Simple Summary: Rectal cancer (RC) is the third most common gastrointestinal malignancy, with a rising incidence, particularly among the population under 50 years old. Extramural venous invasion (EMVI) is defined as the presence of malignant cells in veins beyond the muscularis propria near the primary colorectal tumor. The aim of our retrospective study was to assess the prognostic value of separate pathological EMVI reporting in operative RC samples and to determine its relationship with standard pathohistological and surgical parameters among a selected cohort of RC patients from our institution. Finally, EMVI is proved not only to be a feature of aggressive tumor behavior, but also as a separate and independent parameter in patients with rectal cancer. The results we obtained strongly suggest the importance of separately reporting extramural vascular invasion (EMVI) from lymphovascular invasion (LVI) in daily practice, as well as that EMVI could be a good addition to TNM staging. Background/Objectives: Vascular invasion, especially extramural vascular invasion (EMVI), has emerged as a prognostic parameter for rectal cancer (RC) in recent years. Prediction of recurrence and metastasis development poses a significant challenge for oncologists, who need markers for prediction of adverse outcome. The aim of this study was to examine the prognostic significance of pathohistologically detected EMVI in untreated rectal cancer and its implications in separate reporting. Methods: We examined 100 untreated RC patients who underwent curative resection from January 2016 to June 2018 with a follow-up of 5 years. Patients were divided into equal EMVI− and EMVI+ groups based on histological re-examination of H&E-stained postoperative surgical samples. Results: The presence of EMVI within the selected cohort was significantly associated with female gender, T3/T4 and N1/N2 post-operative stages, positive lymph nodes, lymph node ratio LNR2 and LNR3 groups, abundant tumor-infiltrating lymphocytes, positive lympho-vascular invasion (LVI), perineural (PNI), and circumferential resection margin (CRM) (p < 0.05 in all tests). Within EMVI+ patients, local recurrences and/or metastases and death outcomes were more frequent events (p = 0.029 and p = 0.035, respectively), while survival analyses revealed that EMVI+ patients had significantly shorter overall survival (OS, p = 0.040) and disease-free survival (DFS, p = 0.028). Concerning LVI, differences in OS between LVI+ and LVI− patients were not statistically significant (p = 0.068), while LVI+ patients had significantly shorter DFS (p = 0.024). Moreover, univariate COX regression analysis demonstrated the negative impact of EMVI on OS (HR: 2.053, 95% CI: 1.015–4.152; p = 0.045) and DFS (HR: 2.106, 95% CI: 1.066–4.870; p = 0.038), which was not the case for LVI + RC patients. Conclusions: The obtained results strongly suggest the significance of separate reporting of EMVI from lympho-vascular invasion, as it is potentially a surrogate marker for adverse prognosis and outcome. Acromegaly due to ectopic secretion of growth hormone-releasing hormone (GHRH) is a rare disorder. The signs and symptoms of ectopic acromegaly are indistinguishable from acromegaly due to a somatotroph adenoma. A 35-yearold female presented with secondary amenorrhea for 10 years, intermittent headache, and reduced vision in both eyes for 4 years, which worsened over 4 months before presentation. Additionally, she was diagnosed with uncontrolled diabetes mellitus. On examination, she had coarse facial features, a fleshy nose, and acral enlargement. She had diminished visual acuity (left>right) and bitemporal hemianopia on perimetry. Biochemical investigations revealed elevated IGF-1 [588 ng/ml, reference range (RR) 100--242], markedly elevated basal growth hormone (>80 ng/ml; RR, 0.12--9.88), and hyperprolactinemia in the tumoral range (832 ng/ml; RR, 5--25). MRI sella demonstrated a 22x30x34mm sellar-suprasellar mass with T2 hypointensity. Chest imaging revealed a 75x87x106mm left lung mass, which was found to be a welldifferentiated neuroendocrine tumor (NET) on biopsy. Plasma GHRH levels were elevated [38,088 ng/l; RR, <250--300], and a diagnosis of ectopic acromegaly secondary to lung neuroendocrine tumor was considered. During workup, the patient developed in-hospital pituitary apoplexy, which improved with medical management. After a left pneumonectomy, her clinical features of acromegaly improved, her diabetes underwent remission, and there was a marked reduction in plasma GHRH and pituitary size. Histopathology was suggestive of a neuroendocrine tumor, with immunohistochemistry positive for GHRH and negative for prolactin. Her