Abstract: The secretion in vitroof HCG and proteins was studied in fragments of placenta from women in the first trimester of pregnancy by a pulse-chase system. A 10-min pulse with [3H]leucine was used. It was concluded that the approximate half-time of release for HCG was 150 min. Proteins precipitable with trichloroacetic acid had a bi-exponential pattern, the half-times of release being 100 and 270 min. These rates of release indicate that the HCG produced by the early placenta was rapidly passed into the circulation rather than stored.

Abstract: In this paper we validate the finite element code RAPLICASOL, which models radiofrequency wave propagation in edge plasmas near ICRF antennas, against calculations with the TOPICA code. We compare the output of both codes for the ASDEX Upgrade 2-strap antenna, and for a 4-strap WEST-like antenna. Although RAPLICASOL requires considerably fewer computational resources than TOPICA, we find that the predicted quantities of experimental interest (including reflection coefficients, coupling resistances, S- and Z-matrix entries, optimal matching settings, and even radiofrequency electric fields) are in good agreement provided we are careful to use the same geometry in both codes.

Abstract: The research program of the TCV tokamak ranges from conventional to advanced-tokamak scenarios and alternative divertor configurations, to exploratory plasmas driven by theoretical insight, exploiting the device's unique shaping capabilities. Disruption avoidance by real-time locked mode prevention or unlocking with electron-cyclotron resonance heating (ECRH) was thoroughly documented, using magnetic and radiation triggers. Runaway generation with high-Z noble-gas injection and runaway dissipation by subsequent Ne or Ar injection were studied for model validation. The new 1 MW neutral beam injector has expanded the parameter range, now encompassing ELMy H-modes in an ITER-like shape and nearly non-inductive H-mode discharges sustained by electron cyclotron and neutral beam current drive. In the H-mode, the pedestal pressure increases modestly with nitrogen seeding while fueling moves the density pedestal outwards, but the plasma stored energy is largely uncorrelated to either seeding or fueling. High fueling at high triangularity is key to accessing the attractive small edge-localized mode (type-II) regime. Turbulence is reduced in the core at negative triangularity, consistent with increased confinement and in accord with global gyrokinetic simulations. The geodesic acoustic mode, possibly coupled with avalanche events, has been linked with particle flow to the wall in diverted plasmas. Detachment, scrape-off layer transport, and turbulence were studied in L- and H-modes in both standard and alternative configurations (snowflake, super-X, and beyond). The detachment process is caused by power 'starvation' reducing the ionization source, with volume recombination playing only a minor role. Partial detachment in the H-mode is obtained with impurity seeding and has shown little dependence on flux expansion in standard single-null geometry. In the attached L-mode phase, increasing the outer connection length reduces the in-out heat-flow asymmetry. A doublet plasma, featuring an internal X-point, was achieved successfully, and a transport barrier was observed in the mantle just outside the internal separatrix. In the near future variable-configuration baffles and possibly divertor pumping will be introduced to investigate the effect of divertor closure on exhaust and performance, and 3.5 MW ECRH and 1 MW neutral beam injection heating will be added., This work was supported in part by the Swiss National Science Foundation.

