Abstract: Background: Atrial fibrillation is associated with higher mortality. Identification of causes of death and contemporary risk factors for all‐cause mortality may guide interventions. Methods and Results: In the Rivaroxaban Once Daily Oral Direct Factor Xa Inhibition Compared with Vitamin K Antagonism for Prevention of Stroke and Embolism Trial in Atrial Fibrillation (ROCKET AF) study, patients with nonvalvular atrial fibrillation were randomized to rivaroxaban or dose‐adjusted warfarin. Cox proportional hazards regression with backward elimination identified factors at randomization that were independently associated with all‐cause mortality in the 14 171 participants in the intention‐to‐treat population. The median age was 73 years, and the mean CHADS 2 score was 3.5. Over 1.9 years of median follow‐up, 1214 (8.6%) patients died. Kaplan–Meier mortality rates were 4.2% at 1 year and 8.9% at 2 years. The majority of classified deaths (1081) were cardiovascular (72%), whereas only 6% were nonhemorrhagic stroke or systemic embolism. No significant difference in all‐cause mortality was observed between the rivaroxaban and warfarin arms (P=0.15). Heart failure (hazard ratio 1.51, 95% CI 1.33–1.70, P<0.0001) and age ≥75 years (hazard ratio 1.69, 95% CI 1.51–1.90, P<0.0001) were associated with higher all‐cause mortality. Multiple additional characteristics were independently associated with higher mortality, with decreasing creatinine clearance, chronic obstructive pulmonary disease, male sex, peripheral vascular disease, and diabetes being among the most strongly associated (model C‐index 0.677). Conclusions: In a large population of patients anticoagulated for nonvalvular atrial fibrillation, ≈7 in 10 deaths were cardiovascular, whereas <1 in 10 deaths were caused by nonhemorrhagic stroke or systemic embolism. Optimal prevention and treatment of heart failure, renal impairment, chronic obstructive pulmonary disease, and diabetes may improve survival. Abstract: Tissue hypoxia occurs in various conditions such as myocardial or brain ischemia and infarction, sepsis, and trauma, and induces cellular damage and tissue remodeling with recapitulation of fetal-like reprogramming, which eventually results in organ failure. Analogies seem to exist between the damaged hypoxic and developing organs, indicating that a regulatory network which drives embryonic organ development may control aspects of heart (or tissue) repair. In this context, thyroid hormone (TH), which is a critical regulator of organ maturation, physiologic angiogenesis, and mitochondrial biogenesis during fetal development, may be of important physiological relevance upon stress (hypoxia)-induced fetal reprogramming. TH signaling has been implicated in hypoxic tissue remodeling after myocardial infarction and T3 prevents remodeling of the postinfarcted heart. Similarly, preliminary experimental evidence suggests that T3 can prevent early tissue hypoxia during sepsis with important physiological consequences. Thus, based on common pathways between different paradigms, we propose a possible role of TH in tissue hypoxia after sepsis with the potential to reduce secondary organ failure. 307. Floating mass in the aortic arch: An interesting case report.

Abstract: Abstract: The finding of a floating mass in the aortic arch is rare and the management remains controversial. The gold standard for visualization of atheromatic plaque is computed tomography scanning. However, in some patients transthoracic harmonic imaging from a suprasternal window can reliably visualize protruding aortic arch atheromas, thus making an excellent screening test. We report a case of a ruptured floating atheromatous aortic plaque in a 77-year-old man with severe aortic valve stenosis and known coronary artery disease presenting with progressing dyspnea. Both procedures, atheromatic plaque removal and aortic valve replacement, were undertaken during the same surgical operation, which was completed with success and the patient was discharged in a good state of health. Abstract: One of the major concerns in attitude control of Kinetic-EnergyWeapon (KEW) vehicles proposed for exoatmospheric engagements is that some error sources such as vehicle center of gravity (CG) variations and thrust misalignments of divert thrusters may induce excessive parasitic torques. These errors may result in significant attitude perturbations which are undesirable for target tracking of strapdown seekers. In an effort to identify these error sources in a quantitative way, it has been proposed to explicitly estimate them by using the data obtained from KineticKill-Vehicle Hover Interceptor Test (KHIT) flights. An estimation procedure based on Extended Kalman Filtering (EKF) is developed for the post-flight data reduction that includes the estimation of CG location and thrust misalignments. Some modifications in EKF algorithms are made to reduce the sensitivity of the estimator to uncertainties in divert thrust timing. The proposed estimation scheme is then applied for processing simulated flight data and actual test data. The data reduction results are provided in this paper.