final diagnosis was ectopic acromegaly due to GHRH secreting a lung NET with pituitary somatotroph and lactotroph pituitary hyperplasia and apoplexy in the hyperplastic pituitary. Escherichia coli is a Gram-negative bacterium and part of the intestinal microbiota. However, it can cause various diarrhoeal illnesses, i.e., traveller's diarrhoea, dysentery, and extraintestinal infections when the bacteria are translocated from the intestine to other organs, such as urinary tract infections, abdominal and pelvic infections, pneumonia, bacteraemia, and meningitis. It is also an important pathogen in intensive care units where cross-infection may cause intrahospital spread with serious consequences. Within this study, four E. coli isolates from the tracheal aspirates of a tracheotomised paediatric patient on chronic respiratory support were analysed and compared for antibiotic resistance and virulence potential. Genomes of all four isolates (5381a, 5381b, 5681, 5848) were sequenced using Oxford Nanopore Technology. According to PFGE analysis, the clones of isolates 5681 and 5848 were highly similar, and differ from 5381a and 5381b which were isolated first chronologically. All four E. coli isolates belonged to an unknown sequence type, related to the E. coli ST131, a pandemic clone that is evolving rapidly with increasing levels of antimicrobial resistance. All four E. coli isolates in this study exhibited a multidrug-resistant phenotype as, according to MIC data, they were resistant to ceftriaxone, ciprofloxacin, doxycycline, minocycline, and tetracycline. In addition, principal component analyses revealed that isolates 5681 and 5848, which were recovered later than 5381a and 5381b (two weeks and three weeks, respectively) possessed more complex antibiotic resistance genes and virulence profiles, which is concerning considering the short time period during which the strains were isolated. Cellular processes are controlled by the thermodynamics of the underlying biomolecular interactions. Frequently, structural investigations use one monomeric binding partner, while ensemble measurements of binding affinities generally yield one affinity representative of a 1:1 interaction, despite the majority of the proteome consisting of oligomeric proteins. For example, viral entry and inhibition in SARS-CoV-2 involve a trimeric spike surface protein, a dimeric angiotensin-converting enzyme 2 (ACE2) cell-surface receptor and dimeric antibodies. Here, we reveal that cooperativity correlates with infectivity and inhibition as opposed to 1:1 binding strength. We show that ACE2 oligomerizes spike more strongly for more infectious variants, while exhibiting weaker 1:1 affinity. Furthermore, we find that antibodies use induced oligomerization both as a primary inhibition mechanism and to enhance the effects of receptor-site blocking. Our results suggest that naive affinity measurements are poor predictors of potency, and introduce an antibody-based inhibition mechanism for oligomeric targets. Simple Summary: Adolescents and young adults (AYA) with cancer often fall through gaps between children’s and adults’ cancer services. They are consequently under-represented in clinical trials, and their survival is often inferior to that of children or adults with the same tumor type; in this paper, we use the example of central nervous system germ cell tumors (CNS-GCT), as a model of AYA tumor to illustrate this challenge. We describe how we have built bridges between pediatric and adult oncology, how this can apply to other types of brain tumors, and discuss ways to promote cancer care in the AYA population.Adolescents and young adults (AYA) with cancer are under-represented in clinical trials and have thus not benefited from the same improvement in outcomes as either younger or older patients. Central nervous system germ cell tumors (CNS-GCT) represent an ideal model of AYA tumor as their incidence peaks during adolescence and young adulthood. Since the early 90’s, SIOP (International Society of Pediatric Oncology) has launched two successive European trials: SIOP CNS-GCT96 (January 1996 to December 2005) and SIOP CNS-GCTII protocols (October 2011 to July 2018), for CNS-GCTs. With the removal of the upper age limit in the SIOP CNS-GCTII trial, and closer collaboration between pediatric and adult oncologists within AYA multidisciplinary tumor boards, the proportion of adults enrolled in France has dramatically increased over time. The current article will use the example of CNS-GCT to illustrate how to build a bridge between pediatric and adult oncology, how this can apply to other types of brain tumors, and how to promote cancer care in the AYA population.