Abstract: Within the EUROfusion MST1 work package, a series of experiments has been conducted on AUG and TCV devices to disentangle the role of plasma fueling and plasma shape for the onset of small ELM regimes. On both devices, small ELM regimes with high confinement are achieved if and only if two conditions are fulfilled at the same time. Firstly, the plasma density at the separatrix must be large enough (), leading to a pressure profile flattening at the separatrix, which stabilizes type-I ELMs. Secondly, the magnetic configuration has to be close to a double null (DN), leading to a reduction of the magnetic shear in the extreme vicinity of the separatrix. Abstract: Abstract: An overview of several spectral line shapes studies of common interest in astrophysical and laboratory plasmas is presented. For lines dominated by Stark broadening, approaches taking into account the dynamics of numerous perturbers are sometimes required. We briefly recall ab initio simulation techniques and model microfield methods used for such conditions. Together with the impact approximation, such models may also be used for studying the effects on a line profile of a magnetic field of the order of the tesla, allowing the diagnostic of stellar objects or magnetic fusion devices. Abstract: In this work we investigate the influence of low frequency turbulence on Doppler spectral line shapes in magnetized plasmas. Low frequency refers here to fluctuations whose typical time scale is much larger than those characterizing the atomic processes, such as radiative decay, collisions and charge exchange. This ordering is in particular relevant for drift wave turbulence, ubiquitous in edge plasmas of fusion devices. Turbulent fluctuations are found to affect line shapes through both the spatial and time averages introduced by the measurement process. The profile is expressed in terms of the fluid fields describing the plasma. Assuming the spectrometer acquisition time to be much larger than the turbulent time scale, an ordering generally fulfilled in experiments, allows to develop a statistical formalism. We proceed by successively investigating the effects of density, fluid velocity and temperature fluctuations on the Doppler profile of a spectral line emitted by a charge exchange population of neutrals. Line shapes, and especially line wings are found to be affected by ion temperature or fluid velocity fluctuations, and can in some cases exhibit a power-law behavior. These effects are shown to be measurable with existing techniques, and their interpretation in each particular case would rely on already existing tools. From a fundamental point of view, this study gives some insights in the appearance of non-Boltzmann statistics, such as Lévy statistics, when dealing with averaged experimental data. The broadening mechanisms of these lines are examined for plasma conditions relevant to Tokamak divertors (electron densities and temperatures in the ranges 1019–1021 m−3 and 1–10eV). Under these conditions, the broadening mechanisms affecting the above lines are examined with and without the retaining of the magnetic field. The uncertainties on the deduced plasma parameters are also qualitatively estimated when only Stark and Doppler effects are retained while Zeeman and fine structure effects are ignored. The results presented are based on theoretical calculations carried out with the PPP line shape code and an atomic data basis built using Cowan’s code and the most accurate available atomic data for carbon. Owing to the competition between Stark and Doppler broadening mechanisms, the C IV n =6–7 (λ ∼7726Å) line profile depends on both the plasma electron density and temperature and the C3+ ion temperature. C IV n =6–7 line spectra, measured during an X-point MARFE phase in the JT-60U divertor using a high-resolution visible spectrometer, are fitted assuming the same temperature for both electrons and ions in order to determine the average electron density along the line of sight. These average electron densities deduced from the fit of C IV n =6–7 spectral line shapes corroborate those obtained using C IV line intensities and collisional-radiative modeling. The effect of fluctuations on the neutral density, ionisation source and Balmer α radial profiles are investigated. Abstract: Background: Existing internet-based prevention and treatment programmes for binge eating are composed of multiple distinct modules that are designed to target a broad range of risk or maintaining factors. Such multi-modular programmes (1) may be unnecessarily long for those who do not require a full course of intervention and (2) make it difficult to distinguish those techniques that are effective from those that are redundant. Since dietary restraint is a well-replicated risk and maintaining factor for binge eating, we developed an internet- and app-based intervention composed solely of cognitive-behavioural techniques designed to modify dietary restraint as a mechanism to target binge eating. We tested the efficacy of this combined selective and indicated prevention programme in 403 participants, most of whom were highly symptomatic (90% reported binge eating once per week)., Method: Participants were randomly assigned to the internet intervention ( n = 201) or an informational control group ( n = 202). The primary outcome was objective binge-eating frequency. Secondary outcomes were indices of dietary restraint, shape, weight, and eating concerns, subjective binge eating, disinhibition, and psychological distress. Analyses were intention-to-treat., Results: Intervention participants reported greater reductions in objective binge-eating episodes compared to the control group at post-test (small effect size). Significant effects were also observed on each of the secondary outcomes (small to large effect sizes). Improvements were sustained at 8 week follow-up., Conclusions: Highly focused digital interventions that target one central risk/maintaining factor may be sufficient to induce meaningful change in core eating disorder symptoms.

169. Efficacy of a transdiagnostic cognitive-behavioral intervention for eating disorder psychopathology delivered through a smartphone app: a randomized controlled trial.