(ΠΔ 96/2015/ΦΕΚ 163Α'/20,08,2014) Περίληψη: Τα σύγχρονα οπλικά συστήματα αποτελούν πολύπλοκα συναρμολογήματα επιμέρους δομικών στοιχείων. Παρά τις εξελίξεις στον τομέα της πολεμικής βιομηχανίας παρατηρούνται πολλές φορές αστοχίες σε δομικά στοιχεία, ικανές να καταστήσουν ένα σύστημα μη λειτουργικό και να θέσουν σε κίνδυνο τόσο την αποστολή όσο και το προσωπικό. Η παρούσα μεταπτυχιακή διατριβή έχει ως θέμα την ανάλυση αστοχιών οπλικών συστημάτων (failure analysis of military systems). Πραγματεύεται δηλαδή, το σύνολο των τεχνικών, των μεθόδων και των κριτηρίων ελέγχου που ένας μηχανικός πρέπει να εφαρμόσει προκειμένου να εξακριβώσει «το πως και το γιατί» ένα σύστημα, υποσύστημα, συγκρότημα ή εξάρτημα σταμάτησε να επιτελεί την λειτουργία για την οποία έχει κατασκευαστεί. Γενικά, η ανάλυση αστοχίας ενός συστήματος μπορεί να πραγματοποιηθεί είτε στο στάδιο της σχεδίασης και κατασκευής του, είτε αφού αυτό έχει τεθεί σε λειτουργία και τελικά αστοχήσει σε κάποια από τις φάσεις τις ζωής του. Στην πρώτη περίπτωση, η ανάλυση αστοχίας έχει σκοπό τον έλεγχο της αξιοπιστίας του συστήματος και την πρόβλεψη μελλοντικών αστοχιών, μειώνοντας έτσι τον κίνδυνο πρόκλησης ατυχημάτων σε προσωπικό, την πιθανότητα απώλειας υλικού ή μόλυνσης του περιβάλλοντος, καθώς και το κόστος των επισκευών. Στη δεύτερη περίπτωση αντίστοιχα, η ανάλυση αστοχίας διεξάγεται με σκοπό την εύρεση των αιτιών και των παραγόντων που οδήγησαν ένα σύστημα σε αστοχία, καθώς και τον καθορισμό των απαιτούμενων ενεργειών θεραπείας - επισκευής. Η παρούσα μεταπτυχιακή διατριβή επικεντρώνεται στη δεύτερη κατηγορία ανάλυσης αστοχιών, αναπτύσσοντας μια γενική μεθοδολογία. Είναι σκόπιμο να αναφερθεί ότι, όταν μιλάμε για «σύστημα» εννοούμε: «ένα σύνολο αλληλένδετων εξαρτημάτων/δομικών στοιχείων, τα οποία αλληλεπιδρούν μεταξύ τους κατά έναν οργανωμένο τρόπο προς εκπλήρωση ενός κοινού σκοπού. Τα δομικά στοιχεία ενός συστήματος μπορεί να είναι εντελώς διαφορετικά μεταξύ τους, αποτελούμενα από ανθρώπους, οργανισμούς, διαδικασίες, εξοπλισμό (hardware), λογισμικό (software) και εγκαταστάσεις» (Δάρας, 2014). Στην παρούσα μεταπτυχιακή διατριβή ο όρος «δομικό στοιχείο» θα χρησιμοποιείται για να υποδηλώσει οποιοδήποτε μέρος του κυρίου οπλικού συστήματος, όπως επιμέρους συστήματα, υποσυστήματα, υποσυγκροτήματα και εξαρτήματα. Η περιστασιακή χρήση του όρου «κατασκευή» θα υποδηλώνει την έννοια του οπλικού συστήματος. Ο όρος «αστοχία» (failure) υποδηλώνει την οποιαδήποτε αλλαγή που συμβεί σε ένα δομικό στοιχείο συστήματος ή διαδικασίας και έχει ως αποτέλεσμα τη μείωση της ικανότητάς του να εκτελεί ορθά μία προδιαγεγραμμένη λειτουργία για την οποία έχει σχεδιαστεί και κατασκευαστεί. Από όλα τα δομικά στοιχεία ενός οπλικού συστήματος, η διατριβή αυτή επικεντρώνεται περισσότερο στην αστοχία του υλικού μέρους αυτού (hardware), κάνοντας ωστόσο μια μικρή αναφορά σε αστοχίες του λογισμικού μέρους (software). Επιπλέον, από το σύνολο του υλικού μέρους ενός συστήματος θα αναλυθούν περισσότερο οι αστοχίες των μηχανικών - μηχανολογικών μερών αυτού, αναφέροντας ωστόσο εν συντομία αστοχίες ηλεκτρονικού - ηλεκτρολογικού εξοπλισμού και αναλύοντας μια περίπτωση αστοχίας ηλεκτρονικού εξαρτήματος. Όπως είναι γνωστό, το υλικό μέρος ενός συστήματος κατασκευάζεται από μεταλλικά ή μη μεταλλικά υλικά (πολυμερή, σύνθετα). Η επιλογή του καταλληλότερου υλικού για την κατασκευή ενός δομικού στοιχείου εξαρτάται από μια σειρά λειτουργικών απαιτήσεων που πρέπει να ικανοποιήσει. Επιπλέον, λαμβάνονται υπόψη οι φυσικές και μηχανικές ιδιότητες του υλικού, η σύνθεσή του, οι ιδιότητες της επιφάνειάς του και η μικροδομή του. Όσον αφορά τον τρόπο αστοχίας του υλικού, αυτός εκδηλώνεται διαφορετικά στα μέταλλα απ’ ότι στα σύνθετα ή τα πολυμερή υλικά. Ωστόσο, υπάρχουν κάποιοι μηχανισμοί αστοχίας οι οποίοι είναι κοινοί και στους δύο τύπους υλικών (μεταλλικά και μη). Οι κυριότεροι τρόποι αστοχίας που παρατηρούνται στα μεταλλικά υλικά είναι η κόπωση (fatigue), η θραύση (fracture), η διάβρωση (corrosion), η φθορά (wear), η παραμόρφωση (distortion) ή συνδυασμός των ανωτέρω. Η διατριβή αυτή επικεντρώνεται στα μεταλλικά υλικά, αναφέροντας περιληπτικά και τους τρόπους αστοχίας των σύνθετων υλικών. Οι αστοχίες γενικά μπορεί να οφείλονται σε σχεδιαστικά ή κατασκευαστικά σφάλματα, στον τρόπο χειρισμού του συστήματος, στις συνθήκες λειτουργίας του, στο επίπεδο συντήρησης και επισκευών κ.