154. Unusual ventilation-perfusion patterns in primary lung tuberculosis

Abstract

We report two cases of primary lung tuberculosis in children with unusual perfusion ventilation scintigraphic patterns. In the first case, a mismatch in the right upper lobe suggests an elective compression of the bronchi by the mediastinal lymph nodes; in the second case, the total absence of ventilation and perfusion of the left lobe at scintigraphy illustrates the discrepancy sometimes encountered between chest x-ray and lung scintigraphy., SCOPUS: ar.j, FLWNA, info:eu-repo/semantics/published

168. Determination of right ventricular ejection fraction in children with cystic fibrosis

Abstract

The radionuclide right ventricular ejection fraction (RVEF) determined by means of Krypton‐81 m represents a simple, noninvasive, and accurate procedure to quantify the right ventricular contractility. This procedure was applied to 25 young patients with cystic fibrosis. The RVEF tended to decrease with the progression of the lung disease, as assessed by the clinical S‐K score, the degree of the defects on lung scintigraphy, the Pa O 2, and the lung function tests. However, the decrease of RVEF in patients with marked lung involvement was moderate, and terminal lung disease was sometimes associated with normal right heart contractility. Four patients' positions were tested in search of increased sensitivity of gastroesophageal scintiscanning for the detection of reflux in children: supine, prone, left lateral, and 30° right posterior oblique. The sensitivity was highest when the child was placed in supine position. Using a recently described radioisotopic technique, the separate glomerular filtration rate of 67 kidneys associated with vesicoureteric reflux has been determined. Normal values were found in only half of the kidneys. Abstract

99mTc colloid scintigraphy is a sensitive technique for the detection of gastro-oesophageal reflux, compared to X-ray studies. A quantification index of reflux is used and can be of value when there is concern about the significance of a positive result. Continuous monitoring during one hour increases by 25% the detection of reflux in comparison with a 30 min test. In our series, no case of lung aspiration was recognized using this technique. Abstract

An 11-year-old boy had bone scanning to rule out an osscous lesion of the right arm. He presented progressive pain and hard swelling of the right arm. His past medical history and general physical examination were unremarkable. He trained for karate. The scan demonstrated considerable muscular uptake in both arms. CPK and CPK MB levels were both abnormally high, suggesting muscle injury. After a 10-day rest period the bone scan returned to normal. [ABSTRACT FROM AUTHOR]