We recently reported that atypical teratoid rhabdoid tumors (ATRTs) comprise at least two transcriptional subtypes with different clinical outcomes; however, the mechanisms underlying therapeutic heterogeneity remained unclear. In this study, we analyzed 191 primary ATRTs and 10 ATRT cell lines to define the genomic and epigenomic landscape of ATRTs and identify subgroup-specific therapeutic targets. We found ATRTs segregated into three epigenetic subgroups with distinct genomic profiles, SMARCB1 genotypes, and chromatin landscape that correlated with differential cellular responses to a panel of signaling and epigenetic inhibitors. Significantly, we discovered that differential methylation of a PDGFRB-associated enhancer confers specific sensitivity of group 2 ATRT cells to dasatinib and nilotinib, and suggest that these are promising therapies for this highly lethal ATRT subtype.

BackgroundPatients with incomplete surgical resection of medulloblastoma are controversially regarded as having a marker of high-risk disease, which leads to patients undergoing aggressive surgical resections, so-called second-look surgeries, and intensified chemoradiotherapy. All previous studies assessing the clinical importance of extent of resection have not accounted for molecular subgroup. We analysed the prognostic value of extent of resection in a subgroup-specific manner.MethodsWe retrospectively identified patients who had a histological diagnosis of medulloblastoma and complete data about extent of resection and survival from centres participating in the Medulloblastoma Advanced Genomics International Consortium. We collected from resections done between April, 1997, and February, 2013, at 35 international institutions. We established medulloblastoma subgroup affiliation by gene expression profiling on frozen or formalin-fixed paraffin-embedded tissues. We classified extent of resection on the basis of postoperative imaging as gross total resection (no residual tumour), near-total resection (30 Gy vs no craniospinal irradiation). The primary analysis outcome was the effect of extent of resection by molecular subgroup and the effects of other clinical variables on overall and progression-free survival.FindingsWe included 787 patients with medulloblastoma (86 with WNT tumours, 242 with SHH tumours, 163 with group 3 tumours, and 296 with group 4 tumours) in our multivariable Cox models of progression-free and overall survival. We found that the prognostic benefit of increased extent of resection for patients with medulloblastoma is attenuated after molecular subgroup affiliation is taken into account. We identified a progression-free survival benefit for gross total resection over sub-total resection (hazard ratio [HR] 1·45, 95% CI 1·07-1·96, p=0·16) but no overall survival benefit (HR 1·23, 0·87-1·72, p=0·24). We saw no progression-free survival or overall survival benefit for gross total resection compared with near-total resection (HR 1·05, 0·71-1·53, p=0·8158 for progression-free survival and HR 1·14, 0·75-1·72, p=0·55 for overall survival). No significant survival benefit existed for greater extent of resection for patients with WNT, SHH, or group 3 tumours (HR 1·03, 0·67-1·58, p=0·89 for sub-total resection vs gross total resection). For patients with group 4 tumours, gross total resection conferred a benefit to progression-free survival compared with sub-total resection (HR 1·97, 1·22-3·17, p=0·0056), especially for those with metastatic disease (HR 2·22, 1·00-4·93, p=0·050). However, gross total resection had no effect on overall survival compared with sub-total resection in patients with group 4 tumours (HR 1·67, 0·93-2·99, p=0·084).InterpretationThe prognostic benefit of increased extent of resection for patients with medulloblastoma is attenuated after molecular subgroup affiliation is taken into account. This research aimed to determine the differences between the junior (U20) water polo players of national teams of Montenegro, and Australia regarding their anthropometric characteristics and body composition. The first sub-sample of the subjects consisted of 18 water polo players of the Montenegrin national team, who occupied the sixth position on the FINA World Men’s Junior Water Polo Championship in Kuwait 2019. The second sub-sample consisted of 13 water polo players of the Australian national team, who occupied the eleventh position on the championship. The players were tested at the final preparations just before the World Championship. Anthropometric characteristics were evaluated using a battery of eight tests: body height, body weight, triceps skinfold, biceps skinfold, skinfold of the back, abdominal skinfold, upper-leg skinfold, lower leg skinfold; body composition was evaluated using a battery of three tests: body mass index, fat percentage, and muscle mass. The results of the t-test revealed that the water polo players of the two national teams do not have statistically significant differences in the variables. The results obtained in this research showed average team values for the estimated variables of water polo players of the two national teams (U20) who participated in the World Championship. That can serve as possible model parameters for all teams who want to participate in the U20 water polo World Championship. Differences in classification among water polo players may be related to the motor abilities, as well as technical and tactical parameters, as their anthropometric parameters were found to be similar.