α. Για την εξακρίβωση των πιθανών αιτιών αστοχίας ενός δομικού στοιχείου χρησιμοποιείται μια σειρά καταστρεπτικών και μη τεχνικών ελέγχου. Στην πρώτη περίπτωση ο έλεγχος πραγματοποιείται χωρίς την πρόκληση καταστροφής στο στοιχείο, ενώ στη δεύτερη περίπτωση απαιτείται η λήψη δείγματος (δοκιμίου) από αυτό για την εκτέλεση περαιτέρω εργαστηριακών ελέγχων. Επιπλέον, η διεξαγωγή μιας ανάλυσης αστοχιών πολυσύνθετου συστήματος, όπως είναι τα οπλικά, απαιτεί τη συνεργασία πολλών επιστημόνων και μηχανικών, προκειμένου το αποτέλεσμα αυτής να είναι γρήγορο και αξιόπιστο. Ο αναλυτής συστημάτων θα πρέπει να είναι πλήρως καταρτισμένος στον τομέα του και να διακατέχεται από ήθος και αρχές. Τέλος, με τη διατριβή αυτή συμπεραίνεται ότι κάθε αστοχία είναι μοναδική και πρέπει να αντιμετωπίζεται ως τέτοια. Ακολουθώντας μια συστηματική διαδικασία και μεθοδολογία, προσαρμοσμένη ανά περίπτωση και εκτελώντας μια σειρά από κατάλληλους τεχνικούς ελέγχους (NDT - DT) μπορεί η ομάδα ανάλυσης να καταλήξει σε ένα αξιόπιστο συμπέρασμα για τα αίτια της αστοχίας. Έτσι, η ανάλυση αστοχίας θα μπορεί να αποτελεί εργαλείο για την ορθή επιδιόρθωση της αστοχίας, τη λήψη προληπτικών μέτρων προς αποφυγή μελλοντικών αστοχιών, καθώς και για την αποκατάσταση της αλήθειας μέσω δικαστικών ή διοικητικών διαδικασιών σε περιπτώσεις απώλειας ζωής ή υγείας. Ανάπτυξη της μεταπτυχιακής διατριβής Το 1ο Κεφάλαιο αποτελεί ένα εισαγωγικό μέρος της μεταπτυχιακής διατριβής όπου αναφέρονται ορισμένα ιστορικά παραδείγματα αστοχίας οπλικών συστημάτων, ενώ γίνεται μια σύντομη ιστορική αναδρομή σε κριτήρια αστοχίας υλικών. Στο 2ο Κεφάλαιο περιγράφεται η διαδικασία ανάπτυξης ενός οπλικού συστήματος, μιας και οι περισσότερες από τις αστοχίες που παρατηρούνται στην πρώιμη φάση της ζωής του συστήματος οφείλονται σε σχεδιαστικά και κατασκευαστικά λάθη. Συγκεκριμένα, αναλύεται η έννοια του συστήματος, βασικά στάδια της σχεδίασης και κατασκευής αυτού, η διάρκεια ζωής του, καθώς και η ανοχή του σε ζημία (damage tolerance). Επιπλέον, αναφέρονται εν συντομία οι κύριες κατηγορίες υλικών που χρησιμοποιούνται σε οπλικά συστήματα και ο τρόπος επεξεργασίας και κατεργασίας αυτών. Στη συνέχεια, το 3ο Κεφάλαιο περιλαμβάνει βασικές έννοιες της Μηχανικής των Θραύσεων - Αντοχής των Υλικών, όπως οι έννοιες της παραμόρφωσης, της θραύσης, της σκληρότητας, της δυσθραυστότητας, του ερπυσμού, της κόπωσης, τρόποι διάδοσης της ρωγμής κ.α. Επιπλέον, γίνεται μια απλή αναφορά σε κριτήρια αστοχίας υλικών, ως συμπλήρωμα της ιστορικής αναδρομής των κριτηρίων αυτών στην εισαγωγή της διατριβής. Στο 4ο Κεφάλαιο περιγράφεται η διαδικασία ανάλυσης αστοχιών, περιλαμβάνοντας διάφορες τεχνικές και μεθοδολογίες. Αρχικά γίνεται αναφορά στα είδη και τους τύπους αστοχιών που παρατηρούνται στο υλικό μέρος ενός συστήματος (hardware - μηχανικό, ηλεκτρονικό, ηλεκτρολογικό), αλλά και στο λογισμικό (software), με έμφαση τις μηχανικές αστοχίες. Επίσης, αναλύονται οι τρόποι αστοχίας ενός δομικού στοιχείου με εμβάθυνση σε αυτούς που εμφανίζονται στα μεταλλικά υλικά, όπως μη αναμενόμενη κόπωση, διάβρωση, φθορά, υπερφόρτωση, παραμόρφωση κ.α., αλλά και τα σύνθετα. Επιπλέον, περιγράφονται οι βασικές μέθοδοι και τεχνικές ελέγχου αστοχίας υλικού (καταστροφικές και μη) σε θεωρητικό επίπεδο, καθώς και χρήσιμα εργαλεία - όργανα που πρέπει ένας αναλυτής να έχει για εκτέλεση εργασιών πεδίου. Τέλος, στο κεφάλαιο αυτό αναφέρονται και τα βασικά στοιχεία που πρέπει να περιλαμβάνει η τελική αναφορά αστοχίας, καθώς και τα συνήθη σφάλματα στα οποία μπορεί να υποπέσει ένας αναλυτής αστοχιών. Στο 5ο Κεφάλαιο περιγράφονται κάποιες πραγματικές περιπτώσεις αστοχίας δομικών στοιχείων οπλικών συστημάτων (case studies), στις οποίες αποδεικνύεται ο τρόπος αστοχίας και τα πιθανότερα αίτια που οδήγησαν σε αυτή. Τέλος, παρατίθενται η βιβλιογραφία - ιστότοποι, ο επίλογος, παραρτήματα, πίνακας συντμήσεων και κατάλογος σχημάτων και πινάκων. Summarization: Although today’s weapon systems are becoming more sophisticated and able to perform complicated functions, they are also more complex, consisted of numerous components and thus, usually more susceptible to failures. In order to avoid potential failures, quality control and assurance activities are being carried out during the stage of design and manufacturing. But, these predictive and preventive measures cannot eliminate the presence of systems’ failures. This thesis addresses the failures of weapon systems that may occur during their life-cycle and deals with the set of techniques and methods that must be systematically carried out by an