality in patients with heart failure. Abstract

Aim: To study the clinical value and efficacy of computerized tomography (CT)-monitored microinvasive craniopuncture for traumatic epidural hematoma., Material and Methods: Thirty-three cases of traumatic epidural hematoma patients were selected at our hospital. YL-1-type disposable intracranial hematoma smash needle puncture and drainage of hematoma were used under CT monitoring and urokinase was used for intermittent flushing, and the residual blood clot was then drained. The improvement rate, cure rate, precision and accuracy of the puncture, incidence of bleeding, puncture infection rate, average length of stay, clinical efficacy, and long-term follow-up were evaluated., Results: The improvement rate was 100%, the cure rate was 100% accurate, the precise puncture rate was 100%, the re-bleeding rate was 0%, the local infection rate was 0%, the average length of stay was 7.5 days, and the average medical cost was two thousand U.S. dollars., Conclusion: Under CT surveillance, the YL-1-type disposable intracranial hematoma smash puncture needle aspiration in the treatment of traumatic epidural hematoma is a simple, fast, and accurate positioning procedure. Without craniotomy and blood transfusion, the YL-1-type disposable intracranial hematoma smash puncture needle aspiration is a safe and effective operation, but close attention is needed during the operation.