Concentrated milk systems can be unstable during heat treatment. In this work, we studied the structural changes in the proteins in skim milk (9, 17 and 25% solids) as a result of pH adjustment to pH 7.5 and heating at 75, 85, 95 100, 110 and 121 °C by Fourier transform infrared spectroscopy (FTIR) and SDS-PAGE. The pH adjustment to 7.5 resulted in some dissociation of caseins to serum, predominately in the 9% total solids samples. Furthermore, pH adjustment resulted in an increase in random structures at all concentration levels, due to protein unfolding. Heat treatment affected greatly the protein composition in the serum phase and induced structural modifications. All milk samples when heated to 110°C exhibited loss of serum β-LG, α-LA and caseins (mainly κ-casein) as a result of heat induced aggregation. The intensity of aggregation increased with concentration factor. The trend appeared similar at 121°C but with larger aggregates. As a concentration factor increased, covalent bonds among newly formed structures appeared to prevail.

Abstract The purpose of this study was to analyze the impact of copy number variations (CNV) on sporadic pituitary neuroendocrine tumors (PitNETs) prognosis, to identify specific prognosis markers according to the known clinico-pathological classification. CGH array analysis was performed on 195 fresh-frozen PitNETs (56 gonadotroph, 11 immunonegative, 56 somatotroph, 39 lactotroph and 33 corticotroph), with 5 years post-surgery follow-up (124 recurrences), classified according to the five-tiered grading classification (invasion, Ki-67, mitotic index and p53 positivity). Effect of alterations on recurrence was studied using logistic regression models. Transcriptomic analysis of 32 lactotroph tumors was performed. The quantity of CNV was dependent on tumor type: higher in lactotroph (median(min–max) = 38% (0–97) of probes) compared to corticotroph (11% (0–77)), somatotroph (5% (0–99)), gonadotroph (0% (0–10)) and immunonegative tumors (0% (0–17). It was not predictive of recurrence in the whole cohort. In lactotroph tumors, genome instability, especially quantity of gains, significantly predicted recurrence independently of invasion and proliferation (p-value = 0.02, OR = 1.2). However, no specific CNV was found as a prognostic marker. Transcriptomic analysis of the genes included in the CNV and associated with prognosis didn’t show significantly overrepresented pathway. In somatotroph and corticotroph tumors, USP8 and GNAS mutations were not associated with genome disruption or recurrence respectively. To conclude, CGH array analysis showed genome instability was dependent on PitNET type. Lactotroph tumors were highly altered and the quantity of altered genome was associated with poorer prognosis though the mechanism is unclear, whereas gonadotroph and immunonegative tumors showed the same ‘quiet’ profile, leaving the mechanism underlying tumorigenesis open to question.

The players were tested at the final preparations just before the World Championship in Kuwait in 2019. Anthropometric characteristics and body composition were evaluated using a battery of 11 variables: body height, body weight, triceps skinfold, biceps skinfold, skinfold of the back, abdominal skinfold, upper-leg skinfold, lower leg skinfold, body mass index, fat percentage, and muscle mass. Based on the results, it can be stated that the values of all the variables are highly similar to all of the water polo players of these two countries. The t-test found that the water polo players of the two national teams do not have statistically significant differences in the variables for assessing anthropometric characteristics and body composition. The results obtained in this research can serve as model parameters for the estimated variables for all water polo players (U20) in the World , because the players that have been analysed were the best and the most successful water polo players in their countries, and participants in the World Championship in Kuwait 2019.