engineer in order to confirm “HOW and WHY” a system does not perform its intended function. The objective of failure analysis is to identify the causes and the factors of the failure by analyzing failure mechanisms and modes like overstress, fracture, fatigue, corrosion, wear, deformation, etc and in addition to suggest corrective actions. System failures can be induced by component failures or they can occur as a result of complex component and subsystem interactions, without any parts failing. As far as early failures is concerned, they are usually caused due to manufacturing defects, assembly or implementation faults, design errors, etc, whereas the chance and wear-out failures are generally caused by chance factors (overstresses, inadequate use) and due to wear and lack of maintenance respectively. Moreover, a failure can be occurred in the hardware or in the software part of the system. With regard to hardware failures, they can be observed in the mechanical, electrical, electromechanical or electronic parts of it. This thesis focuses on the hardware failures and especially on mechanical failures. In addition, components can be manufactured by metallic or non metallic materials (composites, polymeric) which perform different failure modes. This thesis deals mainly with metallic materials failures, while it also provides a brief discussion of composites’ failure modes and a respective case study. Finding the root causes of systems failures is far more difficult. Several techniques and methods are used in failure analysis investigation to find out the cause of failure, depending on the severity and type of problem. These techniques range from simple optical macroscopic observations (visual and obvious manifestations) to examinations of samples under a microscope or carrying out other mechanical or chemical analysis. This thesis came to the conclusion that each failure is different and it has to be addressed as an unique incident. Only with a systematic failure analysis which deploys all the available techniques and methods (NDT –DT) depending on the type of component failure and the prevailing circumstances, the analyst can identify the causes and the factors that made the system incompetent to function properly. Failure analysis can provide valuable feedback to the product manufacturers and operators for improving design, manufacturing or even maintenance faults that might have been overlooked. The thesis is divided in five chapters, as follows: Chapter 1: “Introduction”: outlines this thesis and provides some historical examples of military systems’ failures as well as a brief look at criteria of failure throughout the years. Chapter 2: “Development of a system”: discusses the concept of “system”, the stages of its life-cycle, the types of the materials that are used in military systems and the respective treatments that are used in order to obtain desirable properties. Chapter 3: “Basic concepts of Engineering Fracture Mechanisms”: provides a basic background of fracture mechanics and failure criteria. Chapter 4: “Failure Analysis of Systems”: examines the failure analysis purpose and procedure, the fundamental causes and factors of failures, the basic failure modes and mechanisms and finally the basic investigation techniques. Chapter 5: “Case Studies of Military Systems’ Failures”: examines and discusses eight (8) examples of real-world failures of interest and their respective failure analysis. Finally, the Epilogue summarizes the thesis conclusion followed by References and some useful appendices, whereas lists of images and matrices included.

Abstract: The aim of this preliminary study was to investigate the contributions of knowledge and information to the prediction of students' intention to play badminton. The sample, 121 students (53 men and 68 women) 18–25 years of age, were beginning students in a semester badminton course. A questionnaire was completed before and after the course (4 mo.). Hierarchical regression analyses showed strong association between the examined variables of the Planned Behavior Questionnaire, specifically signifying that knowledge, information, attitudes, subjective norms, perceived behavioral control, role identity, and attitude strength could account for anticipation of intention towards playing badminton. Overall, systematic access to information and knowledge appeared to accompany relatively greater intentions.

