Abstract

Eosinophilic esophagitis (EoE) is of increasing prevalence and thought to result from allergic processes. Helicobacter pylori has been inversely associated with allergic diseases, but there is no known relationship between H. pylori, EoE and esophageal eosinophilia. We investigated the association between esophageal eosinophilia and H. pylori infection.

Abstract

BackgroundConstipation, a low fiber diet, sedentary lifestyle and gravidity are commonly assumed to increase the risk of hemorrhoids. However, evidence regarding these factors is limited. We examined the association between commonly cited risk factors and the prevalence of hemorrhoids.MethodsWe performed a cross sectional study of participants who underwent a colonoscopy in a colorectal adenoma prevention trial and who had a detailed assessment of bowel habits, diet and activity. The presence of hemorrhoids was extracted from the subjects’ colonoscopy reports. We used logistic regression to estimate odds ratios and 95% confidence intervals while adjusting for age and sex.ResultsThe study included 2,813 participants. Of these, 1,074 had hemorrhoids recorded. Constipation was associated with an increased prevalence of hemorrhoids (OR 1.43, 95% CI 1.11, 1.86). Of the fiber subtypes, high grain fiber intake was associated with a reduced risk (OR for quartile 4 versus quartile 1 = 0.78, 95% CI 0.62, 0.98). We found no association when comparing gravid and nulligravida women (OR 0.93, 95% CI 0.62–1.40). Sedentary behavior was associated with a reduced risk (OR 0.80, 95% CI 0.65–0.98), but not physical activity (OR 0.83, 95% CI 0.66–1.03). Neither being overweight nor obese was associated with the presence of hemorrhoids (OR 0.89, 95% CI 0.72–1.09 and OR 0.86, 95% CI 0.70–1.06).ConclusionsConstipation is associated with an increased risk of hemorrhoids. Gravidity and physical activity do not appear to be associated. High grain fiber intake and sedentary behavior are associated with a decreased risk of hemorrhoids.

Abstract

Background: A contemporary report describing the burden and expenditures of gastrointestinal (GI) diseases can be helpful for policy makers, administrators, and researchers. Using the most recent data, we estimated the burden and costs associated with GI diseases in the United States., Methods: We generated estimates using data from the Rome Foundation Global Epidemiology Study 2017-2018 (symptoms), National Ambulatory Medical Care Survey 2019 and National Hospital Ambulatory Medical Care Survey 2019 (ambulatory visits), Nationwide Emergency Department Sample 2021 (emergency department visits), National Inpatient Sample 2021 (admissions), Kids' Inpatient Database 2019 (admissions), National Program of Cancer Registries 2001-2021 (cancer incidence), National Center for Health Statistics 2001-2021 (cancer mortality), Centers for Disease Control Wide-ranging Online Data for Epidemiologic Research 2021 (non-cancer mortality), MarketScan Commercial Claims and Encounters data 2002-2021 (endoscopy), MarketScan Medicare Supplemental data 2002-2021 (endoscopy), United Network for Organ Sharing Registry 2023 (transplant), Medical Expenditure Panel Survey 2021 (expenditures), and National Institutes of Health (NIH) 2012-2025 (research)., Results: In 2021, GI health care expenditures totaled $111.8 billion. A GI diagnosis or symptom led to 14.5 million emergency department visits and 2.9 million hospital admissions. There were 315,065 new GI cancers diagnosed. GI diseases caused 281,413 deaths. In 2022, an estimated 23.5 million GI endoscopies were performed. In 2023, the NIH supported $3.6 billion for GI research, which represents 7.4% of the NIH budget., Conclusion: GI diseases are responsible for a considerable and growing burden of health care use and costs. 128. Plasma insulin, glucose, IGF-I, IGF-II, and IGFBP-3 and risk of recurrent colorectal adenomas.

Abstract

Aim: Insulin and insulin-like growth factors (IGF's) are associated with an increased risk of colorectal adenomas. The association with recurrent adenomas is inadequately studied. We prospectively examined the relationship between insulin biomarkers and the risk of recurrent adenomas., Materials and Methods: Our analysis included 167 subjects with one or more adenomas detected on a baseline colonoscopy who had a surveillance colonoscopy within three to five years. We measured serum biomarkers in all subjects at baseline. ELISA was used to measure fasting plasma insulin, IGF-I, IGF-II, and IGF binding protein-3. The hexokinase assay was used to measure fasting plasma glucose by and immuno-turbidimetric assay to measure hemoglobin A1C., Results: Subjects with recurrent adenomas were more likely to be male, overweight and have ≥3 adenomas at baseline. We found no significant associations between insulin (OR=1.6, 95% CI 0.7-3.5), glucose (OR=1.4, 95% CI 0.7-3.1), IGF-I (OR=0.7, 95% CI 0.3-1.5), IGF-II (OR=1.0, 95% CI 0.5-2.3), IGFBP-3 (OR=0.1, 95% CI 0.5-2.1), or anthropometric measures and recurrent adenomas., Conclusion: Our results do not support a role for insulin biomarkers and recurrent colorectal adenomas.