Background and Study Aim: The aim of this research was to determine the differences between the young soccer players (U19) in terms of anthropometric characteristics and body composition. We considered players of three the most successful clubs in three countries of the southern region of the Balkan Peninsula. Material and Methods: The first sub-sample of the subjects was consisted of 19 players of FC Radnicki from Serbia of the average age 17.84±.37. The second sub-sample was consisted of 19 players of FC Sarajevo from Bosnia and Herzegovina of the average age of 17.53±.51. The last sub-sample of the examinees was consisted of 13 players of FC Shkupi from North Macedonia of the average age 18.00±.00. Anthropometric characteristics in the body composition were evaluated by a battery of 11 variables. The significance of the differences between the young soccer players in the anthropometric characteristics and variables for assessing body composition were determined by ANOVA and LSD Post Hoc test. Results: The young soccer players of the three mentioned clubs have statistically significant differences by the two variables that estimate fat percentage and muscle mass. The young soccer players of FC Radnicki are significantly better in variable fat percentage than other players. The young soccer players of FC Skhupi have significantly less muscle mass than other players. Conclusions: The results can be useful for coaches of other clubs for making a comparison of their soccer players with the young soccer players in this research.

Abstract Background A pyogenic liver abscess (PLA) represents a pus-filled cavity within the liver parenchyma caused by the invasion and multiplication of bacteria. The most common offender isolated from the PLA in children is Staphylococcus aureus. Abiotrophia defectiva is a Gram-positive pleomorphic bacterium, commonly found in the oral cavity, intestinal, and genitourinary mucosa as part of the normal microbiota. It has been proven to be an etiological factor in various infections, but rarely in cases of PLA. The case presented here is, to the best of our knowledge, the first pediatric case of PLA caused by A. defectiva. Case presentation A 13-year-old Caucasian boy presented with a two-day history of abdominal pain, fever up to 40 °C, and polyuria. Contrast-enhanced computed tomography (CT) scan revealed a single, multiloculated liver lesion, suggestive of a liver abscess. The boy had sustained a bicycle handlebar injury to his upper abdomen 3 weeks before the symptoms appeared and had been completely asymptomatic until 2 days before admission. He was successfully treated with antibiotic therapy and open surgical drainage. A. defectiva was isolated from the abscess material. Histopathology report described the lesion as a chronic PLA. Conclusions A. defectiva is a highly uncommon cause of liver abscess in children. In such cases, various predisposing factors should be considered, including antecedent blunt abdominal trauma.

Abstract New security systems, methods or techniques need to have their performance evaluated in conditions that closely resemble a real-life situation. The effectiveness with which individual identity can be predicted in different scenarios can benefit from seeking a broad base of identity evidence. Many approaches to the implementation of biometric-based identification systems are possible, and different configurations are likely to generate significantly different operational characteristics. The choice of implementational structure is, therefore, very dependent on the performance criteria, which is most important in any particular task scenario. The issue of improving performance can be addressed in many ways, but system configurations based on integrating different information sources are widely adopted in order to achieve this. Thus, understanding how each data information can influence performance is very important. The use of similar modalities may imply that we can use the same features. However, there is no indication that very similar (such as keyboard and touch keystroke dynamics, for example) basic biometrics will perform well using the same set of features. In this paper, we will evaluate the merits of using a three-modal hand-based biometric database for user prediction focusing on feature selection as the main investigation point. To the best of our knowledge, this is the first thought-out analysis of a database with three modalities that were collected from the same users, containing keyboard keystroke, touch keystroke and handwritten signature. First, we will investigate how the keystroke modalities perform, and then, we will add the signature in order to understand if there is any improvement in the results. We have used a wide range of techniques for feature selection that includes filters and wrappers (genetic algorithms), and we have validated our findings using a clustering technique.