Abstract: Significant evidence shows that elevated heart rate (HR) is an independent risk factor in patients with coronary artery disease (CAD) and influences their prognosis. In addition, patients with chronic obstructive pulmonary disease (COPD) have more frequent episodes of angina and their compliance with heart rate agents, such as beta blockers, is poor. The purpose of the multicenter observational RYTHMOS study was to evaluate the role of heart rate management in the prognosis and quality of life in patients with CAD and COPD.Baseline data from 280 patients, enrolled in 22 hospitals representing all types of hospital and all geographical areas of the country, were analyzed. All patients had either a prior myocardial infarction or angiographically documented CAD, and COPD verified either after spirometry or from a clinical evaluation by pulmonologists.The mean age of the enrolled patients was 71.8 ± 9.3 years, 76% were males, mean body mass index was 28.6 ± 7.9 kg/m2, 76.3% had hypertension, 31% had diabetes mellitus, and 53.5% of them suffered from heart failure. About 31% of the patients had an angina episode the week before the enrollment and the Canadian Cardiovascular Society (CSS) classification was class I, II, III and IV in 55%, 30%, 14% and 1%, respectively. The mean resting HR was 72.5 bpm; 51% of the patients had resting HR70 bpm and 22% of them had HR80 bpm. Only 52.8% of the study patients were receiving beta-blockade (BB) therapy; they were more likely to have resting HR70 bpm (57.4% vs. 42.7%, p0.001). 16.4% of the patients were receiving ivabradine and they had a higher initial HR compared to the others (78.5 vs. 71.3, p0.001). Multivariate analysis showed that diabetes mellitus was independently associated with HR70 bpm. Patients with resting HR70 bpm had significantly more frequent angina episodes (p0.001), were less satisfied with treatment (p0.001), and had a lower quality of life (p0.001).The baseline data of this study showed that patients with CAD and COPD present inadequate HR control and frequent angina episodes. Apart from the special characteristics of these patients related to COPD management, underuse of BB therapy largely contributes to the inadequate control of HR. Patients with HR70 bpm had significantly worse quality of life.

Abstract: Direct oral anticoagulants (DOACs) are the standard treatment for thromboembolic protection in atrial fibrillation (AF) patients. Epigenetic modifications, such as DNA methylation and microRNAs, have emerged as potential biomarkers of AF. The epigenetics of DOACs is still an understudied field. It is largely unknown whether epigenetic modifications interfere with DOAC response or whether DOAC treatment induces epigenetic modifications. To fill this gap, we started the miR-CRAFT (Circulating microRNAs and DNA methylation as regulators of Direct Oral Anticoagulant Response in Atrial Fibrillation) research study. In miR-CRAFT, we follow, over time, changes in DNA methylation and microRNAs expression in naïve AF patients starting DOAC treatment. The ultimate goal of miR-CRAFT is to identify the molecular pathways epigenetically affected by DOACs, beyond the coagulation cascade, that are potentially mediating DOAC pleiotropic actions and to propose specific microRNAs as novel circulating biomarkers for DOAC therapy monitoring. 318. Exercise capacity in patients with @b-thalassemia major: Relation to left ventricular and atrial size and function

Abstract: Objectives The objective of this study was to examine the association between exercise capacity and echocardiographic indexes of atrial and ventricular function and size in patients with @b-thalassemia major. Background In patients with @b-thalassemia major, the assessment of cardiac function with echocardiography alone does not always correspond to their functional status. Peak oxygen uptake and anaerobic threshold, on the other hand, constitute 2 objective and reproducible determinants of exercise capacity in patients with heart failure. Methods and Results Forty consecutive patients (22 women and 18 men, 18 to 30 years old) who were in stable condition while receiving regular transfusions and 30 age- and sex-matched control subjects were studied. At 2 to 3 days after the last transfusion, each subject underwent complete echocardiographic study followed by cardiopulmonary exercise testing. Left atrial volumes (maximal [Vmax], at onset of atrial systolic [Vp], and minimal [Vmin]) and left ventricular volumes were measured with the biplane area-length method, and left atrial active emptying fraction (ACTEF) and left ventricular ejection (LVEF) fraction were calculated. Peak oxygen uptake (V@?o"2 max) and anaerobic threshold (AT) were also estimated. After transfusion, patients with @b-thalassemia major had reduced V@?o"2 max and AT and greater left atrial volume in comparison with control subjects. Also, ACTEF and LVEF were significantly lower in the patient group. Moreover, V@?o"2 max and AT were inversely related to Vmax (r = -0.74 and r = -0.80, respectively) and directly related to ACTEF (r = 0.85 and r = 0.82, respectively) in @b-thalassemia major, whereas they were poorly related to LVEF (r = 0.50 and r = 0.53, respectively). In the control group, V@?o"2 max and AT parameters were related to Vmax and ACTEF in a similar way to that in the @b-thalassemia group. Conclusions In patients with @b-thalassemia major, exercise capacity does not correlate with left ventricular dimensions and function. On the contrary, left atrial size and systolic dysfunction are probably predictors of decreased exercise capacity. (Am Heart J 1998;136:988-90.)