Abstract: Background: Although effective treatments exist for diagnostic and subthreshold-level eating disorders (EDs), a significant proportion of affected individuals do not receive help. Interventions translated for delivery through smartphone apps may be one solution towards reducing this treatment gap. However, evidence for the efficacy of smartphones apps for EDs is lacking. We developed a smartphone app based on the principles and techniques of transdiagnostic cognitive-behavioral therapy for EDs and evaluated it through a pre-registered randomized controlled trial., Methods: Symptomatic individuals (those who reported the presence of binge eating) were randomly assigned to the app ( n = 197) or waiting list ( n = 195). Of the total sample, 42 and 31% exhibited diagnostic-level bulimia nervosa and binge-eating disorder symptoms, respectively. Assessments took place at baseline, 4 weeks, and 8 weeks post-randomization. Analyses were intention-to-treat. The primary outcome was global levels of ED psychopathology. Secondary outcomes were other ED symptoms, impairment, and distress., Results: Intervention participants reported greater reductions in global ED psychopathology than the control group at post-test ( d = -0.80). Significant effects were also observed for secondary outcomes ( d 's = -0.30 to -0.74), except compensatory behavior frequency. Symptom levels remained stable at follow-up. Participants were largely satisfied with the app, although the overall post-test attrition rate was 35%., Conclusion: Findings highlight the potential for this app to serve as a cost-effective and easily accessible intervention for those who cannot receive standard treatment. The capacity for apps to be flexibly integrated within current models of mental health care delivery may prove vital for addressing the unmet needs of people with EDs.

Abstract: Despite their potential as a scalable, cost-effective intervention format, self-guided Internet-based interventions for eating disorder (ED) symptoms continue to be associated with suboptimal rates of adherence and retention. Improving this may depend on the design of an Internet intervention and its method of content delivery, with interactive programs expected to be more engaging than static, text-based programs. However, causal evidence for the added benefits of interactive functionality is lacking. We conducted a randomized controlled comparison of an Internet-based intervention for ED symptoms with and without interactive functionality. Participants were randomized to a 4-week interactive (n = 148) or static (n = 145) version of an Internet-based, cognitive-behavioral program. The interactive version included diverse multimedia content delivery channels (video tutorials, graphics, written text), a smartphone app allowing users to complete the required homework exercises digitally (quizzes, symptom tracking, self-assessments), and progress monitoring features. The static version delivered identical intervention content but only via written text, and contained none of those interactive features. Dropout rates were high overall (58%), but were significantly-yet slightly-lower for the interactive (51%) compared to the static intervention (65%). There were no significant differences in adherence rates and symptom-level improvements between the two conditions. Adding basic interactive functionality to a digital intervention may help with study retention. Abstract: Purpose: Digital interventions that consider end-user needs, preferences, and concerns may address suboptimal rates of e-health uptake, usage, and engagement. We explored target-user perspectives of e-health treatment and prevention programs for eating disorders (EDs), with a focus on investigating (1) perceived advantages and barriers of e-health; (2) help-seeking intentions; and (3) preferences for different digital functionality, device types, and content-delivery formats., Methods: Survey data were analysed from 722 community-based participants. Participants were categorized into one of four groups based on symptom presentation and severity, ranging from low risk to probable bulimia nervosa or binge-eating disorder., Results: e-health advantages that received the highest endorsement (~ 84%) were "always there in times of need" and "travel not required". e-health barriers that received the highest endorsement (~ 50%) were concerns about data privacy and the accuracy of content presented. Nearly three-quarters reported an intention to use an e-health platform for preventing or treating EDs. Preference ratings were highest for programs to be available on all digital devices (relative to restricting the program to one type of device) and for content to be presented via graphics and video tutorials (rather than audio-based). e-health functionality that received highest preference ratings (~ 80%) were added clinician support, tailored feedback, strategies to change unhelpful ED thoughts, screening scales to assess symptoms, ED psychoeducation, and just-in-time intervention prompts. Preference and intention ratings were strikingly similar across all subgroups., Conclusion: Findings may inform the development and design of e-health platforms that meet the needs of people at different stages of an ED., Level of Evidence: Level V, cross-sectional descriptive study.

