335. Risk factors for postoperative complications in posterior scoliosis correction surgeries of pediatric patiens

Abstract

Objective To investigate the postoperative complication rate in pediatric patients who underwent posterior scoliosis correction surgeries, and to identify its risk factors. Methods This study was a retrospective cohort study. Anesthetic data of pediatric patients who underwent posterior scoliosis correction surgeries from January 2015 to December 2017 in Peking Union Medical College Hospital were collected and analyzed. The clinical variables were subsequently compared between patients in whom postoperative complication occured (complication group) and those in whom did not (non-complication group). Binary Logistic regression analysis was performed to identify risk factors of postoperative complication in these patients. Results The study enrolled 580 patients. The postoperative complication rate was 5.7% and the independent risk factors leading to it included preoperative Cobb angle larger than 69.5° (OR=3.043, 95% CI:1.436-6.448, P＜0.01) and perioperative allogeneic transfusion(OR=2.364, 95% CI:1.095-5.102, P＜0.05). The postoperative transfer rate to Intensive Care Unit(ICU)(P＜0.01) in complication group was higher, with longer hospital stay length (P＜0.001) than that in the non-complication group. Conclusions The risk factors for postoperative complication in pediatric patients receiving posterior scoliosis correction surgeries include preoperative Cobb angle larger than 69.5° and perioperative allogeneic transfusion.

Abstract

Objective To investigate the perioperative rate of allogeneic red blood cell (RBC) transfusion in adolescent scoliosis(AS) that underwent posterior correction surgeries, and to identify its risk factors. Methods This study was a retrospective cohort study. Anesthetic data of adolescent scoliosis patients who underwent posterior correction surgeries from January 2015 to May 2017 in Peking Union Medical College Hospital were collected and analyzed. The clinical variables were subsequently compared between patients who received allogeneic RBC transfusion(transfusion group) and those who did not (non-transfusion group). Binary logistic regression analysis was performed to identify risk factors in perioperative allogeneic RBC transfusion in these patients. Results The study enrolled 469 patients. The perioperative rate of allogeneic RBC transfusion was 30.5% and the independent risk factors leading to it included corrected body mass index (BMI) (OR=0.887, 95% CI: 0.815-0.966, P＜0.01),other type of scoliosis (OR=3.847, 95% CI: 1.660-8.917, P＜0.01) (compared with idiopathic scoliosis), preoperative anemia (OR=6.322, 95% CI: 2.053-19.473, P＜0.01), the number of fused levels more than 12.5 (OR=3.554, 95% CI: 2.174-5.885, P＜0.001), preoperative Cobb angle larger than 64.5° (OR=2.381, 95% CI: 1.356-4.178, P＜0.01) and osteotomy (OR=7.551, 95% CI: 3.258-17.503,P＜0.001). The postoperative transfer rate to intensive care unit(ICU)(P＜0.001) in transfusion group was higher, with longer hospital stay length(P＜0.001) and higher cost(P＜0.001), than that in the non-transfusion group. Conclusions The risk factors for perioperative allogeneic RBC transfusion in AS include corrected BMI, other type of scoliosis, preoperative anemia, the number of fused levels more than 12.5, preoperative Cobb angle larger than 64.5° and osteotomy.

Abstract

The Higgs boson decay channel, $H\to\gamma\gamma$, is one of the most important channels for probing the properties of the Higgs boson. In the paper, we reanalyze its decay width by using the QCD corrections up to $\alpha_s^4$-order level. The principle of maximum conformality has been adopted to achieve a precise pQCD prediction without conventional renormalization scheme-and-scale ambiguities. By taking the Higgs mass as the one given by the ATLAS and CMS collaborations, i.e. $M_{H}=125.09\pm0.21\pm0.11$ GeV, we obtain $\Gamma(H\to \gamma\gamma)|_{\rm LHC}=9.364^{+0.076}_{-0.075}$ KeV., Comment: 6 pages, 6 figures

Abstract

The predictive power of perturbative QCD (pQCD) depends on two important issues: (1) how to eliminate the renormalization scheme-and-scale ambiguities at fixed order, and (2) how to reliably estimate the contributions of unknown higher-order terms using information from the known pQCD series. The Principle of Maximum Conformality (PMC) satisfies all of the principles of the renormalization group and eliminates the scheme-and-scale ambiguities by the recursive use of the renormalization group equation to determine the scale of the QCD running coupling $\alpha_s$ at each order. Moreover, the resulting PMC predictions are independent of the choice of the renormalization scheme, satisfying the key principle of renormalization group invariance. In this letter, we show that by using the conformal series derived using the PMC single-scale procedure, in combination with the Pad\'e Approximation Approach (PAA), one can achieve quantitatively useful estimates for the unknown higher-order terms from the known perturbative series. We illustrate this procedure for three hadronic observables $R_{e^+e^-}$, $R_{\tau}$, and $\Gamma(H \to b \bar{b})$ which are each known to 4 loops in pQCD. We show that if the PMC prediction for the conformal series for an observable (of leading order $\alpha_s^p$) has been determined at order $\alpha^n_s$, then the $[N/M]=[0/n-p]$ Pad\'e series provides quantitatively useful predictions for the higher-order terms. We also show that the PMC + PAA predictions agree at all orders with the fundamental, scheme-independent Generalized Crewther relations which connect observables, such as deep inelastic neutrino-nucleon scattering, to hadronic $e^+e^-$ annihilation. Thus, by using the combination of the PMC series and the Pad\'e method, the predictive power of pQCD theory can be greatly improved., Comment: 7 pages, 1 figure. Abstract

We study the problem of named entity recognition (NER) from electronic medical records, which is one of the most fundamental and critical problems for medical text mining. Medical records which are written by clinicians from different specialties usually contain quite different terminologies and writing styles. The difference of specialties and the cost of human annotation makes it particularly difficult to train a universal medical NER system. In this paper, we propose a label-aware double transfer learning framework (La-DTL) for cross-specialty NER, so that a medical NER system designed for one specialty could be conveniently applied to another one with minimal annotation efforts. The transferability is guaranteed by two components: (i) we propose label-aware MMD for feature representation transfer, and (ii) we perform parameter transfer with a theoretical upper bound which is also label aware. We conduct extensive experiments on 12 cross-specialty NER tasks. The experimental results demonstrate that La-DTL provides consistent accuracy improvement over strong baselines. Besides, the promising experimental results on non-medical NER scenarios indicate that La-DTL is potential to be seamlessly adapted to a wide range of NER tasks., Comment: NAACL HLT 2018