Abstract: Purpose: Epicardial adipose tissue (EAT) thickness, measured by echocardiography, is associated to the presence of coronary artery disease (CAD) and severe aortic stenosis (AS). EAT thickness is commonly referred as the diameter of the echo-free space between the right ventricular wall and the visceral layer of the pericardium in parasternal long axis view, using the aortic annulus as an anatomic landmark (EAT-1). We aimed to demonstrate that the direct measurement of the adipose tissue thickness visualized in the space between the ascending aorta and the right ventricle (EAT-2) might be considered an alternative method. Methods: We measured EAT-1 and EAT-2 in 130 pts with severe cardiac disease referred for cardiac surgery: 53 pts with isolated AS, 49 pts with severe CAD, and 28 pts with both severe AS and CAD (AS+CAD); and in 50 control subjects matched for age, sex and BMI. The two measurements were obtained at end-systole in 3 cardiac cycles (figure). Results. Both EAT-1 and EAT-2 measurements had an excellent reproducibility. With respect to controls pts had significantly increased EAT-1 (2,4 ± 0,5mm vs 6 ± 2mm; p<0,05) and EAT-2 (3 ± 1,2mm vs 12 ± 3mm; p<0,05). EAT-1 and EAT-2 were not statistically different in controls. EAT-2 was significantly higher than EAT-1 in CAD, AS, and AS+CAD pts (p<0,05). Interestingly, EAT-2, but not EAT-1, was significantly increased in AS+CAD pts with respect to EAT-2 of pts with isolated AS and isolated CAD. Conclusions: Our data demonstrate that EAT-2, as well as EAT-1, is a valuable method to measure EAT thickness. Further, EAT-2 seems to better recognize EAT increase, in pts with AS+CAD. Comprehensively, EAT-2 is greater than EAT-1. The larger space between ascending aorta and right ventricle, allowing EAT expansion, could justify our observation.

Abstract: Sepsis and septic shock result in impaired microcirculation and red blood cell rheology which lead to tissue hypoxia and multi-organ failure. Early administration of triiodothyronine prevents tissue hypoxia in experimental sepsis. In this context, a clinical trial was initiated to test the efficacy of acute triiodothyronine administration to combat tissue hypoxia in critically ill COVID19 patients. Here, we provide preliminary data from interim analysis of this study showing a novel acute effect of triiodothyronine on erythrocyte sedimentation rate which may have an important therapeutic impact on red blood cell rheology and tissue hypoxia in sepsis and particular in COVID19 critical illness. 341. Prevalence of depression and associated factors in patients hospitalized with heart failure.

Abstract: OBJECTIVE To estimate the prevalence of depression and to identify associated factors in patients hospitalized with heart failure (HF) using the Patient Health Questionnaire-9 (PHQ-9). METHOD The study sample consisted of 152 patients with HF who were hospitalized in the Department of Cardiology of a tertiary hospital in Athens, Greece over a period of one year. Data were collected using a questionnaire consisting of three parts; socio-demographic and clinical characteristics, results of laboratory and diagnostic work-up retrieved from the hospital records, and the PHQ-9 scale. RESULTS The prevalence of major depression (PHQ-9 score ≥10) was 34.2%. Among the factors examined, worse overall perceived health (OPH) (p<0.001) and New York Heart Association (NYHA) class (p<0.001) were strongly associated with major depression. Age (p=0.015) was also associated with major depression, with the younger (<60 years) and the older ≥80 years) patients being more depressed. Sleep duration (p=0.028) was associated with major depression, with those sleeping <6 hours and >8 hours being more depressed. After stepwise logistic regression, OPH, NYHA class and age remained statistically significantly associated with major depression. CONCLUSIONS Measures must be taken to identify, evaluate and manage depression in patients hospitalized with HF, the prevalence of which is particularly high in this population. 342. Triiodothyronine for the treatment of critically ill patients with COVID-19 infection: A structured summary of a study protocol for a randomised controlled trial.