The goal of this research was to assess the quantitative and qualitative changes imposed by training and competition with help of selected variables of morphological characteristics and body composition which were measured in elite football players of First Montenegrin Telecom League at two different time points during the competition period. In this study 28 male football players (22.5±5.87 yrs.) of OFK Titograd from Podgorica who compete in the First Montenegrin Telecom League were enrolled. The measurements were carried out in January and then again, after 30 days, in February. Morphological characteristics in the body composition were evaluated by a battery of 11 variables: body height, body weight, waist circumference, triceps skinfold, biceps skinfold, back skinfold, abdominal skinfold, body mass index, fat percentage and muscle mass. The differences in morphological characteristics and the composition of the body in two periods during the competition period were determined by using a discriminatory parametric procedure with t-test for small independent samples. It was found that in football players of OFK Titograd in 30 days significant changes are observed for 3 variables, for upper arm skinfolf - triceps, back skinfold and abdomen skinfold.

The aim of this study is to observe the trend of anthropometric characteristics in Montenegrin recruits born from 1957 to 1969 from Bar. The sample of respondents in this study numbered a total of 3,670 future soldiers with an average age of 18.19±0.69, who were tested for military service in the period from 20 February 1979 to 21 April 1987. The average body height of the total sample of future soldiers was 175.82±7.04, the average body weight was 68.33±9.73, and the average value of body mass index was 22.06±2.58. The highest body height (178.06±8.67) was in the group of recruits born in 1960. The highest body weight (75.14±5.87) and body mass index (23.96±1.84) was in the group of recruits born in 1957. The value of the body mass index of all future soldiers shows that everyone was in the zone of normal weight. It should be noted that some of these respondents were measured before the age of 18. This means that their growth was not complete. There is a possibility that they on average had a higher body height with the completion of growth and development than this research shows. The results show that all generations of young men from Bar were at that time of a normal weight. This fact is not surprising because it is known that life was different then than it is now. The results of this study are very important in monitoring the trend of these 3 variables in the Montenegrin population, but they also have some limitations. As mentioned above, the rule for recruiting future soldiers was to be tested before the age of 18, when growth and development were not complete yet, and the results after the completion of the growth and development of the respondents would probably be slightly different from these results. Montenegrin magazine for sports science and medicine “Montenegrin Journal of Sports Science and Medicine” (MJSSM), is the only magazine in Montenegro with the index in two world databases: Web of Science and Scopus. Articles published in this magazine stem from different fields of sports science: biomechanics, physiology, sports medicine, anthropology, methodology and other fields related to sports. In the previous three editions of this magazine, 30 articles have been published which are classified according to the following science fields: Sports Medicine, Sports Training, Anthropology of Sport; Sports Psychology; Sports Physiology; Systematic Analysis of Sports Articles. The analysis revealed that the articles in the field of Sports Training are the most numerous, therefore the most cited works as well as the best positioned. Sports Training is an integral part of a triage technology which enables an athlete’s transformation processes of certain segments of the anthropological status as well as an improvement of technical and tactical elements which ultimately lead to the realization of the top sports achievements. Also, results published in the magazine “Montenegrin Journal of Sports Science and Medicine” in the field of Sports training could be beneficial in monitoring athletes, notifying corrections during training methods, estimating and improving capability differences among athletes from different sports orientations.

The goal of this research was to determine the differences in the morphological characteristics and body composition among the top football players of the two clubs, Bosnia and Herzegovinian football club Zrinjski Mostar and Kosovian football club Besa Peje. A sample of 46 subjects was divided into two sub-samples. The first sub-sample of the subjects consisted of 28 players of CSC Zrinjski Mostar of the average age 24.36±4.14, the champion of the Bosnia and Herzegovina in the season 2016/17, while the other sub-sample consisted of 18 players of FC Besa Peje of the average age of 21.83±3.17, the winner of Kosovo Cup in the season 2016/17. Football players were tested immediately after the end of the competition season 2016/17. Morphological characteristics in the body composition were evaluated by a battery of 11 variables: body height, body weight, waist circumference, triceps skinfold, biceps skinfold, back skinfold, abdominal skinfold, body mass index, fat percentage and muscle mass. The differences between the players of the top two football clubs in the morphological characteristics and variables for assessing body composition was determined by a t-test for independent samples. It was found that the football players of CSC Zrinjski and FC Besa Peje have statistically significant differences by the three variables that estimate the body weight, waist circumference and muscle mass.