172. Collisional redistribution of hydrogen line radiation in low- and moderate-density magnetized plasmas.

Abstract: A computer simulation technique has been applied to the modeling of radiation redistribution functions in low- and moderate-density magnetized hydrogen plasmas. The radiating dipole is described within the Heisenberg picture, and perturbations by the plasma microfield are accounted for through a time-dependent Stark effect term in the Hamiltonian. Numerical applications are presented for the first Lyman and Balmer lines at plasma conditions relevant to tokamak divertors and magnetized white dwarf atmospheres. In both cases, the collisional redistribution of the radiation frequency is shown to be incomplete. Comparisons with a previously developed impact model are performed, and results are discussed.

Abstract: Objectives: Internet-based psychoeducational and self-help platforms hold promise for alleviating existing help-seeking barriers and addressing the unmet needs of people with eating disorders (EDs). In this paper, we report data related to the reach, engagement, and visitor profile of Break Binge Eating, an online platform designed to provide evidence-based information and self-help strategies for people at all stages of an ED., Method: Two sources of data were presented: (a) usage data from platform visitors generated through Google Analytics; and (b) characteristics of a sample of platform visitors (n = 786)., Results: In 13 months, approximately 46,311 unique users worldwide have accessed this platform, with usage rates rapidly increasing each month. Most visitors came from organic searches (when ED-related information is directly searched in a browser). Self-help content was the most accessed material, and 81% of the sample stated that their reason for accessing the platform was to get help. Sample visitors were highly symptomatic; 52% met criteria resembling a threshold ED and 87% engaged in at least one ED behavior in the past month. Across different symptomatic subgroups, ∼50% were unsure whether they needed help, ∼80% were not receiving any help, and ∼75% were highly concerned with their symptoms., Conclusion: This online platform has broad reach and is engaging its intended audience. Abstract: Background: This study was designed to estimate the rates of late genitourinary (GU) and rectal toxicity after magnetic resonance image (MRI)-guided prostate brachytherapy exclusively or in conjunction with external beam radiation therapy (EBRT)., Methods: Between November 1997 and April 2002, 201 patients with category T1C prostate carcinoma (according to the 2002 American Joint Committee on Cancer staging criteria), prostate specific antigen levels < 10 ng/mL, and biopsy Gleason score 3 + 4 disease were treated with MRI-guided brachytherapy exclusively or in conjunction with EBRT. The MRI-guided technique was designed to spare the urethra based on delivery of the prescription dose to the peripheral zone exclusively. The Kaplan-Meier method was used to estimate rates of freedom from late GU and rectal toxicity. Comparisons were made using a log-rank test., Results: At a median follow-up of 2.8 years (range, 0.5-5.0 years), the 4-year estimates of rectal bleeding requiring coagulation for patients who underwent implantation therapy, compared with patients who received combined-modality therapy, were 8% versus 30%, respectively (log-rank P value = 0.0001). Although erectile dysfunction was common (range, 82-93%), with the use of sildenafil citrate (Viagra), it was estimated that at least two-thirds of patients had erectile function comparable to or superior to baseline function, independent of whether they received monotherapy or combined-modality therapy (P = 0.46). The 4-year estimate of freedom from radiation cystitis was 100% versus 95% (P = 0.01) for patients who received monotherapy and patients who received combined-modality therapy, respectively. No urethral strictures were observed, and no patients underwent postimplantation transurethral resection of the prostate., Conclusions: In the current study, rectal bleeding after MRI-guided prostate brachymonotherapy was infrequent, and urethral and bladder toxicity is reported to be rare and may be attributed to the urethral-sparing technique of the MRI-guided approach., (Copyright 2003 American Cancer Society.)

Abstract: The secretion in vitro of HCG and proteins was studied in fragments of placenta from women in the first trimester of pregnancy by a pulse-chase system. A 10-min pulse with [3H]leucine was used. It was concluded that the approximate half-time of release of HCG was 150 min. Proteins precipitable with trichloroacetic acid had a bi-exponential pattern, the half-times of release being 100 and 270 min. These rates of release indicate that the HCG produced by the early placenta was rapidly passed into the circulation rather than stored.