Abstract: Objectives: Tissue hypoxia is the main cause of multi-organ dysfunction in sepsis. However, effective pharmacological treatments to combat sepsis-induced tissue hypoxia are not available. Emerging experimental and clinical evidence reveals an evolutionary conserved action of thyroid hormone (TH) to adapt injured tissue to hypoxic conditions via its action on p38 MAPK, Akt signaling pathways. In addition, TH has favorable effects on the immune system and viral load in infected tissue. Non-Thyroid Illness Syndrome is common in sepsis, acute myocardial infarction and trauma and is associated with increased mortality. Thus, TH may be a novel treatment in the setting of critical illness due to viral infection in which hypoxia prevails. The present study aims to address the efficacy and safety of acute administration of triiodothyronine (T3) in critically ill COVID-19 infected patients requiring mechanical respiratory support or Extra Corporeal Membrane Oxygenation (ECMO).Trial Design: This study is a phase II, parallel, 2-arm (1:1 ratio), multi-centre, prospective, randomized, double-blind, placebo controlled trial.Participants: Male and female patients aged over 18 years old who are diagnosed with pulmonary infection due to COVID-19, admitted to Intensive Care Unit and requiring mechanical ventilation or ECMO will be enrolled in this trial. Patients will be excluded in cases of pregnancy, severe systemic disease with life expectancy less than 6 months, participation in another trial of an investigational drug or device, corticosteroid and/or sympathomimetic use before initiation of treatment. All data will be collected in electronic CRF files. Participants will start to be recruited from the ICU center of "ATTIKO" University Hospital in Greece. We aim to include two more clinical sites in the trial one from Greece and one from Germany INTERVENTION AND COMPARATOR: Intervention: T3 Solution for injection 10 μg/ml. The dose administered will be 0.8g/kg i.v. bolus and will be followed by an infusion of 0.113g. kg-1.h-1 i.v. for 48 hours (therapeutic dose). After the first 48h, a maintenance dose will be administered corresponding to 50% of the therapeutic dose (0.057g. kg-1.h-1 i.v.). Drug administration will stop after successful weaning or end of follow up (maximum 30 days). Comparator: Placebo with composition and dosage identical apart from the active substance.Main Outcomes: The primary outcome assessed in the present study will be the percentage of patients successfully weaned after 30 days of follow-up. Successful weaning is defined as no requirement for ventilatory support after extubation (mechanical support) or support from ECMO for 48 hours.Randomisation: An allocation sequence to one of the groups will be prepared by the Sponsor of the study. A 1:1 treatment allocation will be adopted. An electronic CRF will be used incorporating IWRS in order to assure proper randomization and unblinding in emergency cases. The representative of the sponsor will get a copy of randomization codes. The information of the randomization codes will then be locked in the database until the time at which an interim analysis or final analysis is performed.Blinding (masking): Participants, caregivers, and all investigators assessing the outcomes will be blinded to group assignment.Numbers To Be Randomised (sample Size): The sample size of 60 patients (that indicates 30 subjects for each group) will have 84% power to detect the estimated difference between the two study groups. 344. Meta-Analysis on Risk Stratification of Asymptomatic Individuals With the Brugada Phenotype.

Abstract: The prognosis of asymptomatic subjects remains the most controversial issue in Brugada syndrome (BS). A meta-analysis on the prognostic role of spontaneous type 1 electrocardiographic (ECG) pattern and programmed ventricular stimulation (PVS) in asymptomatic subjects with Brugada electrocardiogram was performed. Current databases were searched until March 2014. Fourteen prospective observational studies were included in the present meta-analysis, accumulating data on 3,536 asymptomatic subjects (2,820 men) with BS phenotype. The mean follow-up period varied from 20 and 77 months. Data regarding 1,398 asymptomatic subjects with spontaneous type 1 ECG pattern of BS were retrieved from 6 studies. During follow-up, arrhythmic events (sustained ventricular tachycardia/fibrillation, appropriate device therapies, or arrhythmic death) occurred in 42 patients (3%). The meta-analysis of these studies demonstrated that asymptomatic subjects with spontaneous type 1 ECG pattern of BS exhibit an increased risk of future arrhythmic events (odds ratio = 3.56, 95% confidence interval 1.70 to 7.47, Z = 3.37, p = 0.0008); 1,104 asymptomatic subjects with BS ECG pattern from 12 studies underwent PVS and were available for analysis. During follow-up, arrhythmic events occurred in 36 subjects (3.3%). Inducible ventricular arrhythmias at PVS were predictive of future arrhythmic events (odds ratio = 3.51, 95% confidence interval 1.60 to 7.67, Z = 3.14, p = 0.002). In conclusion, this meta-analysis showed that asymptomatic subjects with either spontaneous diagnostic ECG pattern or inducible ventricular arrhythmias at PVS are at increased risk. Abstract: Abstract: Background: Patients with chronic heart failure (HF) are characterized by alterations in matrix metalloproteinases (MMPs) and tissue inhibitors of metalloproteinases (TIMPs) levels. However, the impact of hypolipidemic and antioxidant treatment in MMPs and TIMPs is unknown. In the present study, we sought to compare the effects of statins and xanthine oxidase inhibitors on circulating levels of MMPs and TIMPs in patients with chronic HF. Methods: Forty-two clinically stable patients with mild to moderate HF were randomized to receive rosuvastatin 10 mg or allopurinol 300 mg daily and followed up for 1 month. Serum levels of MMP-2, -9, TIMP-1, and -2 were measured before and after treatment. Results: Levels of MMP-2 and -9 were significantly decreased in the rosuvastatin group (from 251±52 ng/ml and 400±206 ng/ml to 215±47 ng/ml and 309±166 ng/ml, p <0.001 and p <0.05 respectively), but not in the allopurinol group. In the rosuvastatin group, TIMP-2 levels were significantly increased (from 85±17 ng/ml to 93±16 ng/ml, p <0.05), while TIMP-1 remained unchanged. In the allopurinol group, no significant changes were observed regarding the levels of TIMPs. Conclusions: Short-term rosuvastatin but not allopurinol administration decreases MMP-2 and -9 and increases TIMP-2 levels. [ABSTRACT FROM AUTHOR]

Abstract: Abstract: Background: Depression has been associated with increased inflammatory process. Although anti-depressive medication has anti-inflammatory effect in major depression, its role in patients with heart failure (HF) is unknown. In the present study we evaluated the impact of antidepressive medication on the expression of proinflammatory cytokines and acute phase response proteins, in patients with HF and major depression. Methods: The study population consisted of 250 patients with HF (154 suffering from major depression). Patients with major depression were under selective serotonin reuptake inhibitors (SSRIs, n =120) or tricyclic antidepressants (TCA) and/or serotonin/norepinephrine reuptake inhibitors (SNRIs) (n =34), for at least 6 months. Results: Levels of TNF-α, IL-6, CRP and fibrinogen were not significantly different between HF patients with depression under treatment and those without depression (p =NS for all). However, TNF-α and CRP levels were significantly lower in patients receiving TCA/SNRI compared to patients receiving SSRIs or those without depression (p <0.05 for all). Similarly, patients under TCA/SNRI had significantly lower heart rate compared to those treated with SSRIs or those without depression. In multivariate analysis, treatment with SNRI/TCA was an independent predictor for log(TNF-α) (β =0.036(SE:0.016) and log(CRP) (β =0.099(SE:0.048), p =0.041). Conclusions: In the present study we demonstrate for the first time that treatment of patients with HF and major depression with TCAs/SNRIs, is associated with lower levels of TNF-α and CRP, suggesting that the type of antidepressive treatment may have a significant effect on the underlying inflammatory process. Abstract: The effects of an intracoronary infusion of NG-monomethyl-l-arginine (LNMMA) followed by intracoronary administration of nitroglycerin in non-stenotic proximal and distal coronary segments were studied in 11 patients with coronary artery disease and in 19 subjects with “normal arteriograms”. In normal subjects, LNMMA induced significant constriction (p<0.01) of proximal and distal vessels in non-smokers and smokers. In normal non-smokers, the reduction in coronary luminal diameter of proximal segments was significantly greater compared to normal smokers (p<0.05). In patients with coronary artery disease, LNMMA induced significant constriction of proximal and distal vessels in smokers, and only distal constriction in non-smokers (p<0.01). The reduction in coronary luminal diameter of the distal segments in normal smokers, and in both groups in patients with coronary artery disease was significantly greater compared with proximal segments (p<0.05). Therefore, the difference in vasomotor response to LNMMA in relation to smoking is localised to the proximal coronary segments. [Copyright &y& Elsevier]

Abstract: Background: Proinflammatory cytokines such as tumor necrosis factor alpha (TNF-α) and interleukin-6 (IL-6) are important mediators of immune response, associated with endothelial dysfunction in patients with coronary artery disease (CAD) or heart failure. We compared endothelial function and levels of IL-6 and TNF-α between patients with ischemic heart failure, dilated cardiomyopathy, CAD and healthy controls. Methods: The population consisted of 20 patients with dilated cardiomyopathy, 48 patients with ischemic cardiomyopathy, 26 patients with CAD and normal left ventricle function and 14 healthy controls. Forearm blood flow was measured using venous occlusion strain gauge plethysmography. Forearm vasodilatory response to reactive hyperemia (RH%) or to nitrate were considered as indexes of endothelium-dependent and endothelium-independent dilation, respectively. Results: Levels of IL-6 were significantly higher in ischemic cardiomyopathy compared to CAD patients (P<0.05) or controls (P<0.05) and in patients with dilated cardiomyopathy compared to controls (P<0.05). TNF-α levels were significantly higher in both groups with ischemic or dilated cardiomyopathy compared to CAD (P<0.05) or controls (P<0.05). RH% was significantly lower in ischemic and dilated cardiomyopathy compared to CAD (P<0.05) or controls (P<0.001) and higher in dilated than ischemic cardiomyopathy (P<0.05). Conclusions: Impaired endothelial function and increased inflammatory process were found in both types of heart failure. A greater endothelial dysfunction was observed in patients with ischemic heart failure compared to those with dilated cardiomyopathy, implying that the underlying atherosclerosis may participate in this process. Abstract: Basic fibroblast growth factor (bFGF) has been implicated in the pathogenesis of coronary atherosclerosis and in angiogenesis. We assessed the changes in serum bFGF before, immediately after, and 6 months after coronary angioplasty (PTCA). Using the ELISA methods we measured plasma bFGF in 28 patients who underwent PTCA, in 20 patients with coronary artery disease who underwent elective coronary angiography and in 28 healthy subjects. Before PTCA and coronary angiography, bFGF plasma levels were similar in both patient groups (4.4±1.0 vs. 3.3±0.5 pg/ml), but were significantly higher compared with those of the control group (0.8±0.1 pg/ml, P<0.05). By 24 h, 3 months and 6 months after PTCA, bFGF levels had decreased significantly in the PTCA group (3.2±0.6, 1.7±0.3 and 2.7±0.6 pg/ml, respectively, P<0.05). In conclusion, these findings show that bFGF levels are elevated in patients with coronary artery disease. Following PTCA, bFGF levels decreased significantly and remained stable for 6 months after the procedure. Thus, bFGF level may change in response to PTCA in patients with coronary artery disease and stable angina. [Copyright &y& Elsevier]
